目的探讨荷负电气溶胶促进烧伤患者头部取皮区创面愈合的效果。方法将120例患者随机分为气溶胶组(61例)和对照组(59例),两组均采用电动取皮机取头皮,厚度0.25mm。取皮后对照组采用常规处理;气溶胶组采用气溶胶对头部取皮区创面进行治疗护理,每次2h,每天2次,直至创面愈合。结果两组创面疼痛消失、创面渗出物消失情况及愈合时间比较,差异有显著性意义(P〈0.05、P〈0.01)。结论荷负电气溶胶能有效促进烧伤患者头部取皮区创面愈合。  相似文献

荷负电气溶胶治疗大鼠烧伤创面后表皮干细胞的变化   总被引:1,自引:0,他引:1  
目的探讨荷负电气溶胶对烧伤创面表皮干细胞影响以及其促进创面愈合的机制。方法将清洁级健康Wistar大鼠48只随机分为A、B两组,每个动物背部两侧分别制成实验创面或对照创面,实验创面以荷负电气溶胶照射,A组动物每日照射2次,B组每日照射1次;利用干细胞表达β1整合素的特性,加入整合素β1抗体,通过免疫组织化学染色检测创面表皮干细胞含量的变化,比较各时相点两组创面间的差异。结果A、B两组各时相点实验创面的表皮干细胞数目是同组对照创面的1.5到2倍;而A组实验创面的表皮干细胞数又较B组实验创面增多。结论经用荷负电气溶胶照射大鼠烧伤创面能促进表皮干细胞的增殖。  相似文献

目的 观察荷负电气溶胶治疗对大鼠烫伤创面愈合过程中白细胞介素(IL)-8、IL-10表达的影响,探讨荷负电气溶胶治疗促进创面愈合的作用机制.方法 制作SD大鼠深Ⅱ度烫伤模型,采用分组对照方法,将40只鼠随机分为治疗组(n1=20)和对照组(n2=20).治疗组应用荷负电气溶胶治疗,每次1.5 h,每天2次,直至创面愈合;对照组不作荷负电气溶胶治疗.伤后第1~11天分别取创面标本制作切片,采用免疫组织化学和图像分析方法,检测创面愈合过程中IL-8和IL-10表达水平.结果 创面平均愈合时间治疗组为(7.00±1.15)d,对照组为(9.00±1.34)d,治疗组创面愈合时间明显提前(P<0.01).免疫组织化学显示,两组IL-8均在伤后第1天开始表达.主要位于多核粒细胞和单核细胞;第3天表达明显增多达高峰,并见大量成纤维细胞表达,治疗组的峰值明显低于对照组,差异有统计学意义(P<0.01),第5~11天表达水平迅速下降.两组IL-10伤后第1天在淋巴细胞和单核细胞均有表达;第3天开始有角质细胞表达并达高峰,第5~11天表达水平缓慢下降,但治疗组要明显高于对照组,第3~11天差异有统计学意义(P<0.01).结论 荷负电气溶胶治疗能有效抑制创面IL-8的表达及促进IL-10的表达,缩短炎症进程,从而加速创面愈合.
Abstract:
Objective To discuss the influence of aerosol bioelectricity on the expression of interleukin (IL) -8 and IL-10 in wound healing of burned rats. Methods The deep Ⅱ degree scalding models were established in Sprague Dawley (SD) rats. Rats were randomly divided into experimental group (n1 =20) and control group (n2 =20). The rats in experimental group were treated with aerosol bioelectricity.Samples were collected at the first to eleventh day post-scalding. Immunohistochemistry and image analysis methods were conducted to examine the expression of IL-8 and IL-10 in both experimental and control groups. Results The average wound healing time in experimental group was 7. 00 ± 1. 15 days, and that in control group was 9. 00 ± 1. 34 days. IL-8 and IL-10 were observed mainly in polylmorphonuclear and mononuclear cells in both experimental and control groups on the 1 st day. On the third day, fibroblasts abounded, IL-8 expression was increased evidently and reached a peak. The peak value (6. 73 ± 1. 36) in experimental group was lower significantly than that in control group ( 2. 85 ± 0. 72, P < 0. 01). From the 5th to 11th day, IL-8 expression was declined rapidly. IL-10 was expressed in keratode cells and had the peak value in experimental group (1. 24 ±0. 15) and control group (5. 69 ± 1. 32) on the 3rd day. IL-10 expression was declined gradually from the 5th to 11th days. The expression level of IL-10 in experimental group was significantly higher than in control group from the 3rd day to 11th days post-scalding (P<0. 01). On the 3rd day, both IL-8 and IL-10 in experimental and control groups were expressed abundantly , and there was negative relationship between them (r = - 0. 862, P < 0. 01). Conclusion Aerosol bioelectricity can indicate active cells proliferation through down-regulating the expression of IL-8 and up-regulating the expression of IL-10, accelerating burned wound healing.  相似文献

目的 观察脂质体为主要成份的外用药物速愈平对 °烧伤创面的治疗作用。方法  1998年 10月~ 1999年 10月 ,选择患者伤前身体健康 ,伤后 2 4小时内入院 ,创面深浅 °混合为主 ,左右两侧创面比较对称 ,深度基本一致 42例 ,采用自身对照方法 ,清创后右侧创面外用速愈平 ,左侧创面外用磺胺嘧啶银霜 (SD- Ag)。定期观察创面 ,计算愈合率 ,记录创面情况 ,直至患者创面痊愈出院。结果 平均每例用于观察的速愈平使用创面为 (11.2± 7.3) % ,SD- Ag使用创面为 (9.4± 5 .8) % ;速愈平治疗创面愈合较 SD- Ag快 ,速愈平平均愈合时间为 (18.4± 4.7)天 ,SD- Ag平均愈合时间为 (2 3.5± 7.9)天 ,经统计学处理有显著性差异 (P<0 .0 5 )。速愈平用药后出现创面疼痛、过敏和痂裂出血等情况较 SD- Ag少。所有患者都无严重的不良反应 ,未见用药后发生造血系统、肝、肾功能等实验室检查异常。结论 速愈平对促进 °烧伤创面愈合、创面疼痛、过敏和痂裂出血等情况较少 ,有其独到之处。  相似文献

为研究丹参对Ⅱ度烧伤创面皮肤活力的影响,探讨其对创面愈合的作用,以20只日本大耳白兔制成Ⅱ度烧伤模型,分为丹参治疗组和对照组,对创面组织琥珀酸脱氢酶活性、过氧化脂质含量、含水量及组织形态学进行了动态观测。结果发现,治疗组琥珀酸脱氢酶活性高于对照组,在受伤8小时以后的各时间点两者均有显著差异(P<0.05);而创面过氧化脂质含量及含水量治疗组低于对照组,在伤后8,24及48小时两组均有显著差异;组织学观察治疗组创面愈合过程优于对照组。认为,丹参有助于保护烧伤创面残存的上皮细胞,提高愈合质量  相似文献

目的 了解重组人粒细胞巨噬细胞集落刺激因子(rhGM-CSF)凝胶剂治疗深Ⅱ度烧伤创面的有效性和安全性. 方法采用多中心、随机、双盲、安慰剂平行对照的研究方法,将深Ⅱ度烧伤创面作为目标创面,把321例烧伤患者(实际完成302例)分为用药组200例,创面涂rhGM-CSF凝胶;对照组102例,创面涂安慰剂.观察用药后两组患者的全身情况和不良反应,以及不同时相点创面愈合时间、愈合率、总有效率. 结果用药后两组患者生命体征,血、尿常规及肝、肾功能,创面分泌物、创缘反应比较,差异均无统计学意义(P>0.05),无不良反应.用药组创面愈合时间的中位数为17 d,低于对照组(20 d,P<0.01).用药第8、14、20、28天,用药组平均创面愈合率分别为24.5%、70.5%、95.3%、99.6%,均高于对照组(15.1%、51.4%、84.6%、97.1%,P<0.01).用药8、14、20 d用药组的总有效率显著高于对照组(P<0.01). 结论 rhGM-CSF凝胶剂能促进深Ⅱ度烧伤创面愈合,并且有一定的安全性.  相似文献

目的回顾性分析几种深Ⅱ度烧伤创面的修复方法,探讨改善创面微循环对创面愈合的意义. 方法 (1)对于笔者单位烧伤患者的深Ⅱ度创面,应用削痂疗法治疗614例、磨痂疗法治疗32例、清创后异体皮覆盖86例、外用磺胺嘧啶银后创面暴露1 836例、外用中药京万红烫伤膏包扎治疗408例.统计、分析各种疗法的治疗效果.(2)制作大鼠深Ⅱ度烫伤模型.伤后5 min内分别由其尾静脉注入等渗盐水(对照组,10只)、巴曲酶(治疗组,10只),创面均外用磺胺嘧啶银.测定两组大鼠伤前及伤后0.5-72.0 h的创面皮肤血流灌注单位,计算其伤后14、18 d的创面愈合率、收缩率及创面愈合时间.用组织学方法观察两组大鼠创面愈合后的皮肤毛囊数. 结果 (1)削痂疗法术后2-3周创面愈合,其中烧伤总面积50%~79%TBSA的患者治愈率94.8%,总面积80%~98%TBSA者治愈率93.4%.磨痂疗法磨痂+异体皮覆盖术后(13.8±2.1)d创面愈合,无瘢痕形成.清创后异体皮覆盖其中82例患者术后(18.0±2.3)d创面愈合.外用磺胺嘧啶银后暴露其中1 658例患者用药后(26.0±3.2)d痂下愈合.外用京万红烫伤膏后包扎患者多有细菌感染,其中下肢创面愈合时间为(26.0±2.8)d.(2)治疗组大鼠伤后2.0-72.0 h创面局部血流灌注单位均明显高于对照组(P<0.01).伤后14、18 d,治疗组创面愈合率明显高于对照组(P<0.01),但两组创面收缩率接近(P>0.05).治疗组创面愈合时间短于对照组(P<0.01).伤后30 d,对照组大鼠真皮层中残存少量毛囊,数量明显少于治疗组(P<0.01). 结论深Ⅱ度烧伤后早期采用削痂、磨痂或清创后覆盖异体皮的方法处理创面,可减轻感染、缩短疗程、提高治愈率和愈合质量.使用巴曲酶可改善深Ⅱ度烧伤创面微循环,加快愈合速度.  相似文献

目的探讨浅Ⅱ度烧伤创面愈合过程中相关生长因子及其受体的作用。方法采用大鼠浅Ⅱ度烫伤模型,观察了大鼠浅Ⅱ度烫伤后创面愈合过程中组织学、炎性细胞、表皮细胞周期的变化,检测了创面相关生长因子及受体的基因表达。结果炎性细胞到达创面依次为中性粒细胞、巨噬细胞和淋巴细胞;表皮细胞周期变化为伤后3天细胞增殖活跃,伤后7天细胞分裂明显增多;伤后1,3天 PDGF(血小板源性生长因子)、PDGFR(血小板源性生长因子受体)和 EGFR(表皮生长因子受休)基因表达明显增强,伤后3,5天 EGF(表皮生长因子)、TGFβ-R_2(转化生长因子β受体α)基因表达增强,伤后5,7天 TGFβ-R_1(转化生长因子β受体1)基因表达增强,伤后7,10天 TGFβ_1(转化生长因子β)基因表达增强。结论烧伤诱导相关生长因子及受体的基因表达,并且具有时相性和可逆性;相关生长因子基因表达与表皮细胞周期间可能存在相互调控作用,生长因子是调控创面愈合的主要因素。  相似文献

目的 探讨深Ⅱ度烧伤患者伤后 2 4h内创面行削痂术的安全性和临床疗效。 方法12例有削痂手术指征并在伤后 2 4h内行削痂术的深Ⅱ度烧伤患者为A组 ;14例削痂条件相似并按常规在伤后 4~ 6d行削痂术的深Ⅱ度烧伤患者为B组。比较两组休克期补液量、休克征象发生率、回吸收期的生命体征、尿量及愈合时间。 结果 两组患者在休克期补液量、休克征象发生率方面差异均无显著性意义 (P >0 .0 5 ) ;A组休克期尿量明显增多 ,回吸收期的体温、心率与B组明显不同(P <0 .0 5~ 0 .0 1) ;A组创面平均愈合时间较B组短 (P <0 .0 1)。 结论 深Ⅱ度烧伤创面于伤后2 4h内削痂是安全的 ,并能缩短创面愈合时间。  相似文献

目的 了解自体脂肪颗粒及微粒皮混合移植修复大面积烧伤深度创面的效果. 方法选择20例重度烧伤患者,采用自身同体对照法,将患者双侧肢体或躯干对称部位创面分为脂肪颗粒+微粒皮组和微粒皮组,分别行自体脂肪颗粒+微粒皮(体积比1:1)混合移植和自体微粒皮移植.术后30、45、60 d计算2组创面愈合率;术后7、14、21、28 d取创面组织,行HE染色和增殖细胞核抗原(PCNA)免疫组织化学观察. 结果移植术后30、45、60 d,脂肪颗粒+微粒皮组创面愈合率分别为(56.3±3.1)%、(76.4±6.1)%、(96.2±1.5)%,均明显高于微粒皮组的(28.3±2.0)%、(47.3±4.8)%、(85.4±2.2)%(P<0.01).HE染色显示脂肪颗粒+微粒皮组创面上皮化早于微粒皮组,胶原纤维排列较整齐.脂肪颗粒+微粒皮组PCNA阳性细胞较微粒皮组多,主要分布于表皮基底层. 结论自体脂肪颗粒+微粒皮混合移植可促进创面愈合.  相似文献

烧伤后24小时内削痂对深Ⅱ度创面局部炎症反应的影响   总被引:17,自引:1,他引:16  
目的探讨烧伤后24h内行削痂手术对深Ⅱ度创面局部炎症反应以及组织损害的影响。方法选择12例烧伤患者,伤后24h内对深Ⅱ度创面行削痂手术,将患者同一创面分为3个标本采集区,即手术前、手术后和未手术区。手术前标本取自削痂术前创面,手术后和未手术区创面标本于伤后5—7d获取。采用组织培养和比色法测定创面组织释放白细胞介素(IL)8、髓过氧化物酶(MPO)和丙二醛(MDA)的水平,通过HE和Masson染色对创面组织坏死程度进行形态学观察。结果削痂手术后,患者创面组织局部释放IL8、MPO、MDA的水平分别为(6.83±1.85)μg/L、(4.07±0.87)U/g、(8.94±5.66)μmol/g,与未手术创面比较明显降低(P<0.01)。形态学观察显示,手术前创面有凝固性坏死灶;未手术创面炎症反应明显,坏死组织范围扩大;削痂手术后创面局部的炎症反应得到改善,未见坏死组织范围扩大。结论伤后24h内行削痂手术,可以改善深Ⅱ度烧伤创面局部的炎症反应,防止创面进行性加深,有利于创面及早愈合。  相似文献

目的 前瞻性评价早期VSD治疗深Ⅱ度烧伤创面的临床疗效,为其临床应用提供依据.方法 选择笔者单位2009年5月-2010年3月收治的双下肢烧伤后3 h内入院、总面积小于10%且各下肢深Ⅱ度面积大于1%TBSA的患者22例.依照部位对称、深度相同、面积相近等同体对照原则,将每例患者创面分为VSD治疗组(应用VSD治疗)与对照组(应用10 g/L磺胺嘧啶银霜换药).观察2组患者创面的水分蒸发量、肿胀程度、细菌定植情况、疼痛程度、愈合时间及愈合质量并进行比较分析.数据行t检验与秩和检验.结果 21例患者完成试验,均在伤后4 h内完成创面处理.VSD治疗组正常皮肤及覆盖敷料前创面的水分蒸发量与对照组相近(t值分别为1.310、-0.911,P值均大于0.05);创面覆盖敷料2 h后,敷料表面的水分蒸发量[(44.3±3.9)mL·h-1·m-2]明显少于对照组[(66.1±6.4)mL·h-1·m-2,t=-11.39,P<0.01].伤后3、7 d,VSD治疗组大腿周径较伤后5 h分别增加了(3.48±0.35)、(2.51±0.21)cm,明显小于对照组的(8.02±0.41)、(3.99±0.32)cm(t值分别为4.110、3.569,P值均小于0.01).2组创面入院时及伤后10 d细菌培养阳性率组间比较,差异均无统计学意义(Z值分别为-0.894、0.000,P值均大于0.05);2组伤后10 d细菌培养阳性率均较各组入院时显著降低(Z值分别为-3.220、-3.870,P值均小于0.01).VSD治疗组创面伤后10 d的pH值(7.12±0.06)呈现弱酸性,对照组(7.41±0.13)则为中性.VSD治疗组伤后1、3、7 d创面疼痛程度轻于对照组(t值分别为-16.132、-21.230、-16.453,P值均小于0.01).2组创面愈合时间比较,差异无统计学意义(t=1.186,P>0.05).伤后2、3个月VSD治疗组创面愈合质量评价为佳(100.00%、100.00%),明显优于对照组(19.05%、85.71%,Z值分别为-11.638、-3.870,P值均小于0.01).结论 早期VSD治疗不能使深Ⅱ度烧伤创面愈合时间提前,但能显著提高其愈合质量,是处理深Ⅱ度烧伤创面的有效方法之一,值得临床关注与进一步研究.  相似文献

目的探讨应用异种(猪)脱细胞真皮基质一次性包扎治疗深Ⅱ度烧伤创面的临床应用效果。方法1997年1月—2004年1月,应用异种(猪)脱细胞真皮基质一次包扎治疗50%~95%总体表面积(TBSA)、深Ⅱ度烧伤的患者67例[异种(猪)脱细胞真皮基质治疗组];同期保痂治疗的50%~95%TBSA、深Ⅱ度的患者10例(保痂治疗组),观察创面愈合时间和愈合质量及其并发症发生情况。治愈患者经过3个月~2年的随诊,观察瘢痕增生情况。结果异种(猪)脱细胞真皮基质治疗组深Ⅱ度创面中途基本不需换药,创面愈合时间缩短,平均(12.2±2.6)d,而保痂治疗组愈合时间为(27.4±3.5)d,差异具有统计学意义(P<0.05);同时,异种(猪)脱细胞真皮基质治疗组瘢痕增生情况较保痂治疗组明显减轻或者无瘢痕增生。结论一次性覆盖异种(猪)脱细胞真皮基质可有效地治疗深Ⅱ度烧伤创面,能加快创面愈合,减轻瘢痕增生,从而降低烧伤感染和炎症反应综合征的发生。  相似文献

This article summarizes methods of repair of massive and deep wounds, elucidates how to improve wound healing quality and avoid scar deformity after deep hum. A part of denatured dermis (non-necrotic)in deep partial-thickness burn, "mixed degree" burn, even in full-thickness burn wounds before forming eschar can be preserved and covered with autolo-gous skin, thereby to avoid secondary damage to the structure of subcutaneous tissue and the junction of dermis-adipose, thus to result in good functions, appearance, and survival rate. After skin grafting, wound healing quality and appearance are im-proved, joint function and elasticity of skin are enhanced, the degree of scar contracture is relieved due to preservation of nor-mal adipose tissue after escharectomy. The study of composite artifical skin will be actively developed in the future. Tissue-en-gineering skin and stem cells can be successfully used in pa-tients with deep burns for starless healing with restoration of physiological functions in a short period.  相似文献
