Training in liposuction for dermatologists

Dermatologists helped to develop the specialty of liposuction surgery and contributed to many of the innovations and instrumentation in this field. Dermatologists were trained side by side with other cosmetic surgeons as the procedure was introduced into the United States. Dermatologists have been important in teaching liposuction with the American Society of Liposuction Surgery and have been in the forefront of this developing new area of cosmetic surgery. Liposuction surgery for dermatologists is equal to that for all other surgical specialists.     

对矿山救护队员心肺复苏技能培训的研究分析

从2005年开始长期对矿山救护队员进行CPR的培训及训练。根据《美国心脏学会2005年心肺复苏和心血管急救国际指南》，国家矿山医疗救护中心（煤炭总医院）经验丰富的临床医师负责讲课，并指导用模拟人实际操作。5年来分批次直接培训全国24个省、市、自治区的重点60余支矿山救护队中核心成员500余名，再通过这些救护队核心成员进行基层的培训及演练。2006年起在全国矿山救援竞赛中进行CPR竞赛，本文作者参与第6～8届CPR竞赛的裁判工作。竞赛结果表明，第七和第八届竞赛中CPR操作扣分均显著低于第六届差异有统计学意义（P〈O．05），但第七、第八届间差异无统计学意义（P〉0．05）。实践表明，因大部分矿山救援队队员来自矿工，缺乏医学知识，系统培训非常必要，以点带面的方法行之有效。     

The leprosy elimination campaign in Jigawa,Nigeria: an opportunity for integration

Integration of leprosy control into the general health services is regarded as an important condition for increasing the accessibility and sustainability of leprosy services. However, it is often difficult to embark on such an integration process. In Jigawa State in Northern Nigeria, the leprosy elimination campaign was used as the initiator and catalyst for the integration process. In this article, this challenging process is described and analysed. Available information is used to identify the constraints that emerged and to assess the consequences of integration for important aspects of leprosy control, such as case detection and case-holding and the accessibility and quality of the provided services. Some lessons from this experience are drawn that can be helpful for integration in other States or countries.     

Prevention-of-impairment-and-disabilities activities in leprosy after integration: role for physical medicine and rehabilitation personnel

Referral options for specialist care for prevention of impairment and disabilities are imperative in order to make an integrated leprosy control system work. This requires an understanding of the disease, in addition to the special skills for managing specific disabilities. Physical medicine and rehabilitation (PMR) personnel are better equipped to handle leprosy-related disabilities. They are well versed with biomechanical aspects of deformities, and are competent to provide splints, orthoses, etc. to the needy persons, and they can assess sensory motor functions and deformities. If PMR personnel can be trained in deformity correction they can become valuable resource persons for secondary and tertiary care of leprosy-affected persons. PMR persons, therefore, have the opportunity to volunteer themselves for this job to fill the void created by the fading out of leprosy surgeons. They will also have to bear additional responsibility to train general health care workers so as to empower them to look after the needs of those disabled by leprosy, many of whom will continue to be available for a number of years to come.     

Leprosy control: the rationale of integration

After considering the situation and the perspectives of integration and the drawbacks that a vertical approach can represent for leprosy control, the author proposes the framework of control programmes as a systemic model for comprehensive health care. The structure that health services in developing countries are adopting in order to implement PHC allows for an horizontal integration of specific activities; conversely, activities which have already proved their value for leprosy control can easily enlarge their scope and include other prevalent conditions. Integration leads to an improvement in patients' and health workers' attitudes; provided that the necessary supervision is guaranteed, integration is feasible and warrants more effective patients' care and a better exploitation of resources in order to reduce the specific risk in the community.
