Chronic macular detachment following pneumatic retinopexy   总被引:1,自引:0,他引:1  
In a consecutive series of 73 retinal detachments managed with pneumatic retinopexy, three (4.1%) of 73 eyes sustained chronic detachment of the posterior retina involving the macula even though all retinal breaks were closed. This shallow subretinal fluid persisted for 12 to 21 months but reabsorbed spontaneously. Two cases presented with a detached macula, one of which had pre-existing macular degeneration. The other case presented with an attached macula but it became detached immediately after pneumatic retinopexy. The visual acuities in the two patients who did not have macular pathology before the development of retinal detachment were 20/50 and 20/40 even with persistent subretinal fluid under the macula. In both cases the visual acuity improved to 20/30 after resolution of the subretinal fluid. Patients with a longstanding component to the retinal detachment and small retinal breaks may be at risk of developing chronic macular detachment following pneumatic retinopexy. Pockets of subretinal fluid can persist following scleral buckling, with or without drainage of subretinal fluid. However, it is unknown whether scleral buckling has a lower incidence of this complication than pneumatic retinopexy.  相似文献

BACKGROUND: Although pneumatic retinopexy was introduced for the repair of primary retinal detachments, we have had excellent long-term success in employing this technique along with laser photocoagulation following failure of routine scleral buckle surgery in nonvitrectomized eyes over the last 10 years. PATIENTS AND METHODS: We categorized a consecutive series of 40 eyes that failed primary scleral buckling surgery and had at least six months follow-up. Eyes were separated into two groups: those with 1) subretinal fluid persisting or developing during the first 14 days after surgery or 2) those accumulating subretinal fluid at least 14 days after initially successful anatomic reattachment of the retina. RESULTS: In these groups, 36 of the 40 eyes (90%) were successfully reattached using outpatient pneumatic retinopexy alone. Complications were limited to the production of new retinal breaks in 5 patients. The 4 pneumatic retinopexy failures were all subsequently treated successfully with either scleral buckle revision or vitrectomy. CONCLUSION: We believe that laser pneumatic retinopexy repair of recurrent retinal detachments following scleral buckle and without significant proliferation vitreoretinopathy (PVR) should be considered ahead of conventional surgical intraoperative techniques. Laser pneumatic retinopexy may be a very successful procedure for the treatment of recurrent retinal detachments after failed scleral buckle surgery. In a consecutive series of 40 eyes with recurrent retinal detachment, we were able to repair 36 with pneumatic retinopexy alone.  相似文献

There have been 26 series (1,274 eyes) published on the use of pneumatic retinopexy for selected retinal detachments. Eighty percent were reattached with a single procedure and 98% with reoperations. New retinal breaks occurred in 13% and proliferative vitreoretinopathy in 4%. The three complications reported with pneumatic retinopexy but not with scleral buckling are subretinal gas, gas entrapment at the pars plana, and subconjunctival gas. Twenty-six complications of pneumatic retinopexy, most of which may also occur with scleral buckling, are discussed as to incidence, cause, prevention, and management.  相似文献

目的:探讨不放液巩膜外加压术治疗陈旧性视网膜脱离的临床疗效。方法:回顾性分析不放液单纯巩膜外加压术治疗32例32眼陈旧性视网膜脱离的临床病例资料,观察术后最佳矫正视力、手术并发症和视网膜复位情况。结果:术后随访6~12(平均9.2)mo,最佳矫正视力>0.1者17例17眼,2例2眼术后短期内眼压升高,首次手术解剖复位30例30眼,成功率94%。2例2眼术后视网膜下增殖继续发展,行玻璃体切除术后视网膜复位。结论:不放液巩膜外加压术对视网膜下增殖不严重的陈旧性视网膜脱离简单有效。  相似文献

1,000 consecutive patients operated for retinal detachment were studied prospectively to determine the incidence of complications. Two parts of the operation were responsible for most of the vision-threatening complications: subretinal fluid drainage and the scleral buckle. In an attempt to reduce the complications of retinal detachment surgery, we have utilized a simplified procedure of transconjunctival cryotherapy and intravitreal gas injection with postoperative positioning. This study was limited to detachments with one or more breaks within one clock hour located within the superior eight clock hours of the fundus without signs of proliferative vitreoretinopathy. In a series of 20 consecutive patients, retinal reattachment was initially achieved in all cases. There were two recurrences that were reattached with scleral buckling. The final cure rate for the single pneumatic procedure, with six months follow-up, was 90%. No major complications were observed. This preliminary report suggests that pneumatic retinopexy has the advantages of reduced tissue trauma, no hospitalization, minimal complications, and reduced expense. The major disadvantage is the need for postoperative positioning for five days.  相似文献

目的探讨环扎加压术治疗陈旧性视网膜脱离的疗效。方法陈旧性视网膜脱离患者60例(60只眼)进行环扎加压术治疗,术中巩膜外放液48只眼(80%),不放液12只眼(20%),C3F8玻璃体腔内注入10只眼(16.7%)。结果术后随访6-12个月(平均8.6个月),44只眼(73.3%)视网膜完全复位,14只眼(23.3%)明显好转,2只眼(3.3%)手术后视网膜未复位,后行玻璃体手术。手术后视网膜脱离治愈率73.3%,有效率96.7%。结论环扎加压术治疗不伴有视网膜前膜形成的陈旧性视网膜脱离,疗效良好,创伤小,可作为首先选择的术式。  相似文献

Pneumatic retinopexy. A collaborative report of the first 100 cases   总被引:5,自引:0,他引:5  
The preliminary experience of one surgeon, using pneumatic retinopexy for 20 consecutive cases of rhegmatogenous retinal detachment, was previously reported. The collaborative experience of six surgeons using pneumatic retinopexy for 100 cases is now reported. This series includes cases with pseudophakia, aphakia, macular detachment, macular breaks, vitreous hemorrhage, trauma, and old detachments. Initially, 91% were reattached, but seven recurrences yielded a 6-month follow-up cure rate of 84% with pneumatic retinopexy. With subsequent scleral buckling, 98% were reattached. Postoperative complications included proliferative vitreoretinopathy (3%), macular pucker (3%), and new/missed retinal breaks (7%). A review of the literature disclosed postoperative new/missed retinal breaks in nine series: 21, 3, 8, 4, 9, 7, 5, 4, and 13%. There were no cases of glaucoma, cataract, subretinal gas, endophthalmitis, or extension of the detachment into a previously attached macula. Pneumatic retinopexy offers the advantages of reduced tissue trauma, fewer complications, no hospitalization, and less expense. The major disadvantage is the need for postoperative positioning.  相似文献

目的探讨巩膜扣带术治疗陈旧性孔源性视网膜脱离的临床疗效。方法对41例(46眼)巩膜扣带术治疗陈旧性孔源性视网膜脱离的临床资料,及其临床特征、手术方式、手术并发症及手术效果等进行回顾性分析。结果30眼采用巩膜环扎外加压术,12眼采用巩膜环扎术,4眼采用巩膜外加压术治疗;术中36眼放液,10眼未放液治疗。一次手术成功39眼(84.78%)。术中是否选择放液对一次成功的视网膜复位率无明显影响。最终随访44眼视网膜复位,治愈率达到95.65%。术后35眼(76.09%)视力较术前提高。结论巩膜环扎,外加压术联合术中放液或不放液是治疗陈旧性孔源性视网膜脱离的有效方法。  相似文献

目的 观察视网膜裂孔冷凝,巩膜环扎外加压手术不放液治疗脉络膜脱离型孔源性视网膜脱离的疗效。方法 对28眼脉络膜脱离型孔源性视网膜脱离进行裂孔冷凝,巩膜环扎,外加压,术中不引流视网膜下液,对其治疗结果进行分析。结果 28眼中一次成功25眼,视网膜下液在1~2天吸收为13眼,3天~1周内吸收为12眼,术后均有不同程度的视力提高。结论 在成功封闭裂孔后,脉脱型孔源性视网膜脱离视网膜下液可自行吸收,手术成功率高,可有效减少放液所带来的并发症。  相似文献

目的 观察巩膜外垫压术对激光光凝效果不佳局限性孔源性视网膜脱离(RRD)患者的临床疗效。设计回顾性病例系列。研究对象2017年1月至2021年1月空军军医大学西京医院眼科就诊的既往视网膜激光光凝术治疗局限性RRD效果不佳患者19例(19眼)。方法所有患者均行巩膜外垫压术治疗,术后补充视网膜激光光凝。随访6个月~2年,观察治疗效果。主要指标术后视网膜复位情况、视力及并发症情况。结果19眼中,垫压术后视网膜完全复位17眼(89.5%);2眼(10.5%)垫压嵴明确,但嵴上局部视网膜下存在积液,2周内视网膜下液完全吸收。2个月后2眼增生性玻璃体视网膜病变发生致视网膜脱离复发,均为首次激光后1周内行垫压术并术后2次补充激光光凝患者,行玻璃体切割硅油填充术后复位,后硅油取出稳定。末次随访时,视网膜复位19眼(100%),其中经一次巩膜外垫压稳定17眼(89.5%)。17眼(89.5%)视力无明显变化;2眼(10.5%)视力下降。结论巩膜外垫压术处置视网膜激光光凝术治疗局限性RRD效果不佳患者远期疗效肯定。局限性RRD不恰当使用激光光凝非但无效,还会有一些负面影响,视网膜激光光凝斑密集、重复激光、...  相似文献

Although scleral buckling yields good results in retinal reattachment surgery, we search for less laborious methods which could give the same high cure rate. Fifty-one consecutive patients with phakic, rhegmatogenous retinal detachment and breaks within 60 degrees of the upper retinal quadrants were treated by retino-cryopexy, drainage of subretinal fluid (under an operating microscope), and intravitreal injection of air (0.8 to 2.0 ml). Follow-up examinations (after 6 to 12 months) disclosed retinal reattachment in 44 of 51 eyes (86%). Cases with macular detachment showed good visual recovery, and 15 of 23 eyes (65%) achieved 0.5 (20/40) or more in visual acuity. Postoperatively, new retinal breaks occurred in 6%, proliferative vitreoretinopathy in 2%, and macular pucker also in 2%. Pneumatic retinopexy with air is an easy procedure, it is associated with few complications, and enables rapid postoperative recovery.  相似文献

目的:观察巩膜扣带术治疗陈旧性视网膜脱离的效果。方法:选取38例38眼陈旧性视网膜脱离患者,术中直视下定位、冷凝视网膜裂孔。针对不同病例36眼于裂孔处缝合硅海绵(其中19眼加缝硅胶环扎带),另2眼单纯行环扎术。26眼进行了视网膜下液引流术,7眼在手术前、后联合了激光光凝术。结果:其中33眼单次手术视网膜解剖复位,首次手术复位率87%,2眼再次手术后复位,手术最终解剖复位率92%。术中及术后无严重并发症发生。结论:通过个体化设计手术方案,巩膜扣带术治疗伴有视网膜下增生的陈旧性视网膜脱离可获得较高的手术治愈率。  相似文献

目的:探讨球形孔源性视网膜脱离的手术方法选择与手术预后,方法:回顾性分析1999年在我院手术的球形视网膜脱离患者161例161眼资料。最终手术采用环扎加压不放液53眼,环扎加压放液75眼,玻璃体手术33眼,平均随访3个月。结果:出院时手术复位159眼,复位率98.8%,随访期间复发2眼,最终手术成功157眼,成功率97.5%,术后视力有明显提高,结论:球形孔源性视网膜脱离由于其发病急,就诊快,只要手术方法选择适当,大部分还是可以取得成功,对于膜形成严重,裂孔多,大,靠后,或再手术病例,采用玻璃体手术,其术后解剖复位率和视功能恢复还是相当满意的。  相似文献

目的:观察巩膜扣带术治疗孔源性视网膜脱离的效果。方法:孔源性视网膜脱离患者94例96眼,术中直视下定位、冷凝视网膜裂孔。91眼行巩膜表面节段性外加压,其中37眼联合环扎术,另5眼单纯行环扎术。67眼进行了视网膜下液引流术,19眼在手术结束时行玻璃体腔气体充填。结果:90眼单次手术视网膜解剖复位,首次手术复位率94%,4眼再次手术后复位,手术最终解剖复位率98%。术中及术后无严重并发症发生。结论:巩膜扣带术治疗孔源性视网膜脱离安全有效。  相似文献

目的 对原发怍(孔源性)视网膜脱离行巩膜扣带术治疗并对其效果进行评价。方法 对118例118眼原发性视网膜脱离病人行巩膜扣带术治疗。术中直接检眼镜定位,放视网膜下液,巩膜外液氮冷凝,巩膜扣带或环扎术。结果 术后随访1周至半年,视网膜裂孔封闭、完全复位107眼(90.68%),视力提高90眼(76.27%)。结论 巩膜扣带术是治疗原发性视网膜脱离常用有效的方法。  相似文献

ObjectiveTo report the anatomic and visual results of primary pars plana vitrectomy (PPV) without scleral buckling to repair primary rhegmatogenous retinal detachments in pseudophakic eyes.DesignNonrandomized, prospective, comparative clinical trial.ParticipantsTwo hundred eighty-three consecutive patients (294 eyes) with pseudophakia, peripheral retinal tears, and new rhegmatogenous retinal detachments were treated according to the surgery protocol.InterventionPatients underwent PPV with fluid-gas exchange and endolaser to repair the retinal detachment. Two hundred sixty-four patients (275 eyes) were followed from 6 months to 6 years and 8 months with an average follow-up of 19 months.Main outcome measuresReattachment of the retina and visual outcome were compared to previously published studies.ResultsOf 97 eyes with a macula-attached rhegmatogenous retinal detachment, 88 eyes (91%) were reattached with a single operation, and of the 178 eyes with a macula-detached retinal detachment, 153 (86%) eyes were reattached with a single operation. In 241 (88%) of 275 eyes, the retina was reattached with a single operation, and in 265 (96%) of 275 eyes, the retina was ultimately reattached with subsequent operations. The median initial visual acuity was 20/300, and the median final visual acuity was 20/40. The rate of reattachment with one operation was similar for eyes with an anterior chamber intraocular lens (91%) and for eyes with a posterior chamber intraocular lens (88%). Refractive error measurements obtained in 81 eyes were essentially unchanged. The mean change in refractive error was −0.15 diopter. Seventeen eyes (6%) developed macular puckers requiring surgery, 46 eyes (17%) developed cystoid macular edema, and 6 eyes (2%) developed full-thickness macular holes.ConclusionPrimary PPV with fluid-gas exchange and laser is a safe, effective method to repair primary pseudophakic retinal detachments. The anatomic reattachment rate and the visual acuity obtained with this technique appear to be at least as good as those reported in the literature for scleral buckling, PPV with scleral buckling, and pneumatic retinopexy.  相似文献

PURPOSE: To evaluate the effectiveness of pneumatic retinopexy as an alternative technique for rhegmatogenous retinal detachment (RD) repair in teenagers. DESIGN: Retrospective, noncomparative, interventional case series. METHODS: We conducted a review of 19 patients (19 eyes) younger than 20 years of age who had undergone pneumatic retinopexy as the initial procedure for rhegmatogenous RD between 1995 and 2002. RESULTS: Fourteen boys and five girls (mean age of 17.1 years) were included in this study. Two eyes had a history of trauma. Seventeen eyes (89.5%) had myopia of > -3 diopters. The averaged refraction of the operated eyes was -6.11 diopters (range, -1.25 to -9.5 diopters). Macular detachment was found in 13 eyes (68.4%). Subretinal fibrosis was present in six eyes (31.6%). Pneumatic retinopexy resulted in reattachment in 16 eyes (84.2%) with one injection of gas. One patient required a second injection of gas. Two patients needed additional scleral buckling to attach the retina. Displaced subretinal fluid and delayed resorption of subretinal fluid were noted in four eyes. Mean follow-up period was 42.7 months (range, six to 75 months). CONCLUSIONS: Rhegmatogenous RD in teenagers can be treated with pneumatic retinopexy with an overall successful rate as in adults. More attention is required regarding the postoperative stage and follow-up period to ensure surgical success.  相似文献

目的:观察分析巩膜扣带术不放液治疗陈旧性视网膜脱离的临床效果。方法:回顾分析我科46例(46眼)陈旧性视网膜脱离患者经巩膜扣带术不放液进行治疗。病例资料,男39例,女7例。年龄9~18岁15例,18~35岁20例,35岁以上11例。提供病史2mo~2a。视网膜下方2个象限浅脱离35眼占76%,其视网膜小圆孔、多筛孔36眼占78%,所有患者脱离的视网膜下均伴有黄色机化条索,46眼均行外路手术不放视网膜下液,采用单纯巩膜外加压16眼(35%),环扎联合加压30眼(65%)。结果:随访1~12mo,46眼中有44眼1次手术达到视网膜解剖复位(96%),术后矫正视力0.1以上36眼。结论:巩膜扣带术不放液治疗好发于青少年陈旧性视网膜脱离,简单有效。  相似文献

Purpose: To compare the anatomical results of scleral buckling with and without retinopexy and to assess the effect of retinopexy on the scleral buckling outcome. Methods: This randomized clinical trial was performed on 55 patients. Twenty‐two eyes were treated with scleral buckling (segmental or encircling) with or without drainage of subretinal fluid without any type of retinopexy (group 1); 33 patients received transscleral retinal cryopexy around retinal break(s) in addition to the former procedure. The two groups were matched regarding age, sex, myopia, aphakia, stage of proliferative vitroretinopathy (PVR) and number, type and location of the break(s). Results: In the non‐retinopexy group, 19 patients (86%) had complete retinal reattachment and one patient had partial reattachment after 34–48 months of follow‐up. One patient did not develop attachment because of missed break out of the buckle, and one had no attachment at all because of PVR. Overall success rate was 91% (20 of 22) in this group. In the retinal cryopexy group, 26 patients (79%) had complete retinal reattachment and two had partial reattachment during 35–56 months of follow‐up. In two patients, no attachment was achieved because of missed break out of the buckle; three patients developed redetachment after 1 and 3 months because of PVR. Overall success rate was 85% (28 of 33). The anatomical results in these two groups were the same statistically. Conclusion: With the permanent scleral buckling technique, retinal cryopexy adds no benefit to the success rate of anatomical retinal reattachment.  相似文献

目的探讨应用外放液、玻璃体腔注射平衡液、冷凝、巩膜外加压联合玻璃体腔注射C3F8治疗上方球形视网膜脱离的疗效。方法30例(30只眼)上方球形视网膜脱离先经巩膜切开排放视网膜下液,玻璃体腔注射平衡液恢复眼内压,再行视网膜裂孔定位,巩膜外冷凝、外加压。对此时视网膜下液仍较多者,或者经放视网膜下液并玻璃体腔注射平衡液后视网膜下液迅速积聚者,或者形成放射状邹褶较重者,玻璃体腔注射C3F80.4~0.6ml。结果30只眼上方球形视网膜脱离经巩膜扣带术联合玻璃体腔注射C,F。治疗,随访3~50个月,平均24个月,一次手术视网膜复位27只眼(90%),3只眼术后7~10d下方视网膜形成新裂孔(10%),2只眼经玻璃体切割联合注气术视网膜复位,1只眼放弃手术。最终视网膜复位29只眼(96.7%)。结论应用外放液,玻璃体腔注射平衡液后,行巩膜扣带术联合玻璃体腔注射C3F8是治疗上方球形视网膜脱离的一种实用且有效的手术方法。  相似文献
