The authors present the clinical case of a 61-year-old patient with Hand-Schüller-Christian disease associated with multisystemic involvement. The onset of such puzzling symptoms and the extremely rarity of this disease in a patient of such advanced age resulted in a delayed diagnosis and subsequently delayed treatment of the patient.  相似文献

We report a case of idiopathic midline destructive disease in a 57-year-old man. The patient had a non-specific histological pattern in biopsies obtained from the nose and upper lip, characterized by a granulomatous reaction with progressive destruction of the tissues. The patient's general medical history was non-contributory. Clinical and laboratory data did not support any feasible etiology for this destructive process. The patient was treated with prednisone until the discovery of type II diabetes mellitus (never diagnosed before) and was then in turn treated only with oral antidiabetic therapy. Follow-up controls revealed progressive reduction of the symptoms and of the nasal and lip lesions and total remission of symptoms up to 2 years after the onset of the disease. We discuss the diagnostic and subsequent therapeutic problems in the management of the midline necrotizing lesions.  相似文献

This case has been reported because of its rarity and atypical presentation. A 45 year old man presented in the outpatient department with a gradually increasing swelling over the antero-medial aspect of right ankle joint. This had developed over the last 3 months and the patient was experiencing pain while walking. On laboratory evaluation, he only had an elevated erythrocyte sedimentation rate. X-ray showed an extensive lytic lesion of the talus. Chest X-ray was normal. The Mantoux skin test was negative. Ziehl–Neelsen staining of aspirated fluid revealed acid-fast bacilli. Sequestrectomy with curettage with subsequent bone grafting was undertaken as an alternative to talectomy. Postoperatively a below knee cast was applied for 12 weeks with 20 months of antituberculous chemotheraphy, which resulted in a good clinical outcome. The lytic lesion had healed on X-ray.  相似文献

Tetanus is a life-threatening infection that is rare in the developed world. Trismus and dysphagia are the most common presenting symptoms, and thus oral and maxillofacial surgeons may be involved in the early stages of evaluation of the patient. Early diagnosis helps initiate prompt management and referral for intensive therapy. This article is a case report of severe generalized tetanus with a discussion of its diagnosis, pathophysiology, and management.  相似文献

Tongue piercings can be associated with local and systemic complications. Local complications occur frequently immediately after the surgery but also long-term local effects can cause problems such as speech and swallowing difficulties. Aspiration, transmission of infectious diseases, hypersensitivity reaction belong to the systemic complications. In the presented paper an unusual case of cancer development in a 26-year-old man who had a metal piercing inserted for 5 years in the right anterior third of the tongue. Despite of intense concommitant chemoradiotherapy the patient died 18 months from the first symptoms. In prevention of various complications it would be the best solution spread information about the risks of the tongue piercing especially within teenage population.  相似文献

Luxation injuries such as intrusion are commonly seen in the primary dentition. Intrusion drives the tooth deeper into the alveolar socket, which results in damage to the pulp and peridontium. Difficulty in gaining compliance from a very young child and the risk of damaging the permanent tooth germ makes the management of these injuries challenging. Careful clinical and radiographic examination along with regular follow-up is essential. A case of intrusive luxation to the maxillary central incisor in a 3-year-old patient is reported. Spontaneous reeruption was noted 4 months after injury, but the tooth had developed an abscess and external root resorption. Hence, extraction of the tooth was done and an anterior esthetic fixed space maintainer was placed. Traumatic injuries to the primary dentition should not be ignored by the parents or by the dentist. The paper also includes a literature review of intrusive luxation in the primary dentition.  相似文献

Congenital absence of teeth is a hereditary phenomenon passed through generations by an autosomal dominant pattern. Such a patient often presents with poor esthetics, mastication and disturbed social behavior. ObjectivesThe most common configuration of the mandibular second molar is the presence of two roots, although the presence of one and three roots has also been reported. The objective of this work was to present a rare anatomic configuration with four roots diagnosed during an endodontic therapy.Materials and methodsEndodontic treatment was performed using a dental operating microscope. The anatomic configuration was confirmed with a cone beam computed tomography image analysis. An electronic database search was conducted to identify all the published reported cases of mandibular second molars with four roots.ResultsThe cone beam computed tomography analysis showed the presence of three mature normal length roots plus a smaller and conical radix entomolaris. The authors were unable to identify a single case in population's characterization researches with large samples by search in the available literature. Only one in vivo case report was available in the literature.ConclusionsA configuration with four roots is rare for the mandibular molars. This configuration has an incidence of 0.04% in the first lower molar with only three in vivo case reports available. For the second lower molar, no incidence ratio was found.  相似文献

The present case report aims to describe a typical situation of an adult patient seeing her dentist for replacement of a missing tooth, but also requiring information regarding the possibilities for a return to the original occlusion as perceived by the patient based on family photos. The importance of the communication regarding the consequences of blindly following the patient's demand for a replacement of a missing tooth compared to the treatment possibilities when involving orthodontics is stressed. The significance of maintaining a high information level in the undergraduate training regarding treatment possibilities is stressed. The logic in dividing the treatment into separate phases defined by the solution of specific problems is demonstrated and minimizes the chair time as well as the treatment time.  相似文献

Eagle’s syndrome (ES) occurs when an elongated styloid process or calcified stylohyoid ligament causes recurrent throat pain or foreign body sensation, dysphagia, or facial pain. Additional symptoms may include neck or throat pain with radiation to the ipsilateral ear. The symptoms related to this condition can be confused with those attributed to a wide variety of facial neuralgias. ES can be diagnosed radiologically and by physical examination. The treatment of ES is primarily surgical. The styloid process can be shortened through an intraoral or external approach. In this paper a case of ES exhibiting unilateral symptoms with bilateral elongation of styloid process is reported and the literature is reviewed.  相似文献

Purpose

Temporal hypertrophy is a rare clinical entity, especially the bilateral variety, whereas masseteric hypertrophy is a commonly occurring disorder. Very few bilateral cases are reported in literature. We are reporting a case with bilateral temporalis muscle hypertrophy with peculiar symptoms which is not reported so far in literature.  相似文献

Aim

To align a dilacerated maxillary central incisor and partially impacted canine with unilateral extraction in a young patient with skeletal deep bite.

Materials and methods

A 14 year old male patient reported to the hospital with skeletal deep bite (basal plane angle-17°), severe horizontal pattern of growth (Go-Gn to Sn -22°), upright maxillary incisors (U1 to NA -26°) and retroclined lower incisors (L1 to NB -11°). The maxillary left central incisor was dilacerated, and the maxillary left canine was partially impacted. Unilateral extraction of the left maxillary premolar and left mandibular central incisor was done. A canine disimpaction spring was used to align the impacted canine. An anterior bite plane was given to open the bite.

Results

Superimposition of lateral cephalogram (T1, T2) revealed bite opening, normal overjet and overbite. There was backward rotation of the mandible and increase in lower anterior facial height. There was no evidence of root resorption or loss of vitality in the dilacerated tooth. Clinically the canine was well aligned in the arch.

Conclusion

Orthodontic management of a dilacerated incisor can be done without root resorption or loss of vitality. The partially impacted canine was well aligned in the arch. Unilateral extraction can produce good treatment results.  相似文献
