[目的]探讨空心螺钉内固定治疗儿童胫骨远端骨骺骨折的临床疗效。[方法]回顾性分析2018年12月—2021年12月采用闭合或切开复位后空心螺钉内固定儿童胫骨远端骨骺骨折70例患者的临床资料,评价临床及影像效果。[结果] 70例患者均顺利完成手术治疗,术中未出现血管、神经损伤等并发症。平均随访时间(8.4±2.1)个月,与术前相比,术后3个月和末次随访时VAS评分均显著下降[(5.9±1.0)分,(1.3±0.3)分,(1.2±0.2)分,P<0.001],而AOFAS评分[(34.8±7.9)分,(82.7±8.9)分,(94.1±4.1)分, P<0.001]、踝关节背伸-跖屈ROM [(36.5±4.3)°,(63.8±10.4)°,(74.9±11.5)°, P<0.001]和内翻-外翻ROM [(30.7±6.9)°,(54.8±8.9)°,(65.8±9.0)°, P<0.001]。影像方面,与术前相比,术后骨折力线对合显著改善(P<0.05),mLDTA角和a ADTA角显著增加(P<0.05)。所有患者骨折均在10周左右骨性愈合。[结论... 相似文献

胫骨结节骨折是临床上少见的骨折 ,青少年多因间接暴力引起骨骺撕脱性骨折 ,成人的胫骨结节骨折则为直接暴力所致。自 1998年 2月~ 2 0 0 0年 5月 ,我们采用螺钉加钢丝固定 ,治疗 4例 ,取得良好的效果 ,现报告如下。1 临床资料4例全为男性 ,其中年龄 16~ 17岁者 3例 ,1例为 34岁。损伤原因 :起跳损伤 2例 ,快跑摔倒 1例 ,34岁者为膝部砸伤。按Ogden分型 ,ⅠB型 2例 ,ⅡA型 1例 ,ⅢA型 1例。X线片可见胫骨结节骨折 ,骨块向上移位 ,髌骨上移。2 治疗方法2 1 手术方法 膝前侧正中切口。显露髌骨、髌腱、胫骨结节及两侧股四头肌… 相似文献

目的 探讨儿童胫骨结节骨骺骨折的特点、治疗及预后.方法 1995年1月至2007年12月,胫骨结节骨骺骨折患儿28例,其中资料完整者25例,男23例,女2例;年龄12~16岁.平均13.5岁;左侧10例,右侧15例,均为单侧受累.损伤方式:打篮球时受伤10例,跳跃时受伤7例,踢足球时受伤8例.根据Ogden分型标准,1A型1例,1B型4例,2A型5例,2B型7例,3A型4例,3B型4例.除1例1A型患儿骨折采用闭合复位长腿石膏前后托固定外,余24例均采用切开复位拉力螺钉内固定.结果 所有患儿均获得随访,随访时间14个月~7年,平均43个月.术后3个月患儿膝关节活动范围0°~140°,术后6个月恢复伤前运动水平.按照Mosier临床评价标准,结果均为优.所有病例手术切口均一期愈合,均未发生胫前间室综合征.骨折愈合时间2~5个月,平均3个月,伸膝装置序列正常,无一例患儿发生切口感染、内固定物失效或肢体畸形.结论 儿童胫骨结节骨骺骨折是一种不常见的特殊类型的骺损伤.在伸膝装置突然加速或减速的运动中,当髌韧带的牵拉力超过胫骨结节骨骺、周围软骨膜以及邻近骨膜的结合力时,就会发生胫骨结节的骨骺骨折.该骨折多见于13~16岁的男性患儿,单侧发病为主;可采用拉力螺钉内固定治疗,术后患儿膝关节功能正常. 相似文献

[目的]探讨切开复位可吸收钉固定治疗儿童胫骨远端骨骺损伤的疗效。[方法]回顾分析本院2006年1月~2011年12月采取切开复位可吸收螺钉固定方法治疗的胫骨远端骨骺损伤患儿37例,其中15例获得随访;手术年龄5~15岁,平均12岁,其中男11例,女4例;左侧11例,右侧4例。按Salter-Harris骨骺损伤分型:Ⅱ型7例,Ⅲ型4例(包括1例Tillaux骨折),Ⅳ型2例,三平面骨折2例。所有患儿随访14~75个月,平均36.1个月。手术在C型臂X线机透视下进行,切开复位达到解剖复位后可吸收螺钉固定。术后石膏托固定6~8周。以改良Weber评估标准评价踝关节功能及愈合情况;拍摄双下肢全长正位、双踝关节正侧位X线片,评估切开复位可吸收钉治疗儿童胫骨远端骨骺损伤的疗效。[结果]本研究所有患侧与健侧下肢影像学分析结果显示:下肢机械轴与水平线成角,胫骨长度健侧与患侧比较差异有统计学意义(P<0.05)。而正侧位踝穴角患侧与健侧比较差异无统计学意义(P>0.05)。对获得随访的15例按改良Weber评分,优13例,良2例。所有患儿踝关节活动均不受限,均能参加正常的体育活动。X线片检查所有病例均骨性愈合,其中2例发生骨骺早闭,有骨桥形成,踝关节内翻畸形,肢体短缩并伴有下肢机械轴线的偏移。[结论]可吸收钉对胫骨远端骨骺损伤的固定可靠,对骨折愈合和踝关节功能恢复而言,具有良好的效果。 相似文献

One type 1A fracture was treated nonoperatively with east immobilization, and other 24 fractures with open reduction and internal fixation. Results All of 25 cases were followed from 14 months to 7 years (average 43 months). The range of motion of knee joint was 0°-140° 3 months after operation, and all of them recovered to pre-injury levels of activity by 6 months after operation. According to Mosier clinical assessment system, the final outcome was evaluated as excellent in all. Wound healed primary, no compartment syndrome was found. The time of bony union ranged from 2 to 5 months, with the mean time of 3 months. No complications of infection, ex-tremities deformity and inner fixation failure were found. Conclusion The tuberosity avulsion fracture in children is an uncommon and special type of epiphyseal injury. These fractures occur in the narrow range of 13 to 16 years of age. The majority of patients are males, and most of them are unilateral involved. 目的 探讨应用可吸收螺钉治疗Watson-DoneⅡ、Ⅲ型青少年胫骨结节撕脱骨折的疗效.方法 对8例青少年胫骨结节撕脱骨折采用切开复位可吸收螺钉固定,定期随访,按照HSS膝关节评分评估膝关节功能.结果 术后随访6~18个月,平均8.3个月.骨折均获得一期愈合,平均愈合时间8周.膝关节屈伸活动范围平均123°,HSS评分均为优.结论 切开复位可吸收螺钉固定治疗Watson-Jove Ⅱ、Ⅲ胫骨结节撕脱骨折,无须二次手术取出内固定,并获得良好的疗效. 相似文献
