We report about the case of a 4½-year delayed screw fixation of a tibial posterior cruciate ligament (PCL) avulsion fracture in combination with a posterolateral stabilization. After open reduction and rigid screw fixation, combined with a posterolateral stabilization using a semitendinosus tendon autograft, the patient returned to full activity in sports and daily life. If the PCL substance is sufficient, a delayed refixation in an old PCL avulsion injury seems to be a viable alternative to PCL reconstruction.  相似文献

Reports of isolated avulsion fracture at the planter lateral base of the first metatarsal without injury of the tarsometatarsal joint are very rare. A 24 year old man sustained an avulsion fracture at the plantar lateral base of the first metatarsal. Normal alignment of metatarsal bones and tarsometatarsal joint was maintained. In this paper, we describe internal fixation of the displaced fragment using x ray and minimally invasive surgery with good results.  相似文献

This study reports a case of unusual vascular complication related to the staple fixation for the tibial avulsion fracture of the posterior cruciate ligament (PCL). The patient, who experienced recurrent hemarthrosis 12 months after staple fixation for the avulsion fracture of the PCL, was successfully managed by removing the staple and suturing the bleeding focus of the popliteal artery. Injury to the popliteal artery by the prominent staples could be the culprit causing the recurrent hemarthrosis. This type of delayed popliteal artery injury should be kept in mind in open reduction and internal fixation for the tibial avulsion fracture of the PCL.  相似文献

The posterior cruciate ligament (PCL) plays a major role in knee stabilization, and clinical studies have shown an increase in incidence of its injury. Due to the surrounding neurovascular elements in the popliteal space, open approaches to repair such injury are difficult to perform. The “safe postero-medial approach” to PCL avulsion fracture is a simple approach, does not require exploration of the neurovascular elements, and produced satisfactory results in the majority of patients.  相似文献

Posterior cruciate ligament (PCL) avulsion fractures are uncommon injuries, and their treatment, still, remains difficult. There are procedures described in the literature which are minimal invasive and use either sutures or screws to stabilize the avulsed fragment. The purpose of this study was to present an innovative arthroscopic technique using a suspensory device with specific biomechanical properties. The aim of this study was to determine whether this technique was effective in the treatment of PCL avulsion fractures and applicable in everyday clinical practice.  相似文献

Avulsion fractures of the posterior cruciate ligament (PCL) at the tibial insertion site are extremely rare in children. Because the avulsed osteochondral fragments can be poorly seen on plain radiographs, correct diagnosis is often delayed. Delayed diagnosis of PCL avulsion fracture can lead to difficulties in treatment due to secondary changes in the osteochondral fragment and ligament substance. Three cases of tibial avulsion fractures of the PCL in children surgically treated in a single medical centre are reported in the present study. Level of evidence V.  相似文献

We present the case of an 18-year-old man involved in a fall with blunt abdominal trauma. The patient had hypovolemic shock and findings of an acute abdomen. Initial computed tomography (CT) showed pulmonary contusion, pneumohemothorax, hemoperitoneum, hepatic contusion, right kidney laceration and vascular avulsion, rupture of the mesenteric vein, rupture of the right rectus muscle with bowel hernia, and infrarenal aortic dissection. There were no signs of limb or medullar ischemia. After hemodynamic stabilization and surgical repair of the associated lesions, the dissection was successfully treated with a self-expanding aortic Wallstent. Postprocedure CT showed a well-positioned patent stent and the patient was discharged asymptomatic. Percutaneous endovascular stent implantation is minimally invasive and seems to be a safe treatment for traumatic dissection of the abdominal aorta.  相似文献

Posterior cruciate ligament(PCL) injuries are less frequent than anterior cruciate ligament(ACL) injuries, but are presumably more common than once thought. Thirty-nine patients with PCL injuries identified on MR images were studied. The criteria for PCL injury were complete tear, partial tear, and avulsion fracture. The approximate site of a partial tear was categorized as proximal, midsubstance, distal, or combination. Fourteen patients(35.9%) had complete tears of the PCL, 21 patients(53.8%) had partial tears, and four patients(10.3%) had avulsion fractures. A total of 12 patients (30.7%) had isolated PCL injuries, while the remaining 27 patients demonstrated evidence of other coexistent knee injuries, such as meniscal tears and ligamentous injuries. Of coexistent knee injuries, meniscal tears(18 patients, 46.2%) were most often seen.  相似文献

Injury to the ACL or PCL of the knee most commonly involves a tear of the collagenous fibers of the ligament. Less frequently, a cruciate ligament injury involves an avulsion fracture at the origin or insertion of the ligament, usually from the insertion site on the tibial surface. Avulsion fractures of the cruciate ligaments are important, as they can be identified on radiographs, allowing a specific diagnosis. Although more common in children, when they occur in adults, they are more commonly associated with other injuries. The treatment of cruciate ligament avulsion fractures is different than the treatment of intrasubstance tears of the cruciate ligaments. These injuries can be treated conservatively or surgically with good outcomes. Recently arthroscopic fixation of these injuries with various fixation devices has become more frequent. Treatment largely depends on the type of fracture, particularly, the size, displacement, comminution, and orientation of the avulsed fracture fragment, in addition to the integrity of the attached cruciate ligament. This review article covers the anatomy and biomechanics of the cruciate ligaments, their injury patterns, and approach to management.  相似文献

Ipsilateral femur fracture, patellar fracture, and tibial avulsion fractures of anterior and posterior cruciate ligament injuries are reported. We know of no other report of an injury such as this in the literature. We sutured the tibial avulsion fractures by transtibial suturing technique and internally fixated the femur by intramedullary interlocking nailing 2 weeks after the trauma. After 1 year the results were evaluated as very good. Early surgical repair is valuable in these injuries.  相似文献

目的 探讨股骨近端防旋髓内钉(PFNA)微创植入治疗高龄股骨粗隆间骨折的手术技巧及临床疗效.方法 采用PFNA微创植入治疗67例70岁以上高龄粗隆间骨折患者,重视围手术期处理,总结患者临床资料.结果 术中出血60~100 ml,无输血及死亡病例.67例均获随访,时间12~36个月,骨折均愈合.结论 对高龄粗隆间骨折患者选用PFNA微创植入,手术创伤小,安全性高;掌握微创技巧、重视围手术期处理是预防并发症、降低死亡率的关键.  相似文献

目的 探讨C形臂X线机辅助下L形解剖锁定钢板微创治疗Rüedi-AllgöwerⅡ、Ⅲ型闭合性Pilon骨折的临床疗效。方法 在2013-01至2018-12武警第二机动总队医院采用术中分次C形臂X线机透视骨折部位,有限切开组织,骨折复位满意,L形锁定钢板固定良好,微创治疗Rüedi-AllgöwerⅡ、Ⅲ型闭合性Pilon骨折24例患者。结果 24例均获得随访,时间6~24个月,平均(15±3.25)个月,骨折愈合佳,无切口感染、血管损伤及踝关节僵硬等并发症;仅有1例出现创伤性关节炎,后经康复治疗1年后恢复正常。根据Mazur踝关节功能评分以及骨折的复位愈合程度评定:优16例,良5例,可3例,优良率为87.5%。结论 C形臂X线机辅助下L形锁定钢板微创治疗Rüedi-AllgöwerⅡ、Ⅲ型闭合性Pilon骨折,临床观察,疗效满意。

目的探讨微创内固定支架经皮微创内固定治疗C型老年桡骨远端骨折的临床效果。方法回顾性分析2015年3月—2018年3月厦门大学附属成功医院收治C型老年桡骨远端骨折患者97例,男性62例,女性35例;年龄60~78岁,平均67.4岁。按固定方式不同分为外固定支架组(48例)和微创组(49例),外固定支架组行外固定支架固定治疗,微创组行微创内固定支架经皮微创内固定治疗。比较两组患者手术效果、影像学指标、骨折端骨痂生长愈合情况及术后并发症情况。结果术后6个月,微创组优良率(93.88%)高于外固定支架组(79.17%)(P<0.05),桡骨高度(12.42±4.60)mm高于外固定支架组(8.53±5.59)mm,尺偏角(22.50±5.96)°、掌倾角(11.06±5.82)°大于外固定支架组尺偏角(18.93±5.20)°、掌倾角(8.45±4.78)°(P<0.05);微创组术后1、2个月骨折端骨痂生长愈合情况Lane-Sandhu X线片评分分别为(1.67±0.29)分、(3.78±0.66)分,优于外固定支架组(1.15±0.37)分、(2.82±0.54)分(P<0.05);微创组术后并发症发生率(6.12%)低于外固定支架组(20.83%)(P<0.05)。结论微创内固定支架经皮微创内固定治疗C型老年桡骨远端骨折患者手术效果好,有利于骨折端骨痂生长愈合,术后并发症较少,临床治疗可参考。  相似文献

The aims of this study were to assess the incidence of reverse Segond fracture, to examine the associated ligamentous injuries, and to examine how often reverse Segond fracture coexists with a knee dislocation. At a level 1 trauma center, an 11-year period of emergency department multidetector-row computed tomography (MDCT) examinations for knee trauma was evaluated for reverse Segond and Segond fractures. Surgical findings served as the reference standard for intra-articular injuries. The hospital discharge register was searched for the diagnosis of knee dislocation from August 2000 through the end of August 2011. A total of 1,553 knee MDCT examinations were evaluated. Ten patients with a reverse Segond fracture were found, comprising 0.64 % of emergency room acute knee trauma MDCT examinations. Seven patients who had a reverse Segond fracture were operated: Three had an avulsion fracture of the anterior cruciate ligament, one had an avulsion fracture of posterior cruciate ligament, two had a lateral meniscal tear, and two had a medial collateral ligament tear. The ratio of reverse Segond fractures to Segond fractures was 1:4. None of the 71 knee dislocation patients had a reverse Segond fracture. Reverse Segond fracture is a rare finding even in a level 1 trauma center. Cruciate ligament injuries appear to be associated with avulsion fracture, but every patient does not have PCL injury, as previously reported. Our results do not support the association of knee dislocation with reverse Segond fracture.  相似文献

目的探讨应用半月板缝合系统(Fast-fix)微创治疗内半月板纵裂的临床效果。方法 2015年6月~2015年12月笔者科室应用半月板缝合系统治疗内侧半月板纵裂患者8例,均为男性,年龄18~29岁,平均20岁。术后采用Lysholm评分系统进行疗效评价。结果本组8例均获随访2~7个月,平均4.5个月。全部病例疼痛、活动度较术前明显改善。结论半月板缝合系统微创治疗内侧半月板纵裂疗效满意,能明显改善疼痛、关节活动度,是一种值得选择的治疗方式。  相似文献

Partial articular-surface tendon avulsion (PASTA) lesions of the supraspinatus muscle represent a common cause for shoulder impairment and a preceding pathology for full-thickness tendon tears. Arthroscopic tendon repair is a possible surgical method of treatment. The purposes of cuff repair are anatomical tendon healing, prevention of tear size progression to completion and reduction of shoulder pain. In this report, we describe a transtendon arthroscopic technique of transosseous refixation of articular-side partial tears leaving the bursal layer of the supraspinatus tendon intact. A curved hollow needle is used to perform an all arthroscopic transosseous mattress suture. Thus, anatomical tendon-to-bone contact of the rotator cuff to the footprint is restored. Preliminary clinical results of 16 patients are convincing with significant pain relief and functional improvement.  相似文献

Total knee arthroplasty represents a well-established and successful procedure; however, ligament incompetence is known to negatively affect surgical outcomes. Here we present an unusual case of early total knee arthroplasty failure secondary to femoral posterior cruciate ligament (PCL) avulsion and associated lateral collateral ligament (LCL) tear, treated successfully with primary PCL repair and LCL reconstruction. For LCL reconstruction, a peroneus longus allograft was passed through an anterior to posterior bony tunnel in the fibular head and docked into a horizontal femoral tunnel.
