The so-called head joints consist of the segments occiput/C1, C1/C2, C2/C3. The existence of cervicogenic headaches is not fully accepted by the different medical schools. Morphological alterations in the upper cervical spine are seldom causes of complaints, similar to X-ray diagnosed anomalies of the position of the joints in the upper spine. The vertebrogenic headaches (upper cervical syndrome, cervicocephalic syndrome, cervicocranial syndrome) are discussed as to anamnesis, the topic of pain, and the functional findings. According to Gutmann a distinction is made between the headache caused by blockages, the headache caused by hypermobility, and the headache caused by tensed muscles. It has been criticized that headache specialists diagnose tension headaches based to a large extent on the anamnesis and not the clinical examination. The hope for medical high technology should not put a damper on clinicians’ professional skills.     

Grundlagen radiochirurgischer Systeme und ihre Anwendungen in der Arthroskopie

Zusammenfassung Elektrochirurgische Methoden werden in allen Bereichen der Medizin eingesetzt und haben in der jüngsten Vergangenheit auch in der arthroskopischen Chirurgie große Verbreitung gefunden. Dies ist maßgeblich auf die Entwicklung handlicher und relativ preiswerter Systeme mit bipolarer Funktionsweise in einem hochfrequenten Spektrum zurückzuführen. Ein sich zwischen den eng benachbarten Elektroden aufbauendes lokales Plasmafeld an der Sondenspitze ermöglicht die Desintegration von Molekülen im behandelten Gewebe. Durch den räumlich begrenzten Stromfluss werden zahlreiche Nachteile und Gefahren monopolarer oder Lasersysteme vermieden. Es besteht allerdings eine erhebliche Diskrepanz zwischen dem umfangreichen praktischen Anwendungsspektrum dieser Systeme einerseits und dem Wissen über die mittel- und langfristigen biologischen Auswirkungen im behandelten Gewebe andererseits. Klinische und experimentelle Untersuchungen existieren bislang v. a. für die häufig gestellten Indikationen Knorpelglättung bei Chondromalazie Grad II-III und Kapsel- bzw. Bandschrumpfung. Prinzipielle Einigkeit über diese Indikationen besteht jedoch nicht, zumal sich sowohl im Gelenkknorpel als auch in den Strukturen der Gelenkkapsel in bis zu mehreren Millimetern Tiefe erhebliche Absterberaten von Chondrozyten bzw. Fibroblasten zeigen, deren Vitalität jedoch für das Aufrechterhalten der Stabilität dieser Strukturen essentiell ist. Während andere Arbeitsgruppen zu weniger kritischen Ergebnissen kommen, lehnen manche Autoren den Einsatz von Radiochirurgiesystemen in der Arthroskopie solange gänzlich ab, bis nicht in weiteren Untersuchungen die langfristige Ungefährlichkeit bewiesen ist. Leider gibt es bislang technisch kein funktionierendes Rückkopplungssystem, das über die Gewebeerwärmung und andere wichtige Parameter während der Anwendung zuverlässig informiert. Die praktische arthroskopische Handhabung der Instrumente stellt also ebenso wie die Indikationsstellung hohe Ansprüche an die Erfahrung und die Fertigkeiten des Operateurs. Abstract Electrosurgical devices are being used in each medical subdiscipline, and, only recently, they have found widespread distribution in the field of arthroscopic surgery, as well. This development is mainly due to the construction of small and relatively cheap devices that work in a bipolar mode at high frequency. A local plasma field is being built up between the electrodes at the tip of the probe which has enough energy to allow the desintegration of molecules within the treated tissue. The local current flow helps to minimize the numerous disadvantages of monopolar or Laser-based systems. Nevertheless, there is considerable discrepancy between the wide spectrum of clinical use on the one hand and the knowledge of mid- or even long-term biological effects on the treated tissue on the other. So far, clinical and experimental studies exist for the frequent indications of chondroplasty and capsular or ligamental shrinkage. But, still, there is no agreement upon these indications. Studies have shown a considerable number of dead chondrocytes or fibroblasts within a depth of several millimeters within the tissue of the joint capsule or the articular cartilage after electrosurgical treatment. But especially these cells are thought to be essential for the maintenance of the tissue. While the results of other studies seem to be less critical, some authors propose not to use electrosurgery in arthroscopy at all, until further studies prove the safety of these devises. Unfortunately, there is no reliable feedback system for the temperature increase within the treated tissue or other important parameters. The use of these instruments affords a high degree of experience from a skilled arthroscopic surgeon.     

Die Formen der Hypermobilität und ihre klinische Einordnung

Hypermobility is found in a number of different clinical conditions and described by terms such as increased laxity, hyperextensibility or increased flexibility. Local hypermobility is limited to a single joint or spinal movement segment; it is always pathologic, and mostly the result of a trauma. Generalized pathological hypermobility affects large body areas and is symptomatic in certain neurological and orthopaedic diseases, the main pathogenic factor being generalized low muscle tone. Collagen deficiency is involved in the pathogenesis only in certain hereditary syndromes, such as Marfan or Ehlers-Danlos syndrome. Hypermobility is also found in physiological conditions, such as pregnancy, and after special training in dancers, musicians, sportspersons in several disciplines and acrobats. In such cases hypermobility is reversible. Benign or constitutional hypermobility can be considered to be in the upper limits of normality. Persons so affected can remain in perfect health, but are more susceptible to overstrain, after which they are liable to suffer from signs of recurring dysfunction of the musculoskeletal system.

Die Gründe für den Niedergang der forschenden pharmazeutischen Unternehmen in Deutschland sind zahlreich. Als wichtigste sind zu nennen eine zu geringe öffentliche Forschungsförderung, eine zu spät einsetzende Förderung der Biotechnologie sowie eine nachlassende Investition in die Arzneimittelforschung und -entwicklung durch fortgesetzte Kostendämpfungsmaßnahmen der Sozialpolitik.Eine Übersicht der OECD für den Zeitraum von 1992 bis 2001 deutet darauf hin, dass der durchschnittliche Preisanstieg bei Arzneimitteln wesentlich geringer ausfiel als in Kanada, den USA, Australien und den übrigen europäischen Märkten.
