PURPOSE: To compare three-dimensional (3D) time-of-flight (TOF) MR angiography, contrast-enhanced MR angiography, digital subtraction angiography (DSA), and rotational angiography for depiction of stenosis. MATERIALS AND METHODS: The study had Ethics Committee approval, and each patient gave written informed consent. Forty-nine patients (18 women, mean age, 67.2 years +/- 9.1 [+/- standard deviation], and 31 men, mean age, 63.1 years +/- 8.0) with symptomatic stenosis of internal carotid artery (ICA) diagnosed at duplex ultrasonography underwent transverse 3D TOF MR angiography with sliding interleaved kY acquisition and coronal contrast-enhanced MR angiography, followed by DSA and rotational angiography within 48 hours. MR angiography was performed at 1.5-T with a cervical coil. Contrast-enhanced MR angiograms were obtained after a bolus injection of 20 mL of gadobenate dimeglumine. Maximum ICA stenosis on maximum intensity projection and source images was quantified according to NASCET criteria. Correlations for 3D TOF MR angiography, contrast-enhanced MR angiography, DSA, and rotational angiography were determined by means of cross tabulation, and accuracy for detection and grading of stenoses were calculated. Data were evaluated with analysis of variance, Wilcoxon signed rank test, and McNemar test, all at significance of P < .05. RESULTS: Ninety-eight ICAs were evaluated at contrast-enhanced MR angiography, DSA, and rotational angiography, and 97 were evaluated at 3D TOF MR angiography. Correlations for contrast-enhanced MR angiography, 3D TOF MR angiography, and DSA relative to rotational angiography were r2 = 0.9332, r2 = 0.9048, and r2 = 0.9255, respectively. Lower correlation (r2 = 0.8593) was noted for contrast-enhanced MR angiography and DSA. Respective sensitivity and specificity for detection of hemodynamically relevant stenosis relative to rotational angiography were 100% and 90% for contrast-enhanced MR angiography, 95.5% and 87.2% for 3D TOF MR angiography, and 88.6% and 100% for DSA. Four of 31 severe stenoses were underestimated at DSA, and three were underestimated at contrast-enhanced MR angiography. Three severe stenoses were underestimated at 3D TOF MR angiography, and one was misclassified as occluded. Of 13 moderate (50%-69%) stenoses, one was overestimated at contrast-enhanced MR angiography, two were underestimated and three overestimated at 3D TOF MR angiography, and two were underestimated at DSA. CONCLUSION: DSA results in an underestimation of ICA stenosis compared with rotational angiography. Contrast-enhanced MR angiography correlates best with rotational angiography.  相似文献

BACKGROUND AND PURPOSEThe purpose of this study was to assess the effectiveness of contrast-enhanced fast three-dimensional (3D) MR angiography in depicting both the carotid and vertebral arteries in their cervical portions and to compare MR angiography with conventional angiography for the evaluation of arteriosclerotic disease.METHODSTwenty-seven patients with ischemic cerebral events in the anterior (n = 18) and posterior (n = 9) circulation underwent contrast-enhanced 3D MR angiography in the coronal plane. MR angiograms were examined in a blinded fashion by two observers independently. Stenosis was classified according to the appearance of the residual lumen (no stenosis, mild stenosis, moderate stenosis, severe stenosis, occlusion). Conventional angiography was used as the standard of reference.RESULTSProximal great vessels and carotid siphons were not assessable on MR angiograms in 35% of cases owing to limited coverage. All cervical and petrous segments of the internal carotid arteries (ICAs) and 93% of the extracranial vertebral arteries were assessable. Flow-related artifacts were observed in seven cases of severe stenosis, including three with signal void at the site of narrowing and four with signal loss in the distal ICA. Interobserver agreement was good and significant. Overall agreement between 3D MR angiography and conventional angiography was good for the anterior and posterior circulations despite a tendency toward overestimation of stenoses on MR angiograms. Clinically relevant stenoses and occlusions were correctly identified on 3D MR angiograms, providing good sensitivity and specificity.CONCLUSIONContrast-enhanced 3D MR angiography is a promising tool for assessing arteriosclerotic lesions of supraaortic vessels. Further studies with larger groups are required to determine its value for patient care.  相似文献

目的 探讨磁共振血管造影(MRA)、CT血管造影(CTA)在诊断颈动脉狭窄及内膜切除术后随访中的价值。方法 11例22支颈动脉行MRA、CTA、CT仿真内窥镜(CTVE)、多普勒超声(DUS)检查。12支结果与DSA对照。3支颈动脉重度狭窄者行内膜切除术,MRA采用二维、三维时间飞越法。CTA经最大强度投影(MIP)及遮盖表现显示法(SSD)三维重建。应用导航功能行CTVE成像。结果 颈动脉轻度狭窄8支,中度狭窄3支,重度狭窄5支,闭塞1支,5支正常。12支颈动脉与DSA对照:CTA评估血管狭窄与DSA相符者11支,MRA与DSA相符者9支,DUS与DSA相符者7支。CTA、CTVE显示斑块、壁血栓6支,3支手术证实。术后MRA示狭窄解除,CTA示斑块消失。结论 MRA、DUS可用于颈动脉狭窄的筛选及术后随访。CTA评估血管狭窄程度与DSA、手术比较有较好的一致性,并能直接显示钙化斑块。  相似文献

目的:探讨3D TRICKS MRA对颈动脉粥样硬化狭窄的诊断价值。方法:对43例临床疑诊颈动脉粥样硬化狭窄的患者行2D TOF MRA和3D TRICKS MRA检查,其中31例同期行DSA检查,以颈总动脉法(CC法)计算狭窄率。结果:颈动脉闭塞2支,重度狭窄10支,中度狭窄31支,轻度狭窄25支,正常18支。2D TOF MRA、3D TRICKS MRA与DSA对照,显示狭窄部位均与DSA所示相符。2D TOF MRA高估狭窄程度6支,3D TRICKS MRA高估狭窄程度1支。以DSA为标准,2D TOF MRA诊断颈动脉狭窄的敏感度、特异度、诊断符合率分别为91.6%,85.7%,90.3%(K=0.737);3D TRICKS MRA诊断颈动脉狭窄的敏感度、特异度、诊断符合率分别为97.9%,100%,98.4%(K=0.955)。两种方法对诊断颈动脉狭窄的敏感度、特异度和诊断符合率差异均具有统计学意义(P〈0.05)。结论:3D TRICKS MRA与DSA对评估颈动脉粥样硬化狭窄具有极好的一致性(K=0.955),明显优于2D TOF MRA(K=0.737,P〈0.05),是对颈动脉粥样硬化狭窄筛查、术前评估、术后随访的最佳检查方法,能基本替代DSA检查。  相似文献

Conventional time-of-flight (TOF) MR angiography (MRA) in carotid artery stenosis relies on flow-related enhancement to produce signal from vascular structures. Intravoxel phase dispersion, due to vortices, causes loss of signal and is the reason for the tendency to overestimate the degree of stenosis. In contrast-enhanced MRA, intravascular signal is mainly dependent on T1 shortening of the blood. We compared first-pass contrast-enhanced MRA (contrast-enhanced 3D gradient echo, ce3D GRE) and contrast-enhanced 2D TOF (ce2D TOF) sequences with an unenhanced 2D TOF in 13 patients with carotid artery stenosis, assessing delineation of the carotid bifurcation, enhancement of veins and grade of stenosis. The contrast-enhanced techniques produced more morphological detail, the ce3D GRE being superior to the ce2D TOF. Four carotid arteries were reclassified into lesser stenosis categories using the ce3D GRE technique. However, seven carotid arteries (27 %) were rated as nondiagnostic on the ce3D GRE, mainly due to masking of the carotid bifurcation by veins. The latter can be avoided by decreasing the acquisition time; on our 1.5-T system we could achieve a minimum time of 23 s per 3D GRE. Further reduction of acquisition time would be necessary to incorporate this method into clinical routine, requiring higher-performance gradients, which are not available in many UK hospitals. PURPOSE: To evaluate whether and to what extent greater number of projection images obtained at three-dimensional (3D) time-of-flight (TOF) magnetic resonance (MR) angiography versus conventional digital subtraction angiography (DSA) causes overestimation of internal carotid arterial (ICA) stenosis. MATERIALS AND METHODS: DSA (two or three projections), rotational angiography (16 or 32 projections), and 3D TOF MR angiography (12 projections) were performed in 47 stenotic ICAs of 38 symptomatic patients. Two observers independently measured maximum stenosis, and the mean differences among MR angiography, DSA, and rotational angiography were compared. RESULTS: Three rotational and five MR angiograms were nondiagnostic. Seven MR angiograms of ICA stenoses showed a signal void and were excluded from analysis. On the remaining 32 angiograms, mean differences in maximum stenosis for observers 1 and 2, respectively, were 7% (95% CI: 3%, 12%) and 8% (95% CI: 3%, 13%) at MR angiography versus DSA and 2% (95% CI: -2%, 7%) and -1% (95% CI: -5%, 3%) at MR angiography versus rotational angiography. ICA stenosis was graded significantly higher at MR angiography versus DSA, whereas, it was not overestimated at MR angiography versus rotational angiography. The difference in maximum stenosis at MR angiography versus DSA was significantly different from that of MR angiography versus rotational angiography. CONCLUSION: Apparent overestimation of ICA stenosis at 3D TOF MR angiography versus conventional DSA may be partly explained by the greater number of projection images available at 3D TOF MR angiography.  相似文献

BACKGROUND AND PURPOSE: Since 1996, several preliminary studies have shown the usefulness of contrast material-enhanced MR angiography for imaging supraaortic vessels. The aim of this study was to compare the accuracy of contrast-enhanced 3D MR angiography with that of digital subtraction angiography (DSA) in the evaluation of carotid artery stenosis. METHODS: A blinded comparison of first-pass contrast-enhanced MR angiography with conventional DSA was performed in 120 patients (240 arteries). MR angiography was performed with a 1.5-T magnet with gradient overdrive equipment, by using a coronal radiofrequency-spoiled 3D fast low-angle-shot sequence after the intravenous injection of gadodiamide. The guidelines of the North American Symptomatic Carotid Endarterectomy Trial for measuring stenosis of the internal carotid artery were applied on maximum intensity projection (MIP) images and conventional catheter angiograms. RESULTS: Grading of stenoses on MR angiograms agreed with grading of stenoses on DSA images in 89% of arteries. In the severe stenosis group (70-99%), agreement was 93%. All internal carotid occlusions (n = 28) and seven of nine pseudo-occlusions were accurately detected with contrast-enhanced MR angiography. The correlation between MR angiography and DSA for determination of minimal, moderate, and severe stenoses and occlusion was statistically significant (r = 0.91, P<.001). CONCLUSIONS: This investigation with a large number of patients confirms that contrast-enhanced MR angiography could become a diagnostic alternative to DSA in the treatment of patients with carotid artery disease.  相似文献

PURPOSE: Our goals were to assess image quality of time-resolved contrast-enhanced MR angiography (CE MRA), by using 3D data acquisition along with a parallel imaging technique that can improve temporal resolution and to compare this technique with 3D-time-of-flight (TOF) MRA in the postoperative assessment of extracranial (EC)-intracranial (IC) bypass surgery. METHODS: On a 1.5T imaging system, we performed CE MRA by using a 3D fast field-echo sequence in combination with a parallel imaging technique, to obtain images in the coronal plane centered at the postoperative site. Our patient group comprised 17 patients, including 13 after superficial temporal artery-middle cerebral artery (MCA) anastomosis, 3 after external carotid artery-MCA anastomosis, and one after extracranial vertebral artery-posterior cerebral artery anastomosis. Visualization of the anastomosis and the distal flow on the CE-MRA images was assessed comparatively with that on 3D-TOF MR angiograms obtained at the same time. In 6 patients, we also compared the efficiency of visualization on CE-MRA images with that on conventional angiograms. RESULTS: A temporal resolution of 0.8 s/frame could be achieved with the technique employed. The bypass was better demonstrated postoperatively on CE-MRA images than on 3D-TOF MR angiograms in 13 patients (76%), whereas the 2 methods were equivalent in 4 patients (24%). Good correspondence of results was observed in the 6 patients for whom CE MRA and conventional digital subtraction angiography (DSA) images were compared. CONCLUSION: CE MRA by using the parallel imaging technique can increase image acquisition speed with sufficient image quality. This technique is at least equivalent to 3D-TOF MRA to evaluate the postoperative status of EC-IC bypass.  相似文献

We review our preliminary experience with the use of three-dimensional (3D) time-of-flight (TOF) magnetic resonance (MR) angiography (MRA) in the assessment of intra- and extracranial aneurysms. Six patients were examined: Five had intracranial aneurysms and one had a cervical carotid pseudoaneurysm. A 3D rephased gradient recalled echo pulse sequence and maximum intensity projection (MIP) reconstruction algorithm were used. Magnetic resonance angiography, spin echo MR, and conventional angiography were retrospectively reviewed with specific regard to individual vessel visualization, aneurysm depiction, and presence of artifact related to acquisition techniques or MIP reconstruction. All aneurysms were detected on MRA, and anatomical correlation with conventional angiography was excellent. Significant problems included loss of visualization of small vessels, intraluminal signal loss in large vessels, subacute thrombus simulating flow on MIP reconstructions, and limited projections obtainable with MIP techniques. Adequate MRA assessment of aneurysms can be obtained using a combination of T1-weighted spin echo images and 3D TOF MRA. Review of all components of the MRA is required. MRA may be useful in screening asymptomatic patients for intracranial aneurysms as well as in the follow-up of patients treated with balloon occlusion.  相似文献

BACKGROUND AND PURPOSE: Time-of-flight (TOF) magnetic resonance angiography (MRA) is commonly used to visualize the carotid arteries; however, flow void artifacts can appear. Our purpose was to determine the frequency and diagnostic meaning of flow voids by using real patient data, as part of a larger study of MRA compared with the criterion standard, digital subtraction angiography (DSA). METHODS: In 1997-2000, 390 consecutive patients with sonographic findings suggestive of carotid artery stenosis were included in this study. All patients subsequently underwent three-dimensional (3D) TOF MRA and conventional DSA. The frequency of flow void artifacts on 3D TOF MRA images were compared with stenosis measurements on DSA images. RESULTS: We recorded 107 flow voids (16%) during 3D TOF MRA of 662 carotid arteries. DSA images were available for comparison in 102 cases. The median percentage of stenosis in this subgroup of flow voids on MRA images was 80%, compared with measurements on DSA images according to the North American Symptomatic Carotid Endarterectomy Trial (NASCET) criteria. Stenoses ranged from 36% to 100% (occlusion). Three flow voids (2.9%) were in the 0-49% range; 11 (10.8%), in the 50-69% range; and 86 (84.3%), in the 70-99% range. Two flow voids (2.0%) represented occlusions. The positive predictive value of a flow void artifact for the presence of severe (70-99%) stenosis was 84.3% (95% CI: 77.3%, 91.4%). CONCLUSION: Flow void artifacts represented severe stenosis in most of the arteries. According to our data, the assumption that flow voids on 3D TOF MRA images represent severe stenosis is justified.  相似文献

To evaluate the efficacy and reliability of 3D time-of-flight MR angiography (TOF MRA) as a noninvasive procedure, 27 patients with acute subarachnoid haemorrhage (SAH) were studied with MRA immediately before or after intra-arterial digital subtraction angiography (DSA). 3DTOF MRA was performed with an axial slab of 60 mm centred on the circle of Willis and isotropic voxels. DSA showed 22 aneurysms and 1 dural arteriovenous fistula in 21 patients; the aneurysms ranged in size from 2 to 8 mm. MRA failed to show 2 small aneurysms, at the origin of the posterior and anterior communicating arteries. The 3D display of the intracranial vessels obtained with maximum intensity projection (MIP) or targetted MIP sometimes rendered the aneurysms better than DSA. We studied eight consecutive patients with clinical and intra-arterial angiographic (DSA) diagnosis of internal carotid artery (ICA) dissection to analyse the accuracy of MRA in dissections, using a new technique with presaturation pulses. Spin-echo images of the head were followed by three-dimensional time-of-flight (TOF) MRA at the site of the dissection, with and without a special caudal saturation pulse in addition to the cranial one. The accuracy of MRA was assessed in 64 segments of 16 ICA, all examined with DSA and MRA. High-signal intramural haematoma in the ICA at the level of the dissection was observed in all patients either on the maximal-intensity projection (MIP) reconstructions or on the partitions with this presaturation pulse technique. MRA had a sensitivity to detect dissected vessels of 100 %. Specificity for vessels correctly identified as not having a lesion was also 100 %. There was good correlation between DSA and MRA in demonstrating the site of the dissected ICA segment and the degree of stenosis. PURPOSE: Spontaneous dissection of the carotid and vertebral arteries represents a rare pathology. Its pathogenesis, probably multi-factorial, can be related to neck trauma and to a genetic basis, extensively demonstrated in the cases of association with type IV fibromuscular dysplasia, Ehlers-Danlos syndrome, Marfan's syndrome, pseudoxanthoma elasticum and osteogenesis imperfecta. Onset symptoms include neck, facial pain and diffuse headache. Cranial nerve palsy has been also described. The aim of this study was to evaluate MR angiography and MR findings in a population of patients with carotid and vertebral artery dissections. MATERIALS AND METHODS: We reviewed the MR angiography and MR exams of 15 patients (10 males, 5 females, average age 42.0, age range 30-55) with carotid and/or vertebral artery dissection; the serial MR follow-up studies were also examined. RESULTS: Twelve internal carotid arteries (ICA) showed the presence of an irregular stenosis; three of these showed complete occlusion. In two patients both ICA were involved. Six vertebral arteries (VA) showed irregular stenosis (only one patient suffered from both CI and VA dissection); in one case there was also a pseudoaneurysm of the VA. Follow-up studies showed vessel lumen re-opening in 5/11 ICA (one patient underwent vascular stent positioning) and in 3/6 VA. CONCLUSIONS: The present study confirms the usefulness of MR angiography and MR in the diagnosis and follow-up of patients with carotid and vertebral artery dissection.  相似文献

BACKGROUND AND PURPOSE: The aim of this study was to determine the feasibility and usefulness of contrast-enhanced MR angiography (CE-MRA) for the follow-up of intracranial aneurysms treated with detachable coils, by comparing CE-MRA with digital subtraction angiography (DSA) and 3D time-of- flight (TOF) MRA. METHODS: Thirty-two patients with 42 treated aneurysms were included in the study; 6 had been treated for multiple aneurysms. All MRAs were performed with a 1.5T unit within 48 hours of DSA. We performed 2 types of acquisition: a 3D TOF sequence and CE-MRA. Twenty-eight patients were included 1 year after endovascular treatment, and 4 patients, after 3 years or more. DSA was the technique of reference for the detection of a residual neck or residual aneurysm. RESULTS: Compared with DSA, the sensitivity of MRA was good. For the detection of residual neck, there was no significant difference between the results of 3D TOF MRA (sensitivity, 75%-87.5%; specificity, 92.9%, according to both readers) and CE-MRA (sensitivity, 75%-82.1%; specificity, 85.7%-92.9%). For the detection of residual aneurysm, sensitivity and specificity of both techniques were the same, respectively 80%-100% and 97.3%-100%. Therefore, CE-MRA was not better than 3D TOF MRA for the detection of residual neck or residual aneurysm. For large treated aneurysms, there was no difference between decisions regarding further therapy after CE and 3D TOF MRA, even though CE-MRA with a short echotime and enhancement gave fewer artifacts and better visualization of recanalization than 3D TOF MRA. The interpretation of transverse source images and the detection of coil mesh packing seemed easier with 3D TOF imaging. CONCLUSION: This prospective study did not show that CE-MRA was significantly better than 3D TOF MRA for depicting aneurysm or neck remnants after selective endovascular treatment using coils. For aneurysms treated with coils, 3D TOF MRA seems a valid and useful technique for the follow-up of coiled aneurysms.  相似文献

The aim of this study was to compare image quality, level of diagnostic confidence and interobserver agreement in assessment of carotid stenosis with contrast enhanced MR angiography (CE MRA) in comparison with 2D time of flight MR angiography (2D TOF MRA). 60 carotid arteries in 30 patients were examined by three observers. Image quality and diagnostic confidence were assessed on the basis of a visual analogue scale. Interobserver variability was assessed with the help of intraclass correlation coefficient. Median values on the visual analogue scale for image quality and diagnostic confidence were higher for CE MRA compared with 2D TOF MRA for all three observers. Higher intraclass correlation values were recorded for interobserver variability for CE MRA compared with 2D TOF MRA both for visual estimation of carotid stenosis as well as for measurement of carotid stenosis on the basis of North American Symptomatic Carotid Endarterectomy Trial (NASCET) and European Carotid Surgery Trial (ECST) criteria. CE MRA provides better image quality, higher level of diagnostic confidence and more interobserver agreement compared with 2D TOF MRA.  相似文献

目的 分析三维对比剂增强MR血管成像 (3DCE MRA)显示的颈部动脉常见病变 ;与DSA比较 ,明确 3DCE MRA诊断颈部动脉病变的价值。方法 对 741例超声多普勒怀疑颈部动脉疾病的患者进行 3DCE MRA成像 ,并根据其病变表现总结分类。 2 0 6例病人同时行DSA检查 ,将颈部动脉分为颈总、颈内、颈外、锁骨下及椎动脉 5个部位 ,由 2位放射学专家独立评估DSA及 3DCE MRA血管资料。结果  3 6 7%病例 (2 72 / 741)显示无异常。 63 2 9% (469/ 741)显示颈部动脉有病变 ,共计 82 7段 ,其中动脉粥样斑块占 3 4 2 2 % (2 83段 ) ,动脉狭窄占 3 4 46% (2 85段 ) ,动脉闭塞占 3 3 8% (2 8段 ) ,动脉发育纤细占 14 87% (12 3段 ) ;动脉开口变异占 3 87% (3 2段 ) ,动脉扭曲占 3 63 % (3 0段 ) ,动脉瘤占 0 72 % (6段 ) ,动脉夹层占 1 2 1% (10段 ) ,肿瘤包绕或推压动脉占 1 45% (12段 ) ,术后复查占2 18% (18段 )。对 2 0 6例的 412段血管与DSA比较 ,3DCE MRA在颈总动脉、颈内动脉及椎动脉的阳性检出率差异有显著性意义 (P <0 0 1) ,主要是对动脉粥样斑块 (2 56段与 2 83段 )和动脉狭窄 (2 58段与 2 85段 )存在一定的高估。但对 412段血管整个样本 ,两种方法在同一部位诊断各种血管病变差异无显著性意义 (P >0 0  相似文献

BACKGROUND AND PURPOSE: For intracranial aneurysms treated with Guglielmi detachable coils, long-term follow-up is mandatory because coil compaction may occur and aneurysms may recur. The purpose of this study was to establish a noninvasive technique to visualize residual flow in coiled aneurysms. METHODS: We designed a 3D time-of-flight (3D-TOF) MR angiography (MRA) technique targeted to depict coiled aneurysms that employed a very short TE (1.54-1.60 ms) and a high spatial resolution (0.3 x 0.3 x 0.3 mm3 with zero-filling) to diminish spin dephasing. To diminish spin saturation, image volume was carefully positioned so that the neck of the targeted aneurysm was within 2 cm of the inflow portion along the stream of blood. Fifty-one MRA images of 39 coiled aneurysms in 39 patients were compared with digital subtraction angiography (DSA) images. DSA and MRA findings were interpolated retrospectively for parent and branch arteries' patency, as well as residual flow in aneurysms. In the latest 11 MR studies, a dark-blood 3D turbo spin-echo sequence was added to MRA to negate the effect high-signal-intensity thrombus. RESULTS: MRA visualized all parent and branch arteries with DSA confirmation. MRA visualized residual flow more frequently (38 studies) than did DSA (25 studies). Residual flow space visualized with MRA was always similar to or larger than that with DSA. The dark-blood sequence completely suppressed intraluminal high signal intensity on MRA images and confirmed that the high signal intensity was not due to thrombus. CONCLUSION: TOF MRA targeted to depict coiled intracranial aneurysms is noninvasive and superior to DSA in visualization of residual flow and, hence, useful for follow-up of coiled aneurysms.  相似文献

目的评估透视触发和并行采集技术用于高分辨率三维增强颈动脉磁共振血管成像术(3DCEMRA)的可行性。方法80个临床诊断或怀疑颈动脉狭窄的病人接受3DCEMRA检查。使用透视触发软件触发启动颈动脉3DCEMRA扫描,同时采用K空间椭圆形中心填充法和加速因子为2的并行采集技术。对显示的各段动脉和有无静脉早期显影做分析。动脉狭窄分成动脉管壁不规则、轻度狭窄、严重狭窄和闭塞。颈动脉3DCEMRA的显示结果与其他检查结果做了对照。结果所有病例均顺利触发和完成颈动脉3DCEMRA检查。3DCEMRA显示了80例病人总共800支动脉段(100%显示率),所有病例在动脉显示区域内静脉均未显影或显影很淡,对诊断不构成影响。结果显示有680段动脉正常、41段动脉管壁不规则、24段动脉轻度狭窄、51段严重狭窄和4段闭塞。另外10段颈动脉狭窄处溃疡形成,12例患者除见颈动脉狭窄以外,还发现合并椎动脉和/或锁骨下动脉狭窄。36例病例,与其他血管成像技术作了比较,3DCEMRA的检查结果与之完全符合,未出现高估或低估血管狭窄程度。结论透视触发并行采集高分辨率3DCEMRA简单可行,成像时间短,空间分辨率高,能清楚显示颈动脉,它将在颈动脉狭窄的诊断中起重要作用。  相似文献

目的探讨三维对比增强磁共振血管成像(3D CE-MRA)在颈部动脉血管狭窄诊断中的临床应用价值。方法对23例临床拟诊颈部动脉血管狭窄行数字减影血管造影(DSA)的患者行颈部3D CE-MRA。将两种方法检查结果进行相关性比较。结果 23例患者共230个节段血管,3D CE-MRA显示了227个节段,共诊断出74处(32.6%)狭窄,其中28处轻度狭窄,22处中度狭窄,20处重度狭窄,4处闭塞;DSA共显示了230个节段的血管,共诊断出69处(30.4%)血管狭窄,其中24处轻度狭窄,23处中度狭窄,19处重度狭窄,3处闭塞。与DSA相比,3DCE-MRA对颈部动脉轻度、中度、重度狭窄及动脉闭塞的显示敏感性均为100%,特异性分别为85.71%、90.91%、90%和75%,两种检查方法对颈部动脉狭窄程度的判断有良好的一致性(κ=0.921,P=0.000)。结论 3.0T 3DCE-MRA能够可靠的评价颈部动脉狭窄性病变,基本可以替代DSA检查。  相似文献

PURPOSE: The purpose of this work was to compare Gd-enhanced breath-hold fast imaging with steady-state precession (Gd-FISP) with unenhanced time-of-flight (TOF) sequences in evaluating internal carotid arteries (ICAs). METHOD: Thirty patients underwent three unenhanced TOF sequences [2D traveling saturation (Travelsat); 3D tilted optimized nonsaturated excitation (TONE); TOF 3D Multislab] and two breath-hold 3D Gd-FISP sequences with automated intravenous contrast agent injection (axial and coronal). ICAs were classified as normal (no stenosis); with mild (<30%), moderate (30-70%), or severe stenosis; or occluded (100%). Digital subtraction angiography (DSA) with aortic arch injection was used as a reference technique. RESULTS: DSA revealed 20 normal ICAs; 11 mild, 9 moderate, and 14 severe stenoses; and 2 occlusions. DSA and all MR angiography (MRA) sequences diagnosed the occlusion of four common carotid arteries. The TOF 2D overestimated 10 stenoses, TOF 3D TONE 9, and TOF 3D Multislab 5; Gd-FISP 3D overestimated only 2 of them, reaching the highest sensitivity and specificity for severe stenoses. Significant differences were found between the overestimation of Gd-FISP and each of the three unenhanced sequences (0.0020 < p < 0.0313, Wilcoxon and McNemar tests). Severe artifacts were observed with TOF techniques only. CONCLUSION: Gd-FISP is an interesting, largely artifact-free improvement for MRA of ICAs.  相似文献
