It is generally accepted that the first recorded outbreaks of foot and mouth disease (FMD) in South America occurred around 1870. The disease emerged almost simultaneously in the province of Buenos Aires (Argentina), in the central region of Chile, in Uruguay and in southern Brazil, due to the introduction of livestock from Europe. Argentina set up an agency for the control and eradication of FMD in 1961, Brazil began disease-control activities in Rio Grande do Sul in 1965, Paraguay and Uruguay initiated similar programmes in 1967, Chile in 1970 and Colombia in 1972. A common characteristic was observed in all early national FMD programmes, namely, they were developed, financed, operated and evaluated by the public sector, without major participation from the private sector, except when buying vaccines and abiding by the regulations. In 1987, the Hemispheric Foot and Mouth Disease Eradication Plan (PHEFA: Plan Hemisférico para la Erradicación de la Fiebre Aftosa) was launched and the private sector played a prominent role in achieving the eradication and control of FMD in several countries. However, this model of co-participation between the public and private sectors has suffered setbacks and a new approach is being developed to find ways in which local structures and activities can be self-sustaining.  相似文献

This document outlines the methodology of the Salud, Bienestar y Envejecimiento (Health, Well-Being, and Aging) survey (known as the "SABE survey"), and it also summarizes the challenges that the rapid aging of the population in Latin America and the Caribbean imposes on society in general and especially on health services. The populations of the countries of Latin America and the Caribbean are aging at a rate that has not been seen in the developed world. The evaluation of health problems and disability among older adults in those countries indicates that those persons are aging with more functional limitations and worse health than is true for their counterparts in developed nations. In addition, family networks in Latin America and the Caribbean are changing rapidly and have less capacity to make up for the lack of protections provided by social institutions. The multicenter SABE study was developed with the objective of evaluating the state of health of older adults in seven cities of Latin America and the Caribbean: Bridgetown, Barbados; Buenos Aires, Argentina; Havana, Cuba; Mexico City, Mexico; Montevideo, Uruguay; Santiago, Chile; and S?o Paulo, Brazil. The SABE survey has established the starting point for systematic research on aging in urban areas of Latin America and the Caribbean. Comparative studies of these characteristics and with this comparative nature should be extended to other countries, areas, and regions of the world in order to expand the knowledge available on older adults.  相似文献

OBJECTIVE: To learn if there is social inequality in health and in the utilization of health services among the elderly in six cities of Latin America: Buenos Aires, Argentina; S?o Paulo, Brazil; Santiago, Chile; Havana, Cuba; Mexico City, Mexico; and Montevideo, Uruguay. METHODS: This study used data from a project called Health, Well-being, and Aging in Latin America and the Caribbean (known as the "SABE project"). To investigate the presence of social inequality in health an ordinal probit model was used, with health status indicated by the following dependent variables: activities of daily living, instrumental activities of daily living, physical mobility, and self-rated health. To measure social inequality in the utilization of health services two characteristics were assessed: one for outpatient medical services (whether or not individuals received outpatient care, and how many times), and one for hospital admissions (whether or not individuals were hospitalized). For outpatient services, we estimated a negative binomial hurdle model, and for hospital admissions, a logit model was constructed. RESULTS: Our main results suggest the presence of social inequality in health in all the cities, with better-off socioeconomic groups having better health. The difference in health was less marked in Havana, Buenos Aires, and Montevideo, and it was more pronounced in S?o Paulo and Mexico City. With respect to the utilization of health services we found inequalities in the use of outpatient services in Santiago, Mexico City, and S?o Paulo. In Santiago and Mexico City, more schooling was associated with a higher number of expected outpatient medical visits; however, the opposite was found in S?o Paulo. For hospital admissions, inequality was found only in S?o Paulo, with individuals with more years of schooling being much more likely to be hospitalized. CONCLUSION: To a certain extent, our results reflect the socioeconomic and demographic characteristics of the countries in which each of the six cities is located. The cities in the countries with the worst social indicators (high income inequality and low human development index) tended to have the greatest inequalities in health and in the utilization of health services.  相似文献

This article presents the results of the comparative research project "Managed Care in Latin America: Its Role in Health Reform". The project was conducted by teams in Argentina, Brazil, Chile, Ecuador, and the United States. The study's objective was to analyze the process by which managed care is exported, especially from the United States, and how managed care is adopted in Latin American countries. Our research methods included qualitative and quantitative techniques. Adoption of managed care reflects transnationalization of the health sector. Our findings demonstrate the entrance of large multinational financial capital into the private insurance and health services sectors and their intention of participating in the administration of government institutions and medical/social security funds. We conclude that this basic change involving the slow adoption of managed care is facilitated by ideological changes with discourses accepting the inexorable nature of public sector reform.  相似文献

Foot and mouth disease (FMD) was first recognised in South America in 1870, almost simultaneously in the province of Buenos Aires (Argentina), in the central region of Chile, in Uruguay, in southern Brazil and coincidentally, on the northeastern coast of the United States of America. The epidemiology of the disease was unknown and no government action was taken following the initial outbreaks. This resulted in the disease spreading to other areas of Chile, as well as to Peru, Bolivia and Paraguay, reaching Venezuela and Colombia in the 1950s, and Ecuador in 1961. The entire continent was affected in the 1960s when national FMD control programmes were initiated, with the exception of Guyana, Surinam, French Guiana and Patagonia. In the 1970s, steps were taken to implement a regional control and eradication strategy in view of the impact of production and trade on the persistence of the virus. The Plan Hemisférico de Erradicación de la Fiebre Aftosa (PHEFA: Hemispheric FMD Eradication Plan), public- and private-sector policies, new diagnostic tools, the oil-adjuvanted FMD vaccine and regional strategies played a part in improving the epidemiological situation during the 1990s. A setback was encountered in 2000 and 2001, with outbreaks due to virus types A and 0 recorded in Argentina, Uruguay and Brazil.  相似文献

The process of health care reform benefits tremendously from comparing characteristics and performance across nations. This paper studies market-oriented health insurance reforms in three Latin American countries: Argentina, Chile and Colombia. Chile allowed private health insurers to compete for workers payroll contributions in the 1980s, permitting the modernization of the private health sector but relatively impoverishing the public health sector as a consequence of selection practices by private carriers. In the 1990s, Argentina and Colombia started liberalizing the health insurance sector but using policies to avoid the adverse effects encountered in the Chilean experience. These policies are scrutinized while challenges for these and future health insurance reform processes are discussed.  相似文献

Our research methods included qualitative and quantitative techniques. The adoption of managed care reflects the process of transnationalization in the health sector. Our findings demonstrate the entrance of the main multinational corporations of finance capital into the private sector of insurance and health services, and these corporations' intention to assume administrative responsibilities for state institutions and to secure access to medical social security funds. International lending agencies, especially the World Bank, support the corporatization and privatization of health care services, as a condition of further loans to Latin American countries. We conclude that this process of change, which involves the gradual adoption of managed care as an officially favored policy, reflects ideologically based discourses that accept the inexorable nature of managed care reforms.  相似文献

ABSTRACT

To identify factors associated with hysterectomy, data collected from 1999–2000 were assessed from seven cities of the Health, Well-Being and Aging in Latin America and the Caribbean Study on 6,549 women, aged 60 years and older. Hysterectomy prevalence ranged from 12.8% in Buenos Aires (Argentina) to 30.4% in Bridgetown (Barbados). The median age for having had a hysterectomy ranged from 45 to 50 years across the cities and was 47 years in the pooled sample. Ethnic differences in hysterectomy rates were partially explained by differences across cities. Factors significantly associated with lower odds for hysterectomy included older age, household crowding conditions, and having public/military or no health insurance, compared to having private health insurance. Women who had three or more children were less likely to have had a hysterectomy, a finding that differs from most previous studies. Socioeconomic position related to rates of hysterectomy in late life rather than hysterectomies earlier in life. However, the nature of these differences varied across birth cohorts. The findings suggested that adverse socioeconomic factors were most likely related to hysterectomy risk by affecting access to health care, whereas parity was most likely acting through an effect on decision-making processes.  相似文献

The paper examines the recent reforms of health insurance in Chile and Argentina. These partially replace social health insurance with individual insurance administered through the private sector. In Chile, reforms in the early 1980s allowed private health insurance funds to compete for affiliates with the social health insurance system. In Argentina, reforms in the 1990s aim to open up the union-administered social insurance system to competition both internally and from private insurers. The paper outlines the specific articulation of social and individual health insurance produced by these reforms, and discusses the implications for health insurance coverage, inequalities in access to healthcare, and health expenditures.  相似文献

This stakeholder assessment of HIV/AIDS service providing institutions in Argentina offers insights into the HIV/AIDS crisis in Spanish-speaking Latin America from an institutional level and makes recommendations for strengthening the work and functioning of these institutions. This stakeholder assessment was conducted to determine how HIV/AIDS prevention and management in Argentina affects and is affected by relevant HIV/AIDS institutions. Semi-structured qualitative interviews were conducted with 13 key leaders of organizations working in HIV/AIDS prevention in Buenos Aires including non-governmental, governmental, and academic institutions. Analyses of interviewee comments are presented according to four major themes: network connections, program resources, perceptions of success, and challenges. Key findings include the need for strengthening formal networks, increasing the involvement of other non-AIDS related social institutions in promoting HIV prevention, finding more sustainable funding options, working more effectively with the public sector to create policies and regulations favorable to the HIV/AIDS field, and addressing the lack of perceived susceptibility to HIV/AIDS in Argentine culture. From leaders' comments, recommendations are made for strengthening the HIV/AIDS network among key institutions including adapting the UNAIDS "Three Ones" principal to create one crosssector office responsible for coordinating HIV/AIDS work, formalizing agreements with institutions outside of Buenos Aires, increasing the role of schools in HIV/AIDS awareness, and designing programs that address lack of perceived susceptibility to HIV/AIDS among Argentines.  相似文献

Eighth and ninth grade students (n = 1272) from six Latin American cities (Buenos Aires, Argentina, Guatemala City, Guatemala, Havana, Cuba, Lima, Peru, Panama City, Panama, and Santiago, Chile) completed a questionnaire to assess the association of city, gender, socioeconomic status (SES), and age with the prevalence of overweight, satisfaction with weight, and recent attempted weight loss. Logistic regression was used to model these relationships.Prevalence of overweight varied appreciably across cities. Overweight was more common among males from the higher than from the lower SES stratum, while among females this trend was reversed. Satisfaction with weight was lower among adolescents from wealthier than from poorer families, and was lower among females than among males. Attempted weight loss was generally more frequent among females than among males, and among older adolescents.Results suggest a need for age and gender-specific, culturally appropriate overweight prevention programs for adolescents and their families.  相似文献

Private health insurance is playing an increasing role in both high- and low-income countries, yet is poorly understood by researchers and policy-makers. This paper shows that the distinction between private and public health insurance is often exaggerated since well regulated private insurance markets share many features with public insurance systems. It notes that private health insurance preceded many modern social insurance systems in western Europe, allowing these countries to develop the mechanisms, institutions and capacities that subsequently made it possible to provide universal access to health care. We also review international experiences with private insurance, demonstrating that its role is not restricted to any particular region or level of national income. The seven countries that finance more than 20% of their health care via private health insurance are Brazil, Chile, Namibia, South Africa, the United States, Uruguay and Zimbabwe. In each case, private health insurance provides primary financial protection for workers and their families while public health-care funds are targeted to programmes covering poor and vulnerable populations. We make recommendations for policy in developing countries, arguing that private health insurance cannot be ignored. Instead, it can be harnessed to serve the public interest if governments implement effective regulations and focus public funds on programmes for those who are poor and vulnerable. It can also be used as a transitional form of health insurance to develop experience with insurance institutions while the public sector increases its own capacity to manage and finance health-care coverage.  相似文献

The 2015 meeting of the Intestinal Transplant Association was held in Buenos Aires, Argentina. This was the 14th International Small Bowel Transplant Symposium, and it was the first meeting organized as a joint venture of the Transplantation Society, the Intestinal Transplant Association, and the Argentinean Transplant Society (Sociedad Argentina de Trasplantes). Innovative aspects of the classic meeting format included workshops sessions, debates, and multicenter studies. Since Brazil's adoption of universal health care in 1988, the country's health care system has consisted of a mix of private providers and free public providers. We test whether income-based disparities in medical visits and medications remain in Brazil despite universal coverage using a nationally representative sample of over 48,000 households. Additional income is associated with less public sector utilisation and more private sector utilisation, both using simple correlations and regressions controlling for household characteristics and local area fixed effects. Importantly, the increase in private care use is greater than the drop in public care use. Also, income and unmet medical needs are negatively associated. These results suggest that access limitations remain for low-income households despite the availability of free public care.  相似文献

OBJECTIVES: To estimate antigen-specific and series-complete vaccination coverage among children aged 13 to 59 months in Buenos Aires; to compare the results of a community-based household survey with coverage rates obtained from administrative records; and to identify risk factors for incomplete vaccination. METHODS: Census tracts in Buenos Aires were surveyed systematically in March and April, 2002. Three children aged 13 to 24 months and 25 to 59 months were surveyed per block in each census tract. Written documentation of vaccination was required. Risk factors associated with incomplete vaccination were identified with univariate analysis and multivariate logistic regression. RESULTS: A total of 1391 children were surveyed. Antigen-specific coverage ranged from 69.4% (95% CI 66.7%-72%) for Haemophilus influenzae type b vaccination to 99% (95% CI 98.4%-99.6%) for BCG vaccination. Except for measles, coverage estimates found in the survey did not differ substantially from those obtained from city health authority records. Multivariate logistic regression analysis showed child's age (P < 0.001) and vaccination provider (public or private) (P = 0.001) to be risk factors associated with incomplete vaccination. Not being the first child (P < 0.001) was associated with incomplete coverage under the long-standing program. Living in the Northern zone of the city (P = 0.001), being uninsured (P = 0.02), and lower educational level of the primary caregiver (P = 0.04) were risk factors associated with incomplete coverage under the current vaccination program. CONCLUSIONS: Although coverage rates for some vaccines were high, complete vaccination coverage remains low among children aged 13 to 59 months in Buenos Aires. Increasing coverage will require better access to vaccination, particularly in sections of the community with risk factors.  相似文献

Abstract

The concept of ‘harm’ underpinning current drug harm reduction policies is most often limited to viral infections and other health consequences for drug users. This paper analyses harm reduction policies in Argentina, with the purpose of challenging and extending this narrow conception of harm to encompass all harms inflicted on drug users, in a context of criminalization of drug use and poverty. Faced with a steep rise in poverty, rapid changes in drug use practices, the quality and prices of drugs, and patterns of morbidity and mortality, Argentina has implemented harm reduction policies specifically for drug users who mostly live in the Greater Buenos Aires impoverished areas. These changes, as well as the Latin American tradition of social and health policies that focus on the collective, subjective, and political-economic aspects of harm, highlight some tensions between the individualistic, public health model structured in the North and its application in Argentina.  相似文献
