目的通过对根管治疗失败病例根管内微生物进行培养检测,了解失败病例根管内菌群特征,同时对检测出的细菌进行药敏试验,为临床根管治疗提供参考。方法选择86例根管治疗失败病例共86颗惠牙,去除根管内充填物后,对根管内细菌取样,进行培养和鉴定,用琼脂稀释法对根管内分离的优势菌进行药敏试验。结果根管治疗失败病例根管中微生物菌群组成以兼性厌氧菌为优势菌,主要以1-2种革兰阳性菌为主,肠球菌是最常检出的细菌。失败病例根管内检出的兼性及专性厌氧菌对青霉素G、甲醛甲酚、甲硝唑敏感。结论细菌与根管治疗失败密切相关,根管治疗失败病例根管内微生物组成有其独特性。临床应选择敏感性药物进行治疗。  相似文献

Apical periodontitis, an inflammatory process around the apex of a tooth root, is primarily a sequel to microbial infection of the pulp space. The microbial flora is composed of a restricted group of the total oral flora, selected by environmental pressures of anaerobiosis, nutrition and competition with other species and inhabits the root canal as a biofilm of coaggregated communities in an extracellular matrix. The untreated infected canal is generally composed of a polymicrobial mix with approximately equal proportions of Gram-positive and Gram-negative species, dominated by obligate anaerobes.
The type of microbial flora in the root-filled tooth with persistent apical periodontitis has very different characteristics. These infections are characterized by one or just a few species, predominantly Gram-positive micro-organisms with an equal distribution of facultative and obligate anaerobes. Enterococcus faecalis has been a conspicuous finding in most studies.
Because the primary aetiological problem is infection, endodontic treatment is directed at control and elimination of the root canal flora by working in a sterile way. Based on current knowledge, the best available method for obtaining clean, microbe-free root canals is by instrumentation with antimicrobial irrigation reinforced by an intracanal dressing with calcium hydroxide.  相似文献

Studies of the microbiota from the canals of teeth with failure of endodontic therapy have revealed that it differs markedly from that of untreated necrotic dental pulps. This study aimed to evaluate the microbiota of 30 root-filled teeth with persisting periapical lesions and to test the antibiotic susceptibility of the most prevalent species. Microbial samples, isolation and speciation were done using advanced microbiologic techniques for anaerobic species. A total of 55 bacterial species were isolated, 80% were gram-positives and 58% facultative anaerobic microorganisms. The bacterial genera most frequently recovered were Enterococcus, Streptococcus, Peptostreptococcus and Actinomyces. Antibiotic sensitivity of Enterococcus faecalis and Peptostreptococcus spp. was accomplished with the E-test system. All species studied were susceptible to benzylpenicillin, amoxicillin, amoxicillin combined with clavulanate. However, 20% of the E.faecalis strains were resistant to erythromycin and 60% to azithromycin. It was concluded that microbial flora in canals after endodontic failure comprised predominantly facultative anaerobes and gram-positive species. E.faecalis was the species most frequently isolated and showed erythromycin and azithromycin resistance among the isolates.  相似文献

OBJECTIVE: The purpose of this study was to determine the composition of the microbial flora present in teeth after the failure of root canal therapy in a North American population. These results were then compared with those of the previous Scandinavian studies. STUDY DESIGN: Fifty-four root-filled teeth with persistent periapical radiolucencies were selected for retreatment. After removal of the root-filling material, the canals were sampled with paper points, and by reaming of the apical dentin. Both samples were grown under aerobic and strict anaerobic conditions. Then the bacterial growth was analyzed. RESULTS: The microbial flora was mainly of 1 to 2 strains of predominantly gram-positive organisms. Enterococcus faecalis was the most commonly recovered bacterial species. CONCLUSIONS: Bacteria were cultivated in 34 of the 54 teeth examined in the study. E faecalis was identified in 30% of the teeth with a positive culture.  相似文献

A large body of evidence indicates that microorganisms are the primary causative agents of endodontic treatment failures. This study intended to assess the occurrence of nine putative endodontic pathogens in root-filled teeth associated with periradicular lesions in a South Korean population using a culture-independent molecular approach. Fourteen root-filled teeth with persistent periradicular diseases were selected for retreatment. After removal of the root canal filling, the canals were sampled, and a polymerase chain reaction assay using taxon-specific oligonucleotide primers was used for microbial detection. Bacteria were present in all cases, as revealed by amplification using ubiquitous 16S rDNA primers. The most frequently detected taxon was Enterococcus faecalis (64%), followed by Streptococcus spp. (21%) and Tannerella forsythensis (14%). The results of this study using a highly sensitive identification method are concurrent with those from other geographical locations using diverse identification methods in that E. faecalis is the main species found in cases of root-filled teeth associated with periradicular lesions.  相似文献

Microbiological examination of infected dental root canals   总被引:14,自引:0,他引:14  
OBJECTIVES: The aim of this study was to investigate the root canal microbiota of primary and secondary root-infected canals and the association of constituent species with specific endodontic signs and symptoms. METHODS: Microbial samples were taken from 60 root canals, 41 with necrotic pulp tissues (primary infection) and 19 with failed endodontic treatment (secondary infection). Strict anaerobic techniques were used for serial dilution, plating, incubation and identification. RESULTS: A total of 224 cultivable isolates were recovered belonging to 56 different bacterial species. Individual root canals yielded a maximum of 10 bacterial species. Of the bacterial isolates, 70% were either strict anaerobes or microphilic. The anaerobes most frequently isolated were: Peptostreptococcus micros (35%), Fusobacterium necrophorum (23.3%), Fusobacterium nucleatum (11.7%), Prevotella intermedia/nigrescens (16.7%), Porphyromonas gingivalis (6.7%) and Porphyromonas endodontalis (5%). The root canal microflora of untreated teeth with apical periodontitis was found to be mixed, comprising gram-negative and gram-positive and mostly anaerobic microorganisms and usually containing more than 3 species per canal. On the other hand, facultative anaerobic and gram-positive bacteria predominated in canals with failed endodontic treatment, which harbored 1-2 species per canal. Suggested relationships were found between anaerobes, especially gram-negatives, and the presence or history of pain, tenderness to percussion and swelling (P<0.05). In particular, associations were found between: a) pain (n=29) and P. micros (P<0.01), P. intermedia/nigrescens and Eubacterium spp. (both P<0.05); b) history of pain (n=31) and P. micros (P<0.01) Porphyromonas and Fusobacterium spp. (P<0.05); c) tenderness to percussion (n=29) and Porphyromonas spp. (P<0.01), Peptostreptococcus and Fusobacterium spp. (P<0.001); d) swelling (n=20) and Peptostreptococcus spp. (P<0.01), Porphyromonas and Enterococcus spp. (P<0.05); e) wet canals (n=33) and Porphyromonas and Fusobacterium spp. (P<0.05); f) purulent exudate (n=20) and Porphyromonas, Peptostreptococcus and Fusobacterium spp. (P<0.05); previous endodontic treatment and Enterococcus faecalis, Streptococcus spp., P. micros, F. necrophorum (P<0.05). CONCLUSIONS: Our findings indicate potential complex interactions of species resulting in characteristic clinical pictures which cannot be achieved by individual species alone. They also indicate that the microbiota of primary infected canals with apical periodontitis differs in number and in species from the secondary infected canals by using the culture technique.  相似文献

AIMS: The aim of this study was to determine the occurrence and role of yeasts, enteric gram-negative rods and Enterococcus species in root-filled teeth with chronic apical periodontitis, and the antimicrobial effect of iodine potassium iodide (IKI) irrigation. METHODOLOGY: Forty symptom-free root-filled teeth with chronic apical periodontitis were included in the study. The patients were divided into two groups. In group A the canals were filled with calcium hydroxide for 10-14 days after cleaning and shaping; in group B the canals were irrigated with IKI for 5 min after cleaning and shaping followed by a permanent root filling. Microbiological samples were taken from the canals before and after the chemomechanical preparation and after iodine irrigation (group B). RESULTS: Microbes were isolated from 33 of 40 teeth in the initial sampling. Yeasts were isolated from six teeth, three of them together with E. faecalis. Enteric rods (Escherichia coli, Klebsiella pneumoniae and Proteus mirabilis) were present in three teeth and E. faecalis was isolated from 21 of the 33 culture positive teeth, 11 in pure culture. Growth was detected in 10 teeth of the second samples. Six of the 10 cases were E. faecalis, with five being a pure culture. All third samples (after IKI) except one were negative. The number of microbial cells per sample did not correlate with lesion size. Two flare-ups were recorded, both in teeth with a mixed infection. CONCLUSION: The high prevalence of enteric bacteria and yeasts in root-filled teeth with chronic apical periodontitis was established. IKI improved the antimicrobial effect of the treatment.  相似文献

Objective. The purposes of this study were to determine what microbial flora were present in teeth after failed root canal therapy and to establish the outcome of conservative re-treatment.Study design. Fifty-four root-filled teeth with persisting periapical lesions were selected for re-treatment. After removal of the root filling, canals were sampled by means of advanced microbiologic techniques. The teeth were then re-treated and followed for up to 5 years.Results. The microbial flora was mainly single species of predominantly gram-positive organisms. The isolates most commonly recovered were bacteria of the species Enterococcus faecalis. The overall success rate of re-treatment was 74%.Conclusions. The microbial flora in canals after failed endodontic therapy differed markedly from the flora in untreated teeth. Infection at the time of root filling and size of the periapical lesion were factors that had a negative influence on the prognosis. Three of four endodontic failures were successfully managed by re-treatment.  相似文献

The objectives of the present study were to identify enterococcal species isolated from the canals of root-filled teeth with periapical lesions using biochemical and molecular techniques, and to investigate the genetic diversity of the isolates. Twenty-two Enterococcus strains, isolated from the canals of root-filled teeth with persisting periapical lesions, were identified to species level using rapid ID 32 STREP galleries and partial 16S rDNA sequencing. To subtype the strains, genomic DNA from the isolates was analyzed by pulsed field gel electrophoresis (PFGE) after digestion with SmaI. Intragenic regions of two genes, ace and salA, were sequenced for further differentiation of the isolates. All strains were identified as Enterococcus faecalis by both commercial kit and partial 16S rDNA sequencing. PFGE with SmaI of 22 isolates demonstrated 18 macrorestriction profiles, whereas 13 distinct genotypes were identified after analysis of the ace and salA composite sequences. Most of the isolates from distinct patients had different PFGE profiles. Moreover, in two cases, different E. faecalis strains were found in different root-filled teeth from the same mouth. E. faecalis was the only enterococcal species isolated from the canals of root-filled teeth with periapical lesions. Genetic heterogeneity was observed among the E. faecalis isolates following PFGE and sequence-based typing method. Furthermore, the genetic diversity within root canal strains was similar to previous reports regarding E. faecalis isolates from different clinical and geographic origins.  相似文献

The objective of the present study was to investigate the presence of nine bacterial species in root-filled teeth associated with periapical lesions using a polymerase chain reaction analysis and to correlate these species with clinical features of the cases. DNA was extracted from 45 canal samples of root-filled teeth with periapical lesions. A PCR assay using species-specific primers of 16S rDNA and the downstream intergenic spacer region was used for microbial detection. Enterococcus faecalis was the most prevalent species, detected in 77.8% of the study teeth, followed by Peptostreptococcus micros, detected in 51.1%. Porphyromonas gingivalis, Porphyromonas endodontalis, Prevotella intermedia, and Prevotella nigrescens were detected in 35.6%, 22.2%, 11.1%, and 11.1% of the sampled teeth, respectively. Moreover, PCR detected Filifactor alocis in 26.7%, Treponema denticola in 24.4%, and Tannerella forsythia in 4.4% of the samples. T. denticola and P. micros were statistically associated with tenderness to percussion (p < 0.05). P. nigrescens was associated with the presence of spontaneous pain and abscess (p < 0.05). P. endodontalis and P. nigrescens were associated with purulent exudates (p < 0.05). Synergistic relationship was also observed between some species. The results of this study indicated that E. faecalis was the most frequently identified test species by PCR in teeth with failing endodontic treatment.  相似文献

目的:通过对根管治疗失败病例根管内微生物进行分子生物学检测,研究失败病例根管内各种细菌的检出率及优势菌;分析临床症状和体征与特殊微生物的关系.方法:选择40 例根管治疗失败病例共40 颗患牙,根据症状分为疼痛组、窦道组和无症状组,去除根管内充填物,根管内细菌取样,PCR检测鉴定.结果: 根管治疗失败病例根管主要呈混合感染,共检出6 种细菌,粪肠球菌是最常检出的细菌,产黑普氏菌与疼痛、衣氏放线菌与瘘管有一定的相关性.结论:根管治疗失败的主要原因是根管内的微生物感染持续存在,根管治疗失败病例根管内微生物组成有其特殊性.  相似文献

Abstract The occurrence of bacteria in 12 endodontically induced periodontal lesions associated with sinus tracts was examined. The microbial flora encountered in the sinus tract was compared with that of the root canal of the involved teeth which had not experienced any prior endodontic therapy. All microbiological samples taken from the sinus tract and from the root canal system contained bacteria. Seventy-one strains were detected in the extraradicular lesions. Of the anaerobic species, Fusobacterium nucleatum (7 strains), Prevotella intermedia (4 strains) and P. oralis (4 strains) were most frequently found. In the group of the facultative anaerobes Streptococcus spp. were predominant. Ninety-four strains were isolated from the root canal system of the 12 teeth. P. intermedia (6 strains), P. buccae (5 strains), F. nucleatum (5 strains) and Lactobacillus plantarum (5 strains) were most common. In 9 cases, species present in the root canal could be revealed in the extraradicular lesions. It was concluded that a variety of microorganisms were capable of colonizing endodontically induced, extraradicular lesions clinically characterized by sinus tracts.  相似文献

Enterococcus faecalis is the most commonly found species in root-filled teeth evincing recalcitrant periradicular lesions and as a consequence, a role in causation of endodontic treatment failure has been suggested. The purpose of this study was to evaluate the prevalence of this bacterial species in root-filled teeth with or without periradicular lesions. Identification of E. faecalis was carried out by polymerase chain reaction (PCR) or conventional culture procedures. Overall, E. faecalis was detected by species-specific 16S rRNA gene-based PCR in 40/50 teeth (80%), while culture revealed the occurrence of this species in 8/50 teeth (16%). PCR was significantly more effective than culture in detecting this bacterial species (p < 0.001). Of 27 root-filled teeth with no periradicular lesions, E. faecalis was found in 22 cases (81.5%) by PCR and in five cases (18.5%) by culture. Of 23 root-filled teeth with periradicular lesions, E. faecalis was identified in 18 cases (78%) by PCR and in three cases (13%) by culture. Regardless of the identification technique used, no significant difference was observed when comparing the occurrence of E. faecalis in root-filled teeth with and without periradicular lesions (p > 0.05). Although these findings apparently put into question the status of E. faecalis as the main species causing endodontic treatment failure, other related factors still need to be clarified before this assumption turns into certainty.  相似文献

AIMS: To identify Gram-positive rods from root canals of teeth with apical periodontitis and to examine their associations with other species. METHODOLOGY: Consecutive root canal samples (RCSs) from 139 teeth undergoing root canal treatment were analyzed prospectively for cultivable microbes. Gram-positive rods in the first RCS submitted after chemo-mechanical preparation were categorised to genus level by selective media and gas-liquid chromatography (GLC), and identified to species level by sodium dodecyl sulphate-polyacrylamide gel electrophoresis (SDS-PAGE). Associations between organisms were measured by odds ratios (OR). RESULTS: In the first samples submitted a total of 158 Gram-positive rods, 115 Gram-positive cocci, 26 Gram-negative rods and 9 Gram-negative cocci, were identified. At genus levels Gram-positive rods were classified into: Lactobacillus spp. (38%), Olsenella spp. (18%), Propionibacterium spp. (13%), Actinomyces spp. (12%), Bifidobacterium spp. (13%) and Eubacterium spp. (6%). The most frequent species were Olsenella uli, Lactobacillus paracasei and Propionibacterium propionicum. In subsequent samples taken during treatment, Gram-positive rods were also identified, although the number of strains was considerably reduced. Positive associations were observed between members of the genus lactobacilli and Gram-positive cocci (OR>2). CONCLUSIONS: Olsenella uli and Lactobacillus spp. predominated over other Gram-positive rods. A possible association exists between Lactobacillus spp. and Gram-positive cocci in root canals of teeth with apical periodontitis receiving treatment.  相似文献

AIM: To identify the cultivable bacterial flora in root filled teeth with persistent periapical lesions and to locate their distribution within the root canal system using an in vitro sampling protocol. METHODOLOGY: Eight freshly extracted root filled teeth were collected from the Oral and Maxillo-facial Surgery Department, Eastman Dental Hospital. Seven teeth were associated with persistent apical periodontitis and also showed evidence of coronal leakage. Teeth were transferred to an anaerobic chamber immediately after careful extraction and sectioned transversely to give a crown (in all but one case) and two root segments (coronal and apical). Two samples were obtained from each segment, one from dentine and the other from the restoration or gutta-percha (GP) root filling (46 sites in total). The samples were dispersed, serially diluted and cultured on blood agar and fastidious anaerobic agar (with 5% defibrinated horse blood). The primary growth was subcultured to obtain pure isolates, which were identified by routine microbiological techniques and commercial enzyme tests. RESULTS: A total of 252 strains were isolated from all the teeth. Of all the isolates, the most prevalent bacteria were Gram-positive facultative anaerobes 189/252 (75%) with staphylococci (48/252, 19%), streptococci (44/252, 17%), enterococci (20/252, 8%) and Actinomyces species (20/252, 8%) being found in most of the teeth (6/8, 6/8, 5/8, 5/8 and 7/8, respectively). Of the obligate anaerobes (17%), peptostreptococci (7%) were also present in most teeth (7/8). A statistical association between bacterial flora and site (crown/coronal/apical) or surface (dental/GP/restoration) could not be shown. CONCLUSIONS: The predominant group of bacteria in root filled teeth with persistent apical periodontitis and coronal leakage was Gram-positive facultative anaerobes of which staphylococci followed by streptococci and enterococci were the most prevalent.  相似文献

AIM: This study was undertaken to investigate combinations of bacteria found in root-canal infections of teeth with periapical bone destruction without clinical signs and symptoms. METHODOLOGY: Endodontic samples from 58 root canals were cultured anaerobically and microorganisms were counted and identified. Eighty-one combinations of microorganisms were found and tested for a symbiotic relationship using the Fisher's exact test and Odds ratio calculation. RESULTS: All samples contained microorganisms with a median CFU mL(-1) of 8x10(4) per sample. Strict anaerobic species accounted for 87% of the microflora. The most prevalent bacteria were Prevotella intermedia, Peptostreptococcus micros and Actinomyces odontolyticus, present in 33, 29 and 19%, respectively, of the cultured canals. A significant relationship (P<0.05) and an Odds ratio >2 were found between P. intermedia and P. micros, P. intermedia and P. oralis, A. odontolyticus and P. micros, Bifidobacterium spp. and Veillonella spp. Conclusions: These results indicate that endodontic pathogens do not occur at random but are found in specific combinations.  相似文献

目的分析乳牙根尖周炎感染根管菌丛,为进一步防治提供依据。方法用直接酶法对8例乳牙急性根尖周炎、14例乳牙慢性根尖周炎感染根管菌丛成分进行分析。结果共检出240株细菌,厌氧菌200株,主要分布于消化链球菌、类杆菌、韦永氏菌、真细菌、丙酸杆菌、放线菌和梭杆菌。急性炎症组类杆菌、梭杆菌的检出率明显高于慢性组(P<005),主要差异在牙龈类杆菌和具核梭杆菌;慢性炎症组小韦永氏菌的检出率明显高于急性组(P<0.05)。结论乳牙急、慢性根尖周炎感染根管内是以厌氧菌为优势菌的混合感染。牙龈类杆菌、具核梭杆菌可能与乳牙根尖周炎的急性症状有关。小韦永氏菌在乳牙慢性根尖周炎中的作用不容忽视  相似文献

Data from culture studies have revealed that Enterococcus faecalis is occasionally isolated from primary endodontic infections but frequently recovered from treatment failures. This molecular study was undertaken to investigate the prevalence of E. faecalis in endodontic infections and to determine whether this species is associated with particular forms of periradicular diseases. Samples were taken from cases of untreated teeth with asymptomatic chronic periradicular lesions, acute apical periodontitis, or acute periradicular abscesses, and from root-filled teeth associated with asymptomatic chronic periradicular lesions. DNA was extracted from the samples, and a 16S rDNA-based nested polymerase chain reaction assay was used to identify E. faecalis. This species occurred in seven of 21 root canals associated with asymptomatic chronic periradicular lesions, in one of 10 root canals associated with acute apical periodontitis, and in one of 19 pus samples aspirated from acute periradicular abscesses. Statistical analysis showed that E. faecalis was significantly more associated with asymptomatic cases than with symptomatic ones. E. faecalis was detected in 20 of 30 cases of persistent endodontic infections associated with root-filled teeth. When comparing the frequencies of this species in 30 cases of persistent infections with 50 cases of primary infections, statistical analysis demonstrated that E. faecalis was strongly associated with persistent infections. The average odds of detecting E. faecalis in cases of persistent infections associated with treatment failure were 9.1. The results of this study indicated that E. faecalis is significantly more associated with asymptomatic cases of primary endodontic infections than with symptomatic ones. Furthermore, E. faecalis was much more likely to be found in cases of failed endodontic therapy than in primary infections.  相似文献

The purpose of the present study was to investigate the correlation between the composition of the bacterial flora isolated from infected root canals of teeth with apical periodontitis with the presence of clinical signs and symptoms, and to test the antibiotic susceptibility of five anaerobic bacteria mostly commonly found in the root canals of symptomatic teeth against various substances using the E-test. Microbial samples were taken from 48 root canals, 29 symptomatic and 19 asymptomatic, using adequate techniques. A total of 218 cultivable isolates were recovered from 48 different microbial species and 19 different genera. Root canals from symptomatic teeth harbored more obligate anaerobes and a bigger number of bacterial species than the asymptomatic teeth. More than 70% of the bacterial isolates were strict anaerobes. Statistical analysis used a Pearson Chi-squared test or a one-sided Fisher's Exact test as appropriate. Suggested relationships were found between specific microorganisms, especially gram-negative anaerobes, and the presence of spontaneous or previous pain, tenderness to percussion, pain on palpation and swelling amoxicillin, amoxicillin + clavulanate and cephaclor were effective against all the strains tested. The lowest susceptibility rate was presented by Prevotella intermedia/nigrescens against Penicillin G. Our results suggested that specific bacteria are associated with endodontic symptoms of infected teeth with periapical periodontitis and the majority of the anaerobic bacterial species tested were susceptible to all antibiotics studied.  相似文献

AIM: The aim of this study was to determine the periapical status and the quality of root-canal treatment amongst an adult population attending the dental school in Nice, France during 1998. METHODOLOGY: Patients who attended the dental school for the first time during 1998 were included. Panoramic radiographs, taken by a trained radiology assistant, were used in this study. The periapical areas of all teeth with the exception of third molars, were examined and the technical quality of root fillings were evaluated for both apical extension and density. Statistical analyses were conducted using anova, Chi-square, Fisher's PLSD and Cohen's Kappa tests. RESULTS: The survey involved 344 patients: 180 females and 164 males. Males had significantly fewer natural remaining teeth than females (P<0.03). Similarly, the average number of root-filled teeth was lower for males (P<0.01). Nonroot-filled teeth (n=6126) had significantly fewer signs of periapical pathology than root-filled teeth (n=1429) (1.7% vs. 31.5%, P<0.0001). The majority of root fillings were poorly executed. There was a significant correlation between the presence of periapical pathology and inadequate root-canal fillings (P<0.001). CONCLUSIONS: The results of the present study indicate that many root-canal treatments were technically unsatisfactory in terms of quality and treatment outcome. There is a need for endodontic retreatment in the population examined.  相似文献
