目的探讨房室折返性心动过速(AVRT)和房室结折返性心动过速(AVNRT)患者的个性特征。方法采用龚耀先修订的艾森克个性问卷量表,对83例AVRT、105例AVNRT患者射频消融前后和50例对照组正常人的精神质(P)、内外向(E)、情绪稳定性(N)和掩饰倾向(L)值进行测量。结果射频消融前后,各组之间的P、E、N和L值相比较差异无显著性(P>0.05);射频消融前后AVNRT组内女性患者的N分值较男性高(分别为12.93±2.83vs9.88±2.61;12.84±2.87vs9.87±2.64;P均<0.05)。结论AVNRT女性患者具有神经质倾向,这可能是在AVNRT中女性占多数的原因之一。  相似文献

目的:探讨希氏束旁起搏鉴别间隔部隐匿性房室旁道与慢一快型房室结折返性心动过速(AVNRT)的临床价值。方法:采用希氏束逆传不应期心室早搏刺激法将61例患者分别诊断为37例慢一快AVNRT和24例间隔部房室折返性心动过速(AVRT);再对61例患者采用希氏束旁起搏方法进一步检测。结果:采用希氏束旁起搏法检测37例AVNRT患者中有6例未检测成功,其余31例均为逆传房室结图形;24例AVRT患者中4例未检测成功,15例呈逆传旁道/旁道图形,5例呈非逆传旁道/旁道图形。如以逆传旁道/旁道图形为标准,鉴别间隔快旁路引起的AVRT与慢一快型房室结折返性心动过速,敏感性75%,特异性可达1009/6。结论:希氏束旁刺激法对鉴别诊断AVRT与AVNRT有较高的特异性。  相似文献

INTRODUCTION: Para-Hisian pacing during sinus rhythm can help to identify the presence of an accessory pathway (AP). In this maneuver, the retrograde activation time and pattern are compared during capture and loss-of-capture of the His bundle while pacing from a para-Hisian position. However, identification of a retrograde AP does not necessitate that it is operative during the tachycardia of interest; conversely, slowly conducting or "distant" bypass tracts may not be identified. We evaluated the utility of entrainment or resetting of tachycardias from the para-Hisian position to help distinguish atrioventricular nodal reentrant tachycardia (AVNRT) from orthodromic atrioventricular tachycardia (AVRT). METHODS AND RESULTS: Para-Hisian entrainment/resetting was evaluated in 50 patients: 33 with AVNRT and 17 with AVRT. The maneuvers were performed using a standard quadripolar catheter placed at the His position: low output for right ventricular (RV) capture and high output for both RV and His capture. The retrograde atrial activation sequence, SA interval (interval from stimulus to earliest retrograde atrial activation), and "local" VA interval (interval between the ventricular and atrial electrograms at the site of earliest retrograde atrial activation) were compared between His and His/RV capture. The DeltaSA was > 40 ms in patients with AVNRT and was < 40 ms in all but one patient with AVRT. In concert with the DeltaSA interval, the DeltaVA interval was able to fully define the mechanism of the tachycardia in all patients studied. CONCLUSION: Para-Hisian entrainment/resetting can determine the course of retrograde conduction operative during narrow complex tachycardias. It is a useful diagnostic maneuver in differentiating AVNRT and orthodromic AVRT.  相似文献

目的 评价程控刺激不能诱发的房室结折返性心动过速 (AVNRT)射频消融慢径的临床疗效。方法  6 1例有心动过速病史且心电图疑诊为AVNRT的病人 ,电生理检查有房室结双径(DAVNP)但不能诱发AVNRT ,随机分为两组。A组 30例不消融而进行临床随访 ,当心动过速复发且经心电图证实为窄QRS心动过速者接受射频消融阻断慢径。B组 31例接受射频消融以阻断慢径 ,术后临床随访。结果 A、B两组分别有 2 4例和 2 7例病人完成随访。A组 2 4例随访中分别在 1年内发作心动过速 ,再次接受消融阻断慢径后随访 (12 .1± 12 .2 )个月 ,仅 1例复发心动过速 (4.2 % ) ,与消融前比较差异有显著性 (P <0 .0 0 0 1)。B组 2 7例平均随访 (2 4 .2± 17.6 )个月 ,1例复发心动过速 (3.7% ) ,与A组病人消融前相比差异有显著性 (P <0 .0 0 0 1) ,而与其消融后比较差异无显著性 (P >0 .0 5 )。结论 有阵发性心动过速病史且心电图疑诊为AVNRT的病人 ,电生理检查有DAVNP而不能诱发心动过速者 ,射频消融阻断慢径具有良好的临床疗效。  相似文献

回顾性分析经射频消融治疗的418例阵发性室上性心动过速(PSVT)发作时的体表12导联心电图,以了解ST-T改变的临床意义。结果表明305例房室折返性心动过速(AVRT)中,ST段下移≥2mm且持续≥80ms和(或)T波倒置者有181例(59.34%),明显高于房室结折返性心动过速(AVNRT)患者(28/113,24.78%)P<0.005。并且此差异不能被心率所矫正。此外左游离壁旁道患者ST段压低多分布在V3~V6导联,而右后和左后隔旁道患者倒置的T波和(或)ST段压低多发生于I、II、aVF导联。提示PSVT时心电图上ST-T改变可作为区分AVRT和AVNRT的一个有用指标,并且可能还有粗略的旁道定位价值  相似文献

目的分析快慢型房室结折返性心动过速(AVNRT)患者的临床特征、心电网和电生理检查特点、射频消融治疗特点,旨在为临床长RP。心动过速鉴别提供帮助。方法11例经心内电生理检查证实为慢快型房室结折返性心动过速的患者,回顾性分析其临床特征、心电图特点及电生理检查特点及射频消融治疗。结果心动过速表现为窄QRs波心动过速,RP’〉P’R,P。在Ⅱ、Ⅲ、aVF导联倒置,RP’间期为350±25ms,心率为1664-30bpm。11例患者中有3例出现室房逆传跳跃现象。心房程序刺激无明显跳跃现象,11例均可由心房StS:刺激诱发心动过速发作,且容易诱发,容易终止。心动过速发作时,5例CS9.10A波最早,6例HiS的A波最早,其中1例静推ATP心动过速终止。11例患者中9例经房室结改良消融传统慢径获得成功,2例在冠状静脉窦内消融成功,术后随访3个月以上均未再发作心动过速。结论长RP’心动过速的诊断和鉴别诊断有一定困难,如能排除慢旁道和房速,应考虑快慢型房室结折返性心动过速。  相似文献

INTRODUCTION: Simultaneous dual atrioventricular nodal conduction (SDNC) through slow (SP) and fast pathway (FP) is a rare phenomenon observed upon the induction of atrioventricular nodal reciprocating tachycardia (AVNRT). The aim of this study is to report the electrophysiological features of patients showing typical AVNRT induced through SDNC. METHODS AND RESULTS: Among 461 consecutive patients with typical AVNRT submitted to radiofrequency catheter ablation (RFCA), seven patients (1.5%) with SDNC at tachycardia onset (group I: 6 female; age 60-72 years, mean 65.2 +/- 3.8 years) and 118 age-matched controls (group II: 60 female; age 60-88 years, mean 68.4 +/- 6.8 years) were considered. Controls were further subdivided into two subgroups according to age: subgroup A (94 patients, age 60-75 years) and subgroup B (24 patients, age >75 years). The value of the following parameters was significantly higher in group I than in group II and in subgroup A: A-H interval [113 +/- 26 vs. 89 +/- 27 (P < 0.01) vs. 84 +/- 19 (P < 0.001)], ventriculoatrial conduction effective refractory period [355 +/- 85 vs. 293 +/- 87 (P < 0.05) vs. 281 +/- 82 (P < 0.05)], SP conduction time upon AVNRT induction [444 +/- 104 vs. 350 +/- 72 (P < 0.01); vs. 345 +/- 67 (P < 0.001)], AVNRT cycle length [484 +/- 103 vs. 396 +/- 71 ms (P < 0.05); vs. 384 +/- 69 (P < 0.05)], and rate of AVNRT induction from ventricle [71% vs. 10% (P = 0.001); vs. 6% (P = 0.001)]. Differences were mostly not significant between group I and subgroup B. SP location and RFCA success rate were similar in all groups. CONCLUSION: In a population of AVNRT patients, SDNC at AVNRT induction is infrequent and it prevails beyond the fifth decade of life and in females. SDNC is associated with peculiar AVN conduction features, which resemble the age-related modifications of AVN conduction.  相似文献

目的探讨在特殊情况下,单极电图能否代替双极电图运用于房室结折返性心动过速射频消融术中弥补体表心电图中的不足。方法将冠状窦、希氏束及大头消融电极与胸导联相连,记录局部单极电图,运用于5例病人,观察心动过速、射频消融过程中心房波、心室波的关系,并与20例房室结折返性心动过速病人双极记录的特征相比较。结果(1)心动过速均显示出逆A呈向心性分布的特征(100%vs100%,P>0.05);(2)消融过程中单极电图可清楚显示交界心律时的传导情况(100%vs100%,P>0.05)。结论在特殊情况下,使用局部单极电图可代替双极记录,弥补体表心电图在诊断房室结双径路以及射频消融术中的不足,避免完全性房室传导阻滞的发生。  相似文献

INTRODUCTION: We aimed to characterize blood pressure (BP) response at the beginning of atrioventricular nodal reentrant tachycardia (AVNRT) and its relationship to orthostatic challenge and variable atrioventricular interval. METHODS AND RESULTS: In this prospective study of 17 consecutive patients with documented AVNRT, mean BP was analyzed in the supine and upright positions during sinus rhythm, AVNRT, and pacing with atrioventricular delay of 150 msec (AV150) and 0 msec (AV0). Mean BPs were compared at 3-5 seconds, 8-10 seconds, and 28-30 seconds after the onset of AVNRT or pacing. BP decreased immediately after AVNRT initiation, with gradual recovery during the first 30 seconds from 71.9 +/- 16.5 mmHg to 86 +/- 13.8 mmHg, P < 0.01. A similar pattern was observed during AV0, but not during AV150, pacing. While supine, mean BP decrease was more pronounced during AVNRT and AV0 pacing (-26.1% and -32.1%, respectively) than during AV150 pacing (-8%, P = 0.02 and P = 0.07, respectively). This difference subsided 30 seconds after the onset of AVNRT or pacing. When upright, the mean BP time course was similar, but mean BP recovery during AVNRT was slower, and the difference between mean BP during AVNRT and AV150 persisted at 30 seconds. CONCLUSIONS: The initial mean BP decrease during AVNRT recovered gradually within 30 seconds. A short atrioventricular interval is associated with a greater mean BP decrease at the onset of tachycardia. These observations may explain clinical symptoms immediately after the onset of AVNRT.  相似文献

为探讨食管心房调搏揭示室上性心动过速发生机制的价值和局限性,回顾性分析成功射频导管消融的138例隐匿性单房室旁道参与的顺向型房室折返性心动过速和100例单一类型房室结折返性心动过速的食管心房调搏结果。结果显示:前138例中,3例前间隔旁道引起者食管心房调搏均诊断为房室结折返性心动过速余为左右侧其它部位的旁道,诊断正确。后100例中,5例为慢-慢型,2例为快-慢型,食管心房调搏均诊断为房室折返性心动  相似文献

BACKGROUND: Initial reports have shown cryoablation to be safe and efficacious for treatment of atrioventricular nodal reentrant tachycardia (AVNRT). No direct comparisons of cryoablation vs radiofrequency (RF) catheter ablation in pediatric patients have been made. OBJECTIVES: The purpose of this study was to compare the outcomes of cryothermal vs RF catheter ablation for treatment of AVNRT in pediatric patients. METHODS: We retrospectively reviewed consecutive ablation procedures for treatment of AVNRT at a single arrhythmia center. The RF group consisted of patients who underwent RF ablation from 2002 until cryothermy became available. The cryoablation group consisted of patients who underwent cryothermal ablation from 2004 to 2005. The groups were compared for procedural and electrophysiologic outcomes. RESULTS: RF (n = 60, age 14 +/- 4 years) and cryoablation (n = 57, age 14 +/- 4 years) groups had similar demographic and baseline parameters. Procedural times were shorter in the RF group (RF ablation 112 +/- 31 minutes vs cryoablation 148 +/- 46 minutes, P < .001). Fluoroscopy times were comparable (RF ablation 21 +/- 15 minutes vs cryoablation 20 +/- 13 minutes, P = .77). In an intention-to-treat analysis, success of the procedure was 100% for RF ablation and 95% for cryoablation (P = .11). No permanent AV block occurred in either group. Recurrence rates were higher for the cryoablation group, but this did not reach statistical significance (RF ablation 2% vs cryoablation 8%, P = .19). CONCLUSION: Cryoablation appears to be similar to RF for ablation of AVNRT with respect to short-term efficacy and safety of the procedure in a pediatric population. Recurrence rates are higher with cryoablation.  相似文献

INTRODUCTION: Cryoenergy has evolved as a safe and effective alternative for ablation of arrhythmia substrates in adult patients. Due to two specific features, cryomapping and cryoadhesion, this technique appears very attractive for pediatric patients minimizing complications and fluoroscopy time. The aim of the study was to investigate efficacy and safety of cryoenergy in the ablation of supraventricular tachycardia (SVT) substrates in pediatric patients. PATIENTS AND METHODS: Thirty-two patients (mean age: 10.1 +/- 3.5 years) with SVT (accessory pathways: n = 19; atrioventricular nodal reentrant tachycardia (AVNRT): n = 13) underwent electrophysiological study under the guidance of the LocaLisa system. Cryomapping at -30 degrees C was performed to predict cryoablation outcome and to ascertain AV conduction. Cryoenergy was delivered subsequently at the same spot (cryoablation at -70 degrees C) as verified by the LocaLisa system. RESULTS: Successful cryoablation was achieved in 24 of 32 patients (75%). A median of two (1-10) cryoablations were delivered. In the remaining 8 patients, radiofrequency (RF) current application was effective in 5 resulting in an overall success rate of 90.6%. In 4 patients with an accessory pathway cryomapping was not predictive for successful cryoablation (negative predictive value 66.6%). In 3 additional patients with AVNRT transient high-grade AV block occurred during cryoablation despite previous "safe" cryomapping at the same location. No other major complications were noted. CONCLUSION: Cryoablation of SVT substrates in pediatric patients was associated with a lower success rate compared to RF catheter ablation. Cryomapping decreased the number of permanent lesions but did not predict cryoablation outcome in all tachycardia substrates.  相似文献

目的:探讨无房室结双径路特性的房室结折返性心动过速(AVNRT)的电生理特点。方法:所有心动过速患射频消融前常规行心内电生理检查。结果:845例射频病人中325例为AVNRT,其中有21例患房室结功能曲线呈连续性,其电生理特征:希氏束图上心房回波(A)先出现,A波落在室波升支或其前,希氏柬不应期内刺激心室,不能提前夺获心房,射频消融后心房刺激时AHmax明显缩短。结论:伴连续性房室结功能曲线的AVNRT患心房刺激不表现房室结双径路的电生理特性,其消融终点初步定为:心房心室S1S1、S1S2刺激不诱发AVNRT;无AHvH传导曲线跳跃;房室结前传不应期明显缩短。  相似文献

目的利用食管法心脏电生理检查探讨房室结折返性心动过速伴房室阻滞的电生理特点。方法回顾分析经食管法电生理检查中房室结折返性心动过速伴房室阻滞18例患者的资料。结果房室结折返性心动过速伴房室阻滞主要表现为2:1房室阻滞,多在诱发开始时出现数秒至数分钟,也可呈持久性存在,2:1传导转为1:1传导时多经过一过性3:2文氏传导并伴一过性束支阻滞。结论P波极向及P波与QRS波群的关系特点是房室结折返性心动过速伴2:1房室阻滞的诊断依据。以此排除房室折返性心动过速,并注意与房速相鉴别。  相似文献

目的 探讨经导管冷冻消融与射频消融治疗房室结折返性心动过速的有效性和安全性.方法 对304例房室结折返性心动过速行导管消融术病例行回顾性分析,其中冷冻组67例,射频组237例,比较两组成功率、慢径完全阻断率、房室传导阻滞率和复发率的差异.结果 两组消融成功率(冷冻组98.5%与射频组97.0%,P=0.820)、慢径完全阻断率(冷冻组98.5%与射频组91.6%,P=0.088)、房室传导阻滞率(冷冻组0与射频组2.5%,P=0.413)、复发率(冷冻组0与射频组1.7%,P=0.643)差异均无统计学意义,但冷冻组慢径完全阻断率有优于射频组的趋势.结论 冷冻消融治疗房室结折返性心动过速安全有效,较射频消融术有潜在优势.
Abstract:
Objective To compare the efficacy and safety between cryoablation (Cryo) and radiofrequency (RF) ablation for treating patients with atrioventricular nodal reentrant tachycardia (AVNRT). Methods Patients with AVNRT (n=304) were divided into Cryo group (n=67) and RF group (n=237). The procedure success rate, complete slow pathway block rate, atrioventricular block rate and relapse rate were compared between two groups. Results There was no statistically difference between 2 groups in the success rate (Cryo group 98.5% vs RF group 97.0%, P=0.820), complete slow pathway block rate (Cryo group 98.5% vs RF group 91.6%, P=0.088), atrioventricular block rate (Cryo group 0 vs RF group 2.5%, P=0.413), relapse rate (Cryo group 0 vs RF group 1.7%, P=0.643). But Cryo group had more advantage than RF group. Conclusion Efficacy and safety were comparable between cryoablation and radiofrequency ablation for treating patients with AVNRT.  相似文献

INTRODUCTION: Atrioventricular nodal reentrant tachycardia (AVNRT) is the most common regular supraventricular tachycardia in the general population as well as in elderly patients. The purpose of the study was to investigate the success and complication rate particularly regarding the induction of an atrioventricular (AV) block by radiofrequency (RF) ablation in elderly patients with and without a preexisting AV block. METHODS AND RESULTS: Between February 1998 and July 2004, all patients with symptomatic AVNRT referred for slow-pathway ablation in our institution were included and divided into two groups: group 1 patients younger than 75 years (n = 508) and group 2 patients > or =75 years (n = 70). A preexisting prolonged PR interval was present in 17 (3.3%) patients of group 1 and in 26 (37%, P < 0.0001) patients of group 2. Following successful slow-pathway ablation (follow-up time group 1: 37 +/- 22, group 2: 37 +/- 24 months) no induction of an AV block was observed in group 2 but in four patients of group 1 (0.79%) a complete heart block was induced requiring a pacemaker implantation. In group 1, 15 (2.95%) patients with a recurrence of AVNRT were readmitted for a repeat ablation procedure. No recurrences occurred in group 2. CONCLUSION: Despite a higher incidence of preexisting prolonged PR intervals slow-pathway ablation in elderly patients is both effective and safe and should be considered as the first line therapy also in this patient population.  相似文献

目的 通过与国外进口温控射频消融导管的比较,评价国产温控射频消融导管在房室结折返性心动过速(AVNRT)和房室折返性心动过速(AVRT)的临床疗效和安全性.方法 自2008年6月至11月共入选AVNRT或AVRT患者1342例.采用多中心随机平行对照开放试验设计,应用解放军第四军医大学设计的最小化随机统计方法进行动态随机分为二组,国产导管组应用国产温控射频消融导管,进口导管组应用强生或圣犹达等公司的进口温控射频消融导管.采用常规方法在X线透视下进行心内电生理检查和射频消融.结果 国产导管组672例,即时消融成功率97.9%,进口导管组670例,即时消融成功率99.1%,两组成功率均达到96.0%以上,差异无统计学意义.国产导管组与进口导管组的手术时间[(68±36)min与(67±34)min]、曝光时间[(14±14)min与(10±11)min]、放电次数[(4.5±4.5)次与(4.6±3.9)次]、放电时间[(260±218)s与(257±207)8]、导管性能总体评分[(4.4±0.5)分与(4.5±0.4)分],两组间差异均无统计学意义.发生并发症3例,均为心包积液(国产导管组1例,进口导管组2例),经心包穿刺引流后转为正常出院,差异无统计学意义.随访3个月,复发30例,分别为国产导管组14例、进口导管组16例(2.1%与2.4%),差异无统计学意义.结论 国产温控射频消融导管可安全有效治疗AVNRT或AVRT.  相似文献

目的 观察心室融合波伴心房激动提前对间隔旁路逆传的顺向型房室折返性心动过速(OAVRT)的诊断作用。方法 按心内电生理检查标准和射频消融结果,将47例符合人选条件的患者分为两组:房室结折返性心动过速(AVNRT)组和间隔旁路逆传的0AVRT组,分别为24例和23例。心动过速时行心室期前程序刺激(RS2刺激)和心室快速刺激,测量体表心电图上心室融合波之后的心房激动时间是否提前。结果 RS2刺激和心室快速刺激均能形成多个心室融合波。AVNRT组无l例伴有心房激动提前(特异性100%),而OAVRT组在心室刺激成分明显的心室融合波时,心房激动均被提前(敏感性100%)。两组间的差异十分显著(P<0.001)。结论 心室融合波伴心房激动时间提前是诊断间隔旁路逆传OAVRT的可靠指标,具有敏感性和特异性高的特点,而且也可用于未能记录到希氏束电图的患者。  相似文献

BACKGROUND: Differentiating atrioventricular nodal reentrant tachycardia (AVNRT) from orthodromic atrioventricular reentrant tachycardia (AVRT) can be difficult. The His bundle and atria are activated sequentially over the AV node during entrainment of AVNRT from the ventricle but simultaneously during supraventricular tachycardia (SVT). They are activated in parallel during entrainment of AVRT but sequentially during SVT. OBJECTIVE: The purpose of this study was to test the hypothesis that a DeltaHA (HA((entrainment)) - HA((SVT))) cutoff value of 0 reliably differentiates AVNRT from AVRT. METHODS AND RESULTS: Of 61 patients undergoing electrophysiologic evaluation for paroxysmal SVT, retrograde His-bundle potentials were recorded in 57 (93%) and entrainment performed in 49 (34 AVNRT, 15 AVRT). DeltaHA values during entrainment from the ventricle were significantly longer during AVNRT than AVRT (31 +/- 24 ms vs -38 +/- 31 ms, P <.001). All DeltaHA values were positive (minimum: 3 ms) for AVNRT and negative (maximum: -2 ms) for AVRT. DeltaHA of 0 had sensitivity, specificity. and positive predictive value of 100% for correct diagnosis. CONCLUSION: The DeltaHA criterion during entrainment of tachycardia from the ventricle reliably differentiates AVNRT (positive values) from AVRT (negative values).  相似文献

Introduction: Paroxysmal supraventricular tachycardia (PSVT) is often associated with paroxysmal atrial fibrillation (AF). However, the relationship between PSVT and AF is still unclear. The aim of this study was to investigate the clinical and electrophysiological characteristics in patients with PSVT and AF, and to demonstrate the origin of the AF before the radiofrequency (RF) ablation of AF.
Methods and Results: Four hundred and two consecutive patients with paroxysmal AF (338 had a pure PV foci and 64 had a non-PV foci) that underwent RF ablation were included. Twenty-one patients (10 females; mean age 47 ± 18 years) with both PSVT and AF were divided into two groups. Group 1 consisted of 14 patients with inducible atrioventricular nodal reentrant tachycardia (AVNRT) and AF. Group 2 consisted of seven patients with Wolff-Parkinson-White (WPW) syndrome and AF. Patients with non-PV foci of AF had a higher incidence of AVNRT than those with PV foci (11% vs. 2%, P = 0.003). Patients with AF and atypical AVNRT had a higher incidence of AF ectopy from the superior vena cava (SVC) than those with AF and typical AVNRT (86% vs. 14%, P = 0.03). Group 1 patients had smaller left atrial (LA) diameter (36 ± 3 vs. 41 ± 3 mm, P = 0.004) and higher incidence of an SVC origin of AF (50% vs. 0%, P = 0.047) than did those in Group 2.
