Objective: Recently, a new traditional Chinese medicine differentiation theory "Syndrome Element(SE)" has been raised. In this study, the main syndrome element types and their correlations with the results of coronary angiography (CAG) in patients with coronary heart disease (CHD) were investigated. Methods: Epidemiology cross-sectional study method was employed and 324 patients with CHD were enrolled, and their syndrome element types as well as the CAG results were analyzed. The correlations among syndrome element types, Gensini score, and the number of abnormal branches were also analyzed based on the distribution characteristics of syndrome element and coronary angiography results in the 324 cases. Results:According to their occurrence frequency in 324 CHD patients, the top eight major heart syndrome elements were Xin (心) blood stasis (85.8%), Xin qi deficiency (79.6%), Xin heat blockage (41.1%), Xin phlegm with turbid fluid (38.0%), Xin qi stagnation (24.7%), Xin yang deficiency (18.9%), Xin yin deficiency (17.5%) and Xin cold coagulation (4.4%), respectively, which suggested that Xin blood stasis and Xin qi deficiency were the two most common syndrome elements. Also, as coronary artery Gensini score increased, the changing trend of the syndrome element was "Xin yang deficiency with blood stasis" to "Xin phlegm obstruction with heat blockage" to "Xin yin deficiency with blood stasis" to "Xin qi deficiency with blood stasis" to "Xin cold coagulation with phlegm and turbid fluid, "Xin cold coagulation with blood stasis" to "Xin deficiency of qi, yin and yang". As the number of abnormal branches increased, the syndrome element changing trend was simultaneous occurrence of cold and heat syndrome" to "Xin qi and yang deficiency with blood stasis" to "Xin retention of phlegm with turbid fluid" to "Xin cold coagulation in the heart meridian", "Xin deficiency of both qi and yin". In order to approach the effects of Shenmai Injection on SOD activity and MDA level in the senilepatients with coronary heart disease,48 cases,who had stenosis of over 70% in more than one branchesof the coronary arteries,were assigned randomly into a treatment group (given Shenmai Injection plusthe routine treatment) and a control group (given the routine treatment only).The superoxide dismutase(SOD) activity and malondialdehyde (MDA) level were determined before treatment and at the end of a3-week treatment.The results showed that in the treatment group,the SOD activity was significantlyincreased (P<0.05) and the MDA level markedly decreased (P<0.01) in the treatment group.It can beconcluded that Shenmai Injection may enhance the antioxidant ability of the senile patients withcoronary heart disease.  相似文献

Objective: To explore the fundamental theory, methods and data of the quality of life (QOL) of soldiers and its influence factors in the first response army. Methods: Totally 215 soldiers in the first response army of a military area in China were inquired by using self-regulating questionnaire and WHOQOL-BREF (Chinese Edition). Resalts: WHOQOL-BREF had good acceptability, reliability and validity in the first response army. QOL of soldiers in the first response army was middling. The influencing factors of QOL of the soldiers in first army includes self-report health, different areas where the army men come from, different arms of services, whether only child in family. And the difference was statistically significant. Conclusion: WHOQOL-BREF is fit for evaluating the quality of life of soldiers in the first response army. There are differences of the quality of life among the soldiers in the first response army. So we should take appropriate measures to improve the soldiers' quality of life.  相似文献

A clinical study on acupuncture at point Neiguan(PC 6)was conducted on the basis of successfultreatment of patients with angina pectoris and acute myocardial infarct.The effective rate(91.3%)wasfound to be far superior to isosorbide dinitrate and nifedipine(P<0.01).The experimental studies in animalmodels demonstrated that electro-acupuncture could decrease the elevated ST segments in ECGs andreduce the infarct areas induced by coronary ligation.  相似文献

Objective To investigate the variation of sex hormone and its receptor level in elderly male patients with coronary heart disease (CHD) and to evaluate the correlations between CHD and sex hormone as well as sex hormone receptor. Methods Altogether 139 male CHD patients (CHD group) aged 60-92 years and 400 healthy men (control group) aged 60-90 years were included in this cross sectional study. The plasma concentrations of dehydroepiandrosterone sulfate (DHEAS),total testosterone (TT),free testosterone (FT),estradiol (E2),sex hormone binding globulin (SHBG),luteinizing hormone (LH),and follicle-stimulating hormone (FSH) were measured. The androgen receptor (AR) was tested by flow cytometry. Correlations between CHD and levels of sex hormones and AR were analyzed. Results Compared with the control group,the levels of DHEAS,TT,FT,SHBG,and the fluorescence intensity of AR in the CHD group significantly reduced (P0.05),while the levels of FSH and E2 significantly increased (P0.01). Age was negatively correlated with TT (r=-0.28,P=0.00) and FT (r=-0.17,P=0.01),while it was positively correlated with SHBG (r=0.14,P=0.04) and E2 (r=0.33,P=0.00). AR fluorescence intensity was negatively correlated with systolic blood pressure (r=-0.12,P=0.01). Binary logistic regression analysis showed that TT,SHBG,and AR were all negatively correlated with CHD (P0.05). Conclusions Elderly male patients with CHD are found to have low levels of DHEAS,TT,FT,SHBG,and AR,while high concentrations of E2 and FSH. Low levels of TT and SHBG may be the potential risk factors of CHD in elderly men.  相似文献

OBJECTIVE: Breast cancer is an important disease and one where healthcare services have the potential to improve the quality of life. The aim of this study was to assess the relationship between unmet needs and the quality of life (QoL) of breast cancer patients. SUBJECTS AND METHODS: This study comprised one hundred breast cancer patients who attended the Oncology Outpatient Clinic in Cukurova University between April and June 2001. Three questionnaires were used: questionnaires for demographic details, unmet needs and Functional Assessment of Cancer Therapy-General Questionnaire QoL. RESULTS: There was significant correlation between QoL score and unmet needs score (r = 0.263, p = 0.008) and between QoL score and Napier logarithm (monthly income) (r = 0.257, p = 0.011). There was a significant difference in QoL score by occupation (p = 0.007). CONCLUSION: It is important to assess patients psychologically so as to make interventions about their psychosocial needs which affect QoL. This will require the re-evaluation of doctor-patient communication and interventions for improvement of this relationship.  相似文献

Objectives:

To investigate levels of quality of life (QOL) among thalassemia patients at the Hereditary Blood Disorders Center in Al-Madinah Al-Munawarah, Kingdom of Saudi Arabia.

Methods:

A cross-sectional study was performed on 43 transfusion dependent thalassemia patients compared with 43 normal subjects, as a control, using the World Health Organization Quality Of Life - Brief questionnaire between May 2012 and September 2012 at the Hereditary Blood Disorders Center, Maternity and Children Hospital in Al-Madinah Al-Munawarah, Kingdom of Saudi Arabia.

Results:

Forty-three thalassemia patients were examined, 23 males and 20 females, and compared with 43 peers (control group), 24 males and 19 females. There was no statistical difference between patients and controls for psychological domains (53.4 versus 56.9, p=0.059) and environmental domains (56.6 versus 57.0, p=0.884). Patients had better social QOL than the control group (39.3 versus 31.7, p=0.003), while the control group had better physical QOL (55.4 versus 61.9, p=0.047). Among patients, there was no statistical difference in QOL domains for variables of age, desferroxamine use, serum ferritin level, disease severity, presence of complications; splenectomy status, hepatitis C virus status, or family history.

Conclusion:

Quality of life in thalassemia patients is similar to the control group particularly social life, though physical health is less. Improvement of patients care from all aspects will improve their QOL. More studies in this field are needed with a bigger sample size.β-Thalassemia disorders are characterized by defective production of hemoglobin and excessive destruction of red blood cells.1 Thalassemia effects physical health such as physical deformities, growth on retardation, and delayed puberty, may have an impact on physical appearance, for example: bone deformities and short stature, all of which contribute to poor self-image. Other severe complications such as heart failure, cardiac arrhythmia, liver disease, endocrine complications and infections are common among patients with β-thalassemia and may impact negatively on patients’ quality of life (QOL).2 Therefore, β-thalassemia major and its complications carry a profound psychological impact on the patients.3 The aim of regular blood transfusion is to eliminate the primary complication of severe thalassemia by ameliorating anemia and suppressing erythropoiesis.3 Morbidity and mortality related to thalassemia have been reduced significantly with modern medical treatment, and QOL should now be considered an important index of effective health care and an essential outcome when considering options for treatment for individual patients and the allocation of health care resources.4 As per the World Health Organization (WHO) definition, QOL is individuals’ perceptions of their position in life in the context of the culture and value systems in which they live, and in relation to their goals, expectations, standards and concerns.5 In clinical practice the QOL assessments will assist clinicians in making judgments in regards to the areas in which a patient is most affected by disease, and in making treatment decisions.5 Only a few studies have been conducted on QOL worldwide on patients with β-thalassemia. Various studies have shown that thalassemia is stressful, and patients face a variety of physical, psychological, and social problems. The findings also showed that culture and education play a major role in illness experiences.6 For these reasons, we need to have a baseline assessment for our patients so that we can develop strategies to improve their QOL. The present study aimed to compare levels of QOL among patients with β-thalassemia at the Hereditary Blood Disorders Center in Al-Madinah Al-Munawarah, Kingdom of Saudi Arabia with their healthy peers in terms of physical health, psychological, social relationships, and environmental domains.  相似文献

Objective: To seek a new biochemical index for diagnosis of coronary heart disease (CHD) of shen-Yang deficiency syndrom (CHD-SYD). Methods: Sixty-one patients with CHD were divided into 3 groups according to their TCM Syndrome type, 10 patients in the group without Xin-Qi deficiency (Group A), 25 in the group with Xin-Qi deficiency but without Shen-Yang deficiency (Group B) and 26 in the group both with Xin-Qi deficiency and Shen-Yang deficiency (Group C). Levels of 17-hydroxy-corticoste-roid in urine (urinary 17-OHCS) per 24 hrs, and serum level of high density lipoprotein cholesterol (HDL-C) and high density lipoprotein phospholipid (HDL-PL) in them were determined in synchrostep and compared with those in the control group of 23 healthy aged persons, urinary 17-OHCS per 24 hrs was taken as the diagnostic standard to screen a new index for diagnosis of Shen-Yang deficiency Syndrome, and preliminary appraisal to the index was made.
