We studied 58 patients undergoing orthotopic liver transplantation, aged 42 +/- 10 years (mean +/- SD), and weighing 65 +/- 14 kg. Anesthesia was maintained with fentanyl, midazolam, and vecuronium. Serum bicarbonate, serum potassium, serum ionized calcium and pH did not change significantly throughout the study. Usual hemodynamic parameters were recorded. Hemodynamic tolerance was assessed by a trial of clamping of the inferior vena cava, above and below the liver and the portal vein; patients were allocated to two groups: the group without venovenous bypass (NBP, n = 29) consisted of patients whose MAP did not decrease by more than 30% and/or cardiac output did not decrease by more than 50%; the group with venovenous bypass (BP, n = 29) consisted of patients whose MAP decreased by more than 30% and/or cardiac output decreased by more than 50% or required venovenous bypass for easier surgical dissection. After clamping of the vena cava and the portal vein, the cardiac index (CI) and mean pulmonary arterial pressure (MPAP) decreased significantly, whereas systemic vascular resistances (SVR) increased. After unclamping the inferior vena cava suprahepatically and infrahepatically, no hemodynamic change was observed. After unclamping the portal vein, MAP decreased, despite the increase in the CI, because of an significant decrease in SVR; in addition MPAP increased despite the decrease in pulmonary vascular resistances. The decrease in MAP of more than 30% during at least 1 min occurred in 6 patients (20%) in the NBP group and in 6 patients (20%) in the BP group. We concluded that the occurrence of the syndrome of cardiovascular collapse following liver reperfusion was similar whether venovenous bypass was used or not.  相似文献

Removal of the liver to start the anhepatic stage of liver transplantation requires cross-clamping of the portal vein, inferior vena cava, and hepatic artery. Adverse effects occur from engorged splanchnic beds and decreased venous return. A veno-venous bypass from the inferior vena cava and portal vein to the axillary vein is used in an attempt to ameliorate these changes. The purpose of this study was to evaluate the effect of institution of veno-venous bypass on hemodynamics. Eight randomly selected adult patients undergoing orthotopic liver transplantation had general anesthesia induced with thiamylal and maintained with nitrous oxide and isoflurane. Cardiopulmonary data and arterial and mixed venous blood gases were measured prospectively using radial artery and pulmonary artery catheters. Measurements were taken under four conditions: (1) 10 minutes before bypass; (2) after partial bypass (vena cava to the axillary vein); (3) after partial bypass with portal vein clamping; and (4) after full bypass (vena cava and portal vein to the axillary vein). Statistically significant changes seen were a 22% decrease in cardiac output and a 47% increase in systemic vascular resistance (SVR). Bypass flow was lower than predicted. Venovenous bypass ameliorates, but does not fully prevent, the reduction of cardiac output and rise in SVR seen with initiation of the anhepatic stage. However, bypass does prevent the hypotension experienced during cross-clamping and for these reasons should be used routinely.  相似文献

Thirty-eight adult orthotopic liver transplant recipients were studied to compare renal hemodynamics and renal function with (17 patients) and without (21 patients) venovenous bypass. Bypass was used when mean arterial blood pressure decreased by greater than 30% or cardiac index decreased by greater than 50%, or both, during a 5-min trial of clamping of the suprahepatic and infrahepatic vena cava and portal vein. Intraoperative measurements were performed 2 h after induction of anesthesia, 10 min before the end of the anhepatic phase, and 2 h after cava unclamping. During the anhepatic stage, renal perfusion pressure decreased significantly in the group with no bypass (79 +/- 20 vs 60 +/- 17 mm Hg, P less than 0.05) (mean +/- SD), whereas it remained unchanged in the group with bypass (77 +/- 14 vs 74 +/- 16 mm Hg, NS); urinary output was not modified in the bypass group, whereas it decreased significantly in the group with no bypass compared with the dissection phase (0.7 +/- 0.6 vs 1.7 +/- 2.0 mL.kg-1.h-1, P less than 0.05). However, during the postreperfusion phase, urinary output was similar in both groups and was more when compared with the dissection phase (P less than 0.05). Serum creatinine level was increased in both groups on the third postoperative day, but no difference occurred between the groups (bypass group 107 +/- 49 mmol/L; nonbypass group 126 +/- 95 mmol/L). No patient required dialysis in either group in the postoperative period. This study suggests that in patients without preoperative renal failure and who tolerate the trial of clamping well, venovenous bypass is not required to maintain postoperative renal function after liver transplantation.  相似文献

目的 探讨建立放弃静脉转流的小型猪原位肝移植模型的可行性。方法 选用广西巴马小型猪(3-6月龄,8-11kg)为实验动物,共实施原位肝移植10例次。供体肝脏游离、冷灌注、切取、修整以及UW保存按常规方法进行。受体术中肝脏游离后夹闭肝上、肝下下腔静脉和门静脉并切除受体肝脏,各血管断端与供肝相应血管进行端端吻合,肝上下腔静脉和门静脉吻合完毕即开放肝脏血流,在此过程中未使用静脉转流,此后吻合肝动脉、胆管。结果 本组10例次肝移植手术后一周存活率90%(9/10),无肝期时间31.3±2.67min,无肝期血压显著下降(MAP4.5±1.58kPa)但肝血流开放后能较快恢复并渐趋平稳。结论 本组结果表明,非静脉转流条件下的小型猪肝移植模型是一操作方便、易于复制同时又具有较好重复性和稳定性的大动物肝移植模型,能应用于肝移植的系列实验研究。  相似文献

Simplified technique of orthotopic liver transplantation in pigs   总被引:18,自引:0,他引:18  
BACKGROUND: Pig models have become common in transplantation immunological research. However, in pigs, clamping of the venous splanchnic system during orthotopic liver transplantation (OLT) is responsible for high morbidity and mortality rates; therefore, the use of venovenous bypass (VVB) is advocated. Because venous bypass can also cause specific complications, a simplified method for OLT in pigs has been developed and evaluated in terms of morbidity and mortality. METHODS: Twenty-three OLTs were performed between pairs of inbred miniature swine. Donor and recipient pairs (weighing 20-35 kg) were selected at 3-6 months of age. In the donor, the portal vein, infrahepatic vena cava, and suprahepatic vena cava were dissected, whereas the hepatic artery was preserved in continuity with the coeliac trunk and the abdominal aorta up to the iliac bifurcation. In situ cold perfusion was then performed. The recipient was prepared simultaneously by another surgical team. After total hepatectomy and complete portal and caval clamping, the suprahepatic vena cava and portal vein were sutured; VVB was not used. After completion of both venous sutures, the liver graft was reperfused. The infrahepatic vena cava was then anastomosed and unclamped. The donor aorta conduit was implanted end-to-side to the recipient infrarenal aorta, and the biliary reconstruction consisted of a cholecystojejunostomy with a Roux-Y loop. RESULTS: Twenty of 23 (87%) animals survived more than 1 week (7-483 days). The mean anhepatic time was 29.6+/-4.12 min. Although severe hypotension was noted during the anhepatic phase, the hemodynamic status rapidly recovered and stabilized after graft reperfusion. CONCLUSION: Simplified technique without VVB is appropriate for successfully achieving OLT in pigs.  相似文献

BACKGROUND: The pig tolerates simultaneous clamping of the liver pedicle and inferior vena cava poorly, so venovenous bypass has to be used during the anhepatic phase of experimental orthotopic liver transplantation (OLT). The aim of this work is to assess whether clamping of the supracoeliac aorta during the anhepatic phase (AP) of experimental OLT in pigs allows transplantation in stable hemodynamic conditions. METHODS: Fourteen pigs (weight, 16 to 18 kg) received whole liver grafts from 14 age-matched donors and were subsequently divided into two groups: group I, OLT without venovenous bypass during the AP, group II, OLT with supracoeliac aortic clamping during the AP. Variables analyzed were cardiac output (CO) and related variables, mean systemic arterial pressure (MAP), mixed venous oxygen saturation (SvO2), hepatic artery and portal vein blood flow, systemic and hepatic O2 supply and uptake (SDO2, SVO2, HDO2, HVO2, respectively), liver enzymes, glucose, creatinine, and electrolytes. RESULTS: In group I, CO, MAP, and SvO2, decreased during the AP (anhepatic) in comparison with baseline (preanhepatic) values (CO, 3.60+/-0.74, preanhepatic, v. 1.21+/-0.25 L x min(-1), anhepatic; P<.05. MAP, 97+/-12, preanhepatic, v. 43+/-17 mm Hg, anhepatic; P<.05. SvO2, 91.6+/-5.6, preanhepatic v. 70.0+/-12.5%, anhepatic; P<.05), and SDO2/SVO2 increased by 16% (preanhepatic) to 33% (anhepatic; P<.05). In group II, CO decreased during the anhepatic phase by only 21% (3.82+/-0.81, preanhepatic, v. 3.07+/-0.99 L x min(-1), anhepatic; not significant), the MAP increased significantly (100+/-8, preanhepatic, v. 135+/-4 mm Hg, anhepatic; P<.05), and SVO2, SDO2, SVO2, and SDO2/SVO2 remained unchanged. After revascularization, none of these variables differed significantly between groups, and levels of liver enzymes, glucose, creatinine, urea, and electrolytes were similar in both groups, both before and aftertransplantation. CONCLUSIONS: Experimental OLT can be carried out in pigs without venovenous bypass, but it leads to severe hemodynamic disturbances. Clamping of the supraceliac artery during the AP is well tolerated and results in excellent hemodynamic stability, so it may prove to be a useful technique in liver transplantation in animals, such as dogs or pigs, that do not tolerate simultaneous clamping of the liver pedicle and inferior vena cava as well as human beings.  相似文献

目的:观察不转流的经典工原位肝移植的效果,分析不转流对胃肠道及肾脏功能的影响。方法:1999年以来我所共为15例终末期肝病患施行了不转流的经典式原位肝移植术,术中门静阻断时间平均为40.5min,下年以来我所共为15例终末期肝病患施了不转流的经典式原位肝移植术,术中门静脉阻断时间平均为40.5min,下腔静脉阻断时间58.7min;无肝期血压维持在78-96/52-64mmHg.开放门静后从肝下下腔静放血150-20ml结果:15例一月内除1例术后7天死于DIC外,余14例无一例发生与不转流相关的胃肠道和肾功能明显损害的并发症.结论:不转流经典式原位肝移植若无肝期小于1h和血压在80/50mmHg以上,并不增加胃肠道和肾脏功能损害等产发症,有手术时间短和节省费用等优点.  相似文献

BACKGROUND: Cavaplasty orthotopic liver transplantation (OLT) offers advantages for hepatectomy and implantation and eliminates the risk of outflow obstruction. However, it does require clamping of the cava. This study describes the use of a vasopressor without fluid expansion or venovenous bypass (VB) for hemodynamic control during the anhepatic phase. METHODS: The cavaplasty OLT technique was used routinely. A vasopressor was administered if the mean arterial blood pressure (MAP) was less than 60 mm Hg after clamping of the cava. If the MAP did not reach 60 mm Hg after adjusting the dosage of the vasopressor, femoro-axillary VB would be used. VB was also indicated for preexisting cardiac disease or for massive hemorrhage from severe portal hypertension and extensive adhesions. RESULTS: Among all the 121 adult cavaplasty OLTs, 33 were supported with VB and 50 received a vasopressor. The remaining 38 were excluded. However, baseline variables were well matched, except that preexisting cardiac disease was more frequent in the VB group. The median dosage of epinephrine was 0.07 microg/kg/min (range 0.01-0.6). The VB and vasopressor groups were similar in the reduction in mean MAP and the accumulation in arterial lactate upon clamping as well as in the central venous pressure upon unclamping. Postreperfusion hypotension was more frequent in the VB than in the vasopressor group (27.3% vs. 4.0%, P=0.006). There was no primary graft nonfunction or intraoperative right heart failure. One patient in the vasopressor group required postoperative temporary dialysis. Ninety-day patient and graft survival for the VB and vasopressor groups were 97.0% vs. 98.0% and 97.0% vs. 94.0%, respectively. CONCLUSION: Modest doses of vasopressor without volume expansion or VB can maintain hemodynamic stability during the anhepatic phase of cavaplasty OLT.  相似文献

OBJECTIVE: Magnetic rings were used for rapid vascular reconstruction in a canine liver transplantation model. MATERIALS AND METHODS: Thirty-two adult mongrel dogs weighing 13 to 16 kg were randomly selected as donors or recipients of transplantations. The recipients were randomly divided into two groups: group A (n = 10) had magnetic rings used for vascular reconstruction without venovenous bypass; group B (n = 6) had vascular reconstruction performed by continuous suturing with splenojugular venovenous bypass. RESULTS: In group A, the entire operative period was 3.24 +/- 0.49 hours, the durations of clamping the portal vein and the infrahepatic vena cava of the recipient were 5.89 +/- 2.27 minutes and 3.89 +/- 0.73 minutes, respectively. In group B, the entire operative period was 4.12 +/- 0.51 hours with the duration of clamping portal vein and infrahepatic vena cava, 28.33 +/- 6.04 minutes and 12.16 +/- 3.72 minutes (P < .01 vs group A). In group A, mean arterial pressure dropped during the anhepatic phase but recovered quickly after reperfusion. The fluid infusion was about 730.56 +/- 50.56 mL in the group A and a pressor agent was unnecessary. In group B, blood pressure dropped during the anhepatic phase and slowly recovered. The fluid infusion was about 2241.67 +/- 390.78 mL and a pressor agent was used to maintain the blood pressure of the recipient. No twist or thrombus was discovered in the anastomoses group A and the endothelium at the site of anastomosis was entire. In group B, errhysis was common in the anastomotic stomas. Nine of 10 dogs in group A survived more than 3 days, the longest being 8 days, whereas four of the six dogs in group B survived less than 3 days. CONCLUSION: The results showed that the anhepatic time was significantly shortened (about 5.89 +/- 2.27 minutes) in group A compared with group B and venovenous bypass was unnecessary. Magnetic rings could be used for rapid vascular reconstruction in canine liver transplantation model. The long-term results of this procedure should be clarified before it is applied in clinical practice in the future.  相似文献

Objective

The aim of this study was to compare the hemodynamic changes caused by clamping of the inferior vena cava and portal vein in biliary atresia (BA) versus glycogen storage disease (GSD) patients undergoing living-donor liver transplantation (LDLT) without venovenous bypass.

Methods

We reviewed retrospectively the anesthesia charts of pediatric LDLT patients. Age, weight, height, blood loss, blood product use and fluid replacement between groups were compared with Mann-Whitney test, and systolic blood pressure (SBP), heart rate (HR), central venous pressure (CVP) before clamping of the inferior vena cava, and 4 measurements during anhepatic phase and 5 minutes after reperfusion were compared with analysis of variance.

Results

One hundred four BA patients (GI) and 12 GSD patients (GII) showed mean total blood loss among GI to be more than among GII, but the blood products and crystalloids infused during the operation were not significantly different. The changes of SBP, HR, and CVP after clamping of the IVC were significantly different between groups. CVP of GII was lower than GI, indicating that venous return among GII was more affected, subsequently showing lower SBP and higher HR.

Conclusions

Total clamping of the inferior vena cava resulted a greater decrease in CVP in GII with subsequently lower SBP and faster HR compared with GI.  相似文献

Since most of studies investigating cytokine levels during human orthotopic liver transplantation used venovenous bypass (VVB), it may be difficult to distinguish between the increase in proinflammatory mediators induced by VVB, by ischemia-reperfusion injury or by splanchnic venous congestion in the anhepatic phase. The goal of this investigation was to assess the levels of interleukin-6 (IL-6) and soluble interleukin-2 receptors (sIL-2r) during OLT procedures routinely performed without VVB. PATIENTS AND METHODS: Twenty-one consecutive patients underwent OLT with cross clamping of the inferior caval vein without VVB. Soluble IL-2r concentrations were measured by means of luminescence enzyme immunometric assay and IL-6 by means of a sequential immunometric assay. Time points (TP) of sampling were before induction of anesthesia (TP1), after cross-clamping of the inferior vena cava (TP2), 15 minutes after reperfusion (TP3), and 24 hours after the transplant procedure (TP4). RESULTS: Soluble IL-2r increased significantly 24 hours after transplantation (P =.02) compared to TP1, TP2, and TP3. IL-6 increased significantly during the anhepatic period (TP2 vs TP1, P =.003) and again in the reperfusion period (TP2 vs TP3, P =.002). Twenty-four hours after surgery IL-6 declined significantly (TP3 vs TP4, P =.001), but remained significantly higher (P = 0.04) compared to TP1. Furthermore, we examined the relative changes (DeltaTP %) in perioperative levels of cytokines compared with those previously published in studies using VVB. We observed higher values of DeltaTP % of IL-6 in TP2 and TP4 among our group of patient without VVB. The data on sIL-2r were similar, suggesting no major effects of the operative technique on sIL-2r levels. CONCLUSION: The two interleukins showed different perioperative trends. Our data suggest that cross clamping contributes more to cell activation, namely, increased release of IL-6 in the anhepatic phase than the use of VVB. However, no major differences were observed during the reperfusion period. The extent of clinical effect on graft function of higher IL-6 levels in the anhepatic period among recipients not supported with VVB remains to be clarified.  相似文献

目的:分析大鼠位肝移植制作过程中常见失败原因及总结相应的处理策略。
方法:历时10个月,采用改良Kamada二袖套法制作大鼠原位肝移植模型150例。将每完成50例作为1个实验阶段,分早、中、后期3个阶段对失败原因进行分析,总结经验。
结果:大鼠原位肝移植常见失败原因在早期阶段主要以气胸(5.8%),术中出血(10.6%),无肝期过长(50.4%)为主;中、后期主要以肝上下腔静脉吻合失败(21.0%),无肝期过长(20.6%),门静脉及肝下下腔静脉袖套吻合失败(17.6%),胆管重建失败(5.6%)为主。
结论:导致大鼠原位肝移植模型制作失败的原因较多,无肝期是影响成功与否的关键,及时地对失败原因进行分析总结并寻找合理的解决办法,有助于初学者熟练地掌握该技术。  相似文献

BACKGROUND: Although using a Kamada model with a cuff approach for vascular anastomosis shortens the anhepatic time in orthotopic rat liver transplantation (ORLT), cardiac arrest is still often encountered during the anhepatic phase. We hypothesize that low cardiac output is the reason for this. In this study, we used the supra-celiac aorta clamp (SCAC) method during the anhepatic phase in ORLT to evaluate the success rate. MATERIALS AND METHODS: Spraque-Dawley rats were used in this the study. First, we tested the time between the clamping of the supra-hepatic inferior vena cava (IVC) clamped and cardiac arrest in between two groups according to whether SCAC was performed (n = 40) or not (n = 20). Clamping of the supra-hepatic IVC was performed 30 s or 1 min after SCAC (n = 20 for each) in the SCAC group. Second, we performed the Kamada method for ORLT in two groups according to whether SCAC was performed (n = 20, 1-min SCAC) or not (n = 20). Successful ORLT was defined as a recipient rat surviving more then 5 days. RESULTS: The mean time between IVC clamping and cardiac arrest was longer in the group of 1-min SCAC group than in the 30-s SCAC group or without SCAC (19.8 versus 11.45 versus 3.87 min, respectively, P < 0.001). The success rate of ORLT was higher in the SCAC group than in the group without SCAC (85% versus 15%, respectively, P < 0.001). CONCLUSIONS: SCAC can effectively increase the success rate of ORLT by increasing the tolerable time during the anhepatic phase. The reason for this is the avoidance of hypovolumic cardiac arrest during the anhepatic phase.  相似文献

BACKGROUND AND AIMS: The potential advantages of vena cava-preserving recipient hepatectomy in orthotopic liver transplantation are reduced hemorrhage, improved cardiovascular stability and preserved renal perfusion without the requirement of veno-venous bypass as compared with recipient hepatectomy including the vena cava. No detailed information is available on the use of veno-venous bypass during complicated vena cava preserving recipient hepatectomy and liver transplantation. In the present study, the peri- and postoperative courses of adult liver transplant recipients in whom the hepatovenous reconstruction was performed according to three different techniques with and without the use of veno-venous bypass were investigated. PATIENTS/METHODS: During primary orthotopic liver transplantation, an end-to-end (ETE) cavo-caval interposition of the donor vena cava to the recipient's vena cava was performed in 75 patients (group I). In 15 patients, a termino-terminal piggyback (PB) anastomosis was constructed to the remnant of the recipient's hepatic vein (group II), and in 72 transplantations a latero-lateral cavo-cavostomy (LLC) of donor-to-recipient's vena cava (group III) was performed. The use of bypass, operative time and cold ischemia time, perioperative blood product requirements, incidence of relaparotomy, the evolution of postoperative renal function, technical complications and the survival were analyzed and compared using multivariate statistics and actuarial techniques for statistical evaluation. RESULTS: No differences could be found in preoperative patient conditions, donor conditions, operating time, anastomosing time or cold ischemia time. In groups I-III, the veno-venous bypass was used in 50 (67%), 8 (53%) and 6 (8%) cases respectively (P=0.02 for group III). The mean preoperative packed cells requirements were 20.4 vs 29.6 vs 10.8 units (P=0.01 for group III), while postoperative blood product requirements (first 24 h) were 2.6 vs 5.0 vs 0.20 units of packed cells (P=0.02 for group III). Relaparotomy for diffuse retroperitoneal hemorrhage was performed 14 times (19%) in group I, 3 times (20%) in group II and 7 times (8.3%) in group III (P=0.002). The incidence of posteropative early renal dysfunction (increase of > or =1.3 mg% serum creatinine) in group I vs group II vs group III was 24% vs 60% vs 16.7% (P=0.001 for group II) for patients without the use of veno-venous bypass. No significant difference was observed concerning early renal dysfunction in patients where a veno-venous bypass was used. The survival at 12 months was 81% for group I, 86% for group II and 93.0% for group III. In group III there were four complications (P=0.03) at the hepatovenous anastomosis of which two were eventually fatal. CONCLUSION: Preservation of the recipient's vena cava and LLC can reduce, but not avoid, the requirement for veno-venous bypass. In orthotopic liver transplantation, postoperative hemorrhage, as measured by surgical revisions and requirement for blood products, is significantly reduced with LLC with and without bypass. Early renal dysfunction also occurs in the group of LLC as compared with the termino-terminal cavostomy independent of the bypass. A technical failure resulting in patient death can be associated with LLC.  相似文献

Hemodynamic and metabolic changes during orthotopic liver transplantation   总被引:1,自引:0,他引:1  
Hemodynamic and metabolic profiles were obtained on 48 adult patients (mean age 46 years) undergoing orthotopic liver transplantation without using a bypass technique during the anhepatic period. Baseline measurements after induction of anesthesia (A) revealed a high-output circulatory state. During hepatic dissection (B, preclamping control), mean arterial pressure (MAP), mean pulmonary artery pressure (MPAP), pulmonary capillary wedge pressure (PCWP), right atrial pressure (RAP), and elevated cardiac index (CI) were well maintained. Preoperative plasma exchange with fresh frozen plasma in the presence of severe coagulation defects and liberal use of platelet concentrates limited the need for massive blood transfusion, and thus contributed to stable hemodynamics during this stage. Significant cardiovascular changes occurred immediately after clamping of the inferior vena cava and the portal vein (C): there was a marked fall in MAP (-30%), MPAP (-45%), PCWP (-48%), RAP (-40%), and CI (-60%) reflecting the hemodynamic adaptation to the impeded venous return. At the end of the anhepatic period (D), MAP (-21%), MPAP (-38%), PCWP (-39%), RAP (-24%), and CI (-56%) were persistently lowered because no attempts were made to attenuate the clamping response by vigorous volume expansion or infusion of inotropic drugs. The reduction in oxygen availability index (O2AVI) was compensated by enhanced oxygen extraction. Oxygen consumption index (VO2I) fell secondary to the removal of the liver and the decrease in body temperature (BT). BACKGROUND: Venovenous bypass was considered necessary to maintain haemodynamic stability and avoid splanchnic and retroperitoneal congestion during the anhepatic phase of liver transplantation. It was essential for right lobe living donor liver transplantation (LDLT) in which the inferior vena cava needed to be cross-clamped to construct wide and short hepatic vein anastomoses. However, many complications related to venovenous bypass have been reported. This study aimed to determine whether venovenous bypass was necessary for right lobe LDLT. METHODS: Between June 1996 and June 2001, 72 patients underwent right lobe LDLT. The outcomes for the first 29 patients who had venovenous bypass during the operation were compared with those of the remaining 43 patients who did not have venovenous bypass. In patients without bypass, blood pressure was maintained during the anhepatic phase by boluses of fluid infusion and vasopressors. RESULTS: Compared with patients undergoing operation without venovenous bypass, patients who had venovenous bypass required significantly more blood, fresh frozen plasma and platelet infusion, and had a lower body temperature; their postoperative hepatic and renal function in the first week was worse than that in patients who did not have a bypass. The time to tracheal extubation was longer and the incidence of reintubation for ventilatory support was higher with venovenous bypass. Six of the 29 patients with venovenous bypass died in hospital, compared with two of the 43 patients without a bypass (P = 0.05). By multivariate analysis, the lowest body temperature during the transplant operation was the most significant factor that determined hospital death. CONCLUSION: Venovenous bypass is not necessary and is probably harmful to patients undergoing right lobe LDLT, and should therefore be avoided.  相似文献

目的建立一种非静脉转流的简化型猪腹部多器官移植模型。方法采用健康长白幼猪,供、受猪为同一胎猪,按体重相近原则配对。供者器官原位灌洗,摘取全腹脏器以及与之相连的腹主动脉和下腔静脉,修剪成肝、胰、胃、十二指肠以及近端2 m空肠的器官簇,整块移植到切除整个前肠和中肠器官(肝、胰、胃、十二指肠、全部小肠和大部分结肠)的受者腹腔。术中未使用静脉转流。术中监测受者的血流动力学、pH值及电解质变化。结果25次手术中,仅19次手术顺利完成,受者术后存活时间为2~146 h。与无肝前期相比,无肝初期、新肝初期及新肝期的平均动脉压、中心静脉压明显下降(P<0.05);与无肝初期相比,新肝期的平均动脉压、中心静脉压也明显下降(P<0.05);新肝期受者处于严重的代谢性酸中毒状态。结论在非静脉转流下建立简化型猪腹部多器官移植模型是可行的。  相似文献

Sequential or domino liver transplantation is a well-established procedure for patients with familial amyloidotic polyneuropathy (FAP). Donation for domino liver transplantation imposed the resection of the inferior vena cava along with the liver, requiring complete suprarenal vena cava clamping and usually the use of venovenous bypass. We describe a successful case in which it was possible to perform the FAP hepatectomy by the piggyback technique.  相似文献

目的:观察经典式原位肝移植术不作静脉转流的效果,分析不转流对术后胃肠道及肾脏功能的影响。方法:总结自1999年来我所45例经典式原位肝移植术弃用静脉转流的经验。结果:45例术中无肝期时间平均为54.5min,术后与不转流相关的胃肠道和肾功能明显损害的并发症有4例:3例术后近期发生急性肾功能衰竭,2例经血透后恢复,1例死亡;另1例于术后1.5月发生急性坏死性胰腺炎。结论:若无肝期控制在1h内,经典式原位肝移植不作转流,并不显著增加胃肠道和肾脏功能损害等并发症,且有缩短手术时间和节省费用等优点。  相似文献

目的 建立巴马小香猪原位肝移植模型,探究体外静脉转流对于猪肝移植模型建立的重要性.方法 实施以巴马小香猪为实验对象的原位肝移植共30例.供体肝脏游离、灌注、切取以及保存按常规方法进行.受体术中置入左侧髂外静脉、脾静脉/门静脉和左侧颈浅静脉"Y"形插管,用作无肝期的静脉-静脉转流.受体术中肝脏游离完成后阻断门静脉,肝上、肝下下腔静脉后开放左侧髂外静脉、脾静脉/门静脉-左侧颈浅静脉转流.血管和胆道均采取端端吻合.结果 前6例未采取体外静脉-静脉转流的原位肝移植手术的受体术中全部死亡,存活率为0.0%(0/6);采用脾静脉-颈浅静脉转流方式的受体术中存活率为57.1%(8/14),而采用门静脉-颈浅静脉转流方式的受体术中存活率为100.0%(10/10),具有显著性差异(P<0.05).原位肝移植手术时间平均为6.5 h,受体无肝期平均为(33±12)min.供肝热缺血时间平均为2 min,冷缺血时间平均为130 min.结论 采用左侧髂外静脉、门静脉主干-左侧颈浅静脉转流方式的巴马小香猪原位肝移植模型是具有可重复性好,需要的设备少,更加经济,疗效好,是比较理想的猪肝移植模型.可作为肝移植系列研究的平台.  相似文献
