概述了近年来国内外利用功能磁共振成像(functional magnetic resonance imaging,fMRI)技术对传统医学针刺作用机制研究现状以及研究所面临的基本问题,并探讨未来应用前景。通过fMRI对穴位特异性、针刺机制、针刺效应观察等几方面的文献研究得出以下结论:针刺腧穴与脑结构具有区域特异性;针刺效应是通过多个脑功能区的相互作用而产生;针刺效果与脑内部分区域的功能变化有相关性。因此,fMRI技术是研究针刺作用机制的一个有效方法,其作用机制、研究方法有待进一步证实。 相似文献

就近5年来国内外针刺领域利用功能磁共振成像(fMRI)技术所做的研究作了回顾,将包含平行对照的实验研究纳入,综述了利用脑fMRI技术进行针刺作用机制研究的内容,总结了目前各类研究存在的问题及可借鉴之处。当前利用fMRI技术进行的针刺作用机制研究主要包括穴位特异性、针刺疗效机制、针刺个体差异性及针刺持续效应几个方面,早期的研究多关注针刺的即时效应,从即时效应角度证明穴位的特异性,阐明针刺疗效机制。随着针刺持续性效应的提出,更加契合临床的新研究模式被提出。如何在契合临床实际的同时保证研究结果的稳定性是今后需要注意的问题。 相似文献

针刺治疗失眠被广泛应用于临床,通过针刺腧穴可以调节神经中枢代谢功能,但具体作用机制比较复杂。应用功能磁共振成像(fMRI)技术可以从影像学角度研究针刺治疗失眠的脑响应区,进而对其中枢机制进行剖析。分析基于fMRI针刺治疗失眠的相关文献,发现主要集中在静息态及任务态fMRI方面,即血氧水平依赖功能磁共振成像(bold-fMRI)脑激活区的研究。研究结果证实,认知、情绪调控相关脑区神经元被激活,达到治疗失眠的作用。本文对近些年代表性研究进行归纳总结,以期为后续针刺治疗失眠脑效应机制的探索提供参考。 相似文献

目的基于针刺治疗慢性疼痛的功能磁共振(Functional magnetic resonance imaging,fMRI)文献进行述评,为针刺治疗慢性疼痛的机制研究提供思路和借鉴。方法对近10年针刺治疗慢性疼痛的fMRI研究进行回顾,依据病种选择、样本量计算、试验设计、研究结果四方面内容进行述评,分析并总结当前研究现状。结果偏头痛、膝骨关节炎和下腰痛是目前研究中涉及频次最高的3个病种。受试者的疾病亚型、年龄段、利手习惯及fMRI禁忌症在研究中基本都保证了一致性和规范性。但多数研究仍存在样本量计算方式不明确的问题。对照组设置主要包括标准对照、无效对照和安慰对照。针刺效应因素在各研究间存在较大差异。研究中结局指标包括疾病特异性量表、疼痛评分及心理、精神状态的评估。fMRI设计以静息态和单一任务态设计为主,多任务fMRI研究相对较少。研究证实针刺可调节疼痛处理网络的功能连接,有效建立心理物理疼痛稳态。结论运用fMRI探讨针刺治疗慢性疼痛作用机制的研究成果丰硕,未来可通过扩大病种的选择,完善质量控制,关注针刺效应影响因素,丰富数据处理手段,借鉴多学科任务设计方式等方式,促进针刺疗慢性疼痛机制研究的进一步发展。 相似文献

以"针刺""功能磁共振成像""Acupuncture""fMRI""Magnetic Resonance Imaging"等为检索词,检索2003年1月至2014年12月CNKI(中国知网)、PubMed数据库,最终纳入253篇运用功能性磁共振成像(fMRI)技术研究针刺机制的文献。运用文献计量学方法分析发现:近10年内该类研究数量总体呈增长趋势;研究的受试对象以健康人为主,涉及病种以神经系统疾病为主;研究内容以穴位特异性最多,而对穴位配伍规律的研究较少。结果提示,以fMRI为手段的针刺脑功能成像研究在研究内容上相对单一,试验设计和质量控制有待进一步提升。 相似文献

Objective: The objective of this study was to explore the possibility of using functional magnetic resonance imaging(fMRI) technique in the research of central mechanism of acupuncture effects in the treatment of angina pectoris.Methods: Make an introduction about fMRI and its characteristic techniques, advantages, points of paying attention and possible achievements in the studies about central mechanism of acupuncture effects in treatment of angina pectoris.Results: fMRI provided an indirect reflection of neural activity change in the working brain through detection of blood oxygenation level-dependent(BOLD) signal changes.The characteristics of radiation that are free, moderate cost, as well as high spatial and temporal resolution, are considered the strongest points of fMRI.We should pay high attention to the experimental conditions, reasonable operational procedures and standard acupuncture operation when we explore the central mechanism of acupuncture effects in treating angina pectoris based on fMRI technique.Furthermore,the research would be enhanced if we figured out how the acupuncture signals induce the changes of cerebral functional activity, as well as the regulations and the influence factors of these changes, thereby discovering the central mechanism of acupuncture treatment of angina pectoris.Conclusion: Application of fMRI technique in the research on central mechanism of treating angina pectoris by acupuncture therapy is feasible and it would provide a new tool for scientific research. 相似文献

Through searching the literature on the mechanism of acupuncture for primary dysmenorrhea in the last 15 years, this paper summarizes systematically the current study progress on the analgesic mechanism of acupuncture for primary dysmenorrhea, from the perspectives of animal experiment and human trials. In terms of research methods, the behavioral observation, evaluation of central and peripheral biochemical indexes, and molecular biological techniques were mainly used in the animal experiments; whilst the clinical symptoms and evaluation of peripheral serum biochemical indexes were focused on in human trials. As for the research results, the animal experiments showed that acupuncture may play an analgesic role by regulating endocrine, promoting the release of central and peripheral neurotransmitters, regulating immune function, and relieving uterine smooth muscle spasm. However, the human trials have found that acupuncture can produce analgesic effects by regulating serum prostaglandin and ovarian hormone levels, promoting the release of peripheral β-endorphin, improving the uterine artery flow status, and relieving uterine smooth muscle spasm. Although different research methods were used in animal and human studies, the similar results were obtained in acupuncture regulating endocrine levels, promoting release of peripheral neurotransmitters, and alleviating uterine smooth muscle spasms. In addition, at present stage, animal experiments are more than human trials in numbers, and there are relatively few studies on the central mechanism of acupuncture analgesia in human trials. Therefore, in view of this phenomenon, this paper proposes to use functional magnetic resonance imaging (fMRI) technology to explore the central mechanism of acupuncture analgesia in human body, so as to provide a new idea for further exploring the mechanism of acupuncture analgesia in treating primary dysmenorrhea. 相似文献

In this study, we explore various regions of the brains of Alzheimer' s Disease (AD) patients before and after acupuncture treatment of acupoints in the brain in order to determine the effect, if any, of acupuncture on AD. Twenty-six patients with clinically-diagnosed AD underwent functional magnetic resonance imaging (fMRI) while undergoing acupuncture at the four acupoints of Shenmen (HT 7), Zusanli (ST 36), Fenglong (ST 40) and Taixi (KI 3). fMRI Block design paradigm was chosen by electroacupumcture interval stimulation, and the data of fMRI were analyzed by Statistical Parametric Mapping (SPM 99). The result demonstrated that there were right main hemisphere activations (temporal lobe, such as hippocampal gyrus, insula, and some area of parietal lobe) and left activated regions (temporal lobe, parietal lobule, some regions of cerebellum). The activated regions induced by these acupoints were consistent with impaired areas in brain for AD patients, which were closely correlated with the cognitive function (memory, reason, language, executive, etc.). The present study provided the strong evidence that acupuncture had a potential effect on AD, and in partial revealed the mechanism. 相似文献
