The purpose of this study was to evaluate the diagnostic efficacy of magnetic resonance (MR)-guided biopsy of focal liver lesions within a short, wide-bore 1.5-T MR system and to determine the duration and accuracy of needle placement using MR fluoroscopy guidance in 25 patients. Accuracy of needle placement was evaluated in two orthogonal planes, and the out-of-plane angle of needle deflection was measured. Needle positioning was characterised subjectively as centred, peripheral, or exterior relative to the lesion. Exterior positioning was corrected by a step-by-step procedure. Surgical resection (n = 6), previous histologies (n = 8), or clinical/radiological follow-up (n = 11) served as the ‘gold standard’. The guidance needle could be placed successfully using MR fluoroscopy in 20 of 25 patients (80%). Needle placement was rated as ‘centred’ in 11 and as ‘peripheral’ in nine patients. Median needle deflection was 2.6 degrees, with a median deviation of 3.4 mm. In five patients, the direct approach failed or was rated as ‘exterior’; therefore, repositioning after needle stabilisation with a stainless-steel stylet was necessary. The diagnostic yield of all biopsies was: sensitivity 95.5%, specificity 100.0% and accuracy 96.0%. In conclusion, MR-guided biopsies in a short, wide-bore MR system yielded highly reliable biopsy results, and in most cases the direct approach with MR fluoroscopy guidance proved to be fast and accurate.  相似文献

To compare the clinical usefulness of T2-weighted breath-hold sequences for imaging the liver, 33 patients with 97 focal hepatic lesions were studied with a 1.0-T scanner by using T2-weighted breath-hold turbo spin-echo (SE) sequences and T2-weighted breath-hold half-Fourier single-shot turbo SE (HASTE) sequences with and without fat suppression. Images were quantitatively analyzed for liver signal-to-noise ratio (SNR) and lesion-to-liver contrast-to-noise ratios (CNR). Qualitative analysis was performed for lesion conspicuity, motion artifacts, and anatomic sharpness of extrahepatic structures. Breath-hold turbo SE imaging with fat suppression showed the highest CNR for cystic lesions and the best lesion conspicuity for cystic and solid lesions among the four sequences. For solid lesions, there was no significant difference of lesion-to-liver CNR between them. HASTE sequence was superior to turbo SE sequences in terms of motion artifacts; however, the usefulness for evaluating focal hepatic lesions was limited compared with turbo SE sequence with fat suppression. Addition of fat suppression was not helpful for HASTE imaging because of decreased lesion conspicuity and extrahepatic details without the advantage of reducing motion artifacts. This study suggests that turbo SE sequence with fat suppression is most useful for breath-hold T2-weighted liver imaging at 1.0 T. Addition of imaging without fat suppression can be considered for evaluating extrahepatic structures. HASTE sequence may have a role for imaging uncooperative patients due to absence of motion artifacts.  相似文献

RATIONALE AND OBJECTIVES: The purpose of this study was to calculate the gain in signal-to-noise ratio (SNR) of four human abdominal tissues at 3.0 Tesla (T) compared with standard 1.5 T and to validate this calculation in vivo. MATERIALS AND METHODS: The expected gain in SNR at 3.0 T in the liver, pancreas, spleen, and kidney compared with standard 1.5 T was approximated theoretically for a T2-weighted HASTE (half-Fourier acquisition single-shot turbo spin-echo) and a T1-weighted gradient-echo in- and opposed-phase sequence. Fifteen healthy male subjects underwent abdominal MR imaging using a 1.5 T and 3.0 T scanner. Coronal T2-weighted HASTE images and axial T1-weighted gradient-echo in- and opposed-phase images were acquired using the sequence parameters optimized by the vendor. RESULTS: Except for opposed-phased imaging of pancreatic tissue, in vivo adjusted SNR values of all abdominal tissues were significantly higher at 3.0 T for all sequences (P < .05). The highest overall gain in SNR was achieved with the HASTE sequence ranging from 3.8-fold for renal imaging to 7.4-fold for hepatic imaging. The theoretical calculation of SNR gain was in good agreement with the experimentally measured gain in SNR for the HASTE and the in-phase sequence. CONCLUSION: High-field abdominal MR imaging at 3.0 T offers significantly higher SNR compared with standard 1.5 T MR imaging.  相似文献

OBJECTIVE: To compare the image quality and visualization obtained in MR cholangiopancreatography (MRCP) using different high-field strength (1.0 vs. 1.5 Tesla) MR units and to assess the effect of field strength on MRCP. MATERIALS AND METHODS: This study population included 10 healthy volunteers and 37 patients suspected of having pancreatobiliary diseases. MRCP images were obtained using two MR units with different high-field strengths (1.0 and 1.5 Tesla), with half-Fourier acquisition single-shot turbo spin-echo (HASTE) and rapid acquisition by relaxation enhancement (RARE) sequences. The image quality and visualization of each portion of the pancreatobiliary system were graded and recorded using a four-point scale. Additionally, the signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) were measured. RESULTS: The SNR and CNR in HASTE sequences acquired with the 1.5 Tesla (T) unit were significantly higher than those acquired with the 1.0 T unit (p=0.001). In qualitative analysis, there were no statistically significant differences in image quality or visualization of the ducts in either HASTE or RARE sequences between 1.0 T and 1.5 T. CONCLUSION: Our study showed that visual image quality provided by MRCP was equivalent at 1.0 and 1.5 T.  相似文献

The purpose of this study was to quantitatively compare the hepatic contrast characteristics of conventional spin-echo (CSE) and fast spin-echo (FSE) sequences with breath-hold T2-weighted images acquired with half-Fourier turbo spin echo (HASTE). Forty-five patients were examined with a phased-array surface coil. Nineteen patients had focal hepatic lesions, including eight malignant tumors, 10 cavernous hemangiomas, and one hepatic adenoma. Twenty-six patients had no focal hepatic lesions. T2-weighted images with comparable TE were acquired with CSE, FSE, and HASTE pulse sequences. Signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) for liver, spleen, and lesions were measured. FSE demonstrated significantly better quantitative performance than CSE for liver-spleen CNR (P = .0084). No statistically significant difference was demonstrated between FSE and CSE for liver or spleen SNR. FSE demonstrated clear scan time and resolution advantages over CSE. HASTE performed significantly poorer than CSE and FSE for liver-spleen CNR (P < .0001), liver SNR (P = .0002 for CSE and P < .0001 for FSE), and spleen SNR (P < .0001). Optimized FSE images with a short echo train length performed comparably to CSE images of equivalent TE. Liver-lesion CNR was suppressed on HASTE images, suggesting that long echo train length FSE sequences could diminish solid lesion detection compared to CSE and short echo train length FSE.  相似文献

PURPOSE: To develop and evaluate an MR sialography protocol that reproducibly images the parotid and submandibular ducts over time, in 3D. Such a protocol is needed in order to investigate the possible radiation-induced changes to the salivary ducts in patients receiving radiotherapy to the head-and-neck. MATERIALS AND METHODS: MR sialography was performed on a 1.5-T MR scanner. Sequence parameters were optimized on 11 healthy volunteers. A 3D water-selective turbo spin echo (TSE) pulse sequence (TR/TE = 6000 msec/190 msec), using a two-element circular surface coil was applied twice in one MR session. In order to assess the reproducibility, the same procedure was repeated four to six months later. The quality of the MR sialograms was measured subjectively by developing a visibility scoring system and objectively by the means of contrast-to-noise ratio (CNR) of the ducts vs. fat (CNR(duct-fat)). RESULTS: High-quality, 3D MR sialographic images were obtained. The quality of the MR sialograms and the subjective visibility score of the salivary ducts were constant over time. The CNR(duct-fat) varied between volunteers (standard deviation, SD 26%) but it was relatively constant per volunteer (SD 5%). CONCLUSION: The MR sialography protocol presented in this study provides good quality 3D imaging of the major salivary ducts, submandibular duct, and the parotid duct and it can be used for the comparison of the salivary duct system of an individual over time.  相似文献

PURPOSE: The value of the fast half-Fourier single-shot turbo spin echo (HASTE) sequence in T2-weighted MRI of the kidney was evaluated as a substitute for the conventional turbo spin echo (TSE) sequence. METHOD: Forty-five patients with suspected abnormalities of the kidney underwent MRI with a 1.5 T system. Breath-hold HASTE and respiratory-triggered TSE sequences were performed. Qualitative and quantitative analyses were performed for comparison of these sequences. RESULTS: The signal-to-noise ratio (SNR) with HASTE was higher than that with TSE. The lesion-to-kidney contrast-to-noise ratio for solid masses with HASTE was almost equal to that with TSE. For cystic masses, the CNR with HASTE was significantly higher than that with TSE (p < 0.05). Respiratory and chemical shift artifacts were significantly smaller on HASTE than on TSE (p < 0.01). However, the blurring artifact was higher on HASTE than on TSE (p = 0.01). CONCLUSION: The HASTE sequence generates high contrast images and is free of motion and chemical shift artifacts, with much better time efficacy. The sequence provides comparable diagnostic information to TSE sequences.  相似文献

The purpose of this study was to compare the relative usefulness of multishot turbo spin echo (TSE) and half-Fourier single-shot turbo spin echo (HASTE) for determination of optimal breath-hold fast T2-weighted technique in terms of lesion detection, lesion-to-liver contrast-to-noise ratio (CNR), and image quality. The images of TSE with and without fat suppression (FS) and of HASTE with and without FS were retrospectively reviewed for 49 patients with 128 lesions. Without FS, TSE and HASTE images allowed depiction of focal hepatic masses (112 of 128, sensitivity = 87.5%) at the same rate. TSE with FS depicted more focal lesions (115 of 128, 89.8%) than HASTE with FS (109 of 128, 85.2%), but the difference was not statistically significant (P > .05). The CNR of each lesion on HASTE sequences was greater (P < .01) than that on TSE sequences. The CNR of hemangioma was distinct from that of solid tumors and cystic lesions in all sequences, and the range of CNR in each group of pathologies overlapped less and were well separated in the HASTE sequences. HASTE sequences produced better image quality with fewer artifacts (P < .0001). The results of this study suggest that HASTE sequences allow differentiation between solid tumors, hemangiomas, and cystic lesions in terms of CNR, producing fewer image artifacts, with acceptable sensitivity in lesion detection.  相似文献

The aim of this study was to examine the feasibility of a hybrid interventional MR system, which combines a closed bore magnet with a C-arm fluoroscopy unit for percutaneous drainage of abdominal fluid collections. During the past 2 years, we have performed four drainage procedures in four patients (mean age 47 years). Three patients had abscesses (psoas muscle, kidney, subphrenic location) and the fourth patient had a recurrent splenic cyst. All procedures were performed on an interventional MR system consisting of a 1.5-T ACS-NT scanner combined with a specially shielded C-arm. The drainages were guided by T1-weighted fast gradient-echo images, T2-weighted single-shot turbo spin-echo images or both. A standard 18 G (1.2 mm) nonferromagnetic stainless steel needle with a Teflon sheath was used for the punctures following which a 0.89 mm nitinol guidewire was inserted into the fluid collection. Thereafter, the patient was positioned in the immediate adjacent fluoroscopy unit and a drainage catheter was placed under fluoroscopic control. All drainage catheters were successfully placed into the fluid collections, as proven by fluid aspiration and resolution of the collection. The mean time needed for the entire drainage procedure (MR and fluoroscopy) was 110 min. No procedure-related complications occurred. It is feasible to perform drainage procedures on a closed-bore MR scanner. The multiplanar imaging capabilities of MR are particularly helpful for fluid collections in the subphrenic location. So that we might evaluate the ultrafast half-Fourier single-shot turbo spin-echo (HASTE) sequence in T2-weighted MRI of the female pelvis and compare it with the turbo spin-echo (TSE) sequence, we prospectively studied 60 consecutive females with suspected abnormalities of the pelvis. For all MR examinations, we used a 1.5-T superconductive magnet with a phased array coil. The HASTE sequence was applied with TR/effective TE/echo train = ∞/90/64 and a 128 × 256 matri× (acquisition time: .3 sec/slice), conventional TSE imaging with 3,400 to 5,000/132/15 and a 128 × 256 matri× (mean acquisition time: 2 min 4 sec), and high-resolution TSE imaging with 3,400 to 5,000/132/15 and a 300 × 512 matri× (6 min 4 sec). Although the lesion conspicuity for the HASTE sequence was less than that for the high-resolution TSE sequences, artifacts (including ghosting, bowel motion, susceptibility difference, and chemical shift) were negligible on HASTE images of all patients. The lesion conspicuity for the HASTE sequence was significantly better than for the conventional TSE sequence. In spite of the very short acquisition time, the subjective scoring of the overall image quality for the HASTE sequence was significantly higher than for the conventional TSE sequence (P < .01) and were slightly lower than for the high-resolution TSE sequence. Compared with high-resolution TSE, HASTE provided clearer visualization of large leiomyomas and ovarian tumors but slightly poorer visualization of uterine cancer. In occlusion, HASTE sequence generates higher contrast and is free from motion and chemical shift artifact with much higher time efficacy. Because of limited image resolution, the HASTE sequence should be used when the high-resolution TSE imaging is suboptimal.  相似文献

The purpose of this study was to demonstrate the utility of a T2-weighted single shot turbo spin-echo technique — the so-called “Local Look” (LoLo) and more recently renamed “Zoom Imaging” technique — for MR-guided percutaneous interventions. We performed 28 procedures on 22 patients using a 1.5-T system for MR guidance. All procedures were controlled with the LoLo technique, which acquires T2-weighted images in 600 msec. This is achieved by using a small field of view (250 × 125 mm) along with a maximum echo train length, the so-called “single shot method.” To prevent backfolding artifacts, the 90° and 180° pulses were oriented orthogonally to each other. Because signal is created only in the region in which the pulses overlap, no backfolding can occur from outside this area. Half of the biopsies were additionally monitored using a fast gradient-echo sequence, which was compared with the LoLo technique. All of the procedures were technically successful, and there were no procedural complications. The LoLo technique produced images that had good contrast between the lesion and the needle artifact, and the artifact size was smaller than that produced by the gradient-echo technique. Subjective judgment of the ability to accurately delineate the needle tip indicated that the LoLo technique was either superior to (73%) or equal to (27%) the gradient-echo sequence in all cases. The LoLo technique is an accurate and effective method for MR guidance of percutaneous procedures, because it shows good lesion contrast and small needle artifacts. The additional use of a gradient-echo sequence during the procedure planning stage is advisable in more difficult cases, particularly when adjacent blood vessels are a concern. Monitoring of the needle tip is best performed with the LoLo technique.  相似文献

Respiratory and cardiac motion correction may result in better turbo spin-echo (SE) imaging of the lung. To compare breath-hold cardiac-gated black-blood T2-weighted turbo SE and turbo short-inversion-time inversion-recovery (STIR) magnetic resonance (MR) imaging pulse sequences with conventional breath-hold turbo SE and half-Fourier acquisition single-shot turbo spin-echo (HASTE) sequences for lesion conspicuity of focal lung lesions, 42 patients with focal lung lesions were prospectively studied with MR imaging at 1.5 T. Helical computed tomography was used as a reference. In comparison with the conventional breath-hold turbo SE sequence, all black-blood sequences had fewer image artifacts arising from the heart and blood flow. The overall image quality for the black-blood turbo SE and turbo STIR sequences was superior to that for the breath-hold turbo SE and HASTE sequence (P < 0.01). Not only focal lung lesions but also surrounding inflammatory changes were clearly visualized with these two sequences. With the HASTE sequence, although several slices could be obtained in one breath-hold, both the tumor and vessels appeared blurred. We conclude that T2-weighted turbo SE and turbo STIR imaging of the lung with effective suppression of flow and motion artifacts provide high-quality images in patients with focal lung lesions.  相似文献

Thirty-seven consecutive patients with elevated PSA levels and negative tumor prostate biopsies underwent a MR-guided prostate biopsy in a 1.5-T scanner in the supine position. After localization of suspected tumor areas using an endorectal coil and two body-phased array coils, the biopsy device was positioned without any repositioning of the patient. The biopsy device consisted of a mount, a ball joint, a positioning stage and an insertion stage with a needle guide, which was filled with a MR-visible fluid to control positioning of the needle using a balanced steady-state free precession sequence (TrueFISP) and a high-resolution turbo spin echo (T2-TSE) sequence. Core biopsies were taken manually in the magnet. The biopsy needle could be correctly positioned in all cases. Suspected lesions with a diameter ≥10 mm could be successfully punctured. Four to nine (mean =6) biopsies were taken per patient. In 14 patients, prostate cancer was confirmed at histology. Twenty-four biopsies positive for cancer were performed in 14 patients. A correct correlation was found between the site of biopsy and histology. MR-guided prostate biopsy can be effective in increasing primary positive tumor biopsy results in patients with a history of negative tumor TRUS-guided prostate biopsies.  相似文献

MR-guided biopsy of musculoskeletal lesions in a low-field system   总被引:2,自引:0,他引:2
Thirty magnetic resonance (MR)-guided biopsies were obtained from 20 skeletal and 10 soft-tissue lesions in 31 patients using an open 0.2 T MR system equipped with interventional accessories. The results from aspiration (N = 3), core biopsy (N = 15), and transcortical trephine biopsy (N = 12) were evaluated for accuracy and clinical efficacy. Specimens were successfully obtained from 29 patients. Results were clinically effective in 23 patients, rated definitive in 16, nonconclusive in 9, and unspecific in 2 patients. A false diagnosis due to sampling error occurred in 2 patients, and biopsy sampling was impossible in one case. The best diagnostic yield was achieved from nontranscortical biopsies of osteolytic or soft-tissue masses. Results from transcortical biopsies were less specific due to the predominance of benign lesions. MR fluoroscopy for needle guidance was applied in 13 patients. Complete needle placement inside the magnet could be performed in 16 patients. MR-guided biopsy using an open low-field MR imager is feasible and clinically effective and will become a valuable tool in the management of musculoskeletal lesions. J. Magn. Reson. PURPOSE: To find the optimal contrast agent dilution to maximize signal intensity (SI), signal-to-noise ratio (SNR), and contrast-to-noise ratio (CNR) in shoulder MR arthrography using MR systems operating at different magnetic field strengths. MATERIALS AND METHODS: Autoptic human glenohumeral ligaments were inserted in eight egg-shaped 20-mL phantoms filled with saline and gadolinium diethylenetriaminepentaacetic acid bismethylamide (Gd-DTPA-BMA) in different dilutions of 0.5, 1, 2, 2.5, 5, 12.5, 50 mmol/liter, to simulate the shoulder articular capsule. These phantoms were inserted inside two plastic 240-mL phantoms filled with water. MRI was performed on 0.2-, 0.5-, 1.0-, 1.5-, and 3.0-T MR systems using a three-dimensional gradient echo (GRE)-T1-weighted pulse sequence. SI, SNR, and CNR were determined. RESULTS: Peak SI and SNR were found at 5 mmol/liter, with the exception of the 0.2-T scanner, where the maximum was at 2 mmol/liter. Peak CNR was observed at 1 mmol/liter for the 3-T scanner, at 2 mmol/liter for the 0.2- and 0.5-T scanners, and at 5 mmol/liter for the remaining scanners. CONCLUSION: The optimal SI and SNR are provided by 5 mmol/liter contrast agent dilution. Peak CNR was found in a range between 1 and 5 mmol/liter dilutions, depending on the strength of the magnetic field.  相似文献

PURPOSE: To demonstrate the feasibility of detecting atherosclerotic vascular disease using an innovative magnetic resonance angiography (MRA) protocol in combination with a dedicated whole-body MR scanner with new surface coil technology. MATERIALS AND METHODS: A total of 10 volunteers and eight patients with peripheral arterial occlusive disease (PAOD) were examined at 1.5 T. Conventional digital subtraction angiography (DSA) of the symptomatic region was available as a reference standard in all eight patients. Depending on subjects' size, four to five three-dimensional data sets were acquired using an adapted injection protocol. Images were assessed independently by two readers for vascular pathology. Additionally, signal-to-noise ratios (SNRs) and contrast-to-noise ratios (CNRs) were measured. RESULTS: Whole-body MRA yielded excellent sensitivity and specificity of more than 95% for both readers with high interobserver agreement (k = 0.93). Surface coil signal reception rendered a high SNR (mean 151.28 +/- 54.04) and CNR (mean 120.75 +/- 46.47). Despite lower SNR and CNR of the cranial and cervical vessels, a two-step injection protocol exhibited less venous superposition and therefore proved to be superior compared to single-bolus injection. CONCLUSION: Our approach provides accurate noninvasive high-resolution imaging of systemic atherosclerotic disease, covering the arterial vasculature from intracranial arteries to distal runoff vessels. The recently introduced MR scanner and coil technology is feasible to significantly increase the performance of whole-body MRA.  相似文献

The purpose of this study was to assess a new open system with a field-strength of 1.0 T for the feasibility of liver biopsy using the freehand technique with fast continuous imaging. Fifty patients with focal liver lesions measuring 5 to 30 mm in diameter were included in the study. Guidance and monitoring was performed using a 1.0-T open magnetic resonance (MR) scanner (Panorama HFO; Philips Healthcare, Best, The Netherlands). With fast continuous imaging using a T1-weighted (T1W) gradient echo (GRE) sequence after administration of gadolinium (Gd)-EOB-DTPA, the needle was placed into the lesion. An interface for interactive dynamic viewing in two perpendicular planes prevented needle deviations T2-weighted turbo spin echo (TSE) fat-suppressed sequence was added to rule out postinterventional hematoma or biloma. All lesions were visible on the interventional images. Biopsy was technically successful, and solid specimens were obtained in all cases. Forty-six patients showed a histopathologic pattern other than native liver tissue, thus confirming correct position of the needle. Time between determination of the lesion and performance of the control scan was on average 18 min. No major complications were recorded. MR guidance with the new 1-T open system must be considered an attractive alternative for liver punction. An interface for dynamic imaging of needle guidance and T1W-GRE imaging with administration of Gd-EOB-DTPA for contrast enhancement allows the pinpoint puncture of liver lesions.  相似文献
