Treatment of non-seminomatous testicular cancer often leadsto infertility due to anejaculation/retrograde ejaculation andpoor sperm quality. In these men spermatozoa may be obtainedby transrectal electroejaculation (TE), but the optimal strategyfor assisted procreation in these couples is not known. Ouraim was to examine whether TE and conventional in-vitro fertilization(IVF) would be successful. A total of 10 couples, with long-standinginfertility due to anejaculation or retrograde ejaculation aftertreatment for testicular cancer 514 years earlier, werereferred to our unit. All men underwent diagnostic TE undergeneral anaesthesia. Spermatozoa were recovered in nine cases.The antegrade fraction was prepared and used for IVF. Spermquality was variable and conventional IVF was considered impossiblein three cases. Altogether six IVF treatment cycles in six couplesresulted in five pregnancies, of which four resulted in a deliveryand one resulted in a spontaneous abortion. One additional pregnancyis ongoing after transfer of cryopreserved embryos. The fertilizationrate was 54% (33/61) and the cleavage rate was 97% (32/33).No complications relating to the procedure have been encountered. 相似文献

Previous work from our laboratory has revealed that extracellular ATP is a rapid and potent activator of human sperm acrosome reaction and fertilizing ability. In the present study, we assessed the effects of in-vitro sperm incubation with ATP on fertilization and embryo development in couples undergoing in-vitro fertilization (IVF) for male factor infertility. Oocytes from 22 women undergoing ovulation induction were divided in two groups and inseminated in vitro either with selected spermatozoa from the corresponding partner suffering from male factor infertility pre-incubated with ATP (2.5 mM) for 1 h, or with spermatozoa incubated with 0.9% NaCl solution (control group). After insemination, fertilization was assessed by the presence of pronuclei and then by embryo cleavage. The fertilization rate in the group of oocytes inseminated with ATP-treated spermatozoa improved significantly with respect to the control group (65.7 versus 42.5%, P < 0.01). No significant differences were observed in embryo cleavage and embryo quality. Embryos from both treated and control groups were transferred together in 20 transfer procedures, and in two couples fertilization was not obtained. Nine pregnancies occurred: one biochemical, one miscarriage, and seven patients delivered 9 healthy babies. Two pregnancies were twin with an overall pregnancy rate of 40.9% per cycle and of 45% per transfer. In conclusion, the results of the present study demonstrate that, in humans, extracellular ATP induces a significant increase of sperm fertilizing potential, as these findings are a rationale for the use of ATP for in-vitro treatment of human spermatozoa during IVF. 相似文献

The objective of this study was to define several estimatesof the success rate of in-vitro fertilization and embryo transfertreatment in relation to treatment outcome. The data were collectedin a retrospective study of 591 patients treated during 3 yearsin one Belgian university hospital. Several models used forevaluating the treatment of infertility were fitted to the data(logistic model, binomial model, double binomial model, exponentialmodel, the Speirs model. The number of oocytes retrieved andthe proportion of fertilized oocytes at the first attempt canbe used as prognostic factors. Spinal cord repair is a challenging task that has puzzled clinical specialists and scientists for a long time. Zinc plays an important role in regulating the expression of brain-derived neurotrophic factor (BDNF) in nervous system, which can improve the pathological state of neurons and promote regeneration of injured neurons, reduce neuronal apoptosis. Our previous studies demonstrated that the serum zinc levels in SCI model group were significantly decreased and zinc concentrations in spinal cord were gradually increased in 24 h after SCI, which induces the up-regulation of zinc transporter 1 (ZnT-1). The mRNA levels of ZnT1 and BDNF were both increased after SCI, and there is a positive correlation between them. Excess zinc exposure has been proved to be a risk factor for neuron death in brain and spinal cord injuries, but supplement of the right amount of zinc may be useful in promoting the recovery of spinal cord function. 相似文献

This work was undertaken in order to evaluate retrospectively the relative efficacy of ovulation induction by menotrophins and that of in-vitro fertilization and embryo transfer (IVF-ET) in the treatment of unexplained infertility. These two treatments were compared between two groups of patients: 87 couples undergoing 446 cycles of ovulation induction by menotrophins (group A) and 72 couples undergoing 108 cycles of IVF-ET (group B). A total of 30 and 20 pregnancies were achieved in groups A and B for a rate of 34 and 28% per patient, respectively. A similar cumulative pregnancy rate (CPR) was achieved for three cycles of ovulation induction (23%) and one cycle of IVF-ET (22%). The cumulative live birth rate (CLBR) in group A was 22% after three cycles and exceeded that of one IVF-ET cycle (17%). It is concluded that menotrophin treatment in unexplained infertility is highly beneficial and should precede an IVF attempt. 相似文献

目的评估手术治疗下颈椎爆裂骨折合并脊髓损伤的临床效果。方法回顾性分析自2002年以来收治的12例下颈椎爆裂骨折合并脊髓损伤的临床资料。伤后2~8 d,4例采用前路减压植骨融合内固定术,8例采用1期前-后联合减压植骨融合内固定术。采用ASIA分级评估神经功能,用放射学方法评估植骨融合情况,记录围手术期并发症。结果平均随访11.2个月。1例伤后2 d接受手术,术后死亡,6例术后发生并发症,经对症处理治愈。3例术后需要机械通气支持。7例患者有不同程度神经功能恢复(ASIA分级提高1~2级)。随访期间未见内固定失败者。结论手术治疗下颈椎爆裂骨折可以达到神经组织减压和重建颈椎稳定性的目的 ,根据具体病情可以选择单纯前路手术或者1期前后路联合手术。 相似文献

The innate and adaptive arms of the immune system, represented principally by macrophages and by T and B cells, respectively, provide body tissues with mechanisms of defence, protection and repair. In the central nervous system (CNS), probably because of its status of ‘immune privilege’, any immune activity has long been viewed as detrimental. Recent studies have provided evidence, however, that immune activity after traumatic CNS injury may have a beneficial effect, manifested by promotion of regeneration and reduction in the secondary degeneration of neurons that escaped direct injury. Rigorous regulation of immune system activity allows the individual to derive the benefit of such neuroprotection without the risk of detrimental side effects. 背景:依达拉奉是一种自由基清除药,可以减轻受损神经组织水肿和改善脊髓损伤区微环境。
目的:观察依达拉奉联合神经干细胞移植对大鼠脊髓全横断损伤的修复效果。
方法:成年雌性SD大鼠80只,建立胸9脊髓全横断损伤模型,随机分为4组:对照组不做处理;依达拉奉组脊髓损伤后6 h经尾静脉注射依达拉奉;神经干细胞移植组脊髓损伤后6 h脊髓损伤区域注入神经干细胞悬液;依达拉奉+细胞移植组脊髓损伤后6 h神经干细胞移植的同时尾静脉注射依达拉奉。
结果与结论:造模后8周可观察到PKH-26标记的神经干细胞在体内存活并在脊髓内迁移;细胞移植组和依达拉奉联+细胞移植组可见少量连续性神经纤维通过损伤区。荧光金逆行脊髓追踪显示神经干细胞移植组和依达拉奉+细胞移植组可见被荧光金标记的神经锥体细胞穿越损伤区。PKH-26标记的阳性细胞数及荧光金阳性神经纤维数:依达拉奉+细胞移植组最多,依达拉奉组、神经干细胞移植组次之,对照组最少,各组之间差异有显著性意义(P < 0.05);后肢功能运动BBB评分依次为依达拉奉+细胞移植组>神经干细胞移植组>依达拉奉组>对照组。提示依达拉奉能促进神经干细胞在损伤区的存活并向神经细胞分化,依达拉奉联合神经干细胞移植有促进细胞移植修复大鼠脊髓损伤的效果。 相似文献

BACKGROUND:Sufentanil and propofol are both found to have good neuroprotective effects on neurological damage in clinical practice.
OBJECTIVE:To investigate the effect of propofol combined with sufentanil in umbilical cord blood mesenchymal stem cells transplantation for the treatment of spinal cord injury.
METHODS:Sixty-five Wistar rats were selected to make animal models of acute spinal cord injury using Allen’s method. Six hours after modeling, these rats were randomly assigned into combined group (injection of 2×107/L human umbilical cord blood mesenchymal stem cell suspension (0.5 mL) plus injection of 1.0-1.5 mg/kg propofol and 0.5 μg/kg sufentanil via the tail vein), stem cell group (injection of 2×107/L human umbilical cord blood mesenchymal stem cell suspension (0.5 mL) via the tail vein), or control group (injection of 30 μL of LDMEM containing 5% fetal bovine serum). S100β protein level in serum was detected in each group at 15 and 60 minutes after injection. Motor function of rat in each group was assessed by Basso Beattie Bresnahan (BBB) scoring and incline plane test at 1, 2, 4, 6, 8 weeks after modeling. Pathological changes of the spinal cord were observed at 4 weeks after modeling. Expression of vascular endothelial growth factor was detected using western blot assay at 1 and 2 weeks after modeling.
RESULTS AND CONCLUSION:After 15 and 60 minutes of intervention, S100β protein level was lowest in the combined group followed by the stem cell and control groups (P < 0.05). At 2, 4, 6, 8 weeks after modeling, scores on the incline plane test and BBB were ranked as follows: combined group > stem cell group > model group (P < 0.05). At 4 weeks after modeling, severe damage to the spinal cord and few nerve fibers were found in the control group; spinal cord hyperplasia and a few of regenerated axons and PKH-26-positive stem cells appeared in the stem cell group; while in the combined group, there were a large amount of PKH-26-positive stem cells and nerve axon-like structures. At 1 and 2 weeks after modeling, the highest protein level of vascular endothelial growth factor was found in the combined group followed by the stem cell group and control group (P < 0.05). To conclude, these findings indicate that propofol and sufentanil in umbilical cord blood mesenchymal stem cell transplantation therapy can promote the recovery of hindlimb function after spinal cord injury, thereby promoting the functional recovery of rats from spinal cord injury. 相似文献

This study evaluated outcome in 117 couples with unexplained infertility
who underwent 162 attempts at natural cycle in-vitro fertilization (NIVF)
between 1991 and 1993. An egg was obtained in 138 cycles and a single
embryo was transferred in 89 cycles. There were 16 implantations (four
biochemical pregnancies, three clinical abortions and nine live births).
The implantation rate per embryo was 16/89 (18.0%), which translated into a
live birth rate per egg collection of 9/138 (6.5%). The impact factors that
were assessed included oocyte quality, sperm quality, embryo quality and
woman's age. The outcome measures used were fertilization/inseminated egg
and implantation/replaced embryo. All embryo transfers were of single
embryos. We conclude that, in couples with unexplained infertility, outcome
following NIVF is affected by both egg and sperm quality and by the age of
the woman. Embryo quality was independent of the above factors but was also
critical for successful implantation.

During the years 1984–1992, 951 couples completed 2252in-vitro fertilization (TVF) treatment cycles at the In-VitroFertilization Unit of The Chaim Sheba Medical Centre. This studywas conducted to evaluate the success of IVF using the cumulativepregnancy rate (CPR), with special emphasis on the optimal numberof treatment cycles, the age of the patients and female infertilityfactors. It was found that the CPR showed a constant rise duringthe six initial IVF treatments (56% CPR), and plateaued in thesubsequent three cycles (63% CPR). Various female infertilityfactors did not influence this rate. Women >40 years of agehave a significantly lower CPR. Thus, it was concluded thatthe CPR in IVF declined after the sixth initial treatment cycle,and in women >40 years of age. The infertility factor didnot significantly influence CPR. 相似文献

背景:脊髓损伤后RhoA表达增高是神经再生困难的主要原因之一,本课题在前期研究证明自聚合肽纳米纤维材料能较好地促进脊髓损伤后结构与功能修复的基础上,构建了自聚合肽纳米纤维材料与RhoA-siRNA复合材料用于修复脊髓损伤。
目的:探讨自聚合肽纳米纤维材料介导siRNA干扰RhoA表达促进脊髓损伤修复。
方法:54只昆明小鼠随机数字表法分为4组。假手术组仅作脊髓暴露,其他3组在切除1 mm脊髓组织制备全横断脊髓损伤模型后,分别于损伤腔内填充生理盐水、自聚合肽纳米纤维支架或含有RhoA特异性siRNA复合物。通过FAM标记检测siRNA转染效率;免疫组化检测RhoA表达;免疫组化和定量分析检测再生神经纤维;行为学检测评定后肢功能恢复。
结果与结论:移植FAM标记的含有RhoA特异性siRNA复合物后,在脊髓内的神经纤维及大脑皮质运动区神经元胞体内均可检测到FAM荧光信号,提示siRNA可从复合材料中释放到组织中并成功导入靶细胞。与SAPNS组和生理盐水组相比,含有RhoA特异性siRNA复合物和自聚合肽纳米纤维支架组可显著降低RhoA在神经元的表达,提高脊髓损伤区内NF阳性神经纤维的密度,促进后肢功能的恢复。结果提示通过自聚合肽纳米纤维支架的介导,含有RhoA特异性siRNA复合物能有效干扰RhoA表达,从而促进脊髓损伤的修复。 相似文献

背景:电针在脊髓损伤的康复过程中有其一定的临床作用,同时以嗅鞘细胞为代表的细胞治疗在部分患者的康复过程中亦起到了一定的作用,两者是否具有协同应用尚不清楚。
目的:观察督脉电针联合嗅鞘细胞移植对前脊髓综合征大鼠神经营养因子3水平及P75NTR表达的影响。
方法:将40只成年大鼠随机分为对照组、督脉电针组、嗅鞘细胞移植组、督脉电针+嗅鞘细胞移植组,经嗅鞘细胞移植和督脉电针治疗2周后 ELISA法检测脊髓组织神经营养因子3水平,免疫组化检测P75NTR的表达。
结果与结论:督脉电针+嗅鞘细胞移植组神经营养因子3水平较对照组及其他实验组高(P < 0.05);嗅鞘细胞移植组和督脉电针+嗅鞘细胞移植组的脊髓损伤部位以及相邻的组织内均有P75NTR表达,且督脉电针+嗅鞘细胞移植组阳性表达明显较嗅鞘细胞移植组多。实验证实督脉电针联合嗅鞘细胞移植能够明显增高大鼠前脊髓综合征邻近组织的神经营养因子3水平;督脉电针可以有效促进移植的嗅鞘细胞在宿主内存活。 相似文献

Subacute combined degeneration of the cord which occurred as a result of dietary deficiency of folic acid in a patient with a psychotic illness, is described. A significant improvement in the neurological deficit occurred following treatment with folic acid. 目的:利用Scopus数据库文献检索和深度分析功能,对干细胞移植治疗脊髓损伤研究的6年文献资料进行多角度的探讨分析。
设计:文献计量学分析。
资料提取:由第一作者以“neural stem cells (干细胞),spinal cord injury or spinal injury (脊髓损伤),cell transplantation (细胞移植),spinal transection (脊髓横断)”为关键词检索Scopus数据库2006-01/2011-12的相关文献,并将分析结果及资料导出,以文字和图表的形式进行统计和计量分析,描述其分布特征。
入选标准:纳入标准:检索干细胞移植治疗脊髓损伤研究相关的文献。文献类型包括研究原著、综述、会议记录及摘要、快报文章、编辑素材。排除标准:与文章目的无关的文献,大于6年较陈旧的文献,未发表的文章以及需电话追踪和手工检索逐一分析的文献。
主要数据判定指标:以文献的类型、出版时间、发表文献的作者分布、学科类别、机构分布、国家地区分布、来源期刊、及文献被引情况进行相关分析。
结果:Scopus数据库2006/2011收录的文献中共检索到1 080篇干细胞移植治疗脊髓损伤研究相关的文献,研究原著以607篇位居首位,其中有15篇可以确定为经典文献。在时间分布上,文献数量2006/2011总体呈上升趋势,来源出版物呈分散情况。高被引研究原著类文章主要发表在Journal of Neuroscience (《神经科学杂志》)上,目前已发表文献中以美国为主,占全球相关领域发稿量的28.5%。中国在过去6年间在数据库中收录文章总量中排在第2名,共发表212篇相关文章,占全球相关文章的19.6%。 《神经外伤杂志》发表文献量22篇,占全部文献的15.09%。其次为Journal of Neuroscience Research《神经科学研究杂志》21篇。

背景:单纯神经干细胞移植已应用于对受损脊髓组织的修复。
目的:以神经干细胞移植同时应用高压氧治疗大鼠脊髓损伤,观察联合作用对脊髓损伤大鼠运动功能恢复的影响。
方法:雌性SD大鼠60只,以半切法制成胸段脊髓半横断大鼠模型。随机分成单纯损伤组、神经干细胞移植组及高压氧治疗组,每组20只。伤后第4周取材行病理切片苏木精-伊红染色及BrdU免疫组织化学染色,第8周取材行辣根过氧化物酶示踪,透射电镜观察轴突的再生情况,通过体感诱发电位观察神经电生理恢复情况。造模后1,2,4,6,8周进行BBB评分和斜板实验等运动功能检测。
结果与结论:观察伤后4周病理切片,单纯损伤组未见神经轴索通过,神经干细胞移植组可见少量神经轴索样结构,高压氧治疗组可见较多神经轴索样结构。BrdU的阳性细胞数及辣根过氧化物酶阳性神经纤维数,高压氧治疗组最多,神经干细胞移植组次之,单纯损伤组最少,且各组之间差异有显著性意义(P < 0.05)。透射电镜下神经干细胞移植组、高压氧治疗组正中横断面可见新生的无髓及有髓神经纤维。高压氧治疗组大鼠体感诱发电位的潜伏期短于神经干细胞移植组,波幅高于神经干细胞移植组(P < 0.05),明显优于单纯损伤组(P < 0.01)。伤后4周神经干细胞移植组、高压氧治疗组大鼠后肢运动功能均有较明显恢复,高压氧治疗组较神经干细胞移植组恢复快(P < 0.05);单纯损伤组亦有所恢复,但程度较轻。提示神经干细胞移植对于脊髓损伤大鼠后肢功能的恢复有促进作用,联合应用高压氧有协同效果。 相似文献

BACKGROUND: There is a high morbidity after spinal cord injury, and the therapeutic strategy is limited to early surgical intervention, medication and post-treatment exercise that only can improve the motor function slightly. However, there is no effective cure method.OBJECTIVE: To study the effect of partition-type spinal cord catheter combined with bone marrow stromal stem cells on T8 spinal cord transection damage in rats.METHODS: Fifty rats were randomized into five groups (n=10 per group): group I, T8 spinal cord transection (5 mm) was made in rats with no treatment; group II, the partition-type tube was inserted into the injured site after modeling; group III, partition-type tube combined with bone marrow stromal stem cells was implanted into the injured site after modeling; group IV, partition-type tube combined with polyglycolic acid fibers was implanted into the injured site after modeling; group V, partition-type tube combined with bone marrow stromal stem cells and polyglycolic acid fibers was implanted into the injured site after modeling. RESULTS AND CONCLUSION: At 2 and 12 weeks postoperatively, Basso, Beattie and Bresnahan scores were significantly higher in the groups III and IV than the groups I, II, IV (P < 0.05). At 12 weeks postoperatively, the latency of motor evoked potential below the injury plane was significantly decreased in group V compared with groups I, II, III, IV (P < 0.05). Immunohistochemical results displayed that in the groups III and V, regenerated nerve fibers grew positively and arranged orderly among the tubes, and there was no obvious winding phenomenon. Under transmission electron microscopy, a certain number of myelinated nerve fibers were found as bridges among groups. These findings indicate that the partition-type chitosan tube combined with bone marrow stromal stem cells has a good connection with the injured spinal cord a good connection to restore part of electrophysiological properties, accelerate the axon regeneration, recover the motor function, thereby providing a new direction for the treatment of spinal cord injury.

We report five cases in which no oocytes were retrieved afterstandard ovarian stimulation for in-vitro fertilization (IVF),and in which it was found that mistakes had been made at thetime of human chorionic gonadotrophin (HCG) administration.In all five cases, oocyte retrieval was achieved after injectingHCG, when necessary, and reprogramming aspiration 2436h later. A mean of 7 ± 3.2 MII oocytes were recoveredper patient and 3.2 ± 0.8 embryos were transferred. Threeclinical pregnancies were obtained, and four healthy infantswere born. In our programme, these were the only cases of emptyfollicle syndrome (EFS) that appeared over a total of 1118 cycles,and were all explained by human error in the administrationof HCG. Our experience shows that human error could be considereda significant factor in the aetiology of empty follicle syndrome,and that EFS may be in part avoided by taking simple preventivemeasures. 相似文献

外加直流电场刺激(EFS)可抑制脊髓损伤(SCI)后损伤组织局部Ca2+内流,有效抑制脊髓继发性损伤的发生。但电场刺激结束后,损伤局部Ca2+会重新开始内流并激发后续病理生理学连锁反应,影响了EFS治疗SCI的效果。聚乙二醇(PEG)是一种亲水性的高分子材料,具有促进细胞膜融合及受损细胞膜修复的作用。本文旨在研究EFS联合PEG治疗Sprague-Dawley(SD)大鼠SCI的效果。脊髓损伤后的SD大鼠被随机分为对照组(无治疗,n=10)、电场组(EFS治疗30 min,n=10)、PEG组(浸润50%PEG的明胶海绵覆盖损伤处5 min,n=10)和联合组(电场刺激和PEG联合治疗,n=10)。术后不同时间进行运动诱发电位(MEP)测量、运动行为学评分以及脊髓切片染色分析。研究结果表明,术后8周联合组大鼠MEP潜伏期差和波幅差以及脊髓空洞面积比均显著低于对照组、电场组和PEG组,而运动功能评分和脊髓神经组织残余面积比均显著高于对照组、电场组和PEG组。以上结果提示,联合治疗方法能减轻大鼠脊髓损伤后的病理损伤程度,促进大鼠电生理及运动功能的恢复,其疗效优于EFS和PEG单独使用时的效果。 相似文献

方法:将48只SD雄性大鼠随机数字表法均分为单纯股骨骨折组和股骨骨折合并脊髓损伤组,通过双侧小切口开放截骨造成股骨中端横行骨折并植入内固定建立股骨骨折模型,改良Allen法自制打击装置造成大鼠T10段脊髓急性挫伤性损伤,两种方法融合制造股骨骨折合并脊髓损伤模型,观察大鼠制模成功后不同时间点大体情况及4周后骨折断端愈合情况。
结果与结论:建模后骨折合并脊髓损伤动物模型均成活,双下肢感觉及运动功能丧失,但可利用双上肢缓慢匍匐向前移动,前3 d进食少,活动少,夜间采用尾端悬吊后,骨折患肢未出现缺血坏死,到达第4周时,单纯股骨骨折组死亡1只,股骨骨折合并脊髓损伤组共死亡4只,成活率为83.33%,髓腔内固定未见脱出,两组骨折断端均有连续骨痂生成,从体积上来看,股骨骨折合并脊髓损伤组骨痂组织明显大于单纯股骨骨折组。证实将改良Allen法与小切口股骨外侧开放截骨法结合后,简单易行,可以成功制作股骨骨折合并脊髓损伤动物模型并成活至第4周。
