It is estimated that 70 million Americans have used the Internet to acquire health-related information. Multiple factors provide the driving force behind this demand for online health information. Information technology is beginning to change the exclusive focus of medicine from curing disease to prevention of disease and enhancing health status. A critical feature of this change is the development of information and services that assist consumers to assume more responsibility for their own health and to actively participate in health care decisions. At the same time, physicians remain skeptical about the advantages of using the Internet for patient education. Some of the issues that this paper addresses include the following: What are the major factors that are leading to the increased use of the Internet by consumers to obtain health-related information? How do physicians view the use of the Internet by patients to obtain health-related information and services? How is the use of the Internet by consumers affecting physician-patient communications? The University Health System Consortium (UHC) represents a strategic alliance of 169 academic health centers and associated institutions engaged in knowledge sharing and idea-generation. The use of the Internet as a tool in the delivery of UHC's products and services has increased dramatically over the past year and will continue to increase during the foreseeable future. This paper examines the current state of UHC-member institution driven tools and services that utilize the Web as a fundamental component in their delivery. The evolution of knowledge management at UHC, its management information and reporting tools, and expansion of e-commerce provide real world examples of Internet use in health care delivery and management. Health care workers are using these Web-based tools to help manage rising costs and optimize patient outcomes. Policy, technical, and organizational issues must be resolved to facilitate rapid adoption of Internet applications.  相似文献

Evidence such as systematic reviews or clinical practice guidelines are information products, and clinicians are consumers of those products; their current proliferation but low uptake by consumers indicates an information oversupply. The costs and benefits of accessing and applying information are at least as important as are the costs and benefits of the treatments the information describes. In the same way that a citation index is a measure of the impact of a scientific paper, an evidence uptake index could measure effectiveness of evidence products in a clinical population. The uptake of evidence-based medicine may be hampered by the perceived high cost of changing to it. At present, most costs are borne by individual clinicians, but individual benefits for clinicians are downplayed in favour of population benefits. Specific strategies to increase evidence uptake into practice include decreasing the "cost of ownership"; increasing the direct or perceived value of evidence resources in routine practice; and customising evidence to suit different users, tasks and clinical contexts.  相似文献

Search engines and Web-based directories play a central role in facilitating access to online health information. In this environment consumers often find and act upon health information of variable quality, with little input from health professionals. As an added concern, when consumers access health information on the Web via a search engine they often fail to discuss the information with a healthcare provider, especially in areas of perceived medical uncertainty. For many health consumers, the Internet may be the primary (or even sole) source of health information, especially where consumers lack access to, or trust in, trained providers. To date the corresponding role of search engine technology use and efficacy has received relatively little attention, however. This study serves as an exploratory technology assessment that explains the application of keyword effectiveness indexing (KEI) analysis in estimating the ability of commercial search engines to return relevant results for healthcare consumers. Findings suggest that KEI can serve as a useful health search profiling tool, though its applicability may vary across domains. Six years have passed since the total hospital information system of Miyazaki Medical College, nicknamed PHOENIX, began its functions for the first time. It started with order entry systems, and has accomplished various systems, leaving one entry system unfinished; the injection order entry system which will be completed in the near future. It was revealed that the waiting period was most reduced at the hospital pharmacy. The waiting period for the visitors was also reduced. Usefulness of the PHOENIX system was greatly advanced by the function of a unique system of personal computer LAN, nicknamed PALM. This personal computer environments consisted of 200 or more Apple Macintosh computers. In this PALM environment, file servers, CD-ROM MEDLINE, clinical information databases, electronic mails (available in LAN and Internet) and sharing of printers are on service 24 hr a day.  相似文献

USA is known worldwide as leaders and innovators in higher clinical pharmacy education and research. The doctor of pharmacy (PharmD) program has been served as the model for others countries. The top-ranked PharmD program is offered in the United States by the School of Pharmacy at the University of California, San Francisco (UCSF). 运输包装是以运输储存为主要目的的产品包装,其性能优劣将直接影响货物流通期间的产品质量。药品开发过程中,国内企业往往较为重视内包装的选择与研究,而容易忽视药品流通过程中潜在的运输风险,忽略对运输包装实际性能的有效检测及评价。本文论述了药品运输包装性能测试的必要性,分析对比了国内外药品申报注册的相关现状,并通过介绍药品运输包装的性能测试中的可参考标准,尤其是美国材料与试验协会《运输容器系统性能检测标准规范》(ASTM D4169),以期为药品运输包装的性能测试及评价提供相应思路及参考。  相似文献

目的研究中国珠海、中国澳门、美国纽约三地的药房医学服务现状及存在的问题,比较三地药房医学服务的水平,并提出相应建议。方法在中国珠海、中国澳门、美国纽约三地通过半结构化问卷访谈了30名药师和70名消费者,并对访谈结果进行对比分析。结果通过对三地的访谈分析发现,三个地区药房医学服务的水平都不高并且服务种类比较单一,地区差异不大,但消费者和药师之间存在认知差异。结论三地药房可以通过提供一般性的医疗检查服务、传递医学保健知识和开设医疗门诊等服务提高药房医学服务的水平,增强药房的综合竞争力。  相似文献

In this paper, we introduce a simulation study to improve the antineoplastic medication preparation and delivery performance at a pharmacy department in a large community hospital. The goal of this work is to help pharmacy reduce patients' average waiting time when receiving chemotherapy. This will be achieved by simulating and analyzing the preparation and delivery procedures to identify process bottlenecks, carry out what-if analysis, predict the impact of improvement effort, and provide recommendations to hospital leadership. Using the simulation model, we discover that by introducing early preparation for the returning patients and dedicating an infusion staff member for medication delivery, patients' waiting time for antineoplastic medications can be reduced substantially. Such improvements do not require additional floor space or significant investment. The recommendation has been accepted by hospital management and implemented in the pharmacy department. The preliminary results have verified the simulation output with the desired improvement predicted by the model.  相似文献
