Pulp stones are usually found in the pulp chamber. Radiographically, they appear as radiopaque structures in the pulp chambers or root canals of individual teeth. Generalized pulp stones throughout the dentition are usually associated with systemic or genetic disorders of the dentine. This report presents an unusual case of a 13-year-old girl with generalized pulp stones with clinically normal crowns. The patient's main complaint was a crossbite affecting all permanent canines. Radiographic examination revealed multiple pulp stones and several teeth with atypically shaped roots. Orthodontic treatment, gingivoplasty, and esthetic restorations were performed, thus using a multidisciplinary approach to establish functionally and esthetically sound dentition.  相似文献

Objective. The purpose of this study was to describe the prevalence of pulp stones in a sample of Jordanian dental patients. The article also presents frequency distributions of this condition among different types of teeth.Study design. Data were collected through radiographic examinations of a random sample of periapical and bitewing films of 4573 teeth from 814 dental records from the Faculty of Dentistry's filing system.Results. Pulp stones were identified in 22% of the teeth examined. First and second molars were the teeth most commonly affected. The organic matrix component of human pulp stones was investigated by immunohistochemistry. Two pulp stones were extracted from the upper molar teeth of two patients suffering from irreversible pulpitis. Both were formed in the center of the pulp cavity and located apart from the dentin walls. After demineralization, serial sections of the stones were prepared and subjected to immunohistochemical procedures using specific antibodies to type I collagen and noncollagenous proteins (osteopontin, osteonectin, and osteocalcin), which are reported to be involved in calcified matrix formation. Type I collagen was localized evenly in the stones, indicating that it is a major matrix component of pulp stones. Strong immunostaining of osteopontin appeared in the peripheral area of the stones, whereas osteonectin and osteocalcin were not detected. We previously reported that dental pulp cells produced osteopontin in vitro. Osteopontin has been commonly found in other pathological calcification, such as urinary stones, atherosclerotic plaques, and dental calculus. Taken together, the present findings suggest that osteopontin produced by dental pulp cells is possibly associated with calcification of the pulp stone matrix.  相似文献

Hypophosphatasia is caused by mutations of the tissue-non-specific alkaline phosphatase (TNSALP) gene with deficiency of dentin structure. The aim of this study was to examine whether TNSALP mutation in dental pulp cells contributes to dentin dysplasia in hypophosphatasia. Mutation analysis showed that compound heterozygous mutations of TNSALP were identified in three hypophosphatasia patients, including 3 novel mutation sites. Exfoliated teeth from the patients showed abnormal dentin mineralization and loss of cementum, as assessed by ground sections and scanning electron microscope analysis. Dental pulp cells isolated from one of the patients showed a significantly reduced TNSALP activity and mineralization capacity when compared with those in dental pulp cells from the unaffected individuals. Our results suggested that dentin dysplasia in hypophosphatasia may be associated with the decreased mineralization ability of dental pulp cells.  相似文献

The patient's dental, medical and family history, as well as the findings from the clinical examination were non-contributory. Biochemical analysis of one pulp stone removed from the right maxillary central incisor revealed large amounts of urates, proteins and phospholipids, but a blood test of the patient showed no metabolic disturbance. We propose that calcification of dental pulp may have a similar pathogenesis as calcified atheromas and could lead to use of routine dental radiographs as a rapid screening method for early identification of potential cardiovascular disease (CVD). Fifty-five dental patients ages 20 to 55 were chosen because pulp stones in pulpally noninflamed teeth were not expected in this age group. They completed a questionnaire regarding their CVD status and that of their parents and siblings. Entry criteria included at least one asymptomatic, minimally restored, noncarious molar and no history of gout, renal disease, or renal lithiasis. Patients' periapical radiographs of record were viewed to determine the presence of pulp stones. There was a significant relationship between pre-existing CVD and pulp stones (odds ratio of 4.4 with a 95% confidence interval of 1.1, 18.7), but no relationship was found for family history of CVD and pulp stones (odds ratio of 1.7 with a 95% confidence interval of 0.5, 5.5). Seventy-four percent (14/19) of patients with reported CVD had detectable pulp stones while only 39% (14/36) of patients without a history of CVD had pulp stones. This pilot study demonstrates that patients with CVD have an increased incidence of pulp stones in teeth with noninflamed pulps compared to patients with no history of CVD. No relationship was found between presence of pulp stones and family history of CVD. The findings suggest that dental radiographic determination of the presence or absence of pulp stones may have possibilities for use in CVD screening.  相似文献

BACKGROUND: Pulp stones are discrete calcified bodies found in the dental pulp. The aims of this study were to calculate the prevalence of pulp stones in young Australian adults using radiographs, and to report any associations between occurrence of pulp stones and sex, tooth type, dental arch, side and dental status. METHODS: From 217 undergraduate dental students, comprising 123 males and 94 females aged between 17-35 years, 3296 teeth were examined under 2x magnification on bitewing radiographs. Pulp stones were scored as present or absent, and associations with sex, tooth type, dental arch, side and dental status noted. RESULTS: Pulp stones were found in 100 (46.1 per cent) of the subjects and 333 (10.1 per cent) of the teeth examined. Occurrences were rare in premolars (0.4 per cent) but significantly higher in molars (19.7 per cent). Pulp stones were significantly more common in first molars than in second molars, and in maxillary first molars than in mandibular first molars. Carious and/or restored maxillary right first molars and maxillary left second molars displayed higher prevalences of pulp stones than unrestored and intact molars. CONCLUSION: Pulp stones may provide useful forensic information when examining dental records to identify deceased persons.  相似文献
