巴金森病与多系统萎缩患者的交感神经皮肤反应研究 总被引:4,自引:0,他引:4
目的:研究原发性巴金森病(IPD)与多系统萎缩(MSA)交感神经皮肤反应(SSR),以搪塞它们自主神经功能障碍的差异。方法:对31例IPD、17例MSA和83位正常人的SSR结果比较,分析PD组和MSA组SSR异常特征和与病程、自主神经症状的相关性。结果:MSA组SSR异常率(76%)显著高于IPD组(45%),以双侧异常多见。3年内病程的SSR异常率为73%,并与自主神经症状相关。IPD组SSR 相似文献

目的 探讨交感神经皮肤反应(SSR)对帕金森病自主神经功能障碍的诊断价值.方法 对解放军第一○二医院神经内科自2006年7月至2008年8月门诊或住院的47例帕金森病患者及与之相匹配的20例健康人进行SSR检测,并对其中有自主神经功能障碍患者与无自主神经功能障碍患者的结果进行对比分析. 结果帕金森病患者与健康人相比,上肢潜伏期明显延长(1.55±0.18 vs 1.42±0.29),波幅明显降低(1.87±0.26 vs 2.56±1.47);下肢潜伏期明显延长(2.13±0.16vs 2.04±0.27),波幅明显降低(0.49±0.21 vs 0.76±0.39),差异均有统计学意义(P<0.05).有自主神经功能障碍患者与无自主神经功能障碍患者相比,上肢潜伏期明显延长(1.56 ± 0.17 vs 1.53 ± 0.15),波幅明显降低(1.75±0.21 vs 1.89±0.33);下肢潜伏期明显延长(2.17 ± 0.18 vs 2.08±0.24),波幅明显降低(0.46±0.20 vs 0.51±0.17),差异均有统计学意义(P<0.05). 结论 SSR检测结果与患者临床表现相一致,其对帕金森病患者的自主神经功能障碍有诊断价值. 相似文献

目的探讨交感神经皮肤反应(SSR)在帕金森病(PD)患者自主神经损害中的评价作用。方法选用36例PD患者和30例健康对照者作为研究对象,行SSR检测,并对患者进行SCOPA-AUT评分及H-Y分级,评价SSR与PD自主神经损害的相关性。结果 PD患者SSR总异常率61.1%;H-Y评分0~1.5分者,SSR异常率为35.7%,H-Y评分2.0分者,SSR异常率77.3%;伴随自主神经症状的PD患者中(PD-AS),SSR异常率85.7%;不伴随自主神经症状者(PD-NAS),SSR异常率26.7%。结论 SSR可客观评价PD患者自主神经功能障碍,并检测PD自主神经的亚临床损害,且随患者运动症状加重,SSR异常越明显。 相似文献

目的 为多系统萎缩 (m ultiple system atrophy,MSA)与帕金森病 (Parkinson disease,PD)的鉴别诊断提供依据。方法 对 12名首发锥体外系症状的 MSA患者和 40名 PD患者的临床症状及主要辅助检查行比较分析。MSA诊断依据 Quinn(1994)提出的临床诊断标准。PD诊断参考英国帕金森病协会 1997年提出的诊断标准。结果 与 PD患者比较 ,MSA患者发病年龄较小 (5 4.3 2± 9.5 0 vs 61.5 6± 9.5 5 ,P<0 .0 5 ) ,主要以行动困难或僵硬为首发症状 (66.7% vs 2 5 % ,P<0 .0 5 ) ;少数以震颤为首发症状 (3 3 .3 % vs 75 % ,P<0 .0 5 ) ,也可以单侧症状发病(5 8.3 % vs 80 % ,P>0 .0 5 ) ,多数对多巴胺反应不良 (66.7% vs 2 0 % ,P<0 .0 5 ) ,植物神经症状、语言障碍比 PD患者显著多见 (分别为 83 .3 % vs 2 5 % ;5 0 % vs12 .5 % ;P<0 .0 5 )。辅助检查 :MRI:大部分 MSA患者有阳性表现 ,橄榄体、脑桥和小脑部位有萎缩 ,PD患者未发现特征性病变。 PET:3例 MSA患者均呈阳性表现。神经心理 :MSA患者未出现智能障碍的情况 (0 % ) ,少数 PD(10 % )患者出现智能障碍 ,但两者比较无统计学意义 (P>0 .0 5 )。结论 依据详细的病史和全面的神经系统查体 ,并结合临床辅助检查 ,可提高 MSA和 PD早期鉴别诊断的准确性 相似文献

目的探讨多系统萎缩和帕金森病患者执行功能障碍特点。方法采用简易智能状态检查量表(MMSE)和蒙特利尔认知评价量表(Mo CA),以及Stroop色词测验(SCWT)、数字符号转换测验(DSST)/图形符号转换测验(GSST)、画钟测验(CDT)和连线测验(TMT)评价34例多系统萎缩患者[以小脑共济失调为主要表现型(MSA-C型)21例、以帕金森病综合征为主要表现型(MSA-P型)13例]和18例原发性帕金森病患者的整体认知功能和执行功能。结果各组受试者Mo CA评分差异有统计学意义(P=0.019),其中PD组和MSA-C型组患者评分低于对照组(P=0.015,0.002)。各组受试者SCWT测验各部分评分(P=0.035,0.013,0.012,0.037)、DSST评分(P=0.000)、GSST评分(P=0.000)、TMT评分(P=0.035)差异均有统计学意义,其中,MSA-C型组和MSA-P型组患者SCWT-A(P=0.004,0.045)、SCWT-B(P=0.001,0.036)和SCWT-D(P=0.023,0.010)评分均高于对照组,PD组、MSA-C型组和MSA-P型组患者SCWT-C评分(P=0.005,0.014,0.003)、DSST评分(P=0.003,0.000,0.000)和GSST评分(P=0.001,0.000,0.000)均高于对照组,仅MSA-P型组患者TMT评分高于对照组(P=0.006)。结论多系统萎缩和帕金森病患者均存在不同程度的执行功能障碍,SCWT和DSST/GSST测验有助于评价此类患者的执行功能障碍。 相似文献

目的探讨交感神经皮肤反应(SSR)与事件相关电位(ERP)对卒中后抑郁患者的诊断价值。方法对55例卒中后抑郁患者和52例正常健康者分别进行了SSR和ERP测定,并将结果加以比较。结果卒中后抑郁组中SSR和ERP测定中的P300成分异常率分别为87.3%(48/55)和83.6%(46/55),两者异常吻合率为76.4%(42/55)。卒中后抑郁组SSR测定结果中,SSR潜伏期和波幅值较对照组延长和降低,其差异具有显著性(P<0.01),ERP测定中N2、P3波潜伏期和P3波幅较对照组分别延长和降低,存在显著性差异(P<0.01)。其中N2、P3波潜伏期与SSR测定中潜伏期以及波幅与波幅之间呈正相关(r=0.29~0.36,P<0.01),而两者潜伏期与波幅之间呈负相关(r=-0.32~-0.33,P<0.01)。结论交感神经皮肤反应和事件相关电位测定可作为卒中后抑郁患者的诊断指标应用于临床。 相似文献

采用放射免疫法检测8例帕金森病(PD)和13例具帕金森综合征的多系统萎缩(MSD)患者脑脊液(CSF)中β-内啡肽(β-EP)、亮脑啡肽(LEK)、强啡肽A_(1-13)(DynA_(1-13))含量。发现患者组CSF中β-EP和LEK含量显著高于对照组,DynA_(1-13)含量变化不显著。帕金森病和多系统萎缩两组间CSF中三种阿片肽含量相差不明显。提示且EP、LEK含量增高可能与PD和MSD患者所共有的肌张力增高和肌强直等锥体外系症状产生有关。 相似文献

目的探讨交感神经皮肤反应(SSR)对抑郁症患者的临床价值。方法对45例抑郁症患者治疗前后和43例正常健康者分别进行SSR测定和汉密尔顿抑郁量表(HAMD)评定,并加以比较。结果抑郁症组治疗前SSR测定的异常率为84.4%(38/45),而治疗后异常率为13.3%(6/45)。治疗前SSR测定波潜伏期及波幅值较治疗后及对照组分别延长和降低,差异有统计学意义(P<0.01);治疗后抑郁症组HAMD评定分值较治疗前降低,差异有统计学意义(P<0.01)。相关分析结果表明,抑郁症患者SSR波潜伏期与HAMD分值呈显著正相关(P<0.01),其波幅与HAMD分值呈显著负相关(P<0.01)。结论交感神经皮肤反应测定可作为判定抑郁症患者自主神经功能的参考指标应用于临床。 相似文献

目的 :研究脑梗死时交感神经皮肤反应 (SSR)的异常变化与梗死部位的关系。方法 :对 64例脑梗死患者测试SSR ,并比较大脑中动脉供血区梗死 (MCAI)与脑干梗死SSR异常的特点。结果 :脑梗死组SSR异常率 62 5 % ,MCAI单侧SSR异常率明显高于脑干梗死组 (P <0 0 5 ) ,双侧SSR异常脑干梗死组显著高于MCAI组 (P <0 0 1)。结论 :脑梗死后存在严重的交感神经功能障碍 相似文献
