Diphenylhydantoin-induced pure red cell aplasia

Pure red cell aplasia is an uncommon complication of diphenylhydantoin therapy. It has not been reported in Indian literature. Awareness of the entity helps in establishing the cause of anaemia in these patients and alerts the physicians to the need of comprehensive haematological monitoring in these patients. A case of 58-year-old male who developed pure red cell aplasia following three months of diphenylhydantoin therapy is reported here.     

胸腺腺瘤合并单纯红细胞再生障碍性贫血

目的：探讨胸腺瘤合并单纯红细胞再生障碍性贫血（PRCA）的诊断与治疗。方法：总结1999年5月～2006年3月本科收治胸腺瘤合并PRCA的临床资料,并结合文献进行讨论,进行文献复习和讨论。结果：70例胸腺瘤中3例合并PRCA占43％（3／70）,其中2例手术切除胸腺及胸腺瘤,分别随访1．5～3年,无胸腺瘤和PRCA复发征象;探查1例内科治疗未见改善。结论：胸腺瘤合并PRCA首选治疗是手术切除胸腺瘤和胸腺组织,其疗效有待进一步观察。

纯巨核细胞再生障碍的特点是骨髓选择性巨核细胞减少，外周血血小板减少不是破坏加速，而是生成不足所致，巨核细胞及血小板不仅有数量的减少，而且有形态的改变．对发病机理、诊断标准、治疗及转归进行了讨论。

Azathioprine-induced pure red blood cell aplasia.

Pure RBC aplasia developed in two renal transplant recipients who were receiving long-term azathioprine therapy. With substitution of cyclophosphamide therapy for azathioprine, erythroid hyperplasia and reticulocytosis developed at three weeks in one patient and at three months in the other. Selective erythroid toxic reaction is a potential problem that must be considered when anemia develops in patients receiving long-term azathioprine therapy.     

Pure white cell aplasia and health food products.

The safety of ''health'' foods and complementary medicine products is increasingly questioned. We report a case of pure white cell aplasia developing in a patient who took a variety of such products leading to an excessive intake of zinc. Recovery was complete following the withdrawal of her ''medications''. The aetiology of the pure white cell aplasia is discussed with respect to the ingredients of the products ingested.     

Pure red cell aplasia and thymoma associated with high levels of the suppressor/cytotoxic T lymphocyte subset

A 64 year old man admitted to hospital with increasing effort dyspnoea and lethargy was found to have a thymoma and pure red cell aplasia. Lymphocytes accounted for 20-30% of marrow cells, and numbers of T8 suppressor/cytotoxic cells in peripheral blood were greatly increased. He remained anaemic after removal of the thymoma despite blood transfusions, and immunosuppression with prednisolone 60 mg and cyclophosphamide 50 mg daily was started. The dose of prednisolone was reduced to 15 mg owing to steroid myopathy and the risk of opportunistic infection. He went into remission, and the dose was further decreased to 10 mg daily.     

Pure red cell hypoplasia secondary to isoniazid

We describe a 77 year old man who developed pure red cell aplasia while receiving antituberculous therapy including isoniazid. Prompt recovery occurred following cessation of isoniazid. In this paper we also review previously described case reports of isoniazid-induced pure red cell aplasia.     

24例纯红细胞再生障碍性贫血临床分析

目的探讨纯红细胞再生障碍性贫血(PRCA)的病因、发病机制、治疗.方法采用回顾性分析的方法,对我院1985～2005年收治住院的24例PRCA患者进行研究.结果 PRCA的发病与多种免疫性疾病相关,免疫球蛋白检查11/16例异常,血清蛋白电泳4/9例γ球蛋白明显增高;O型血患者比例高,占56.5%.治疗有效率80%.结论 PRCA发病与免疫机制有关,O型血患者比例高;本病预后较好,免疫制剂治疗有效.
