目的探讨内科总值班护士长制度对护理质量的控制效果。方法设立总值班护士长,负责内科各个护理单元晚夜班及节假日的护理质量管理,督导护理工作落实情况,协助科室进行危重患者的抢救并解决各类疑难护理问题,实现护理质量24h无缝隙管理。对总值班护士长制度实施前(2005年)和实施后(2010年)的效果进行比较。结果实施总值班护士长制度后,各项护理质量指标达标率逐年提高,且实施后与实施前比较,差异有统计学意义(P<0.05);按照住院患者对护理工作满意率≥90%的标准,实施前后内科护理工作满意度达标率分别为81.25%、97.73%,两者比较差异有统计学意义(P<0.05);实施5年来,共协助科室完成125次危重患者抢救,解决102例疑难护理问题。结论总值班护士长能够有效提高特殊时段的护理质量和护理安全,提高医院整体护理水平。 相似文献

目的 探索追踪方法学在提升内科个案护理查房内涵中的作用.方法 采用由大内科组织主持各专科病区个案护理查房的方法,根据病人具体情况成立跨专业团队并责任分工,负责对个案实施护理服务及质量跟踪督导,并参与查房讨论.结果 病人得到了跨专科护理照护,疑难问题得到了专业指导,护士的临床护理能力显著提高,专业水平得到了扩展,各专科护士能力得到了发挥与应用,团队协作精神得到体现.结论 追踪方法学应用于个案护理查房,可使病人获得跨专业优质护理照护,是培养与提高护士临床护理综合能力的有效手段. 相似文献

目的 :分析综合医院建立多专科一体化糖尿病管理模式的做法及其对非内分泌科糖尿病患者的管理效果。方法 :护理部牵头组建多专科一体化糖尿病管理团队,成立糖尿病管理指导委员会,选拔各科室骨干作为联络护士组成覆盖全院的糖尿病管理小组。对联络护士进行系统的糖尿病管理知识培训,制定并督导普通病区对糖尿病患者实施全程管理。结果 :糖尿病联络护士培训后理论知识良好率较培训前提高,糖尿病知识各单项成绩较培训前提高,差异具有统计学意义(P0.01)。非内分泌科住院的糖尿病患者血糖监测质量明显提高,异常血糖得到及时处理,无不良事件发生。结论 :多专科一体化糖尿病管理提高了非内分泌科护士的糖尿病知识水平,提高了非内分泌科糖尿病患者护理质量,促进了糖尿病患者管理系统化、规范化。 相似文献

Liaison nurses, employed by a home care organization, were introduced into two Dutch hospitals to improve discharge planning for stroke patients. The main aim of the study presented was to gain insight into the effects of liaison nursing on the quality of the discharge process and related outcomes. After the introduction of liaison nursing, hospital nurses completed a questionnaire on satisfaction with the liaison nurse. In addition, both before and after the introduction of liaison nursing, two groups of discharged stroke patients were interviewed by telephone. The records of these patients were also studied with respect to background characteristics and duration of hospital stays. The hospital nurses were, generally, positive about the liaison nurse and the job she did (e.g. they found that home care was better organized). Further, after the introduction of liaison nursing, more patients stated that their post-discharge needs had been discussed not later than 48 hours prior to discharge, and more patients said their aftercare had been discussed with community nurses. However, the number of patients whose medication had arrived at home on time had decreased. The results also indicated that there was no significant difference in the duration of stay between the before and after group. The overall conclusion is that the liaison nurses have been moderately successful in their jobs. However, since the study was conducted in only two Dutch hospitals, findings may not be representative of other settings. Future research on liaison nursing is therefore recommended. 相似文献

目的:探讨并分析责任护士包干负责制在改进护理服务质量中的效果.方法:对2010年7月~2011年6月成为优质护理服务试点病房的15个科室实行责任护士包干负责制,即每个科室根据床位多少分为2~3组,每组由责任组长、责任护士、助理护士组成,采取"扁平化"管理,每人负责5~8例患者,实行8 h在岗、24 h负责制.统计实施责任护士包干负责制后7项护理质量检查评分及患者满意度等,并与实施前(2009年7月~2010年6月)的相应指标作对照分析.结果:实施责任护士包干负责制试点病区的护理质量和患者满意度较实施前均有提高,且护理不良事件发生率较实施前下降(P<0.01).结论:实施责任护士包干负责制可转变护理工作模式,优化工作流程,明确工作职责,增强护理人员工作责任心和主动服务意识,密切护患关系,使临床护理工作更加贴近患者. 相似文献

BACKGROUND: Information about the contributions of acute care nurse practitioners to medical management teams in critical care settings is limited. OBJECTIVE: To examine contributions of acute care nurse practitioners to medical management of critically ill patients from the perspectives of 3 disciplines: medicine, respiratory care, and nursing. METHODS: Attending physicians, respiratory therapists, and nurses in 2 intensive care units were asked to list 3 advantages and 3 disadvantages of collaborative care provided by acute care nurse practitioners. Qualitative methods (coding/constant comparative analysis) were used to identify common themes and subthemes. Overall response rate was 35% (from 69% for attending physicians to 26% for nurses). RESULTS: Responses were grouped into 4 main themes: accessibility, competence/knowledge, care coordination/communication, and system issues. Acute care nurse practitioners were valued for their accessibility, expertise in routine daily management of patients, and ability to meet patient/family needs, especially for "long-stay" patients. Also, they were respected for their commitment to providing quality care and for their communication skills, exemplified through teaching of nursing staff, patient/family involvement, and fluency in weaning protocols. Physicians valued acute care nurse practitioners' continuity of care, patient/family focus, and commitment. Nurses valued their accessibility, commitment, and patient/family focus. Respiratory therapists valued their accessibility, commitment, and consistency in implementing weaning protocols. CONCLUSION: Responses reflected unique advantages of acute care nurse practitioners as members of medical management teams in critical care settings. Despite perceptions of the acute care nurse practitioner's role as medically oriented, the themes reflect a clear nursing focus. 相似文献

目的探讨糖尿病联络护士的培训方法及效果。方法对40名糖尿病联络护士进行糖尿病知识与技能的培训,由其负责培训本病区护理人员,并对糖尿病患者进行健康教育。比较培训前后对糖尿病相关知识与操作技能的掌握情况、糖尿病专科护理质量评分及非糖尿病专科患者对糖尿病健康教育的满意度。结果糖尿病联络护士在培训后糖尿病相关知识和操作技能成绩提高;糖尿病专科护理质量及非糖尿病专科患者对糖尿病健康教育的满意度均提高(P0.05)。结论糖尿病联络护士模式有利于护士综合护理能力提高,有利于提高糖尿病专科护理质量。 相似文献

目的通过设计重症监护病房(intensive care unit,ICU)护理索引卡(简称索引卡),提高护士对危重症患者的病情掌握程度及交接班质量,保证患者安全。方法 2016年2-10月,将索引卡应用于ICU护理工作中,比较使用前后护理人员对患者的病情掌握程度及交接班质量。结果共202例患者使用索引卡,护士对患者的病情掌握程度评分由平均(84.68±5.04)分提高到(91.68±4.71)分(t=7.86,P0.05),交接班质量评分由(87.93±4.61)分,提高到(94.44±2.66)分(t=8.21,P0.05)。结论将索引卡应用于ICU重症患者的临床护理工作中,有助于提高ICU护士对危重症患者的病情掌握程度及交接班质量。 相似文献

目的探讨亚专业化护理模式对护士核心能力的影响,分析和评价新模式的临床应用效果。方法采用“注册护士核心能力量表”评估我科31名护士在亚专业护理模式实施前后核心能力的情况,使用SPSS13.0统计学软件进行配对t检验,以分析护士核心能力的差异,评价新模式的临床应用效果。结果亚专业护理模式前后我科护士核心能力总分差异有显著意义(P=0.004);批判性思维/科研、临床护理、领导能力、人际关系、专业发展及教育/咨询等各个能力得分的差异有显著意义(P〈0.05),法律/伦理能力得分的差异无显著意义(P=0.173)。结论亚专业化护理模式促进了护士核心能力的培养,改进了科室医疗护理质量。 相似文献

目的探讨层级全责一体化护理模式在血液净化中心的应用效果,提高透析护理质量。方法对血液净化中心的维持性血液透析患者实施层级全责一体化护理模式,将血液净化中心护理人员根据职称、学历和能力水平的不同实行结构岗位职责分层,即在病区实行“护士长→护理组长→责任护士→辅助护士→助理护士”的5级层级管理结构。根据维持性规律血液透析患者隔日上午或下午透析的治疗特点,按照区域设置8个层级责任制护理小组,每组各由1名责任护士、1名辅助护士、1名助理护士组成,其中责任护士的工作班次与本组透析患者的透析时间安排相同,固定为隔日早班及下午班。层级责任制护理小组中责任护士和助理护士班次固定3个月,辅助护士班次相对固定。对实施前(2005年)及实施后(2006年)的病房护理管理质量、护理质量结果及患者、护士满意度进行比较。结果实施层级全责一体化护理模式后,病区管理检查分数由实施前(2005年)的93分提高到了实施后(2006年)的97分;消毒隔离检查分数由实施前的96分提高到了实施后的99分;患者满意度由实施前的82%提高到了实施后的92%,护士满意度由实施前的80%提高到了90%。护理质量(患者内瘘穿刺成功率、透析中患者不适症状发生率、护理缺陷发生率)比较差异有统计学意义(P〈O.05)。结论血液净化中心实施层级全责一体化护理模式可以有效提高透析护理质量,实现了一体化、系统的透析护理服务,使血液净化中心的护理管理质量更加系统和深入,促进了血液净化护理队伍自身的健全与发展。 相似文献
