Background

The aim of this analysis was to describe the role of anesthesiology departments in pain medicine and palliative care services in German hospitals.

Method

In the year 2012, all heads of departments of anesthesiology registered with the German Society of Anesthesiology and Intensive Care Medicine were surveyed about structures of pain medicine and palliative care services in their hospitals using a standardized postal questionnaire.

Results

Out of 408 returned questionnaires (response rate 47%) 403 could be evaluated. Of the hospitals 58% had a designated pain medicine service, in 36 (9%) of the hospitals this was organized as an independent department and in 195 (57%) as part of another department, mostly the department of anesthesiology. The “pain clinic” as an outpatient service was the most common form of structure for pain medicine services (41%). Inpatient pain medicine units were available in 77 (19%) of the hospitals and a partial inpatient unit in the form of a day hospital in 26 (7%) of the hospitals. For the care of inpatients from other departments, there was an intrahospital pain consultation service in 166 of the hospitals, which was the only structure for pain medicine in 32 of the 231 hospitals that reported having a designated pain medicine service. In 160 pain medicine services anesthesiologists were the only medical practitioners and in a further 18 both anesthesiologists and other specialists were available (orthopedist/orthopedic surgeons n = 6, internal medicine n = 4, psychiatrist n = 2, general practitioner = 1 and neurologist n = 1). Only two hospitals had no anesthesiologist in the pain medicine team and for the remaining 51 hospitals no information was provided. In 189 of the 231 hospitals with pain medicine services, there was at least 1 physician with special qualifications in pain management. In 97 (44%) of the hospitals psychologists were part of the team with 53 having at least 1 psychologist with a special qualification in chronic pain management. Of the hospitals, 16% had a specialized department for palliative care, in 32% a specialized palliative care service was part of another department, which was the department of anesthesiology in 30%. Of the hospitals 56% had a palliative care consultation service, 41% had a specialized inpatient palliative care unit, 6% an outpatient clinic, 4% a day hospital and in 16% a specialized outpatient palliative care (SOPC) serving the community was incorporated. Inpatient consultation services and the SOPC were more common when the department of anesthesiology was involved in the palliative care services.

Conclusion

In German hospitals, the departments of anesthesiology make a significant contribution to the provision of both pain medicine and palliative care services. Nevertheless, the respective structures of care are often incomplete or even lacking. There were shortcomings in terms of organization and qualification of the team in pain medicine services (e.g. no doctor with special qualifications in pain management or no psychologist). Palliative care services are more often organized as independent departments than as pain medicine services. Engagement of the anesthesiology department in palliative care is linked to a broader scope of the services provided, which might reflect the capacity of many anesthesiologists to work in an interdisciplinary manner and across interfaces.

A 78 year old woman had suffered ten spontaneous bone fractures, the first occurring when she was 57 years old. She is now wheel-chair bound. After 21 years, the underlying osteomalacia due to oligosymptomatic celiac disease with malabsorption was diagnosed. The treatment consists of a gluten free diet and substitution of calcium and vitamin D until there is a normalisation of calcium, vitamin D and alkaline phosphatase. Any spontaneous fracture deserves a careful search for metabolic bone disease. An elevation of alkaline phosphatase indicates osteomalacia rather than osteoporosis.  相似文献

Objective

Prosthetic grafts used for lower extremity revascularization and dialysis access fail because of hyperplastic stenosis and thrombosis. Graft surveillance is advocated to monitor function; however, graft failure can occur between episodic examinations. An innovative sensor with wireless, microchip technology allows automated surveillance with assessment of graft function using a “cloud”-based algorithm. We performed proof-of-concept experiments with in vitro and in vivo models to assess the feasibility such a real-time graft surveillance system.

Methods

A self-monitoring graft system was evaluated consisting of a prosthetic conduit of expanded polytetrafluoroethylene and a sensor unit, and a microsensor, microelectronics, battery, and remote processor with a monitor. The sensor unit was integrated on the extraluminal surface of expanded polytetrafluoroethylene grafts without compromise to the lumen of the conduit. The grafts were tested in vitro in a pulsatile, recirculating flow system under physiologic flow parameters. The hemodynamic parameters were varied to assess the ability to obtain wireless signal acquisition reflecting real-time flow properties in vitro. Segments of custom tubing with reduced diameters were inserted into the model to mimic stenosis proximal and distal to the grafts. After characterization of the initial data, the self-monitoring grafts were implanted in an ovine carotid model to assess proof of concept in vivo with 30-day follow-up of signal acquisition as well as arteriographic and histologic analysis.

Results

In vitro flow data demonstrated the device was able to determine factors related to prosthetic graft function under varied hemodynamic flow conditions. Wireless signal acquisition using Bluetooth technology (Bluetooth SIG, Inc, Kirkland, Wash) allowed remote data analysis reflecting graft flow parameters through changes in microsensor voltage and frequency. Waveform analysis was applied to construct an algorithm using proprietary software and determine a parameter for graft flow characteristics. This algorithm allowed determination of the degree of stenosis and location of stenosis location (proximal or distal) for display on a remote monitor in real time. Subsequent in vivo experiments confirmed the ability of the system to generate signal acquisition through skin and soft tissue under biologic conditions with no arteriographic stenosis and a favorable healing response at 30-day harvest.

Conclusions

Initial in vitro and in vivo experiments demonstrate the ability for a self-monitoring graft system to remotely monitor hemodynamic parameters reflecting graft function using wireless data transmission. This automated system shows promise to deliver real-time data that can be analyzed by cloud-based algorithms alerting the clinician of a change in graft function or development of stenosis for further diagnostic study or intervention before graft failure.  相似文献

Background

The experience gained by the Basel Hirschsprung Competence Center over 20 years is presented.

Materials and Methods

A total of 19,365 rectal mucosal biopsies were investigated in the 20 years between 1987 and 2006. All biopsies of rectal mucosa originated from 6,615 children aged between 1 week and 4 years. Biopsies were collected in teaching hospitals all over Germany and transported on dry ice by Intercity Courier Service. Serial sections of frozen tissue were made using a cryostat. Enzyme histochemical staining was performed.

Results

A total of 935 cases of Hirschsprung’s disease (14%) were observed (769 cases of classical Hirschsprung’s disease, 68 total colon aganglionosis, 98 ultrashort rectum aganglionosis). Total colon aganglionosis was found in 1.0% and the frequency of ultrashort Hirschsprung’ disease was 1.4%. The quality of the histological results was confirmed by a second independent investigator. There were neither false positive nor false negative diagnoses. Enzyme histochemical staining results were readable within 2 h. Acetylcholinesterase, which is significantly increased in Hirschsprung’s disease, was used for nerve fiber staining. Succinic and lacticdehydrogenases and nitric oxide synthase served as confirmatory proof of aganglionosis (elective nerve cell staining of the submucous plexus).

Conclusion

Among 100 children with chronic constipation an average of 12 children were diagnosed with Hirschsprung’s disease. Of these 2% showed total colon aganglionosis or ultrashort Hirschsprung’s disease. Enzyme histochemical diagnosis of Hirschsprung’s disease proved 100% reliable and time saving.  相似文献

Visceral leishmaniasis is a disease caused by the protozoan Leishmania and is transmitted by Lutzomyia longipalpis (sand fly). It is an endemic parasitic infection in numerous areas around the Mediterranean basin. Though immunocompetent patients may not develop the disease, in transplant recipients the use of corticoids and intensified immunosuppressants to prevent graft rejection may accelerate the disease, causing severe damage to the liver, spleen, and hematopoietic system. We report 2 cases of visceral leishmaniasis with an atypical presentation in transplant recipients. The first patient, who had a kidney transplant, was treated successfully with liposomal amphotericin B, and the second patient, a combined kidney-pancreas transplant recipient, suffered a relapse 3 years after treatment. Visceral leishmaniasis should be considered in the differential diagnosis of pancytopenia or unexplained fever occurring after organ transplantation in patients living in endemic areas or returning from endemic countries.  相似文献
