Objective To summarize the experience in surgical treatment of hepatic cavernous hemangioma (HCH). Methods The clinical data of 345 patients who received HCH resection in General Hospital of PLA from 1986 to 2005 were retrospectively analyzed. Results The ratio between male and female patients was 1/1.8. Eighteen patients (5.2%) were incidentally found with HCH during or after operation. Most of the HCH were located in the right lobe, with the proportion of 16.2% (56/345). Ninety-one patients (26.5%) had small HCH (diameter<5.0 cm), 173 (50.3%) had large HCH (diameter ranging from 5.0-10.0 cm), and 80 (23.2%) had giant HCH (diameter>10.0 cm). The mean diameter of the HCH was (8.0±5.0) cm. Three hundred and twenty-three (99.7%) patients were with Child pugh A. Right subcostal incision and enucleation were performed on all patients. The incidence of postoperative complications and mortality were 11.3% (39/345) and 0.3% (1/345), respectively. Caudate lobe resection was performed on 9 of 11 patients with the tumor located in caudate lobe. Conclusions Some HCHs may be easy to be misdiagnosed as hepatic solid tumor. HCH resection (inclu-ding hepatic caudate lobectomy) is safe for patients with HCH, and the most severe operative complication is massive bleeding during hepatectomy.  相似文献

腹腔镜肝切除术治疗肝血管瘤22例临床分析   总被引:5,自引:2,他引:3  
Objective To investigate technical features and efficacy of laparoscopic liver resec-tion for hepatic hemangioma. Methods The clinic data of 22 patients with hepatic hemangioma receiv-ing laparoscopic liver resection in our hospital from March 2007 to February 2008 were retrospectively analyzed. Results Twenty patients received successful total laparoseopie liver resection and 2 conver-sion to laparotomy. Anatomical liver resection was performed in 14 patients including 5 with left hemi-hepateetomy, 5 with left lateral segmentectomy (1 combined with radiofrequeney ablation for the tumor in the right liver lobe) and 4 with segment Ⅵ resection. Eight patients underwent nonanatomi-cal hepatectomy. The liver parenchyma of 10 patients were transected under regional hemi-hepatic blood occlusion, 7 under intermittent Pringle's manoeuvre, and 5 without hepatic blood inflow block-age. The mean operative duration was 209 rain (92-375 min) and mean blood loss 360ml (50-1300 ml). No operative death and postoperative complications occurred, and the patients recovered well. The mean postoperative hospital stay was 6 d (4-10 d). All the patients were followed up for 2-14 months and no recurrence was found. Conclusion The advantages of laparoseopie liver resection for hepatic hemangioma are safe operation, less complications, and quick postoperative recovery. The key points are right choice of surgical approach, effective control of hepatic blood inflow, proper man-agement of cutting surface of liver, and liver parenchyma amputation being performed 0. 5-1 cm to tumor margin in the normal parenchyma or anatomical liver resection.  相似文献

AIM:To analyze our experience in patients with duodenal gastrointestinal stromal tumors(GIST) and review the appropriate surgical approach.METHODS:We retrospectively reviewed the medical records of all patients with duodenal GIST surgically treated at our medical institution between 2002 and 2011.Patient files,operative reports,radiological charts and pathology were analyzed.For surgical therapy open and laparoscopic wedge resections and segmental resections were performed for limited resection(LR).For extended resection pancreatoduodenectomy was performed.Age,gender,clinical symptoms of the tumor,anatomical localization,tumor size,mitotic count,type of resection resectional status,neoadjuvant therapy,adjuvant therapy,risk classification and follow-up details were investigated in this retrospective study.RESULTS:Nine patients(5 males/4 females) with a median age of 58 years were surgically treated.The median follow-up period was 45 mo(range 6-111 mo).The initial symptom in 6 of 9 patients was gastrointestinal bleeding(67%).Tumors were found in all four parts of the duodenum,but were predominantly located in the first and second part of the duodenum with each 3 of 9 patients(33%).Two patients received neoadjuvant medical treatment with 400 mg imatinib per day for 12 wk before resection.In one patient,the GIST resection was done by pancreatoduodenectomy.The 8 LRs included a segmental resection of pars 4 of the duodenum,5 wedge resections with primary closure and a wedge resection with luminal closure by Roux-Y duodeno-jejunostomy.One of these LRs was done minimally invasive;seven were done in open fashion.The median diameter of the tumors was 54 mm(14-110 mm).Using the Fletcher classification scheme,3/9(33%) tumors had high risk,1/9(11%) had intermediate risk,4/9(44%) had low risk,and 1/9(11%) had very low risk for aggressive behaviour.Seven resections showed microscopically negative transsection margins(R0),two showed positive margins(R1).No patient developed local recurrence during follow-up.The one patient who underwent pancreatoduodenectomy died due to progressive disease with hepatic metastasis but without evidence of local recurrence.Another patient died in complete remission due to cardiac disease.Seven of the nine patients are alive disease-free.CONCLUSION:In patients with duodenal GIST,limited surgical resection with microscopically negative margins,but also with microscopically positive margins,lead to very good local and systemic disease-free survival.  相似文献

Objective To discuss the management of solid-pseudopapillary tumor of the pancreas. Methods Four patients of solid-pseudopapillary tumor of the pancreas (SPT) were diagnosed by pathology. One patient was underwent magnetic resonance and two enhanced computer tomography scan. The mass was located in tuberculum (1 case), neck (1 case), and body-tail (2 cases) of pancreases respectively. Four cases were underwent duodenopancreatectomy, local resection of mass, resection of body-tail of pancreases respectively. The maximal diameter of tumor ranged from 5 cm to 16cm, averaged 10.3cm. The blood regular test and biochemistry were normal, and the tumor markers including AFP,CA19-9,CA125,CEA also were normal. The specimens were examined by pathology and immunohistochemistry. Results Four cases all were diagnosed solid-pseudopapillary tumor of the pancreas by pathology. At histologic analysis, the tumor was composed of uniform polygonal cells with moderate to abundant amphophilic cytoplasm and arranged in solid nests with areas of degeneration characterized by separation of the cells into pseudopapillary aggregates with intervening accumulation of mucopolysaccharide rich ground substance. The expressions of Vimentin, α-ACT, α-AAT were positive, and CgA negative. Four cases were followed up for six months and norecurred. Conclusions SPT of pancreases was a kind of tumor of low potential malignancy, and the magnetic resonance and computer tomography were often to be used. The effective therapy was thoroughly resection, and the prognosis in most patients is excellent.  相似文献

目的 对35例膝关节后交叉韧带断裂治疗进行临床分析,重点探讨了有关交叉韧带断裂的治疗问题。方法 经明确诊断后,分析采用胫骨附着处撕脱骨折复位固定手术治疗26例、早期髌韧带中1/3移植重建3例、单纯长腿石膏固定6例。结果 本组病例全部进行随访,随访时间13个月-5年,胫骨附着处撕脱骨折复位固定及髋韧带中1/3移植重建29例为优良、单纯长腿石膏固定6例为差。结论 后交叉韧带断裂后应该及时给予手术修复;膝后外侧手术入路,操作简单,暴露充分;少于3个月的陈旧性病例仍适应手术治疗。  相似文献

目的:探讨不同方法重建指尖离断静脉回流的疗效。方法:2008年3月-2013年2月收治指尖离断患者80例,38例吻合指侧方静脉重建回流,术中吻合动静脉比例1:1或1:2或2:2,平均1:2;22例吻合指腹静脉重建回流,术中吻合动静脉比例1:1;20例未吻合静脉,术中仅吻合1条动脉,行侧切口或甲床放血。观察各组治疗效果。结果:吻合指侧方静脉组手指全部成活,无一例发生回流障碍;吻合指腹静脉组19例发生静脉危象,其中4例手指坏死;未吻合静脉组20例均发生回流障碍,其中6例手指坏死。58例获随访,随访时间6~28个月。吻合指侧方静脉组32例,指尖外形佳、指腹饱满;吻合指腹静脉组14例,指体轻度萎缩,指甲生长不平整;未吻合静脉组12例,指体萎缩明显。吻合指侧方静脉组指甲生长近平整,长度长于其他两组[(14.4±3.2)mm比(12.5±2.3)mm和(12.2±2.2)mm],远侧指间关节活动度大于其他两组[(63±5)°比(48±3)°和(45±7)°],两点分辨觉小于其他两组[(4.6±0.4)mm比(7.1±1.2)mm和(7.3±0.6)mm],感觉级别高于其他两组[S(3.45±0.39)级比S(2.57±0.42)级和S(2.55±0.49)级],差异均具有显著性(P〈0.05)。吻合指腹静脉组和未吻合静脉组在指甲长度、运动和感觉方面差异无统计学意义(P〉0.05)。结论:吻合指侧方静脉能有效解决指尖再植静脉回流问题,可避免回流障碍,成活率高,促进指甲生长,可恢复 DIPJ 活动度及感觉。  相似文献

目的:明确不同固定器械在胫骨干不同骨折类型固定中的特点,以指导临床应用。方法:68例胫骨干骨折,行加压钢板螺钉、交锁髓内钉、单侧外固定架固定后,作临床疗效分析。结果:加压钢板固定组42例,感染5例,骨不连1例,平均愈合时间3.8个月;交锁髓内钉固定组13例,无感染及骨不连,平均愈合时间5.4个月;单侧外固定架组13例,骨不连1例,踝关节背伸受限3例,平均愈合时间4.5个月。结论:胫骨骨折交锁髓内钉固定并发症少,功能恢复好,适用范围广,但要注意及时进行动力加压。加压钢板及外固定架固定应选择各自的最佳适应证,以达到理想的疗效。  相似文献
