PURPOSE: To describe the technique and results of stereotactically guided conformal radiotherapy (SCRT) in patients with craniopharyngioma after conservative surgery. METHODS AND MATERIALS: Thirty-nine patients with craniopharyngioma aged 3-68 years (median age 18 years) were treated with SCRT between June 1994 and January 2003. All patients were referred for radiotherapy after undergoing one or more surgical procedures. Treatment was delivered in 30-33 daily fractions over 6-6.5 weeks to a total dose of 50 Gy using 6 MV photons. Outcome was assessed prospectively. RESULTS: At a median follow-up of 40 months (range 3-88 months) the 3- and 5-year progression-free survival (PFS) was 97% and 92%, and 3- and 5-year survival 100%. Two patients required further debulking surgery for progressive disease 8 and 41 months after radiotherapy. Twelve patients (30%) had acute clinical deterioration due to cystic enlargement of craniopharyngioma following SCRT and required cyst aspiration. One patient with severe visual impairment prior to radiotherapy had visual deterioration following SCRT. Seven out of 10 patients with a normal pituitary function before SCRT had no endocrine deficits following treatment. CONCLUSION: SCRT as a high-precision technique of localized RT is suitable for the treatment of incompletely excised craniopharyngioma. The local control, toxicity and survival outcomes are comparable to results reported following conventional external beam RT. Longer follow-up is required to assess long-term efficacy and toxicity, particularly in terms of potential reduction in treatment related late toxicity.  相似文献

PURPOSE: When compared with radiosurgery, fractionated stereotactic radiotherapy for acoustic neuroma (AN) offers escalation of the tumor dose and potential sparing of auditory and facial nerve functions. METHODS AND MATERIALS: Between 1996 and 2001, 249 consecutive patients have received fractionated stereotactic radiotherapy for AN. One hundred twenty-five patients had follow-up >1 year and were the subject of this report. A noninvasive, repeat-fixation mask allowed simulation by way of spiral CT. Two distinct schedules for total dose and fractionation were used. For an AN <3.0 cm in diameter (volume 1.4 +/- 0.2 cm(3)), patients received 25 Gy given in 5 consecutive daily fractions of 5 Gy (111 patients), and for ANs >or=3.0 cm (volume 8.1 +/- 1.2 cm(3)), patients received 30 Gy given in 10 fractions of 3 Gy (14 patients). RESULTS: The percentage of decrease in tumor size was 12% +/- 2% (range 0-100%) vs. 13% +/- 3% (range 0-38%) for the 25 Gy vs. 30 Gy regimens, respectively. No patient had growth of the AN or developed facial weakness. Two patients developed transient decreases in facial sensation. The rates of hearing preservation were similar for the larger and smaller tumors. CONCLUSION: Fractionated stereotactic radiotherapy may preserve normal function and control both small and large ANs.  相似文献

目的:体部肿瘤立体定向放疗的研究。方法:1997年5月-1998年12月期间治疗32例共35个病灶,根据病变大小进行适形立体定向放疗,射野一般5-7个,治疗次数3-7次,治疗间隔1-3天,每次治疗周边剂量450-1000cGy,总量1890-3420Gy。结果:32例35个病灶根据CT复查肿瘤退缩情况确定CR为28.6%,PR为31.4%,总有效率为60%。1年以上生存率为58.3%。结论:立体定  相似文献

Fractionated stereotactic radiotherapy (FSRT) for optic glioma   总被引:1,自引:0,他引:1  
PURPOSE: To determine efficacy and toxicity of fractionated stereotactic radiotherapy (FSRT) in patients with optic glioma. METHODS AND MATERIALS: Ten patients suffering from optic glioma were treated by FSRT between December 1990 and December 1995 at the German Cancer Research Center (DKFZ) in Heidelberg. Eight patients were treated for progressive recurrent tumor following partial tumor resection and 2 patients were treated postoperatively. Dose distributions were calculated by a 3D treatment planning system (Voxelplan, Heidelberg). Patients were treated with a noninvasive repeatable stereotactic fixation system using a manually driven midsize multileaf collimator attached to a linear accelerator. We applied a median prescribed total dose to the isocenter of 52.4 Gy with a median daily fraction size of 1.8 Gy. RESULTS: All patients treated by definitive radiotherapy remained free from local tumor progression during the follow-up period (range 12-72 months) except the 1 patient treated for recurrence after previous radiotherapy. A complete remission was achieved in 3 patients with subsequent improvement of visual acuity. None of the patients with locally controlled tumor experienced any further impairment of vision. One patient developed new ACTH deficiency. No other clinically significant late effects attributable to radiotherapy were observed. CONCLUSION: FSRT permits treatment of optic glioma with excellent tumor control and without clinically relevant morbidity. Compared to conventional techniques there is the potential of sparing the pituitary gland in chiasmatic lesions.  相似文献

AimsTo assess visual outcome, tumour control and treatment-related morbidity in patients with optic nerve sheath meningiomas (ONSMs) treated with fractionated stereotactic radiotherapy (FSRT).Patients and methodsA retrospective analysis of 45 patients (13 men and 32 women, median age 46 years) with ONSMs (51 optic nerves involved) treated in a single institution between 1997 and 2010 was carried out. FSRT was delivered to a dose of 50 Gy in 30 or 33 fractions as primary treatment in 39 patients and after surgery in six patients.ResultsAt a median follow-up of 30 months (range 1–13 years), the tumour control in 41 evaluable patients (four were lost to follow-up) was 100% at 5 years with no subsequent local or distant recurrence. Of the 46 evaluable optic nerves treated, 41 had residual vision (38 with impaired vision) before radiotherapy and five were blind in one eye. There was no recovery of vision in any of the blind eyes. Of 41 optic nerves with residual vision, 13 had improvement, 24 remained stable and four deteriorated; two patients (4%) developed radiation retinopathy. One patient developed a central retinal artery occlusion in the untreated eye 10 years after treatment.ConclusionFSRT is highly effective at controlling the growth of ONSMs with improvement or stabilisation of visual deficit in 89% of the optic nerves retaining some vision, albeit with a small risk of radiation-induced retinopathy. The results support the use of FSRT as an effective approach in the management of ONSM. The lack of functional benefit in patients with severe visual impairment would argue for earlier institution of treatment before complete visual loss is established.  相似文献

Fractionated stereotactic radiotherapy of small intracranial malignancies   总被引:4,自引:0,他引:4  
Purpose: To retrospectively evaluate the effectiveness of fractionated stereotactic radiotherapy (FSRT) in patients with small intracranial malignancies.

Methods and Materials: From July 1991 to March 1997, 80 patients with a total of 121 brain or skull-base tumors were treated with FSRT alone, and were followed for periods ranging from 3 to 62 months (median 9.8). The majority of patients received 42 Gy in 7 fractions over 2.3 weeks, but in July 1993, protocols using smaller fraction doses were introduced for patients whose radiation-field diameters were larger than 3 cm or whose tumors were close to critical normal tissues.

Results: For 64 patients with metastatic brain tumors the overall median survival was 8.3 months and 1-year actuarial survival rate was 33%. Significant prognostic factors were: the presence of extracranial tumors, pre-treatment performance status, and the lung as a primary site. Patients without extracranial tumors prior to FSRT had a median survival of 21.2 months. For seven patients with high-grade glioma, 1-year actuarial local control rate was 75%, with a median survival of 10.3 months. For patients with skull-base tumors the local control was achieved in 6 of 6 patients (100%), with a median survival of 30.7 months. No one suffered from acute complications, but three patients, two of whom had undergone FSRT as the third course of radiotherapy, developed late radiation injuries.

Conclusion: Overall high local control and low morbidity rates suggest that FSRT is an effective and safe modality, even for those with a history of prior irradiation. However, patients with risk factors should be treated with smaller fraction doses.  相似文献

背景与目的:鼻咽癌复发、残留患者采用常规技术推量,适应症局限,而应用分次立体定向放疗(FS-RT)能得到合理的剂量分布,保护高危器官,提高疗效.本研究旨在评价鼻咽癌患者放射治疗后局部残存、区域复发进行立体定向放射治疗的临床价值和毒副作用.方法:41例鼻咽癌放射治疗后局部残存、区域复发的患者,应用立体定向放射治疗系统,6 MV X线,60%~90%为参考剂量曲线,肿瘤边缘分次剂量为4 Gy,间隔1~2d,总剂量为12~28 Gy(中位数16 Gy)/3~7次(中位数4次)放疗.靶体积为3.56~35.75 cm3,中位靶体积11.02 cm3.患者分为2组:残存组35例为首程适形调强根治性放射治疗后;复发组6例为首程根治性放射治疗后6个月以上区域复发,二程IMRT后;全部病例1个月内经鼻咽镜、CT或MRI检查发现仍有较大肿块残存.结果:FSRT治疗后3个月,CR63.47%(26/41),PR22.0%(9/41),SD14.6%(6/41),全组病例中位随访时间为28个月(3~41个月),残留组1、2、3年总生存率分别为100%、97.1%和91.4%,局控率分别为97.1%、88.6%和88.6%,无瘤生存率分别为94.3%、85.7%和85.7%.复发组1、2年总生存率分别为83.3%和66.7%,局控率分别为66.7%和50.0%.无瘤生存率分别为66.7%和50.0%.结论:FSRT对局部残留或复发鼻咽癌有较好的局部控制效果和较高的安全性,区域复发仍是治疗失败的主要原因.  相似文献

目的 评价分次立体定向放射治疗 (FSRT)对局部残留和复发鼻咽癌的初步疗效。方法 应用FSRT对 30例局部残留 (11例 )或复发 (19例 )鼻咽癌患者分 2个组进行治疗 ,其残留组和复发组中位处方剂量分别为 18Gy和 4 8Gy ,中位分次量分别为 6Gy和 8Gy ,每周 2次。结果 可评价例数共 2 9例 ,FSRT治疗后 3个月残留组的CR和PR各为 6 /11和 3/11,复发组各为 8/18和 8/18。全组病例随诊 3.4~ 37.9个月 (中位值 15个月 ) ,残留组 1和 2年总生存率、无瘤生存率、无局部复发生存率和无远地转移生存率分别为 77.8%和 6 6 .7%、5 6 .8%和 5 6 .8%、88.9%和 88.9%、75 .0 %和75 .0 % ,复发组相应为 6 9.8%和 5 9.8%、5 3.4 %和 4 4 .5 %、76 .6 %和 6 3.8%、70 .5 %和 5 8.7%。残留组和复发组生存患者卡氏评分≥ 90者分别占 8/8和 11/13。结论 FSRT对局部残留或复发鼻咽癌有较好的局部控制效果和较高的安全性 ,区域和远地转移是治疗失败的主要原因。  相似文献

目的:评价对鼻咽癌患者放射治疗后局部残存、区域复发进行分次立体定向放射治疗(FSRT)的临床价值和毒副作用. 方法:对 24例鼻咽癌放疗后局部残存、区域复发的患者,应用奥沃光子公司生产的立体定向放射治疗系统,10MV X射线,80%为参考剂量曲线,肿瘤边缘分次剂量为4-6Gy/次,2-3次/周,总剂量为8Gy-24Gy.患者分为2组:A组18例为首程根治性放射治疗后6个月内经鼻咽镜、CT或MRI检查仍有较大肿块残存;B组6例为首程根治性放射治疗后6个月以上区域复发者.结果: 24例患者CR为66.7%(16/24),PR 29.1%(7/24),NC 4.2%(1/24),CR PR 95.8%(23/24).鼻咽部大出血的发生率为16.7%(4/24). 结论:对鼻咽癌放射治疗后局部残存、区域复发,采用分次立体定向放射治疗推量治疗是一种有效的方法,鼻咽部大出血是其严重的并发症.  相似文献

目的 分析大分割体部立体定向放射治疗肝转移瘤的临床疗效和副反应.方法 回顾性分析2007年前10年内22例不可手术且化疗失败的肝转移瘤患者,共33个病灶接受大分割体部立体定向放射治疗.最常见的原发灶部位分别为乳腺12例和结直肠5例.单纯肝转移患者15例(68%),伴有其他部位转移患者7例(32%).中位病灶最大径2.0 cm(1.0~4.3 cm),中位靶体积3.68 cm~3(0.45~77.29 cm~3).接受一程治疗18例,二程治疗4例.中位总剂量40 Gy(30~60 Gy),中位分割次数为3次(2~6次).结果 中位随访时间13.9个月(5.0~36.4个月),随访率100%.中位生存时间为13.9个月.1、2年局部控制率分别为100%、90%.1、2年总生存率分别为73%、49%,1、2年无疾病进展生存率分别为94%、28%.所有患者均能耐受,无3级以上相关并发症.结论 大分割体部立体定向放射治疗对肝转移瘤是安全有效的,局部控制率高且并发症可接受.  相似文献
