We attempted radiofrequency ablation of the AV junction with a sequential right- and left-sided approach in 78 patients affected by severely symptomatic, drug refractory atrial fibrillation. Stable third-degree A V block was obtained in 99% of cases and, after 3 months, persisted in 92% of cases. Single session, stepwise, radiofrequency modulation of the AV node was attempted in 13 patients with paroxysmal atrial fibrillation. During sinus rhythm, ablation of the slow and fast AV node pathways was performed in order to increase the nodal refractory period or to slow conduction. Clinically successful modulation of AV conduction was achieved in 15% of cases and persisted during a 3-month follow-up. In conclusion. AV junction ablation is a well-established means of treating atrial fibrillation, but implies the implant of a permanent pacemaker. AV node modulation avoids the pacemaker implant, but is efficacious only in a minority of patients. Thus, in patients affected by paroxysmal atrial fibrillation, AV modulation should be attempted first; if this is ineffective. AV ablation can be performed during the same session.  相似文献

Automatic junctional tachycardia is frequently refractory to medical management and difficult to treat with nonpharmacoJogical methods. A 12-year-old female with symptomatic, refractory automatic junctional tachycardia is reported. Earliest atrial activation during supraventricular tachycardia was in the posterior portion of the intraatrial septum. The patient underwent electrophysiological study and successful radiofrequency current ahiation of the ectopic automatic focus within the atrioventricuiar junction. Normal atrioventricular junctional conduction was maintained, and at 7-month foJlow-up the patient has been free of tachycardia.  相似文献

Radiofrequency catheter ablation is now the first line treatment for atrioventricular nodal reentrant tachycardia. The success rate is high with a low incidence of complications. However, a possible proarrhythmic effect of radiofrequency energy has been rarely reported and no study has demonstrated a direct correlation between the anatomic site of the radiofrequency application and the origin of a new post‐ablation arrhythmia. We present a case of a focal atrial tachycardia that occurred after slow pathway radiofrequency catheter ablation for atrial nodal reentrant tachycardia and originating close to the previous ablation site. Radiofrequency (RF) catheter ablation has been widely used in the treatment of cardiac arrhythmias. In atrioventricular nodal reentrant tachycardia (AVNRT), the experience has been predominantly in adults. The cardiac electrophysiological records of 18 consecutive children undergoing RF catheter AV node modification for AVNRT were reviewed. The patients (10 females, 8 males) were 8.2–17.9 years of age (mean 13.6 ± 3.0), weight 15.2–88.1 kg (mean 52.2 ± 20.8), and height 103–190 cm (mean 157.1 ± 21.7). Thirteen were on antiarrhythmic medications (1–3, average 1.5 drugs/day). All drugs were discontinued 48 hours prior to the ablations. The procedures were performed under sedation and local anesthesia. Pre- and post-AV node modification electrophysiological studies were performed in all procedures. The 18 patients underwent a total of 25 procedures (1.39 ± 0.61 per patient): the anterior approach aimed at the antegrade fast pathway in the first four patients and the posterior approach aimed at the slow pathway in the remainder. Thenumber of energy applications was 8–54 (19.8 ± 10.7) per procedure. The maximum energy used in each procedure was 30–50 watts (33.8 ± 8.4). The average energy was 24–50 watts (33.0 ± 6.8). The fluoroscopy time was 7.1–73.4 minutes (29.9 ± 20.0) per procedure, for a total catheterization time of 228–480 minutes (300.3 ± 59.1). Preablation spontaneous or induced AVNRT (cycle length 310.4 ± 55.0 msec) was seen in all except one who had the arrhythmia (cycle length 270 msec) on surface ECG. In 22 of 25 studies, the AH interval measured 67.4 ± 13.2 msec pre- and 98.7 ± 58.4 msec post-AV node modification (P < 0.02). Procedures were initially successful in 16 (89%) of 18 patients. One patient developed complete AV block requiring DDD pacemaker and has since recovered normal AV conduction. Transient third- or second-degree block was seen in four. Other complications included airway obstruction in one and excessive emesis in another. In follow-up of 2–26 months (13.0 ± 7.3), one patient underwent surgical ablation for failed initial RF catheter ablation, and two underwent successful RF procedures for recurrences. RF catheter AV node modification for AVNRT in children is a useful technique. Under ideal circumstances, it is safe and efficacious. Follow-up to determine the potential long-term complications is necessary.  相似文献

Atrial ectopy sometimes appears during RF ablation of the slow pathway in patients with atrioventricular nodal reentrant tachycardia (AVNRT). However, its origin, characteristics, and significance are still unclear. To examine these issues, we analyzed 67 consecutive patients with AVNRT (60 with slow-fast AVNRT and 7 with fast-slow AVNRT), which was successfully eliminated by RF ablation to the sites with a slow potential in 63 patients and with the earliest activations of retrograde slow pathway conduction in 4 patients. During successful RF ablation, junctional ectopy with the activation sequence showing H-A-V at the His-bundle region appeared in 52 patients (group A) and atrial ectopy with negative P waves in the inferior leads preceding the QRS and the activation sequence showing A-H-V at the His-bundle region appeared in 15 patients (group B). Atrial ectopy was associated with (10 patients) or without junctional ectopy (5 patients). Before RF ablation, retrograde slow pathway conduction induced during ventricular burst and/or extrastimulus pacing was more frequently demonstrated in group B than in group A (9/15 [60%] vs 1/52 [2%], P < 0.001). Successful ablation site in group A was distributed between the His-bundle region and coronary sinus ostium, while that in group B was confined mostly to the site anterior to the coronary sinus ostium. In group B, atrial ectopy also appeared in 21% of the unsuccessful RF ablations. In conclusion, atrial ectopy is relatively common during slow pathway ablation and observed in 8% of RF applications overall and 22% of RF applications that successfully eliminated inducible AVNRT. Atrial ectopy appears to be closely related to successful slow pathway ablation among patients with manifest retrograde slow pathway function.  相似文献

Radiofrequeucy ablation of the atrioventricular conduction system (ACS) has become an estoblished theTapy for patients with drug refroctory atrial fibrillation. We observed eight patients with hemodynamic deterioration ofteT radiofrequency oblotion of the otTioventTicular conduction system. As we found hemodynamic deterioration related to worsening mitral regurgitation, we compared the clinical history, eiectrophysiologicai, ond echocardiographic dato from the patients with hemodynamic deteriorotion and worsening mitral regurgitation (group 1) to those without hemodynamic deterioration and stable mitral regurgitation after the procedure (group 2). Eight out of 108 patients (7.4%) undergoing ablation of the ACS deteriorated hemodynamically with acute pulmonary edema in three and congestive heart failure in five patients occurring at a mean of 3 and 8 weeks, respectively, after the procedure. Three of these patients were referred for mitral valve surgery. Two patients underwent ablation using a left-sided approach. A right-sided approach was used in five patients. In one patient, a left- and right-sided approach was used. Compared to group 2 patients, group 1 patients had significantly higher left ventricular end-diastolic diameters (64 ± 6 mm vs 56 ± 9 mm) at baseline despite similar fractional shortening (32%± Il% vs 34%± 13%), left ventricular end-systolic diameters (43 ± 9 mm vs36 ± 7 mm) and degree of mitral regurgitation (1.4 ± 1.1 vs 1.4 ±0.7) on echocardiographic analysis. Thus, hemodynamic deterioration together with progression of mitral regurgitation is a potential complication of ablation of the ACS (up to 7.4%). Patients with high left ventricular end-diastolic diameters ond moderate mitral regurgitation at baseline seem prone to this complication.  相似文献

The anatomical substrate for AV nodal reentrant tachycardia (AVNRT) is well known and is due to anterograde conduction through a siow conducting pathway and retrograde conduction using a fast conducting path way. In this report, we describe a patient with AVNRT who also presented with frequent episodes of paroxysmal nonreentrant tachycardia due to the occurrence of two conducted ventricular beats for each sinus depolarization. Palpitations and arrhythmias were abolished after radiofrequency ablation of the slow pathway.  相似文献

目的:分析心房颤动(简称房颤)经导管射频消融术中出现心房扑动、房性心动过速等规律性快速性房性心律失常(RATs)的预测因素。方法:首次行经导管射频消融治疗的497例房颤患者(阵发性房颤333例,持续性和(或)永久性房颤164例)在三维标测系统及环状标测电极导管指导下行经导管射频消融治疗(包括环肺静脉电隔离、线性消融和(或)碎裂电位消融)。术中记录RATs的发生情况,并分析RATs的可能影响因素。结果:术中共有163例患者[32.8%;阵发性房颤92例,持续性和(或)永久性房颤71例]出现195种RATs,持续性和(或)永久性房颤术中RATs的发生率显著高于阵发性房颤(43.3%比27.6%,P〈0.001)。单因素分析发现心脏外科术后(P〈0.001)、术前有RATs(P=0.010)、持续性和(或)永久性房颤(P〈0.001)、左房内径增大(P〈0.001)、左室射血分数降低(P=0.018)是房颤术中出现RATs的影响因素。Logistic多因素回归分析发现心脏外科术后[优势比(0R)=8.14,95%可信区间(CI):I.69~39.1,P=0.0093、术前有RATsEOR=2.15,95%CI:1.35~3.42,P=0.001]、持续性和(或)永久性房颤[0R=1.71,95%CI:1.06~2.76,P=0.029]、左房内径[0R=1.04,95%CI:1.01~1.08,P=0.025]是术中出现RATs的独立预测因素。结论:房颤射频消融术中常出现RATs,心脏外科术后、术前有RATs、持续性和(或)永久性房颤、左房内径是术中出现RATs的独立预测因素。  相似文献

WICHTERLE, D., et al .: Paradoxical Autonomic Modulation of Atrioventricular Nodal Conduction During Heart Rate Turbulence. Heart rate turbulence (HRT) represents a biphasic chronotropic response of sinus rhythm to a single ventricular premature beat (VPB). It consists of early acceleration and late deceleration of heart rate and is predominantly mediated by the autonomic nervous system. The aim of this study was to investigate if autonomic perturbations after a VPB exert a significant effect on AV conduction. Both surface ECG and the high right atrial electrogram were recorded at a sampling frequency of 1000 Hz in 26 patients (24 men, mean age 49 ± 12 years) referred for electrophysiological evaluation. The stimulation protocol consisted of series of single ventricular extrastimuli delivered from the right ventricular apex at decreasing coupling intervals. A biphasic profile of AV intervals after a single VPB was observed. The response of AV conduction to a VPB was approximately 25 times and 15 times weaker in the early and late phase, respectively, than that of R-R intervals. Thus, AV interval dynamics significantly preceded the change in R-R intervals, which is in conflict with the near to zero phase of transfer function between R-R and AV intervals described in previous studies. A significant AV turbulence was observed consisting of early shortening and later prolongation of AV intervals after VPB. Its magnitude was much smaller than that of HRT. Dynamics of AV delay has little impact on the accuracy of HRT assessment from surface ECG. The significant temporal dissociation of R-R and AV interval dynamics after a VPB remains unexplained. Radiation exposure during fluoroscopic imaging poses potential risks to patients and physicians, especially during protracted cardiovascular or radiological interventional procedures. We describe a woman with refractory paroxysmal supraventricular tachycardia who underwent radiofrequency catheter ablation of the slow pathway involved in atrioventricular nodal reentrant tachycardia. The patient subsequently returned 4 weeks later with acute radiation dermatitis that was retrospectively attributed to a malfunction in the fluoroscopy unit that lacked a maximum current output cut-off switch. Using dose reconstruction studies and her estimated biological response, we determined that she received between 15 and 20 Gy (1 Gy = 100 rods) to the skin on her back during the procedure. The exposure will result in an increase in her lifelong risk of skin and lung cancer. This article underscores the potential for radiation-induced injury during lengthy therapeutic procedures using x-ray equipment.  相似文献

The advent of catheter ablation stimulated extensive research into anatomical localization of the pathways involved in atrioventricular nodal reentrant tachycardia(AVNRT). Conventional electrophysiological methods that attempt to correlate intracardiac electrograms with two-dimensional fluoroscopic anatomy are limited by the relative inaccuracy and poor reproducibility of this technique, and the requirement for high levels of radiation exposure. A new method of nonfluoroscopic electroanatomical mapping utilizes magnetic field sensing with a specialized catheter to construct three-dimensional electroanatomical endocardial maps of selected heart chambers with spatial resolution of < 1 mm. This system can be used in patients undergoing catheter ablation for AVNRT to create accurate maps of Koch's triangle and to guide application of radiofrequency energy. Initial experience in 14 patients suggests efficacy and safety comparable to conventional mapping and ablation techniques. Further evaluation may confirm the potential benefits of this system with respect to success rates, complications, procedure time, and radiation exposure.  相似文献

A healthy 37-year-old male presented with a history of frequent palpitations and sustained wide QRS complex tachycardia with a right bundle branch block and left axis morphology. Serial electrophysiological studies revealed two inducible tachycardias, which were shown to represent atrioventricular nodal reentrant tachycardia and idiopathic left ventricular tachycardia. Transformation from one tachycardia to the other occurred spontaneously as well as following atrial or ventricular pacing. Radiofrequency catheter ablation of the slow atrioventricular nodal pathway resulted in cure of atrioventricular nodal reentrant tachycardia and the prevention of spontaneous recurrence of ventricular tachycardia, suggesting a role of atrioventricular nodal reentrant tachycardia in triggering the clinical episodes of ventricular tachycardia. The patient has remained asymptomatic without antiarrhythmic therapy for 8 months.  相似文献

Paroxysmal supraventricular tachycardia (SVT) may have a variety of hemodynamic effects depending on rate, patient volume status, and presence of structural heart disease or left bundle branch block. We report a case of a patient with atrial tachycardia and dual atrioventricular (AV) nodal physiology who developed profound hypotension during transition from fast to slow AV nodal pathway conduction, despite similar tachycardia cycle length. This case illustrates the potential importance of AV timing in determining the hemodynamic effect of SVT.  相似文献

Radiofrequency lesions in the anterior, superior aspect of the tricuspid annulus result in selective elimination of fast pathway function in patients with typical atrioventricular (AV) nodal reentry tachycardia. This technique is simple and effective, but has been associated with a significant risk of inadvertent complete AV block. The purpose of this study was to compare the safety and effectiveness of two different techniques for radiofrequency catheter ablation of the fast AV nodal pathway. Initially, a fixed power output was used at each target site. This method was compared retrospectively to a newer technique where power output was gradually incremented at each site. Radiofrequency power was initially applied at 10 watts for 10–15 seconds. If no junctional ectopy or a change in PR intervoi was seen, power output was incremented by 2 to 4 watts every 10 to 15 seconds up to a maximum of 30 watts. Thirty seven of 38 (96%) patients treated using this incremental power output were cured of their AV nodal reentry tachycardia. None of these patients developed inadvertent complete AV block. In contrast, 92% of historic controls treated with a fixed power output between 20 and 30 watts achieved a primary success and nine of these 89 (10%) historic controls developed inadvertent complete AV block (P = 0.04). There was no difference in the amplitudes of atrial, His, or ventricular electrograms at the effective sites between the two groups. Conclusions: (1) the anterior approach to radiofrequency catheter ablation of typical AV nodal reentry is associated with a significant risk of inadvertent complete AV block if a fixed power output is used; (2) starting at low power and gradually incrementing the output during radiofrequency energy application reduces the risk of complete AV block; (3) this incremental technique does not compromise efficacy.  相似文献

Radiofrequency ablation of the atrioventricular conduction system was attempted in a 63-year-old man with drug refractory atrial fibrillation. A total of 5 radiofrequency pulses (750 kHz, power setting: 25-50 W, pulse duration: 9-20 sec) were delivered in a unipolar fashion via the distal electrode of a 7 Fr bipolar electrode catheter without induction of permanent AV block. No direct measurements of current (I) and voltage (U) were made. During the fifth pulse catheter disruption occurred at the interface of the shaft and the proximal electrode. Inspection of the catheter shaft revealed carbonized insulation material indicating overheating of the catheter tip. Overheating was presumably due to an impedance rise with unrecognized clot formation on the distal electrode. This led to progressive melting of insulation material during repeated radiofrequency applications and short circuiting of current flow to the proximal ring electrode that resulted in catheter disruption. This case report is the first to describe a serious complication of radiofrequency ablation. The complication might have been prevented by measurements of U and I, reflecting changes in impedance or by measurements of catheter tip temperature (T). It is concluded that measurements of U, I, and/or T are necessary to control the coagulation process thereby reducing the risk of serious complications during transcatheter radiofrequency ablation.  相似文献

Background: Most patients with symptomatic sinus node disease (SND) receive DDDR pacemakers (PM) in order to cover SND and atrioventricular (AV) block from the outset. But the concern about adverse effects of right ventricular pacing (RVP) is increasing. So far, data on the incidence of AV block in SND are based on clinical events. The study undertakes to assess and appraise AV block and atrial tachyarrhythmias (AT) from memory and electrograms of a dual-chamber PM set to an AAIR-DDDR switch mode (AAISafeR).
Methods: A dual-chamber PM incorporating the AAISafeR mode was implanted in 58 patients (70 ± 10 years, 28 males) with SND, but without AV block >I. AV block and AT episodes were retrieved from the PM memory and validated from electrograms. AV block episodes were classified potentially relevant while comprising AV block III or AV block I/II during exercise.
Results: The patients experienced a median of 90 (interquartile range 7–1,084) commutations. Possibly relevant AV block occurred in 32 patients (55%). Validation revealed high-quality PM-based categorization. The RVP prevalence was 0% (0–16%). The median AT prevalence was 0.03 (0–26) min/day. RVP was the only multivariate predictor of AT (P = 0.001).
Conclusions: Potentially relevant AV block occurs frequently in patients with SND. Nonetheless, the RVP prevalence is kept low through the AAISafeR mode. The protection of SND patients with demand-actuated ventricular pacing appears reasonable. The AT prevalence is low in SND patients treated by the AAISafeR mode. Even low RVP proportions appear to favor AT. Prospective evaluation is needed.  相似文献

Patients with atrial fibrillation or atrial flutter (AF) are candidates for radiofrequency (RF) catheter ablation of the atrioventricular (AV) node with the aim being to control heart rate. As patients wilh AF can have markedly impaired ventricular function, information concerning the hemodynamic effects of AV node ablation using RF current would be valuable. Fourteen consecutive patients (mean age 65 ± 3 years) with drug-resistant AF underwent AV node catheter ablation with RF current and had permanent pacemaker implantation. The mean left ventricular ejection fraction (FFJ by two-dimensional echoeardiography immediately before ablation was 42 ± 3% (range 14%–54%) and their mean exercise time was 4.4 ± 0.4 minutes. Complete AV block was achieved in all 14 patients with 6 ± 2 RF applications (range 1–18). There was no evidence of any acute cardiodepressant effect associated with delivery of RF current, and EF 3 days after ablation was 44 ± 4%. By 6 weeks after ablation, the left ventricular EF was significantly improved compared to baseline (47 ± 4% postablation vs 42 ± 3% preahlation; P < 0.05), and this modest increase in EF was accompanied by an improvement in exercise time (5.4 ± 0.4 min). In conclusion, delivery of RF current for AV node catheter ablation in patients with AF and reduced ventricular function is not associated with any acute cardiodepressant effect. On the contrary, improved control of rapid heart rate following successful AV node ablation is associated with a modest and progressive improvement in cardiac performance.  相似文献

To examine the characteristics of Haïssaguerre's slow potential (SP) specific to effective catheter ablation of the slow pathway in AV nodal reentrant tachycardia, the properties of SP and its recording site ware analyzed in 52 patients who underwent successful SP-guided ablation. The properties of SP included the ratio of the amplitude of SP to that of atrial potential (A)(SP/A), the SP duration, the interval between His-bundle potential (HP) and SP (HP-SP), the interval between A and SP (A-SP), the interval between SP and ventricular potential (V) (SP-V), and the ratio of A-SP to the interval between A and the V (A-SP/A-V). The SP recording site was determined by the ratio of the amplitude of A to that of V (A/V) and by the relative position of the ablation catheter on X ray (right anterior oblique projection), expressed as the ratio of the distance between the coronary sinus ostium and SP site to that between the coronary sinus ostium and HP recording site (relative SP position). Twenty-eight slow pathways were ablated with a single energy application, while the other 24 required applications ≥ 2. In all successful applications, SP/A, SP duration, HP-SP, A-SP. SP-V, A-SP/A-V, A/V, and relative SP position were 51 %± 25%, 28 ± 5 ms, -11 ± 9 ms, 57 ± 25 ms, 68 ± 13 ms, 46%± 9%, 15%± 13%, and 51%± 13%, respectively. A significant correlation was observed between the relative SP position and A-SP, and between the relative SP position and A-SP/A-V (r = 0.60 and 0.37, respectively), while it was not between the relative SP position and HP-SP, nor between the relative SP position and SP-V. When the characteristics of SP were comparatively analyzed between the effective and ineffective applications in 24 patients in whom applications ≥ 2 were required, there was no difference observed in HP-SP, A-SP, SP-V, A-SP/A-V, and A/V. However, SP/A, SP duration, and the relative SP position in the effective applications were all greater than those in the ineffective ones (56%± 20% vs 35%± 18%, P < 0.001; 29 ± 4 vs 26 ± 5 ms, P < 0.01; and 52%± 15% vs 33%± 11%, P < 0.001, respectively). These results indicate that SP with an amplitude over a half of A amplitude and recorded at the mid-septum of the tricuspid annulus can be a marker for successful slow pathway ablation. Although the local atrial electrogram appears late as the SP recording site shifts to the lower position, the timing of SP relative to HP and V remained unchanged, suggesting that SP is independent of the local atrial activation.  相似文献
