Objectives Acute atrial fibrillation and flutter are very common arrhythmias seen in emergency department (ED) patients, but there is no consensus for their optimal management. The objective of this study was to examine the efficacy and safety of intravenous (IV) procainamide for acute atrial fibrillation or flutter.
Methods This health records review included a consecutive cohort of ED patients with acute-onset atrial fibrillation or atrial flutter who received IV procainamide at one university hospital ED during a five-year period. The standard clinical protocol involved IV infusion of 1 g of procainamide over 60 minutes, followed by electrical cardioversion if necessary. A trained observer extracted data from the original clinical records. Outcome measurements included conversion to sinus rhythm, adverse events, and relapse up to seven days.
Results The 341 study patients had a mean age of 63.9 years (SD ± 15.5 years), and 56.6% were male. The conversion rates were 52.2% (95% confidence interval = 47% to 58%) for 316 atrial fibrillation cases and 28.0% (95% confidence interval = 13% to 46%) for 25 atrial flutter cases. Mean dose given was 860.7 mg (SD ± 231.2 mg), and median time to conversion was 55 minutes. Adverse events occurred in 34 cases (10.0%): hypotension, 8.5%; bradycardia, 0.6%; atrioventricular block, 0.6%; and ventricular tachycardia, 0.3%. There were no cases of torsades de pointes, cerebrovascular accident, or death. Most patients (94.4%) were discharged home, but 2.9% of patients returned with a recurrence of atrial fibrillation within seven days.
Conclusions This study of acute atrial fibrillation or flutter patients treated in the ED with IV procainamide suggests that this treatment is safe and effective in this setting. Procainamide should be prospectively compared with other ED strategies.  相似文献

As part of a new effort to develop an implantable drug infusion pacing system to treat atrial fibrillation, this study examined the effects of rapid intracardiac procainamide infusion in humans with pacing-induced atrial fibrillation. Twenty patients with atrial fibrillation for >5 minutes during an EP study received 500mg of procainamide either via a peripheral venotis infusion (n=5) or directly in the right atrium (n=15). Peak coronary simis and femoral vein procainamide blood levels (mean ± SEM) during 10, 5, and 3.3 minute central infusions were 17.0 ± 4.1, 25.1 ± 4.5, 45.6 ± 5.1 and 11.3 ± 3.2, 17.1 ± 6.4, 18.7 ± 5.0, respectively. In contrast, peak coronary sinus and femoral procainamide levels following the 5 minute intravenous infusion were 17.7 ± 5.1 and 9.3 ± 2.1. Changes in QT, QTc, QRS, and RI intervals were similar at each infiision rate. Systolic blood pressures (BP) decreased more with higher procainamide infusion rates but similar when comparing intravenous versus central drug administration at the same rate. The mean ± SEM decreases in blood pressure with the 10, 5, and 3.3 min procainamide infusions were 12f5, 20f11, and 39f14, respectively. Conversion to sinus rhythm was not a primary endpoint given the often transient nature of acute atrial fibrillation in this setting. We conclude that significantly higher femoral vein and coronary sinus procainamide levels can be achieved by central rather than peripheral drug infusion. These data support that concept that rapid central infitsion of anti-arrhythmic therapy can result in high intracardiac levels of antifibrillatory agents for the treatment of paroxysmal atrial fibrillation.  相似文献

Radiofrequency (RF) catheter ablation of the atrioventricular node (AVN) and implantation of a ventricular pacemaker can improve cardiac performance in patients with congestive heart failure (CHF) and uncontrolled atrial fibrillation (AF). Alternatively. RF catheter modification of the A VN has been proposed to slow ventricular response during AF without requirement for permanent pacing. Among 44 consecutive patients (mean age 69.7 ± 10.2 years) with drug resistant chronic AF, 22 (group I) had AVN ablation with permanent ventricular pacemaker implantation, while 22 patients had attempted AVN modification. Complete AV block was obtained in all group I patients while only seven (32 %) A VN modification patients (group II) had permanent slowing of ventricular rate. Among patients in group I, mean left ventricular ejection fraction (EF) increased from 32.2%± 8.8% before ablation to 41.9%± 14.6% 4-weeks postablation (P < 0.01); exercise tolerance time (ETT) increased from 2.9 ± 2.2 minutes to 4.5 ± 2.9 minutes (P < 0.01); and quality-of-life score decreased from 66.1 ± 22.6 to 36.9 ± 17.1 (P < 0.01). By comparison, there was only a small increase in ETT in the seven successful group II patients (2.4 ± 1.8 minutes to 3.0 ± 1.9 minutes; P < 0.05) and there was no significant change in EF or quality-of-life. While AVN ablation can occasionally have transient adverse effects, it is more effective than AVN modification for improving cardiac performance in selected patients with CHF and AF.  相似文献

It is well known that the restoration of sinus rhythm is not always associated with the return of effective atrial contraction. Atrial ejection force (AEF) is a noninvasive Doppler derived parameter that measures the strength of the atrial contraction. The aim of the present study was to use pulsed-Doppler echocardiography to determine if different modalities of cardioversion influence the delay in the return of effective atrial contraction after cardioversion. DC shock and pharmacological therapy were compared. Sixty-eight patients were randomly cardioverted, either using DC shock or i.v. procainamide. The patients who were restored to a sinus rhythm had a complete Doppler echocardiographic examination within 1 hour after the restoration, after 24 hours, after 1 month, and after 3 months. AEF was measured and compared in the two groups of patients and within the same group. AEF was greater immediately and at 24 hours after cardioversion in patients who underwent pharmacological therapy compared to patients treated with DC shock (peak A wave, 60 ± 9 vs 31 ± 8 msec, P < 0.001; AEF 11.3 ± 3 vs 5 ± 2.9 dynes, P < 0.001). In both groups, AEF increases over time. In conclusion, AEF is a noninvasive parameter that can be easily measured after cardioversion and can give accurate information about the recovery of left atrial mechanical function. This finding may have important implications for guiding the anticoagulant therapy after cardioversion.  相似文献

High energy internal cardioversion has been proposed as an alternative method to cardiovert drug refractory or external cardioversion refractory atrial fibrillation. However, the safety of this technique has not been clearly evaluated. We reviewed findings in 53 patients who underwent 55 sessions of high energy internal cardioversion (2 patients underwent 2 sessions] for termination of longstanding atrial fibrillation. Shocks energy varied from 70–270 J. Three patients had 3 shocks during the same session, 5 had 2, and 47 only 1. Success rate was 89% (success was defined as immediate conversion to normal sinus rhythm).
Low cardiac output occurred in two patients, and resulted in the death of one of these individuals, a patient with significant hypertrophic cardiomyopathy and heart failure. The other patient recovered completely. In 11% of the cases, shock induced transient atrioventricular block, necessitating ventricular pacing until sinus rhythm was restored. In three patients, a moderate but asymptomatic and uncomplicated pericardial effusion was diagnosed on echocardiogram. Finally, four patients had side effects related to venous puncture, which resolved spontaneously. These results suggest that high energy internal cardioversion is effective for conversion of atrial fibrillation. However, the technique may not be optimal in patients with advanced hypertrophic cardiomyopathy and in such cases the technique should be used carefully and only in the case of failure of external cardioversion; no more than two shocks should be delivered during the same procedure. Temporary ventricular pacing should be provided in all patients and an echocardiogram should be performed before patients are being discharged.  相似文献

Transesophageal Echocardiography in Patients with Atrial Fibrillation   总被引:2,自引:0,他引:2  
Over the past decade transesophageal echocardiography (TEE) has become an integral tool in the management of patients with AF. Thrombi can be readily detected and the function of the left atrium and left atrial appendage (LAA) can be easily assessed by this modality. With this knowledge, informed decisions regarding anticoagulation, cardioversion, and the likelihood of restoring and maintaining sinus rhythm can be made. An alternative TEE-guided strategy for achieving sinus rhythm in patients with AF is now available with efficacy and safety similar to the conventional approach. Refinements of this strategy may include the use of low molecular weight heparin to avoid hospitalization in patients planned for cardioversion. Further uses of TEE in patients with AF are in their early stages including evaluation of patients following pulmonary vein radiofrequency ablation and those undergoing percutaneous or surgical closure of the LAA. The influence of the timing of a nontrunsmitted or transmitted atrial impulse on the atrioventricular (AV) conduction time of the subsequent impulse was studied in nine isolated rabbit hearts. AV conduction curves were determined by applying the atrial extrasfimulus test. The extrasfimulus was delivered preceded or not by an interposed afrial impulse whose coupling interval with respect to the last atria] beat of a basic train was kept constant at 100, 120, 140, 160, 175, 200, 225, 250, and,300 msec. In all experiments, there was a "concealment interval," i.e., the AV effective refractory period was longer than the atrial functional refractory period, and in seven experiments was comprised between 100 and 160 msec. For any given extrastunulus coupling interval in the presence of an interposed nontransmitted atrial impulse, AV conduction time was significantly greater than in its absence; the increase was greater than the longer the nontransmifted atrial impulse coupling interval, i.e., the shorter the subsequent transmitted impulse coupling interval with respect to the previous interposed nontransmitted impulse. The AV conduction curves relating the extrastimulus AV conduction time to its coupling interval with respect to the last atrial impulse of the basic train fitted to a hyperbolic model both in the absence of the interposed atrial impulse and in its presence (mean square residual 31 ± 23 msec2). and the interposed atrial impulse modified the constants of the functions; the slope of the linear transformations was progressively more negative as the interposed atrial impulse was delayed. Furthermore, the effects of the interposed atrial impulse—transmitted or not—on AV conduction time of the subsequent impulse were qualitatively similar, their magnitude depending on the time elapsed between the two.  相似文献

Quadruple Pads Approach for External Cardioversion of Atrial Fibrillation   总被引:2,自引:0,他引:2  
MARROUCHE, N.F., et al. : Quadruple Pads Approach for External Cardioversion of Atrial Fibrillation. This study examined the alternative of transthoracic quadruple pads shock delivery of two simultaneous 360-J shocks to convert refractory AF in patients failing standard external cardioversion. Forty-six patients (  mean age 58 ± 11 years, 23 men  ) with chronic AF (  duration 14 ± 17 months, range 1–60 months  ) were included. The left atrial diameter was  47 ± 7 mm  . The left ventricular ejection fraction was  59 ± 11%  . Antiarrhythmic drugs had failed to convert 44 (96%) of these patients. All patients underwent conventional external transthoracic cardioversion with pads applied in the antero-apical position using energy settings of 200 and 360 J, consecutively. In all patients who failed conventional cardioversion, quadruple pads were applied. Quadruple pads consisted of four pads, two in the antero-posterior position and two in a second apex-posterior position. Standard cardioversion to sinus rhythm was successful in 19 (41%) patients after use of a single 200-J shock and an additional 8 (17%) after a single 360-J shock. The total success rate was 58% after conventional cardioversion. The quadruple pads were successful in 14 (74%) of the remaining 19 patients. Four of the five patients who failed the quadruple pads approach subsequently also failed internal cardioversion. Thus, the cardioversion success rate was increased from 48% using the conventional approach to 89% using the quadruple pads approach. Quadruple pads external cardioversion is highly effective in converting chronic AF refractory to standard shock protocols to sinus rhythm. Moreover, the failure of the quadruple pads approach seems to predict poor response to internal cardioversion.  相似文献

Atrial fibrillation (AF) has been associated with premature beats and decreased atrial conduction velocity. This study examined a new index of dynamic inter-atrial conduction time (iaCT) in patients with paroxysmal AF (PAF). We compared 42 consecutive patients with paroxysmal AF (mean age = 52 ± 16 years) without structural heart disease with 39 age-matched patients (mean age = 49 ± 15 years) who underwent ablation of junctional tachycardias. Prior to investigation, all antiarrhythmic drugs were discontinued for an appropriate period of time. The following measurements were made: baseline iaCT (iaCTb) between high right atrium (HRA) and distal coronary sinus, iaCT during HRA pacing S1S1 600 ms (iaCTS1), maximum prolongation of iaCT during S2 and S3 delivery (iaCTS2, iaCTS3). We then derived the decremental index (DI), the maximum percent prolongation of iaCT = iaCT S3-iaCTS1/iaCTS1%. In patients with PAF, iaCTb was 81.3 ± 24 ms versus 59.5 ± 14 ms in controls (P = 0.0001). Atrial fibrillation was reproducibly and easily induced with a prominent increase in iaCT in 11 patients with AF. In this subgroup DI was 92 ± 17%, compared to 45 ± 21% in the other patients with AF (P = 0.0001) and 21 ± 15% in the control group (P = 0.0001). Spontaneous isolated or repetitive ectopic activity was observed in 11 patients with AF (26%), and decremental atrial conduction was observed in 76% of patients with AF. This study supports the role of dynamic inter-atrial conduction disturbances in patients with lone PAF. The DI may be a new index of vulnerability to paroxysmal AF.  相似文献

Clinical electrophysiology has not yet clearly defined atrial features that can predict spontaneous occurrence of atrial fibrillation (AF). The aim of this work was to identify atrial electrophysiological features that can distinguish Wolff-Parkinson-White patients with spontaneous AF from those without this arrhythmia. Sixty-nine patients with Wolff-Parkinson-White were divided into three groups: group I (16 patients) with spontaneous AF; group II (35 patients) with reciprocating tachycardia but not AF; and group III (18 patients) asymptomatic without documented arrhythmias. Atrial effective refractory periods (ERPs) and intraatrial conduction times in response to premature extrastimuli were analyzed. The latter were evaluated as the A1A2 interval minus the correspondent S1S2 interval (A1A2-S1S2), S2A2 and the interval A1A2 following the shortest S1S2 producing atrial activation (FRP'). All the parameters have been evaluated in two atrial sites and at two atrial pacing cycle lengths (600 and 400 ms). For all the parameters, the difference (“gradient”) was calculated between the values of the same parameter measured at the atrial pacing cycle length of 600 ms and that found at the atrial pacing cycle length of 400 ms in the same recording site in each patient was calculated. Atrial FRP did not differ significantly in the three groups. Intraatrial conduction parameters, evaluated in the high right atrium (HRA), were longer when measured at an atrial pacing of 400 ms and showed a lack of rate adaptation in patients with spontaneous AF. In group I patients in particular, FRP’became longer with the increase of atrial rate, while in groups 2 and 3, it usually shortened. The mean gradient of HRA FRP’was -15.0 ± 19 ms in group I as compared to 5.7 ±13 ms in group II and 6.4± 13 ms in group III (P < 0.001); sensitivity. specificity, and negative predictive value of a negative gradient in the identification of patients with spontaneous AF, were, respectively, 83%, 75%, and 93%. Patients from groups 2 and 3 did not differ in any of the analyzed parameters. Patients with Wolff-Parkinson-White and spontaneous AF showed prolonged intraatrial conduction times and a different behavior in response to modification of heart rate. (PACE 1997;20[Pt. To study the characteristics of the ventriculoatrial conduction system in palienfs capable of rapid antegrade atrioventricuiar conduction, eiectrophysiologic studies were performed in 23 subjects capable of 1:1 atrioventricular conduction at atrial cycle lengths < 300 ms (Group I). and in 23 subjects with normal 1:1 atrioventricular conduction (Group II). During venfricular pacing, ventriculoatrial block at all cycle lengths was seen in 5/23 (22%) in Group I and in 7/23 (30%) in Group II patients (p = NS). In the remainder, the minimum ventricuar pacing cycle length maintaining 1:1 ventriculoatrial conduction was 359 ± 85 ms in Group I, compared to 444 ± 118 ms in Group II (p < .02). Both flat and exponential VA conduction interval curves, drawn as a function of pacing cycle length, were observed in both groups. Discontinuous ventricuioatrial conduction curves were seen in 5/18 (28%) Group I and 1/16 (6%) Group II patients (p = NS). In conclusion, retrograde ventriculoatrial conduction, when present in patients capable of rapid 1:1 atrioventricular conduction, is maintained at shorter cycle lengths than in patients with normal atrioventricular conduction. Quantitative, rather than qualitative, differences distinguish the two groups.  相似文献

Background: The activation of the renin–angiotensin–aldosterone system has been implicated in the progression of atrial structural remodeling during atrial fibrillation (AF). However, consequences of the changes of aldosterone in AF have not been evaluated. Objectives: This study's aim was to evaluate changes of serum aldosterone concentration after successful cardioversion of persistent AF and to determine the prognostic value of these changes. Methods: The prospective, single center study included 45 consecutive patients with nonvalvular persistent AF and preserved left ventricular systolic function, referred for cardioversion. None of the patients were taking aldosterone antagonists. Blood samples for aldosterone measurement were collected twice: 24 hours before and 24 hours after cardioversion. Results: Forty‐three patients were successfully converted to sinus rhythm. On the 30th day following cardioversion, 24 patients maintained sinus rhythm (group A), 19 patients relapsed to AF (group B). Serum aldosterone concentration before cardioversion did not differ significantly between both groups (175.6 ± 112.82 pg/mL vs 125.8 ± 51.2 pg/mL; P = 0.25). However, in group A serum aldosterone level decreased significantly within 24 hours after cardioversion, from 175.6 ± 112.8 pg/mL to 101.4 ± 44.2 pg/mL (P = 0.0034). In group B, the aldosterone level before and after cardioversion did not differ significantly (125.8 ± 51.2 pg/mL vs 118.2 ± 59.6 pg/mL; P = 0.68). Logistic regression analysis revealed that a decrease in plasma aldosterone concentration after direct current cardioversion more than 13.2 pg/mL predicted sinus rhythm maintenance in a 30‐day follow‐up, with 87% sensitivity and 64% specificity. Conclusions: There is a positive correlation between the fall in aldosterone concentration 24 hours after cardioversion and maintenance of sinus rhythm during 30 days of observation. The aim of this study was to evaluate the efficacy of low energy internal atrial cardioversion in restoring sinus rhythm (SR) in patients with chronic atrial fibrillation (AF) persisting > 1 year. Fifteen patients with chronic AF lasting > 1 year (from 13–48 months, mean 24 ± 13 months) were studied. R wave synchronized 3/3 ms biphasic shocks were delivered between right atrial and coronary sinus (left pulmonary artery in five patients) electrodes. Sedatives or anesthetics were administered only at the patient's request. Results: Stable SR was restored in 14 (93%)of 15 patients after shocks with a mean leading edge voltage of 377 ± 77 V (range 260–500) and a mean delivered energy of 7.3 ± 3.4 J (range 2.6–12.9). The procedure was performed without anesthesia in 6 (40%) patients. All successfully cardioverted patients were treated with flecainide, sotalol, or amiodarone. During a follow up of 7.7 ± 7.9 months (range 1–24) AF recurred in five (36%) patients. Three of five AF recurrences occurred within 3 days after conversion to SR. Conclusion: Internal low energy atrial cardioversion is highly effective in restoring SR even in patients with AF lasting > 1 year. The long-term results from the standpoint of freedom from AF recurrences, are satisfactory, although additional antiarrhythmic treatment is required, particularly in the first days after conversion.  相似文献

Although electrical cardioversion of atrial fibrillation (AF) is frequently performed, initial energy requirements for cardioversion of persistent AF is still a matter of debate. The aim of the study was to determine the efficacy of biphasic shocks for transthoracic cardioversion of persistent AF and to predict adequate initial energy. A prospective study enrolled 94 consecutive patients with persistent AF, who were referred for elective cardioversion with a biphasic waveform. The paddles were placed in the anterolateral position. A step-up protocol was used to estimate the cardioversion threshold. The initial shock energy was 50 J, with subsequent increments to 100, 200, and 300 J in the event of cardioversion failure. The mean age of the study group was of about 65 years (6 ± 11 years) and a median duration of AF was 65 days (3–324). Sixty-two out of 94 patients were men, 55% of the study population was classified as having well-controlled hypertension. The overall success rate of cardioversion was 89%, with a mean 2.2 ± 1.4 shocks, and effective J 217.8 ± 113 delivered during repeated cardioversions. The success rate of low energy shocks: 50 and 50 +100 J was 51%. By logistic regression analysis the only independent predictor of success at low energy shock was shorter duration of AF (r =−0.51; P = 0.02). Patients with shorter duration of AF have a higher probability for successful cardioversion with low energy. We attempted radiofrequency ablation of the AV junction with a sequential right- and left-sided approach in 78 patients affected by severely symptomatic, drug refractory atrial fibrillation. Stable third-degree A V block was obtained in 99% of cases and, after 3 months, persisted in 92% of cases. Single session, stepwise, radiofrequency modulation of the AV node was attempted in 13 patients with paroxysmal atrial fibrillation. During sinus rhythm, ablation of the slow and fast AV node pathways was performed in order to increase the nodal refractory period or to slow conduction. Clinically successful modulation of AV conduction was achieved in 15% of cases and persisted during a 3-month follow-up. In conclusion. AV junction ablation is a well-established means of treating atrial fibrillation, but implies the implant of a permanent pacemaker. AV node modulation avoids the pacemaker implant, but is efficacious only in a minority of patients. Thus, in patients affected by paroxysmal atrial fibrillation, AV modulation should be attempted first; if this is ineffective. AV ablation can be performed during the same session.  相似文献

Paroxysmal supraventricular tachycardia (SVT) may have a variety of hemodynamic effects depending on rate, patient volume status, and presence of structural heart disease or left bundle branch block. We report a case of a patient with atrial tachycardia and dual atrioventricular (AV) nodal physiology who developed profound hypotension during transition from fast to slow AV nodal pathway conduction, despite similar tachycardia cycle length. This case illustrates the potential importance of AV timing in determining the hemodynamic effect of SVT.  相似文献

Background: Despite using different electrode positions, “conventional” external DC cardioversion in patients with atrial fibrillation is ineffective in 6%–50% of cases. An alternative when DC cardioversion is not successful is low energy internal cardioversion, which is performed at increased risk. We tested the hypothesis that optimization of electrode pad position under fluoroscopy to encompass as much atrial muscle as possible might improve the success rate of external cardioversion and thus minimize the need for internal cardioversion. Methods: Fifteen (9 males, 6 females) patients (age: 54 ± 15 years, weight: 124 ± 35 kg) with chronic atrial fibrillation (> 8 weeks) who had undergone unsuccessful conventional external cardioversion entered the study. Repeat conventional external cardioversion with electrodes in standard (right anterior and left posterior) positions was followed by “optimized” external cardioversion by positioning electrodes under fluoroscopy (using metallic markers). In case of failure, internal cardioversion was performed. Results: All 15 patients had undergone unsuccessful conventional external cardioversion with 360-J shocks. Eight patients (group A) reverted to sinus rhythm with one or two 360-J shocks using fluoroscopy-guided pad placement (53%). Six of the remaining 7 (86%) patients (group B) had successful internal cardioversion with biphasic shocks (12 ± 3 J). The body weight and body mass index were statistically lower in group A vs group B (106 ± 27 vs 145 ± 33 kg, p = 0.03 and 35 ± 8 vs 45 ± 8 kg/m2, P = 0.48, respectively). There was no statistically significant in age, height, body surface area, duration of atrial fibrillation, amiodarone therapy, ejection fraction, or underlying heart disease. Conclusion: Unsuccessful external DC cardioversion, in some patients, is in part due to suboptimal conventional positioning of electrode pads that can be improved under fluoroscopic guidance by achieving the best possible vector encompassing the right and left atria. The optimized external cardioversion technique may minimize the need for internal cardioversion, which remains an effective approach when external cardioversion fails.  相似文献

Background

The management of emergency department (ED) patients with presumed recent-onset atrial fibrillation or flutter ≤ 48 h in duration varies widely.

Objective and Method

We conducted a prospective study across three affiliated community EDs within a large integrated health care delivery system to describe the management of patients with recent-onset atrial fibrillation or flutter, to determine the safety and effectiveness of ED cardioversion, and to measure the incidence of thromboembolism 30 days after discharge.

Results

We enrolled 206 patients with convenience sampling between June 2005 and November 2007. Mean age was 64.0 ± 14.4 years (range 21–96 years). Patients were grouped for analysis into four categories based on whether cardioversion was 1) spontaneous in the ED (59; 28.6%); 2) attempted with electrical or pharmacological means (115; 56.3%), with success in 110 (95.7%); 3) hoped for during a short stint of home observation (16; 7.8%, 11 of which spontaneously converted to sinus rhythm within 24 h); or 4) contraindicated (16; 7.8%). Of the entire group, 183 (88.8%) patients were discharged home. Adverse events requiring ED interventions were reported in 6 (2.9%; 95% confidence interval [CI] 1.1–6.2%) patients, all of whom recovered. Two (1.0%; 95% CI 0.1–3.5%) patients were found to have an embolic event on 30-day follow-up.

Conclusions

Our approach to ED patients with presumed recent-onset atrial fibrillation or flutter seems to be safe and effective, with a high rate of cardioversion and discharge to home coupled with a low ED adverse event and 30-day thromboembolic event rate.  相似文献
