目的 分析遗传性出血性毛细血管扩张症(hereditary hemorrhagic telangiectasia,HHT)家系ENG、ACVEL1和SMAD4基因突变.方法 收集4个HHT家系临床资料并分析其临床特点,应用直接测序和多重连接探针扩增技术对11例临床确诊及可疑患者的ENG、ACVRL1和SMAD4基因进行突变分析,将结果与HHT基因突变数据库进行对比.结果 家系2先证者及2个妹妹的ENG基因发生了第2外显子c.207G>A(p.L69L)同义突变、第8外显子c.1004A>T(p.Q335L)错义突变、ACVRL1基因第7外显子c.817C>T(L273L)同义突变;家系3先证者及其母亲和弟的ENG基因发生了第8外显子c.1004A>T(p.Q335L)突变;也检测到家系4先证者及其兄的ENG基因第8外显子c.1004A>T(p.Q335L)突变.家系1先证者及其他HHT患者,未检测到基因突变.其中ENG基因第8外显子c.1004A> T(p.335Q>L)为新突变,在200名正常对照中也未检测到该突变.结论 HHT具有遗传异质性,ENG基因第8外显子c.1004A>T(p.Q335L)为HHT新的致病突变.  相似文献

目的 分析3个Fabry病家系GLA基因突变及其与临床表型的关系.方法 应用PCR结合DNA测序技术,检测先证者及相关成员GLA基因编码序列与剪切位点DNA序列变异,分析致病性突变与临床表型关系.结果 在家系1先证者GLA基因第5外显子中发现1个未经报道的错义突变c.797A>C(D266A),家系2先证者GLA基因第5外显子中发现1个错义突变c.644A>G(N215S),家系3先证者GLA基因第2外显子中发现1个无义突变c.355C>T(Ql19X).家系1与家系3先证者主要表现为皮肤损害和慢性肾功能不全,家系2先证者临床则以肥厚性心肌病为特点.结论 首次发现的GLA基因c.797A>C(D266A)突变是第266位密码子第6个被证实的错义突变,已报道的另5种突变均有致病性,在正常非相关对照中未发现该突变,提示GLA基因c.797A>C突变很可能是该家系的致病原因.N215S和Q119X系首次发现于中国Fabry病家系的突变.GLA基因不同位点的突变具有较为显著的表型差异.  相似文献

目的 探讨2例先天性肌强直患者的氯离子通道蛋白-1(chloride channcl 2,CLCN1)基因突变情况和临床特点.方法 收集福建地区1个先天性肌强直家系的先证者和1例散发性先天性肌强直患者的临床资料并进行综合分析.用PCR扩增患者CLCN1基因的全部外显子,通过直接测序检测突变的情况.结果 家系1先证者的CLCN1基因第8外显子存在c.1024 G>A的杂合性错义突变,散发性患者的CLCN1基因第11外显子发现了c.1292 C>T的杂合性错义突变.结论 先天性肌强直症临床表现缺乏特异性,CLCN1基因突变检测是确诊该病的有效方法.  相似文献

目的探讨中国人遗传性非息肉病性结直肠癌(hereditary nonpolyposis colorectal cancer,HNPCC)家系中MSH6基因胚系突变。方法采用PCR-直接测序的方法检测39个无胚系MSH2及MLH1基因突变、符合不同临床标准的中国人HNPCC家系先证者MSH6基因各外显子胚系突变;对137名正常人胚系基因组DNA进行错义突变相应外显子的测序分析。应用Envision二步法检测有突变的先证者肿瘤组织MSH6蛋白表达。结果在39个HNPCC先证者中共发现6个MSH6基因的胚系突变,分别位于第4、6、9和第10外显子;突变类型为4个错义突变、1个无义突变、1个剪接区的插入突变;对4个错义突变的相应外显子的测序分析显示:137名正常人胚系基因组DNA5例具有第6外显子1163密码子处的c.3488A>T的错义突变,约占3.65%(5/137),为单核苷酸多态性(single nucleotide polymorphism,SNP);其余错义突变在正常人群中均未发现。在6例有MSH6基因胚系突变家系的肿瘤组织中免疫组化染色除1例为SNP的肿瘤组织MSH6蛋白阳性表达外,其余均为阴性表达。经过查询国际HNPCC突变数据库及SNP数据库证实上述突变中5个为国际上尚未报道的病理性突变,1个为新发现的SNP。结论MSH6基因胚系突变在符合不同临床标准的中国人HNPCC中均起一定作用,对无MSH2及MLH1基因胚系突变的先证者行MSH6基因胚系突变的测序分析对确诊HNPCC家系是必要的。  相似文献

目的:报告一个脑干听觉诱发电位有异常改变的腓骨肌萎缩症(Charcot-Marie-Tooth disease,CMT)家系,并探讨与连接蛋白32(connexin32,Cx32)基因突变的关系。方法:对整个家系进行临床检查,对先证者进行肌电图及脑干听觉诱发电位检查,并应用聚合酶链式反应-单链构象多态(polymerase chain reaction -single strand conformation polymouphism,PCR-SSCP)技术结合DNA序列分析方法检测了先证者、家系内8人及家系外50名无血缘关系的正常人。结果:先证者肌电图检查示神经传导速度明显减慢,脑干听觉诱发电位示中枢传导延长,该家系中先证者及另3人均出现异常SSCP条带,经测序证实为392T→C(Leu131Pro)突变。结论:Leu131Pro是未报道过的突变,腓骨肌萎缩症患者可以出现脑干听觉诿发电位异常。  相似文献

长QT综合征 KCNQ1基因突变筛查方法   总被引:5,自引:0,他引:5  
目的 研究中国人长QT综合征(long QT syndrome,LQTS)与编码缓慢激活延迟整流钾通道基因(postassium voltage—gated channel,KQT—like subfamily member 1,KCNQl)突变的关系。方法 根据心电图T波的特征对31个家系进行基因分型的初步预测。对10个预测为LQTl家系的家庭成员,用聚合酶链反应-单链构像多态性(polymerase chain reaction—single strand conformation polymorphism,PCR—SSCP)方法进行KCNQ1基因16个外显子及剪接位点的筛查,SSCP异常者进行DNA测序。为避免遗漏心电图表现不典型的LQT1,同时也为了进行方法学比较,对其它21个非LQT1家系只对先证者进行16个外显子的PCR和DNA直接测序。对测序有异常者,分析其家系成员相应外显子的疾病分离情况。若异常只存在于患者,则检查该异常在50个正常对照者中的情况。结果 (1)在心电图预测分型为LQT1的家系中发现了位于第5外显子的S277L(1个家系)和G306V(1个家系)2个错义突变。另外发现了3个多态性,分别为435C→T(1145I)(7个家系)、1632C→A(S546S)(1个家系)、IVS1 9C→G(3个家系)。(2)在心电图分型预测为非LQT1的先证者中只发现了1个剪接突变IVS1 5G→A(2个家系)和1个多态性IVS1 18C→T(1个家系)。3个突变位点均位于KCNQ1基因功能区域,各突变家系内患者存在同样的异常条带或序列,而正常对照无此异常。结论 在中国人LQTS患者中发现TKCNQ1基因上的2个新错义突变、1个剪接突变和4个多态性。结合心电图分型预测,PCR-SSCP法可发现绝大部分突变,是筛查LQTS突变的简便而经济的方法。  相似文献

目的 对1个姨表近亲结婚的遗传性凝血因子Ⅴ (coagulation factor Ⅴ,FⅤ)缺陷症家系进行基因分析,探讨其分子发病机制.方法 检测先证者及其家系成员的凝血酶原时间(prothrombin time,PT)、活化部分凝血活酶时间(activated partial thromboplastin time,APTT)、纤维蛋白原(fibrinogen,FIB)、血浆FⅤ活性(FⅤ∶C)和血浆FⅤ抗原(FⅤ∶Ag)等凝血指标进行表型诊断.用DNA直接测序法分析先证者及家系成员F5基因的全部外显子及侧翼、5’和3’非翻译区,发现突变位点用反向测序予以证实.结果 先证者PT和APTT均明显延长,分别为23.5s和50.5s,其FⅤ∶C(8%)和FⅤ∶Ag(<1%)极度减低;同样先证者的妹妹PT和APTT也显著延长,分别为24.1s和62.4 s,其FⅤ∶C(7%)和FⅤ;Ag(<1%)也极度减低;家系其他成员的PT及APTT均在正常参考范围.先证者的F5基因第5外显子测序发现其29170位为纯合突变T→C,导致190位氨基酸苯丙氨酸变为丝氨酸(Phe190Ser),先证者的妹妹同样为纯合Phe190Ser突变;大哥、大姐、大女儿、小女儿和外甥女均为Phe190Ser杂合突变,其FⅤ∶C也有所减低(分别为57%、73%、72%、66%、75%);两个弟弟为正常野生型,其FⅤ∶C和FⅤ∶Ag均在正常水平.结论 该遗传性FⅤ缺陷症家系先证者及其妹妹F5基因第5外显子存在g.29170T→C纯合型错义突变,导致Phe190Ser.其原因推测与其父母近亲结婚有关.Phe190Ser与该遗传性凝血因子Ⅴ缺陷症家系FⅤ水平降低有关.  相似文献

目的 对两个X连锁隐性遗传少汗性外胚层发育不良(X-linked hypohidrotic ectodermal dysplasia,XLHED)家系进行ED1基因突变分析,为罹患家庭提供遗传咨询及产前诊断.方法 综合应用序列分析及多重连接依赖性探针扩增方法,对两个家系的先证者进行ED1基因突变分析,并针对检测到的突变位点对女性成员进行检测.采集家系1胎儿的羊水细胞进行产前诊断,包括致病突变位点的分析、ED1基因内4个短串联重复序列(short tandem repeat,STR)位点的单倍型连锁分析、性别鉴定及核型分析.结果 家系1先证者缺失ED1基因第1外显子及下游2个STR位点DXS8269,DXS1422区域,其余外显子序列分析未见异常,其女儿为该缺失突变的携带者;结合连锁分析、性别鉴定及核型分析结果,家系1胎儿为男性非ED1基因缺失突变携带者,胎儿足月分娩后随访,为健康个体.家系2先证者经序列分析检测到ED1基因第3外显子c.463C>T(R155C)错义突变,母亲为c.463C>T(R155C)杂合突变携带者.结论 ED1基因第1外显子区域缺失和错义突变R155C是导致2个少汗性外胚层发育不全家系患者临床表型的主要原因,ED1基因的突变检测结合单倍型分析,能准确地对该类家系提供产前诊断.  相似文献

目的 对1个X连锁隐性遗传性少汗性外胚层发育不全(X-linked hypohidrotic ectodermal dysplasia,XLHED)家系进行EDA基因检测,分析基因突变类型,探讨其遗传学因素及发病原因.方法 对该XLHED家系进行家系调查,收集家系患者及部分成员和50名无亲缘关系的正常个体的外周血标本并提取基因组DNA,应用聚合酶链反应扩增EDA基因8个外显子,DNA直接测序进行突变检测.结果 该家系中患者检出EDA基因第3外显子存在c.467G>A突变,导致R156H错义突变.先证者的母亲及姨妈携带c.467G>A杂合突变,家系中正常个体和正常对照个体均未检测到该突变.结论 EDA基因的R156H突变可能是引起该XLHED家系先证者少汗性外胚层发育不全的致病原因.  相似文献

目的 探讨9个遗传性凝血因子Ⅶ( coagulation factorⅦ,FⅦ)缺陷症家系的基因突变类型与临床特征.方法 检测凝血酶原时间、活化部分凝血活酶时间及FⅦ活性和抗原等指标以明确诊断;用PCR法扩增先证者F7基因的全部外显子及侧翼序列、5′和3′非翻译区.PCR产物纯化后直接测序,寻找基因突变,用反向测序证实所发生的突变.结果 9个家系中的先证者凝血酶原时间均延长,FⅦ活性在2.0%~6.0%之间,7例先证者的FⅦ抗原显著减低.共发现8种F7基因突变,其中g.8355 A>T(Gln100Leu)、g.11243T>C(Ser269Pro)和g.11520_11521insT 3种突变为首次发现;6种突变发生在催化区;缺失突变和插入突变各1种,其余均为错义突变;所有的基因突变均来自先证者的父亲和(或)母亲,发现5个家系存在近亲婚配.g.27_28delCT、Cys329Gly、Arg304Trp和His348Gln突变在无亲缘关系的家系中重复出现.不同基因突变类型的临床表型有较大差异:2例His348Gln及1例Arg304Trp纯合突变可表现为轻型和无症状,2例g.27_28delCT纯合、杂合突变分别表现为中型、无症状,4例双杂合突变分别表现为1例(Ser269Pro和Cys329Gly)无症状、2例(Arg304Trp和Cys329Gly与Arg277Cys和g.11520_11521insT)轻型、1例(Gln100Leu和His348Gln)中型.结论 中国汉族人群中存在导致F7基因缺陷的突变热点.遗传性凝血因子Ⅶ缺陷症患者的临床表型与FⅦ活性、F7基因突变类型无明显的相关性.  相似文献

A family is reported in which two sibs, one male and the other female, both died within 24 hours of birth with enlarged polycystic kidneys. Postmortem histology in the second child showed gross renal dysplasia. In both children the pancreas was enlarged, nodular and cystic but the liver appeared macroscopically normal. In the second child, histological examination confirmed pancreatic fibrosis with cystic dilation of ducts, but showed portal fibrosis with bile duct proliferation in the liver.
This combination of findings is very reminiscent of those in a girl and her brother reported by Ivemark et al. (1959). The children reported here also showed absence or hypoplasia of the spleen, cardiac anomalies and other features of the Ivemark syndrome (Ivemark 1955), a quite different, usually sporadic, congenital disorder. It is suggested that the children described here have a distinct lethal congenital disorder, probably inherited in an autosomal recessive manner.  相似文献

Over 200 schizophrenic patients belonging to three major and interrelated pedigree complexes have been investigated over the past 30 years in a North Swedish geographically isolated population, presently numbering about 6,000. An intensive investigation of a number of biochemical correlates and genetic markers in a few selected families belonging to one of the major pedigrees has indicated new strategies for the current research program.
Schizophrenia, as defined operationally, is significantly associated with decreased activities of two enzymes (1) blood platelet monoamine oxidase, (2) plasma dopamine-β-hydroxylase, and (3) with the genetic marker Gc2 (group specific antigen). Both enzymes are subject to genetic variation. A positive score for linkage between schizophrenia and low plasma DBH activity has been calculated, but, so far, available data are insufficient for discrimination between linkage and partial contribution of genetically controlled low plasma DBH to the pathogenesis of the disease. Alternatively, both mechanisms could be involved.
As a model for continued research, schizophrenia is explained as based on a double dominant-recessive genotype (Aabb), representing a vulnerability which in about 50 % of cases develops into clinical schizophrenia. It is suggested that the dominant mutation (A) operates on or affects MAO activity, and that the recessive genotype (bb) is instrumental in low variates of DBH activity and very likely such variates within the normal range of physiological variation. Moreover, it is suggested that the combined effects of MAO- and DBH-reduced efficiency on the metabolism of e.g. dopamine could be an essential pathogenic mechanism for the schizophrenic illness which is segregating in this population.  相似文献

About 1900, modern food selection and processing caused widespread epidemics of the B vitamin deficiency diseases of beriberi and pellagra which, for genetic reasons, often expressed as different diseases ranging from bowel and heart disease to dermatoses and psychoses. But the B vitamins merely help convert essential fatty acids (EFA) into the prostaglandin (PG) tissue regulators and it now turns out that, through hydrogenation, milling and selection of w3-poor southern foods, we have also been systematically depleting, by as much as 90%, a newly discovered trace Nordic EFA (w3) of special importance to primates and sole precursor of the PG3(4) series, even as a concurrent fiber deficiency increases body demand for EFA. Since substrate EFA is processed by many B vitamin catalysts, an EFA deficiency will mimic a panhypovitaminosis B, i.e., a mixture of substrate beriberi and substrate pellagra resembling vitamin beriberi and pellagra but exhibiting as even more diverse endemic disease. This would consitute a second stage of the Modern Malnutrition and explain why some workers now hold the dominant diseases of modermized societies to be new, nutritionally based, pellagraform yet lipid-related and to range, once again, from heart disease to psychosis. It is an assumption that our dominant diseases are unrelated to each other or are merely revealed by our diagnostic acumen and therapeutic success; and that hydrogenating millions of tons of food oils annually, to destroy the rancidity producing w3-EFA, is safe for primates. Extensive beriberiform disease is reported here in 32 typical cases taken from medical practice which responds strikingly to linseed oil supplements (60% w3-EFA) in confirmation of identical results in Capuchins.  相似文献

There are an estimated over 200 million yearly cases of malaria worldwide. Despite concerted international effort to combat the disease, it still causes approximately half a million deaths every year, the majority of which are young children with Plasmodium falciparum infection in sub-Saharan Africa. Successes are largely attributed to malaria prevention strategies, such as insecticide-treated mosquito nets and indoor spraying, as well as improved access to existing treatments. One important hurdle to new approaches for the treatment and prevention of malaria is our limited understanding of the biology of Plasmodium infection and its complex interaction with the immune system of its human host. Therefore, the elimination of malaria in Africa not only relies on existing tools to reduce malaria burden, but also requires fundamental research to develop innovative approaches. Here, we summarize our discoveries from investigations of ethnic groups of West Africa who have different susceptibility to malaria.  相似文献

Sick children were ubiquitous in early modern England, and yet they have received very little attention from historians. Taking the elusive perspective of the child, this article explores the physical, emotional, and spiritual experience of illness in England between approximately 1580 and 1720. What was it like being ill and suffering pain? How did the young respond emotionally to the anticipation of death? It is argued that children’s experiences were characterised by profound ambivalence: illness could be terrifying and distressing, but also a source of emotional and spiritual fulfilment and joy. This interpretation challenges the common assumption amongst medical historians that the experiences of early modern patients were utterly miserable. It also sheds light on children’s emotional feelings for their parents, a subject often overlooked in the historiography of childhood. The primary sources used in this article include diaries, autobiographies, letters, the biographies of pious children, printed possession cases, doctors’ casebooks, and theological treatises concerning the afterlife.  相似文献

Recent advancements in agricultural biotechnology have created a need for analytical techniques to determine introduced proteins in crops enhanced through modern biotechnology techniques. These proteins are expressed in plant tissues and may be present in food ingredients. Immunoassays are ideally suited for protein detection and may be used as both quantitative and threshold methods. Microplate ELISA and lateral flow devices are two of the most commonly used immunoassay formats for agricultural biotechnology applications. This paper provides general background information and a discussion of criteria for the validation and application of immunochemical methods to the analysis of proteins introduced into plants and food ingredients using biotechnology methods. It is the result of a collaborative effort of members of the Analytical Environmental Immunochemical Consortium. This collaborative effort represents the combined expertise of several organizations to reach consensus on establishing guidelines for the validation and use of immunoassays. Further, the paper offers developers and users a consistent approach to adopting the technology as well as aid in producing accurate and meaningful results.  相似文献

The preparation steps usually necessary for obtaining ultrathin frozen sections of biological material (chemical prefixation, enclosing, cryoprotective treatment, freezing, sectioning, and post-staining the sections for transmission electron microscopy) are submitted to a critical analysis. The application of cryo-ultramicrotomy, in particularly for cytochemical purposes, is reviewed. Fundamental considerations of chemical prefixation and poststaining are supported by examples from yeast cytology. Furthermore, the efficiency of the cryo-ultramicrotomy (electron optical resolution of ultrastructural details) is demonstrated on yeast cells and protoplasts.  相似文献

HLA-A,-B,-C,-DRB1 and -DQB1 alleles have been studied in Chimila Amerindians from Sabana de San Angel (North Colombian Coast) by using high resolution molecular typing. A frequent extended haplotype was found:HLA-A*24:02-B*51:10-C*15:02-BRB1*04:07-DQB1*03:02 (28.7%) which has also been described in Amerinndian Mayos Mexican population (Mexico, California Gulf, Pacific Ocean). Other haplotypes had already been found in Amerindians from Mexico (Pacific and Atlantic Coast), Peru (highlands and Amazon Basin), Bolivia and North USA. A geographic pattern according to HLA allele or haplotype frequencies is lacking in Amerindians, as already known. Also, five new extended haplotypes were found in Chimila Amerindians. Their HLA-A*24:02 high frequencies characteristic is shared with aboriginal populations of Taiwan; also, HLA-C*01:02 high frequencies are found in New Zealand Maoris, New Caledonians and Kimberly Aborigines from Australia. Finally, this study may show a model of evolutionary factors acting and rising one HLA allele frequency (-A*24:02), but not in others that belong to the same or different HLA loci.  相似文献

Starting with the integument, we see many organs are contractile sacs or multiples thereof, which tubes or bags constitute the major part of the entire body. Recognition of this basic unit and its characteristics sheds new light, individually and collectively, on many disorders previously considered unrelated. Muscular tears and perforations develop in the walls of these chambers, being no way peculiar to those organs, wherein, hydrochloric acid occurs. So, it is not necessary to explain the absence of excessive acid from patients who exhibit holes in the gastric, uterine, aortic, duodenal, rectal, pulmonary, retina, and other walls. Muscle, not acid is the great common factor relating idiopathic disorders in the gastrointestinal tract to each other and to similar diseases in other systems. When the units are linked together, the lesions tend to appear as arthropathies, i.e. at the joints. Rephrasing common-place observations, frees us from conventional, conceptual cul-de-sacs. An observation is only as good as its interpretation, so all possibilities must be considered, otherwise, we will remain blinded by our misconceptions.  相似文献
