There is limited literature that follows a population of Anterior Cruciate Ligament Reconstruction (ACLR) patients through recovery. Our aim was to examine differences in movement and loading patterns across time and between limbs over four visits during 12 months post‐ACLR. We hypothesized that kinematic and kinetic data during a stop‐jump would have time‐ and limb‐dependent differences through 12 months post‐surgery. Twenty‐three ACLR athletes performed five vertical stop‐jumps at 4, 5, 6, and 12 months post‐op with motion capture and force plate data collection. The peak knee flexion (PKF) was different between the 4 and 12, 5 and 6, and the 5 and 12 month visits with earlier months exhibiting higher PKF. The peak vertical ground reaction force (vGRF) was lower at 4 than at 5 and 6 months. The peak posterior ground reaction force (pGRF) was lower at 4 months than all other visits. Frontal knee and sagittal hip range of motion (ROM) were different between 12 months and each previous visit. Asymmetries were present in peak vGRF, peak knee extension moment and impulse up to 12 months. The loading rate and peak pGRF demonstrated between limb differences up to 6 months; limb stiffness demonstrated differences up to 5 months post‐ACLR. PKF was only asymmetric at the 4 month visit. While some variables improved in the 12 months post‐ACLR, limb asymmetries in peak knee extension moment, peak vGRF and impulse persisted up to 12 months. Background: Distal femoral varus osteotomy (DFVO) is an effective surgical intervention for the management of symptomatic valgus malalignment of the knee. Because it preserves the native knee joint and its ligamentous stability, DFVO is preferred to total knee arthroplasty (TKA) in the young, active population. Purpose: We sought to assess return to work (RTW) and return to sport (RTS) rates following DFVO for valgus malalignment of the knee. Methods: For this systematic review, we searched EMBASE, MEDLINE, and Web of Science from inception through December 31, 2020. English language studies of all levels of evidence explicitly reporting on RTS and RTW rates following DFVO for valgus malalignment of the knee were eligible for inclusion. Results: Seven studies and 127 patients were included in our analysis. Mean age was 32.4 ± 8.8 years with men comprising 46.7% ± 22.3% of study populations. The mean RTS rate was 87.2% ± 10.7%, with a return to preoperative activity levels rate of 65.4% ± 26.8%. The mean RTW rate was 81.8% ± 23.3%, with a return to preoperative activity levels of 72.8% ± 18.1%. The mean reoperation rate was 35.6% ± 18.8% within a mean follow-up period of 5.5 ± 1.9 years. Conclusions: This systematic review of low-level studies found DFVO to be a safe and effective procedure for the management of genu valgum in young, active populations, with most patients returning to sport and/or work, although not all at their preoperative activity levels. A paucity of data surrounds RTS and RTW rates following DFVO. Future studies should explicitly report both return to activity rates and whether patients returned to their preoperative activity levels.  相似文献

Patients with anterior cruciate ligament reconstruction (ACLR) are often psychologically and physically under-prepared for sports participation. This study compared readiness to return to sport based on completion of advanced training after ACLR. Patients with ACLR who self-selected participation in a 6-week group-format advanced training program (TRAINING) were compared to age- and sex-matched patients who did not participate (NoTRAINING). Each group had 23 participants (14 females). Advanced training consisted of plyometric, strengthening, and agility exercises. Baseline and follow-up testing included psychological measures (Anterior Cruciate Ligament Return to Sport after Injury [ACL-RSI]; Tampa Scale for Kinesiophobia [TSK-11]; Knee Activity Self-Efficacy [KASE]; and fear intensity for the primary fear-evoking task or situation) and a hop test battery. Return to sport criteria were ACL-RSI score ≥70 points and limb symmetry index ≥90% on all hop tests. At follow-up, KASE score was higher in TRAINING than NoTRAINING (92.7 vs. 89.1 points; respectively), but ACL-RSI, TSK-11 and fear intensity scores were not significantly different between groups. Return to sport criteria passing rate was not significantly different between groups at baseline (TRAINING: 13%, NoTRAINING: 30%) or follow-up (TRAINING: 52%, NoTRAINING: 43%); however, the distribution of criteria met at follow-up differed with more patients in TRAINING than NoTRAINING meeting hop test criteria (30% vs. 4%, respectively) and more patients in NoTRAINING than TRAINING failing to meet any criteria (25% vs. 0%, respectively). Advanced training after ACLR facilitated readiness for sport participation by improving confidence and hop performance, but may not have a preferential effect on fear.  相似文献

The objective of the study was to determine if psychological readiness for sport and knee self-efficacy assessed early (3 months) after anterior cruciate ligament reconstruction (ACLR) are predictive of self-reported functional outcomes, quadriceps strength, and knee mechanics while running at the time of return to sport training (6 months). Thirty athletes with unilateral ACLR completed the ACL Return to Sport after Injury (ACL-RSI) and Knee Self-Efficacy Scale (K-SES) 3 months after ACLR and completed self-reported functional outcomes, isometric quadriceps strength testing, and three-dimensional running gait analysis 6 months after ACLR. The 3-month ACL-RSI significantly correlated with the 6-month International Knee Documentation Committee (IKDC; r = 0.565, p = 0.001), Knee Injury and Osteoarthritis Outcome Score (KOOS) sport/recreational activities (KOOSSport; r = 0.548, p = 0.002) and quality of life (KOOSQoL; r = 0.431, p = 0.017), and quadriceps strength (r = 0.528, p = 0.003). The 3-month K-SES significantly correlated with the 6-month IKDC (r = 0.528, p = 0.003), KOOSSport (r = 0.430, p = 0.018), KOOSQoL (r = 0.411, p = 0.024), quadriceps strength (r = 0.465, p = 0.010), and knee flexion excursion (r = 0.472, p = 0.008). With multivariate modeling, both the ACL-RSI and K-SES were predictive of the IKDC (R2 = 0.411; p = 0.001). Only the ACL-RSI was predictive of the KOOSSport (R2 = 0.300; p = 0.002), KOOSQoL (R2 = 0.186; p = 0.017), and quadriceps strength (R2 = 0.279; p = 0.003), whereas only the K-SES was predictive of knee flexion excursion (R2 = 0.173; p = 0.022). Athletes with greater psychological readiness for sport and knee self-efficacy at 3 months demonstrated higher scores on self-reported functional outcomes, greater quadriceps strength, and greater knee flexion excursion at 6 months after ACLR. This study indicates that psychosocial measures may be important to include into early post-surgical evaluations to help guide and facilitate interventions to restore subjective and objective knee function.  相似文献

This study tested the hypotheses that participants with high knee-related confidence at the time of return to sport (RTS) after anterior cruciate ligament (ACL) reconstruction (ACLR) would have a greater likelihood of meeting RTS criteria and would have a higher likelihood of second ACL injury. Participants completed RTS testing when cleared to RTS. Success was defined as an IKDC score more than or equal to 90 and all limb symmetry index (LSI) scores more than or equal to 90%. Confidence groups were created based on response to the KOOS Quality of Life subscale. Incidence of second ACL injury was tracked for 24 months after RTS. Associations among confidence, success in meeting RTS criteria, and second ACL injury were evaluated. At RTS, 37.7% were classified with high knee-related confidence, 26.4% met RTS criteria and 22.0% sustained a second ACL injury post-RTS. A higher proportion of confident participants met all RTS criteria (p = 0.001). Confident participants were over two times more likely to suffer a second ACL injury (odds ratio [OR] = 2.40; 95% confidence interval [CI]:1.21–5.20) (p = 0.02) and participants who were both confident and met all RTS criteria were 10 times more likely to suffer a second ACL injury (OR = 9.6; 95% CI:1.1, 84.2) (p = 0.02) than those who passed all RTS criteria but lacked knee-related confidence. Clinical significance: Confident, young athletes at the time of RTS after ACLR are more likely to meet all RTS criteria and more likely to sustain a second ACL injury after RTS. Future work must consider how the inclusion of additional factors into RTS criteria may better identify those at high risk for poor outcome after ACLR.  相似文献

Background

The shoulder is the second most frequently injured joint after the knee in rugby players and labral tears appear to be common. There is limited data available in the literature regarding the mechanisms of posterior labral injury in rugby players and the management of these injuries.

Objective

The aim of this study is to report the clinical presentation, arthroscopic findings, surgical technique for repair, and the functional outcome in elite English rugby players with isolated posterior labral injuries.

Study Design

Case series (level IV evidence)

Materials and Methods

Over a 5-year period we surgically treated 142 elite rugby players, of whom 11 (7.8%) had isolated posterior labral injuries. All these 11 patients had significant contact injury. Only three (24%) patients had a true posterior shoulder dislocation. Pre- and postoperative assessment included Constant score, Oxford shoulder score, and Oxford instability score. We also assessed the time taken to return to preinjury level of fitness and the complications of surgery.

Results

Average follow-up was for 32 months (range 17–54 months). The mean Constant score improved from 66 to 99. The Oxford score indicated improvement, decreasing from 33 to 18; similarly, the Oxford instability score also decreased from 52.2 to 12.3. Return to playing rugby at peak level was at a mean of 4.3 months after arthroscopic repair.

Conclusion

Successful clinical results and rapid return to play can be achieved by appropriate early arthroscopic repair and supervised accelerated rehabilitation for posterior labral tears in elite rugby players.  相似文献

The purposes of this prospective cohort study were (1) to assess if second anterior cruciate ligament (ACL) injury rate 2 years after ACL reconstruction (ACLR) in those who returned to pivoting sport was associated with meeting (a) quantitative return to sport (RTS) criteria, (b) qualitative RTS criteria, and (c) combined quantitative and qualitative RTS criteria, and (2) to determine why athletes did not return to their preinjury (level of) sport. Athletes after ACLR performed RTS tests immediately before RTS: seven movement quantity (strength and hop test battery) and two movement quality (countermovement jump with LESS score and hop-and-hold test) tests. A 2-year postoperative questionnaire asked for RTS, reasons for not returning to the same (level of) sport and second ACL injuries. One hundred and forty-four athletes (82%) completed the questionnaire and 97 of them returned to a pivoting sport. Seven of these athletes had a second ACL injury. Meeting the hop test battery RTS criterion (absolute risk reduction 11%; p = .047) and hop-and-hold test RTS criterion (absolute risk reduction 15%; p = .031) were both significantly associated with a reduced second ACL injury rate. Meeting combined RTS criteria were not significantly associated with second ACL injury rate. Therefore, RTS tests after ACLR should at least comprise a hop test battery or the hop-and-hold test to reduce second ACL injury risk after return to pivoting sport. Also, one-third of all athletes mentioned fear of reinjury as the main reason for not returning to their preinjury (level of) sport. This psychological component should be taken seriously and discussed during rehabilitation.  相似文献

Second anterior cruciate ligament rupture is a common and devastating injury among young women who return to sport after ACL reconstruction, but it is inadequately understood. The purpose of this study was to compare gait biomechanics and return‐to‐sport time frames in a matched cohort of young female athletes who, after primary ACLR, returned to sport without re‐injury or sustained a second ACL injury. Approximately 6 months after primary reconstruction, 14 young women (age 16 ± 2 years) involved in jumping, cutting, and pivoting sports underwent motion analysis testing after physical therapy and impairment resolution. Following objective return‐to‐sport clearance, seven athletes sustained a second ACL rupture within 20 months of surgery (13.4 ± 4.9 months). We matched them by age, sex, and sport‐level to seven athletes who returned to sports without re‐injury. Data were analyzed using a previously validated, EMG‐informed, patient‐specific musculoskeletal model. Compared to athletes without re‐injury, athletes who sustained a second ACL injury received surgery sooner (p = 0.023), had post‐operative impairments resolved earlier (p = 0.022), reached criterion‐based return‐to‐sport benchmarks earlier (p = 0.024), had higher body mass index (p = 0.039), and walked with lower peak knee flexor muscle forces bilaterally (p = 0.021). Athletes who sustained a second injury also tended to walk with larger (p = 0.089) and more symmetrical peak knee flexion angles and less co‐contraction, all indicative of a more normal gait pattern. Statement of Clinical Significance: Delayed return‐to‐sport clearance even in the absence of gait or clinical impairments following primary ACL reconstruction may be necessary to mitigate second ACL injury risk in young women. © 2016 Orthopaedic Research Society. BackgroundTotal hip arthroplasty (THA) is currently performed on active patients with increasing functional demands. Single stage bilateral THA is indicated in younger patients in good general health. Our objective is to evaluate the return to sport (RTS) in patients who underwent bilateral single stage THA compared to unilateral THA.MethodsThis retrospective case control study was conducted between 2013 and 2017. All patients who underwent bilateral single stage THA were included. The control group had unilateral THA performed and was matched based on age, body mass index, gender, and surgery date (2 controls for each bilateral case). All surgeries were performed by a single senior surgeon using the direct anterior approach. The University of California Los Angeles activity score was collected at the last follow-up. A questionnaire regarding RTS, motivation, and satisfaction was assessed.ResultsThirty-two patients were included in the study, of whom 21 were men. The average age was 60.7 ± 9.6 years, body mass index was 26 ± 4 kg/m2, and mean follow-up was 20.1 ± 11.6 months. Twenty-eight patients overall (87%) returned to sport after the procedure. Twenty-five of these (89%) returned to the same sport, and 17 (68%) participated at the same intensity. The average time to RTS was 4 ± 2.8 months. These results were at least as good as those after unilateral THA. The level of motivation of the patient was the only predictive factor for RTS (P < .001).ConclusionBilateral single stage THA via a direct anterior approach allows for RTS and to a similar level in the majority of patients in whom this procedure is indicated.Level of EvidenceComparative retrospective study, Level III.  相似文献

BackgroundThe study’s aim was to summarize the recommendations given by members of the European Hip Society (EHS) regarding sport activities after total hip arthroplasty (THA).MethodsMembers of the EHS were invited to complete an online web-based questionnaire including recommendations for 47 sports disciplines. The questions regarding the specific sports were also divided into 4 subcategories: “allowed,” “allowed when experienced,” “not allowed”, and “no opinion.” Four intervals for resuming the sports activities after THA were evaluated: within 6 weeks after THA, 6 to 12 weeks after THA, 12 weeks to 6 months after THA, and more than 6 months after THA. Consensus on resuming sports was analyzed.ResultsA total of 150 (32.9%) EHS members participated in the survey. Agreement was found for 5 sports activities in the first 6 weeks after THA, for 10 activities, 6 to 12 weeks after surgery, for 26 activities, 3 to 6 months after THA, and for 37 of 47 activities, 6 months after surgery. Sports activities which were not allowed after THA were handball, soccer and football, basketball, full contact sports, and martial arts.ConclusionThis is the first report describing the recommendations of European hip arthroplasty surgeons on resuming sport activity after THA. Most physical activities were allowed for the patients 6 months after THA. The experience of the patient in performing a distinct sport activity did not influence the recommendations to return to former sports activities. European surgeons are progressively mitigating restrictions to sports after THA. Further studies should evaluate the effects of this trend on patients' outcome and implant survival.  相似文献
