轻度认知损害(Mild Cognitive Impairment,MCI)是阿尔茨海默病(Alzheimer Disease,AD)的前驱早期阶段,是介于AD和正常衰老之间的一种认知功能损害状态[1]。Petersen等认为MCI具有以下特征:(1)经常为忘事烦恼;(2)与受教育程度、年龄不相称的记忆损害;(3)保持一般的认知功能;(4)日常生活能自理;(5)没有痴呆。相关研究显示,MCI在老年人中的发生率为5.3%,每年有10%~15%的MCI患者转化为AD,而正常老年人群每年转化为AD仅为1%~2%。  相似文献

目的探讨轻度认知功能障碍老年人(MCI)和阿尔茨海默病(AD)在事件相关电位(ERP)P300检测中的不同表现.方法收集符合ICD-10诊断标准的36例MCI患者和30例AD,并以45例健康老人作对照组(NC).使用美国仪器以及"听觉靶-非靶刺激序列"为诱发事件,完成P300检测.结果 (1)3组和NC组在靶潜伏期Fz脑区N2以及在靶波幅Fz脑区P2P3和非靶波幅Fz脑区P2上均有显著差异(P<0.01).(2)AD主成分N2表现为延迟,与NC组和MCI组有极显著性差异(P<0.01).(3)MCI组和AD组靶波幅P3和非靶波幅P2均见降低,与NC组比较也有显著性差异(P<0.05~<0.01).结论提示作为反映MCI和AD认知功能障碍的客观生理指标,P300有可能作为MCI和AD辅助诊断的一个脑电生理学标志.P300检查可作为老年精神科的必查项目.  相似文献

目的 探讨躯体形式障碍患者认知功能的事件相关电位P300特征.方法 随机将年龄在18~65岁之间符合CCMD-3躯体形式障碍诊断标准患者30例作为实验组,选择30例性别年龄相匹配的健康者作为对照组.各组分别予听觉P300检测,比较P300潜伏期及波幅的差异.结果 实验组PZ点潜伏期[(308±21)ms],均比对照组[(298±22)ms]延长,实验组FZ、CZ、PZ、OZ点[分别为(2.4±1.6)μV、(2.6±2.3)μV、(2.6±3.3)μV、(2.4±1.9)μV]波幅比对照组[分别为(3.9±2.1)μV、(3.8±1.9)μV、(4.2±3.4)μV、(3.7±2.0)μV]降低(P<0.05及0.01).结论 躯体形式障碍患者存在认知功能障碍.  相似文献

目的观察焦虑症患者事件相关电位P300的变化特点.方法对22例焦虑症患者和22例健康成年人,应用Nicolet Bravo脑诱发电位做事件相关电位P300检测.结果与正常组相比,焦虑症组靶N2潜伏期延长(P<0.05),靶P2波幅低(P<0.01),靶P3潜伏期长且波幅低(P<0.01),非靶P2波幅低(P<0.01).P300各指标在男女性别之间的差异无统计学意义(P>0.05).结论焦虑症患者P300有多项指标变化,值得进一步随访.  相似文献

目的 探讨甲基苯丙胺依赖者认知功能状况及事件相关电位P300特征.方法 随机将年龄在18 ~ 65岁之间符合CCMD-3甲基苯丙胺依赖诊断标准患者30例作为实验组,选择30例健康者作为对照组.各组分别予听觉P300检测和蒙特利尔认知评估量表筛查实验,将两组结果加以分析、比较.结果 与对照组相比,实验组Cz点P2、N2、P3及Pz点N1、P2、N2、P3潜伏期明显延长(P<0.05或P<0.01);实验组Cz、Pz点波幅明显降低(P<0.05或P<0.01);实验组蒙特利尔认知评估量表7个因子分及总分值均低于对照组,差异有统计学意义(P<0.05或P<0.01);Pz点潜伏期及波幅与蒙特利尔认知评估量表中7个因子分具有相关性:为Pz点N1、P、N2 、P3潜伏期与相关因子及总分呈负相关(P <0.05或P<0.01);PZ点波幅则与各因子分及总分呈正相关(P <0.05或P<0.01).结论 甲基苯丙胺依赖者存在认知功能障碍.P300电位可作为评价甲基苯丙胺依赖者认知状况的客观指标之一应用于临床.  相似文献

帕金森病(Parkinson disease,PD)是临床常见的神经系统退行性疾病。认知功能障碍严重影响PD患者的生活质量,增加家庭负担。目前临床上尚无相关的生物标志物协助PD认知功能障碍早期诊断。事件相关电位P300是一项灵敏的电生理指标,可以检测PD患者疾病初期隐匿的认知能力下降。本文就P300在PD患者认知障碍中的诊断价值及临床应用进行综述。  相似文献

目的 探讨额叶肿瘤患者认知功能的损害和事件相关电位P300有关参数的变化特点,了解额叶在认知功能及P300产生中的作用.方法 对31例额叶肿瘤患者(左侧15例、右侧16例)及30例健康对照者分别进行Stroop字色干扰等多项认知神经心理学测试和P300检查.结果 与健康对照组相比,额叶肿瘤组认知功能各项测试指标评分均显著降低(P<0.05),P300的N2、P3波潜伏期显著延长(P<0.05),P3波幅显著降低(P<0.05).进一步研究发现,右额叶肿瘤组患者的各项认知测试指标评分均显著低于健康对照组(P<0.05),而左额叶肿瘤组只有词语流畅性指标评分显著低于健康对照组(P<0.05);与健康对照组相比,左、右额叶肿瘤组P300的N2、P3波潜伏期均显著延长(P<0.05),P3波幅均显著降低(P<0.05);左、右额叶肿瘤组患者之间的P300比较则无统计学差异(p>0.05).结论 额叶肿瘤患者认知功能有明显损害,右侧肿瘤患者的认知功能损害更严重:额叶可能与P300发生或传导有关,且左右额叶无明显的差异.  相似文献

目的:探讨躯体形式障碍患者的认知功能状况及其与P300的关系. 方法:对120例躯体形式障碍患者及60例正常对照组分别进行P300检测和瑞文标准推理测验、数字广度与数字符号测验,并对结果进行比较. 结果:躯体形式障碍组的瑞文推理、数字广度、数字符号分数均低于对照组,差异有统计学意义(P<0.01);N1、P2、P3潜伏期较对照组延长,N1N2、N2P2、P2P3波幅低于对照组,差异有统计学意义(P<0.01或P<0.05);瑞文推理分数与N1潜伏期正相关;数字广度分数与P2潜伏期、N2潜伏期正相关,与N1N2波幅负相关;数字符号分数与N1N2波幅、P2P3波幅正相关,与N2P3波幅负相关. 结论:躯体形式障碍患者的认知功能缺陷范围较广,不同的P300成分可能反映躯体形式障碍不同类型的认知功能损害.  相似文献

目的 探讨阿尔茨海默病 (AD)P3 0 0 的特点以及P3 0 0 与简易智力状况检查 (MMSE)评分的相关性。方法 对 32例AD患者和 30名健康老年人应用NicoletBravo脑诱发电位仪进行事件相关电位P3 0 0 检测 ;同时用MMSE进行认知功能评估。结果  (1)AD组MMSE评分 [(16 5 2± 2 17)分 ]低于正常对照组 [(2 6 5 7± 1 4 3)分 ,P <0 0 1]。 (2 )AD组与正常对照组相比 ,靶刺激中的P2 、N2 、P3 潜伏期长 ,P2 、P3 波幅低 ;非靶刺激P2 波幅低 (P <0 0 5~ 0 0 1) ;AD组Cz、Pz、Fz各点间的P3 0 0 指标相比 ,差异均无显著性 (P >0 0 5 )。 (3)AD组P3 0 0 靶刺激中的P2 、P3 波潜伏期与MMSE分值呈负相关 (P <0 0 5~ 0 0 1)。结论 AD患者的P3 0 0 有多指标变异 ,且与其认知功能缺损的程度有关。  相似文献

目的 探讨长期住院的男性精神分裂症患者认知功能的变化与事件相关电位P300的关系.方法入组长期住院男性精神分裂症患者82例和健康对照52人.采用美国Nicolet Viking Quste诱发电位仪记录事件相关电位P300的潜伏期和波幅.采用动物命名测验、范畴流畅性测验、数字划消测验、连线测验(TMT-A、TMT-B)、Stroop测验(单词、颜色、色词干扰测验)、木块图测验、WMS-Ⅲ空间广度测验评估认知功能.结果患者组各项认知功能测验成绩与对照组之间差异均有统计学意义(P<0.01).患者组事件相关电位P300潜伏期较对照组明显延长(t=22.990,P<0.01),波幅较对照组明显降低(t=-9.699,P<0.01).患者组事件相关电位P300潜伏期与数字划消测验及TMT-A呈正相关(r=0.481,P<0.01;r=0.245,P<0.05).事件相关电位P300波幅与数字划消测验呈负相关(r=-0.338,P<0.01).结论长期住院男性精神分裂症患者虽处于稳定期,但仍存在认知功能的损害.事件相关电位P300的潜伏期和波幅可能是精神分裂症认知功能的电生理指标,并与患者的认知量表评估结果之间存在相关性.  相似文献

目的 研究血管性认知功能障碍 (VCI)患者的神经心理学、影像学及事件相关电位的特点并探讨其意义。方法 对 78例VCI患者及 4 9名正常人进行中文版简易智能状态检查 (MMSE)量表、头颅CT、事件相关电位检查。结果  (1)VCI组MMSE总分 (2 4 .35± 3.2 8)分 ,与正常组 (2 5 .0 4± 5 .0 3)分比较差异无显著性(P >0 .0 5 ) ;地点定向、时间定向、短程记忆、计算能力、语言表达、言语复述、图形描画 7项亚项的评分均明显低于正常组 (均P <0 .0 5 )。 (2 )VCI患者脑萎缩明显 ,脑梗死灶位于额、颞叶者及多发性梗死患者MMSE评分降低更显著 (均P <0 .0 5 )。 (3)VCI患者P3 0 0 潜伏期与正常组相比显著延长 (P <0 .0 5 ) ,P3 0 0 潜伏期与MMSE评分呈负相关 (r=- 0 .6 1,P <0 0 5 )。结论 MMSE量表中认知功能亚项的测评 ,有利于早期发现VCI患者的认知功能障碍 ,脑萎缩及脑梗死灶的部位和数目与VCI的程度有关 ;P3 0 0 潜伏期的检测对于早期发现VCI患者的认知障碍及其程度有重要价值。  相似文献

Intracranial recordings of long-latency, event-related potentials were obtained from paralyzed, artificially respirated cats. A modified oddball paradigm was employed in which cats were presented with a randomized series of two tones, a 'frequent' 4 kHz stimulus and a 'rare' 1 kHz stimulus. A tail shock was administered 700 ms after onset of the rare tone. Under these circumstances the stimulus elicited a positive component at the vertex similar to the human P300. Intracranial potentials associated with the rare tone usually manifested components of greater amplitude than did potentials associated with the frequent tone. A positive component occurring in latency between 200 and 350 ms only accompanied the presentation of the rare stimulus. The P300 component, which was positive at the dura, appeared as a negative component within a few millimeters of the surface over a wide area of the marginal and suprasylvian gyri. Changing the probability of the rare stimulus resulted in a reduction in the amplitudes of both the intracranial negative component and the P300 recorded at the skull. Components of large amplitude associated with the rare stimulus were obtained from the region of the hippocampus. These components reversed polarity, sometimes more than once, as the electrode was advanced. Substantial latency differences were often observed between the P300 recorded at the skull and P300-like intracranial components associated with the rare stimulus. These results suggest that the cortices of the marginal and suprasylvian gyri and the hippocampal region contribute to the generation of the cat P300.  相似文献

目的:探讨维汉两民族精神分裂症、抑郁症、焦虑症患者事件相关电位视觉P300的临床应用价值及特点。方法:141例精神分裂症患者(精神分裂症组)、147例抑郁症患者(抑郁症组)、146例焦虑症患者(焦虑症组)和120名健康志愿者(对照组),采用BrainMaster脑诱发电位系统进行视觉P300检测。结果:精神分裂症组P300各指标与对照组比较差异均有统计学意义(P〈0.05);抑郁症组除P2潜伏期外、焦虑症组除N1潜伏期外,其余各指标与对照组差异均有统计学意义(P〈0.05或P〈0.01)。维汉两民族P300各指标间差异均无统计学意义(P〉0.05)。结论:视觉P300能比较客观地反映精神疾病患者的认知功能,亦可用于维族人群。  相似文献

目的探讨酒依赖患者脑干听觉反应(ABR)及P300的特点.方法以click短声、听觉靶刺激和非靶刺激为诱发事件,检测26例男性酒依赖患者及31名男性正常人的ABR和事件相关电位P300.结果 (1)ABR与对照组比较,酒依赖组波Ⅱ~波Ⅶ的绝对潜伏期长,波Ⅱ~波Ⅴ的绝对波幅低,差异均有显著性和非常显著性(P<0.05或P<0.01);(2)P300在靶刺激中,酒依赖组的P2和P3潜伏期[分别为(187±26)ms和(319±27)ms]长于对照组[分别为(171±21)ms和(302±16)ms],P2和P3波幅[分别为(3.9±2.3)μV和(3.5±3.1)μV]低于对照组[分别为(6.0±2.1)μV和(8.2±4.1)μV];在非靶刺激中,酒依赖组的N1潜伏期[(108±14)ms]长于对照组[(98±12)ms];差异有显著性和非常显著性(P<0.05或P<0.01).结论长期大量饮酒不仅损害脑干功能,同时损害皮层认知功能.  相似文献

Objectives

The study aimed to investigate and compare the effect of cognitive reserve (CR) on brain activation in healthy controls (HC) and amnestic mild cognitive impairment (aMCI) patients during 0-back and 1-back tasks measured by event-related potential (ERP).

Methods

The study recorded 85 subjects (39 aMCI patients and 46 their matched controls) with a 64-channel electroencephalogram (EEG). Subjects performed 0- and 1-back tasks.

Results

Compared to HC, aMCI patients showed reduced accuracy, delayed mean correct response time (RT) and decreased P300 amplitude at central-parietal and parietal electrodes. A mediation analysis indicated that higher CR reduced neural inefficiency, which might be associated with better task performance in HC. However, no correlation was detected between CR and neural inefficiency in aMCI patients, whereas higher CR was still related to enhanced accuracy and prolonged RT in aMCI patients.

Conclusions

The present study reported that higher CR could contribute to better task performance via down-regulating neural inefficiency in HC. In addition, higher CR might modulate attention processes in aMCI via a way distinct from that in HC, and eventually result in better task performance.

Significance

The study provided evidence for that improving CR might lower cognitive impairment of healthy elderly and aMCI patients.  相似文献

目的:探讨与颈动脉狭窄(CAS)相关的认知功能障碍。方法:检测40例CAS患者听觉事件相关电位(P300),并与38例对照组对比。结果:CAS组的P300潜伏期异常率为57.5%。CAS组与对照组的P300相比,潜伏期显著延长,波幅降低,且与颈动脉狭窄程度相关。结论:CAS患者存在认知功能障碍,而且可用P300动态观察。  相似文献

OBJECTIVE: Various degenerative cerebral diseases and even depression may cause abnormalities of the cognitive event related potentials (ERPs). We conducted the present study to elucidate the effects of ischemic stroke on the P300 ERP component. MATERIAL AND METHODS: We recorded the P300 wave using an auditory oddball paradigm in 38 consecutive brain infarct patients with mild neurological deficits at 3 and 12 months post-stroke, and in 29 healthy control subjects. RESULTS: Brain infarction slightly prolonged the P300 latency, and the observed delay was related to the presence and degree of post-stroke depression evaluated with the Zung Depression Scale and the DSM-III criteria. Infarction did not affect the P300 amplitude or its distribution on the scalp. The results of the patients with hemispheric brain infarction and those of the patients with brainstem infarction were similar, and also the values of the patients with the left- and right-sided lesions. The normal physiological correlation between subject age and the P300 latency was absent at 3 months post-stroke, but was present at 12 months post-stroke. CONCLUSION: Brain infarction delays the P300 ERP and temporarily distorts its age-related physiology. The increase of the P300 latency seems to be associated with the post-stroke depression.  相似文献

Aims. To investigate the role of centrally recorded P300 in patients suffering from mesial temporal sclerosis‐temporal lobe epilepsy (MTS‐TLE). Methods. Sixteen patients (3 men and 13 women; median age: 32.5 years old) suffering from TLE with MTS and 43 healthy controls (12 men and 31 women; median age: 35 years old) participated in the study. P300 was elicited using an auditory two‐stimulus oddball paradigm. In order to address the aim of the study, we adopted two statistical approaches; hierarchical linear regression analyses and ROC curves. Results. After adjusting for age, MTS patients had a mean reduction of P300 amplitude by 6.93 μV and a mean increase of P300 latency by 38.78 ms, compared to controls. Age and MTS‐TLE status accounted for 32 and 16% of the variance of latency and amplitude, respectively. Diagnostic analyses to detect MTS‐TLE status revealed a sensitivity and specificity of 88 and 65% for amplitude and 81 and 70% for latency, respectively. No association between duration of disease and P300 characteristics were found. Conclusions. This study, along with other studies, contributes to our understanding and clinical significance of centrally recorded P300s in MTS‐TLE patients. Future studies should focus on the association of these P300s with cognition in such patients.  相似文献
