左室特发性室性心动过速折返路径标测和消融点的选择   总被引:4,自引:0,他引:4  
报道 1 0例 (男 8、女 2 )左室特发性室性心动过速 (简称室速 )折返路径标测结果和选择折返路径的不同部位为消融点的消融效果。电生理检查常规插入右室心尖与冠状静脉窦电极 ,并经左、右股动脉分别插入大头电极和2 8 2mm间距冠状静脉窦 1 0极标测电极至左室 ,后者贴靠在室间隔表面。窦性心律时各电极对可依次记录到His束电位 (HP)、左束支电位 (LBP)和左后分支的蒲氏纤维电位 (PP) ,室速时仍可同时记录到上述各电位 ,但顺序相反 ,PP领先 ,HP最后 ;而各部位的V波激动顺序在窦性心律和室速时是相同的 ,都是远端电极 (PP以远 )的V波最早 ,近端电极 (HP)的V波最晚。大头电极置于PP电极对附近。结果 :1 0例中 9例能记录到折返路径各电位心内电图 ,折返路径记录成功率为 90 % ( 9/1 0 )。第 1例大头电极位于PP电极对略上方处放电 ,消融成功 ,但导致完全性左束支阻滞。第 2 ,3例开始在PP电极对略下方处放电 ,但凡未记录到PP的点 ,虽然V波最早 ,都是放电无效点。最后消融成功的点 ,都记录到最领先的PP。第 4例以后 ,都必须记录到最领先的PP后才放电 ,除 1例 2次放电成功外 ,都是 1次放电成功。 1 0例随访至今 3~ 1 8个月 ,未服任何抗心律失常药均无室速发作。结论 :左室标测法不仅对研究左室特发性室速的折返  相似文献

A 69 year old man with ischemic heart disease underwent electrophysiologic evaluation for paroxysmal wide QRS tachycardia, the configuration of which was identical to that recorded during sinus rhythm, that is, right bundle branch block, left anterior fascicular block and anterior myocardial infarction. Electrocardiographic recordings during tachycardia showed atrioventricular dissociation and His bundle activation occurring 5 ms after the onset of the QRS complex recorded on the surface electrocardiogram, consistent with a left posterior fascicular tachycardia. All traditional therapeutic attempts failed to prevent frequent recurrences of tachycardia, which was finally ablated by three 300 J shocks delivered through an electrode catheter positioned in the posterobasal region of the left ventricular septum.  相似文献

Idiopathic Left Ventricular Tachycardia. Introduction: Idiopathic left ventricular tachycardia with a QRS pattern of right bundle branch block and left-axis deviation constitutes a rare but electrophysiologically distinct arrhythmia entity. The underlying mechanism of this tachycardia, however, is still a matter of controversy. This report describes findings in a 42-year-old man who underwent successful radiofrequency catheter ablation of idiopathic left ventricular tachycardia.
Methods and Results: On electrophysiologic study, the tachycardia was reproducibly induced and terminated with double ventricular extrastimuli. Intravenous verapamil terminated the tachycardia whereas adenosine did not. Detailed left ventricular catheter mapping during sinus rhythm revealed a fragmented delayed potential at the mid-apical region of the inferior site near the posterior fascicle of the left bundle branch. At the same site, continuous electrical activity throughout the entire cardiac cycle was recorded during ventricular tachycardia. Repeated spontaneous termination of this continuous electrical activity in late diastole was followed immediately by termination of the tachycardia. Single application of radiofrequency current for 20 seconds at this site completely abolished inducibility of the tachycardia. After catheter ablation, at the identical site of preablation recording of the fractionated potential during sinus rhythm, no fragmented delayed activity could be recorded. There was no complication from the ablation procedure.
Conclusion: The preablation recordings of fragmented delayed potentials during sinus rhythm and continuous diastolic electrical activity during tachycardia, together with ablation characteristics and previously reported electrophysiologic properties of this arrhythmia, may further support microreentry as the underlying mechanism in idiopathic left ventricular tachycardia.  相似文献

Radiofrequency catheter ablation (RFCA) was performed on a 5-year-old boy with congenital junctional ectopic tachycardia (JET) that was refractory to medical management. Because of the lack of retrograde atrial depolarization during tachycardia, radiofrequency energy was delivered during atrial overdrive pacing to confirm the presence of preserved atrioventricular (AV) conduction. Although the procedure was complicated by complete right bundle branch block after ablation of the para-Hissian region, the patient regained sinus rhythm accompanied by normal AV conduction. Rapid atrial pacing during RFCA of JET may be safely used to avoid AV block.  相似文献

OBJECTIVE: To report experience with radiofrequency catheter ablation of fascicular tachycardia including two cases of the rare type of this arrhythmia which arises from the anterior fascicle of the left bundle branch. DESIGN: Review of results of radiofrequency ablation in nine consecutive patients presenting with fascicular tachycardia. SETTING: Regional cardiac centre. INTERVENTION: Percutaneous radiofrequency catheter ablation, performed between 1993 and 1996. RESULTS: Radiofrequency ablation was successful in both patients with tachycardia arising from the anterior fascicle and in six of the seven patients with tachycardia arising from the anterior fascicle and in six of the seven patients with tachycardias arising from the posterior fascicle. Notable differences in the "right bundle branch block" configuration of lead V1 during tachycardia between patients were observed. One patient with incessant tachycardia had marked impairment of ventricular function which returned to normal after ablation. CONCLUSIONS: Radiofrequency ablation is effective in both anterior and posterior fascicular tachycardias. The arrhythmia can cause reversible impairment of ventricular function.  相似文献

The authors report the case of a 57-year-old woman with Ebstein's anomaly and atrial septal defect. She was referred to their hospital for treatment of refractory paroxysmal wide QRS tachycardia. Her 12-leads ECG in sinus rhythm showed ventricular preexcitation of type B Wolff-Parkinson-White syndrome. In a baseline electrophysiological study, a wide QRS tachycardia with right bundle branch block configurations was induced. This tachycardia was orthodromic atrioventricular reciprocating tachycardia with a right inferior accessory pathway. Radiofrequency current was successfully delivered at the inferior site of the atrialized right ventricle. Radiofrequency catheter ablation seems to be useful for supraventricular tachycardia in patients with Ebstein's anomaly and atrial septal defect.  相似文献

BACKGROUND: Idiopathic repetitive monomorphic ventricular tachycardia with an inferior axis and left bundle branch block pattern typically originates from the superior right ventricular outflow tract. When indicated, radiofrequency catheter ablation is usually safe and effective. However, a left ventricular origin has been described recently in adult patients in whom ablation attempts in the right ventricular outflow tract were unsuccessful. Experience in pediatric patients is limited. PATIENTS AND METHODS: Since 1998, 13 young patients suffering from symptomatic ventricular tachycardia episodes with an inferior axis and left bundle branch block pattern underwent an electrophysiological study and radiofrequency catheter ablation. In 2 patients, age 13 and 15 years, no endocardial local electrograms preceding the surface ECG QRS complex could be recorded within the right ventricular outflow tract during ventricular ectopy. Detailed mapping within the left ventricular outflow tract and in the aortic root revealed local electrograms 25 and 53 ms earlier than the QRS complex and a 11/12 and 12/12 lead match during pacing inferior and anterior to the ostium of the left main coronary artery in the left aortic sinus cusp. Earliest activation was recorded 10 and 12 mm away from the coronary artery ostium identified angiographically. In each of the patients, one single radiofrequency current application (60 degrees C, 30 W, duration 30 and 60 s, respectively) resulted in complete cessation of ventricular ectopy. Subsequent selective injection into the left coronary artery did not reveal any abnormalities. During follow-up (2 and 34 months) off any antiarrhythmic drugs, both of the patients are in continuous normal sinus rhythm. CONCLUSION: In young patients with symptomatic idiopathic ventricular tachycardia originating from the left aortic sinus cusp, radiofrequency catheter ablation was safe and effective.  相似文献

A detailed analysis of the ventricular activation along the posterior aspect of the mitral annulus was made using a multipolar catheter positioned in the coronary sinus in a patient with mitral isthmus ventricular tachycardia (VT) associated with a remote inferior myocardial infarction and prior cryosurgical ablation for the elimination of a different preexisting VT. A change in the timing and sequence of the ventricular activation along the isthmus could be observed during induction of the VT and entrainment pacing. A radiofrequency (RF) current application directed at the posterolateral region of the isthmus successfully eliminated this tachycardia. During the RF delivery, complete conduction block was confirmed by a sudden change in the activation sequence during sinus rhythm.  相似文献

We report on a patient without evidence of structural heartdisease who presented with adenosine-sensitive sustained ventriculartachycardia with left bundle branch block and right axis QRSmorphology. Endocardial catheter mapping revealed the originof ventricular tachycardia to be located in the right ventricularoutflow tract, where the earliest endocardial activation duringventricular tachycardia was registered 30–40 ms priorto onset of QRS complex in the surface ECG. Pacemapping providedno additional information; we found a good match between 12lead surface ECGs registered during spontaneous episodes ofventricular tachycardia and those recorded during pacing ina relatively large area in the right ventricular outflow tract.The 10th application of radiofrequency current abolished ventriculartachycardia temporarily, but it recurred within 30 min. Afterthe 28th radiofrequency current delivery ventricular tachycardiawas permanently abolished.  相似文献

Background: Radiofrequency (RF) catheter ablation is a safe and effective cure for many forms of supraventricular tachycardia. Its efficacy in the cure of right ventricular outflow tract tachycardia, and some forms of left ventricular tachycardia in patients with left ventricular dysfunction, has also been shown. In contrast limited data are available to assess the role of RF catheter ablation in treating idiopathic left ventricular tachycardia (ILVT), an unusual form of tachycardia occurring in patients without demonstrable heart disease.
Aim: To examine the efficacy and safety of RF catheter ablation in patients with ILVT.
Methods: Three patients without structural heart disease and with recurrent drug-refractory ILVT (right bundle branch block and left axis morphology) underwent electrophysiologic study (EPS) to initiate and localise the site of origin of their VT. RF catheter ablation of the VT focus was performed, with success being defined as failure to reinduce VT during incremental infusion of isoprenaline.
Results: In all three patients VT was inducible by rapid right atrial pacing and/or programmed ventricular stimulation, and could be terminated by intravenous verapamil. RF catheter ablation was successful in all patients. The site of successful ablation was common to each patient and was localised to the infero-apical aspect of the left ventricular septum. It was characterised by the recording of the earliest presystolic 'P' potential during both sinus rhythm and induced ILVT. No complications occurred during the procedure. During follow-up periods ranging from six to 12 months there were no symptomatic or documented episodes of recurrent ILVT.
Conclusions: We conclude that ILVT can be safely and effectively cured by RF catheter ablation.  相似文献

目的 阐明隐匿性束室纤维介导的心动过速的电生理机制及其导管消融方法。方法 研究病例为男性、29岁,心动过速病史7年余。心动过速不能被腺苷三磷酸和维拉帕米终止,但可被普罗帕酮终止。曾在院外拟诊房室结折返性心动过速,两次行慢径改良术,但心动过速仍反复反作。在我院行电生理检查一次并在非接触球囊导管标测系统(即EnSite,3000)指导下标测和消融。结果 电生理检查示窦性心律时AH为75ms,HV 44ms,心房增频刺激及程序刺激未见心室预激现象。心室600ms起搏示室房分离,RS2程序刺激无逆传A波,提示室房逆传功能较弱。心房与心室均能诱发心动过速,但心室更易诱发。心动过速时室房分离。心动过速可呈窄QRS图形、左束支传导阻滞(LBBB)图形及右束支传导阻滞(RBBB)图形。LBBB和RBBB心动过速可自行转为窄QRS心动过速。窄QRS心动过速及RBBB图形时,心动周期为300ms;LBBB图形时,心动周期为316ms,明显长于前。三种心动过速时,希氏束激动均领先于右束支激动。维拉帕米和腺苷三磷酸不能终止心动过速。专房程序刺激及超速抑制不能终止心动过速,但可被心室程序刺激终止。EnSite 3000标测系统分析发现三种心动过速的最先激动点均位于心动过速的最先活动点均位于右室间隔上部,此处行环状消融后心动过速不再诱发。随访4个月,临床无心动过速发作。结论 该患的心动过速由隐匿性束室纤维介导,其折返环路包括正常的希-浦传导系统、心室和束室纤维,其消融方法与室性心动过速相似;EnSite3000标测系统指导此类心动过速的消融有极大的优越性。  相似文献

BACKGROUND. Radiofrequency energy has been used safely and successfully to eliminate accessory pathways in patients with the Wolff-Parkinson-White syndrome and the substrate for atrioventricular nodal reentrant tachycardia. However, this form of ablation has had only limited success in eliminating ventricular tachycardia in patients with structural heart disease. In contrast, direct-current catheter ablation has been used successfully to eliminate ventricular tachycardia in patients with and without structural heart disease. The purpose of this study was to test whether radiofrequency energy can safely and effectively ablate ventricular tachycardia in patients without structural heart disease. METHODS AND RESULTS. Sixteen patients (nine women and seven men; mean age, 38 years; range, 18-55 years) without structural heart disease who had ventricular tachycardia underwent radiofrequency catheter ablation to eliminate the ventricular tachycardia. Two patients presented with syncope, nine with presyncope, and five with palpitations only. Mean duration of symptoms was 6.7 years (range, 0.5-20 years). Radiofrequency catheter ablation successfully eliminated ventricular tachycardia in 15 of 16 patients (94%). Sites of ventricular tachycardia origin included the high right ventricular outflow tract (12 patients), the right ventricular septum near the tricuspid valve (three patients), and the left ventricular septum (one patient). The only ablation failure was in a patient whose ventricular tachycardia arose from a region near the His bundle. An accurate pace map, early local endocardial activation, and firm catheter contact with endocardium were associated with successful ablation. Radiofrequency ablation did not cause arrhythmias, produced minimal cardiac enzyme rise, and resulted in no detectable change in cardiac function by Doppler echocardiography. CONCLUSIONS. Radiofrequency catheter ablation of ventricular tachycardia in patients without structural heart disease is effective and safe and may be considered as early therapy in these patients.  相似文献

Radiofrequency Ablation of Pseudo-Mahaim Fibers. Introduction: A young woman with refractory recurrent supraventricular tachycardia was referred for catheter ablation. Methods and Results: Electrophysiologic studies revealed the mechanism of tachycardia to be atrioventricular (AV) reentry, utilizing a decrementally conducting atriofascicular accessory pathway as the anterograde limb of the circuit and the normal intraventricular conducting system as the retrograde limb. Pace mapping in the right atrium during sinus rhythm suggested an atrial origin of the accessory pathway several centimeters distant from the AV node. Multiple radiofrequency lesions at the distal insertion of the accessory pathway in or near the right bundle branch failed to abolish preexcitation. In contrast, radiofrequency current applied to the ventricular side of the anterolateral tricuspid ring, adjacent to the atrial origin of the accessory pathway, was successful in abolishing preexcitation and inducible supraventricular tachycardia without affecting AV nodal conduction. Conclusion: Radiofrequency ablation can provide curative therapy for intractable supraventricular tachycardia due to decrementally-conducting atriofascicular accessory pathways. The risk of AV block in such patients as a consequence of the procedure should be quite low.  相似文献

目的探讨经心外膜途径在电-解剖标测系统指导下行射频消融治疗心肌梗死后室性心动过速的可行性和安全性。方法成年中华小型猪共7只,采用经皮穿刺的方法将球囊置于左前降支中下部,封堵150 min建立心肌梗死模型。3~5周后将心肌梗死模型猪,行电生理检查诱发室性心动过速。经胸穿刺进入心包腔,采用电-解剖标测系统在窦性心律下进行心外膜电压标测和线性消融。射频消融后再次行电生理检查,不能再诱发室性心动过速为消融成功。结果存活的心肌梗死模型的猪7只,3~5周后行电生理检查,共诱发出单形性室性心动过速共8种,7种表现为右束支阻滞图形,1种表现为左束支阻滞图形,室性心动过速(VT)周长平均在(338±66)ms。1只猪同时诱发心室颤动,电除颤转复窦性心律。7只猪心包穿刺均成功,完成心外膜电压标测,沿瘢痕区到二尖瓣环或正常心肌区逐点进行线性消融。射频消融后再次行电生理检查,6只猪不能再诱发室性心动过速。结论经胸穿刺进入心包腔行心外膜标测和消融治疗心肌梗死后室速的方法是安全可行的,心外膜标测消融心肌梗死后室速的方法可以作为心内膜消融的一种有效补充方法。  相似文献

Seven of 120 consecutive patients with inducible sustained ventricular tachycardia (from September 1, 1988 to January 1, 1991) had bundle branch reentrant tachycardia and underwent percutaneous radiofrequency ablation of the right bundle branch. The seven patients had been unsuccessfully treated with a mean of 3 +/- 1 drugs. Four patients presented with syncope and three with aborted sudden death. The baseline electrocardiogram revealed a left bundle branch block pattern in three patients and an intraventricular conduction defect in four. The baseline HV interval was prolonged in each case (79 +/- 2 ms). With use of programmed ventricular extrastimuli, sustained bundle branch reentrant tachycardia was inducible in all patients at a mean cycle length of 283 +/- 17 ms (range 230 to 350). Bundle branch reentrant tachycardia characteristics included atrioventricular dissociation, a His deflection that preceded each QRS complex and spontaneous His to His variation that preceded changes in ventricular tachycardia cycle length. A quadripolar catheter was positioned across the tricuspid valve with the distal electrode tip of the catheter near the right bundle branch. One to three applications of continuous unmodulated radiofrequency current at 300 kHz between the distal electrode and a large posterior skin patch resulted in complete right bundle branch block in all patients, after which none had inducible bundle branch reentrant tachycardia on restudy. On restudy, three of the seven patients had ventricular tachycardia of myocardial origin (not bundle branch reentry). Aims This study aimed to clarify the safety and efficacy of selectivefast pathway ablation in patients with atrio-ventricular nodalreentrant tachycardia and a prolonged PR interval during sinusrhythm. Such patients have been reported to have an increasedincidence of complete atrioventricular block. Methods and Results In this study, the earliest retrograde atrial activation duringatrioventricular nodal reentrant tachycardia and right ventricularstimulation was localized. Fast pathway ablation was then performedin five patients with the common form of atrioventricular nodalreentrant tachycardia and a prolonged PR interval. Three ofthe five patients had almost incessant atrioventricular nodalre-entrant tachycardia. Radiofrequency catheter ablation induceda complete ventriculo-atrial block during right ventricularstimulation in four patients and a marked prolongation of ventriculo-atrialconduction during right ventricular stimulation in one. Non-inducibilityof common atrioventricular nodal reentrant tachycardia withand without isoproterenol was achieved in all five patients.The PR interval increased from 254±53ms to 276±48msand the atrio-His interval from 172±46ms to 192±45ms.Second- or third-degree atrioventricular block did not occurduring the ablation procedure. During the follow-up of 19±20months none of the patients developed symptoms suggestive ofatrioventricular nodal reentrant tachycardia or evidence ofsecond- or third-degree atrioventricular block. Conclusion These data suggest that atrioventricular node (retrograde) fastpathway ablation can apparently be safely performed in patientswith common atrioventricular nodal reentrant tachycardia anda prolonged PR interval during sinus rhythm.  相似文献

With the introduction of radiofrequency energy, catheter ablation has become an established technique for managing many cardiac rhythm disturbances. High efficacy and safety have been reported for accessory pathway ablation, selective fast and slow atrioventricular nodal pathway ablation to eliminate atrioventricular nodal reentrant tachycardia (while preserving atrioventricular conduction), atrioventricular junctional ablation to control the ventricular response to atrial tachyarrhythmias, ablation of the right bundle branch to eliminate bundle branch reentrant ventricular tachycardia, and ablation of the site of tachycardia origin in patients with ventricular tachycardia unassociated with structural heart disease. In addition, there has been active investigation into ablation techniques for more complex arrhythmias such as atrial tachycardia, atrial flutter, and ventricular tachycardia associated with structural heart disease.  相似文献

Radiofrequency Catheter Ablation. Radiofrequency catheter ablation techniques are becoming increasingly accepted as the therapy of choice for selected patients with symptomatic arrhythmias. The ability to titrate the power output using radiofrequency current has allowed these ablative techniques to be applied safely in a variety of arrhythmias. In many institutions, radiofrequency catheter ablation has now become standard therapy for controlling medically refractory atrial arrhythmias using atrioventricular (AV) junction ablation and for curing AV nodal reentrant tachycardia and supra ventricular tachycardia due to accessory AV connections. This technology is also being used to treat some forms of ventricular tachycardia such as bundle branch reentry ventricular tachycardia, ventricular tachycardia in structurally normal hearts, and with limited success in patients with ventricular tachycardia and coronary artery disease. 目的:对经射频消融术证实的特发性室性心动过速的病例进行总结分析,探讨室性心动过速的发病状况、心电图特点、消融靶点的确定及消融结果。方法:对68 例特发性室性心动过速的起源部位和体表心电图进行分析,所有患者在诱发出室性心动过速后进行射频消融治疗,观察特发性室性心动过速的射频消融成功率和复发率以及它们和消融靶点的关系。结果:本组特发性室性心动过速患者中右室室性心动过速较左室室性心动过速多见。右室特发性室性心动过速心电图表现为左束支传导阻滞,左室特发性室性心动过速心电图则多表现为右束支传导阻滞。消融靶点的确定右室特发性室性心动过速主要采用起搏标测法,左室特发性室性心动过速主要采用激动顺序标测法。右室流出道室速组在起搏标测时起搏ECG和VT时ECG的12导联QRS波完全相同处消融成功率较高。结论:室性心动过速发作时的体表心电图可初步估计特发性室性心动过速的起源部位,射频消融术治疗特发性室性心动过速成功率高,并发症少。  相似文献
