Objective:To compare the treating effects of different intramedullary nailing methods on tibial fractures in adults.Methods:Literature reports in both Chinese and English languages were retrieved (from the earliest available records to October 1,2013) from the PubMed,FMJS,CNKI,Wanfang Data using randomized controlled trials (RCTs) to compare reamed and unreamed intramedullary nailing for treatment of tibial fractures.Methodological quality of the trials was critically assessed,and relevant data were extracted.Statistical software Revman 5.0 was used for data-analysis.Results:A total of 12 randomized controlled trials,comprising 985 patients (475 in the unreamed group and 510 in the reamed group),were eligible for inclusion in this meta-analysis.The results of metaanalysis showed that there were no statistically significant differences between the two methods in the reported outcomes of infection (RR=0.64; 95%CI,0.39 to 1.07;P=0.09),compartment syndrome (RR=1.44; 95%CI,0.8to 2.41; P=0.16),thrombosis (RR=1.29; 95%CI,0.43to 3.87; P=0.64),time to union (WMD=5.01; 95%CI,-1.78 to 11.80; P=0.15),delayed union (nonunion)(RR=1.56; 95%CI,0.97 to 2.49; P=0.06),malunion (RR=1.75; 95%CI,1.00 to 3.08; P=0.05) and knee pain (RR=0.94; 95%CI,0.73 to 1.22; P=0.66).But there was a significantly higher fixation failure rate in the unreamed group than in the reamed group (RR=4.29; 95%CI,2.58to 7.14; P<0.00001).Conclusion:There is no significant difference in the reamed and unreamed intramedullary nailing for the treatment of tibial fractures,but our result recommends reamed nails for the treatment of closed tibial fractures for their lower fixation failure rate.  相似文献

Objective: To evaluate the clinical feasibility and effect of the computer-assisted auto-frame navigation system for distal locking of tibial intramedullary nails. Methods: The hardware components of the system included a PC computer with a monitor, auto mechanical stereotactical locating cubic frame, foot holder and localization operative apparatus. Special navigation software can be used for registration of X-ray fluoroscopic images and real-tune controlling navigation of tools. Twenty-one cases of close tibial and fibular fractures were treated with closed intramedullary nailing, 6 of which involved in middle third, 12 in middle and lower third, 3 in lower third. C-arm alignment and registration time, fluoroscopic time and drilling time involved in the locking procedure were recorded. The size of unreamed or reamed tibial nails ranged from 8/300-11/330. Results: All distal holes except 1 were locked successfuUy. In 9 of 41 locked holes (21.95%), the drill bit touched the canal of locking hole without damage of the nail and clinical consequences. The fluoroscopy time per pair of screws was 2.23 s±0.31 s . Conclusions: The computer-assisted auto-frame navigation system for distal locking is well designed, easy to operate and do not need additional instruments during the procedure. The developed system enables the physician to precisely navigate surgical instruments throughout the anatomy using just a few computer-calibrated radiographic images. The total time of x-ray exposure per procedure can be significantly reduced.  相似文献

Fracture of femoral shaft in adults is common and mostly managed with intramedullary interlocking nails. Complications during closed intramedullary femoral nailing are uncommon, and mostly of them are caused by technical reasons. We describe a case of closed nailing for a femoral shaft fracture in which a jammed intramedullary guide wire, due to an incarcerated bone fragment at the nail tip, was inadvertently advanced across the knee. Forceful attempt of nail insertion caused this complication, which was probably attributed to nail design.  相似文献

High-energy tibial plateau fracture poses a significant challenge and difficulty for orthopaedic surgeons. Fracture of tibial plateau involves major weight bearing joint and may alter knee kinematics. Anatomic reconstruction of the proximal tibial articular surfaces, restoration of the limb axis (limb alignment) and stable fixation permitting early joint motion are the goals of the treatment. In cases of complex bicondylar tibial plateau fractures, isolated lateral plating is frequently associated with varus malalignment and better results have been obtained with bilateral plating through dual incisions. However sometimes a complex type of bicondylar tibialplateau fractures is encountered in which medial plateau has a biplaner fracture in posterior coronal plane as well as sagittal plane. In such fractures it is imperative to fix the medial plateau with buttressing in both planes. One such fracture pattern of the proximal tibia managed by triple plating through dual posteromedial and anterolateral incisions is discussed in this case report with emphasis on mechanisms of this type of injury, surgical approach and management.  相似文献

Objective To measure the anatomical parameters of the simulated low tibial tunnel of posterior cruciate ligament (PCL) based on knee CT images so as to provide clinical reference for accurate location of the tunnel. Methods The CT images of 201 healthy knee joints collected at Department of Orthopedics, The Second Hospital of Lanzhou University from June 2016 to September 2021 were used for simulation of the PCL low tibial tunnel. The anatomical parameters of the tibial tunnel were measured using the RadiAnt DICOM Viewer. The primary measures included the angle between tibial plateau and tibial tunnel (ATPT) and the perpendicular distances from the tibial tunnel entrance and exit point to the tibial plateau (L1 and L2). The secondary measures included the angle between tibial plateau and posterior slope (PSA), the angle between tibial anatomical axis and central line of tibial tunnel (ATAA), the angle between posterior tibial slope line and the central line of tibial tunnel (APST), the anterior and posterior diameter of tibial plateau (APD), the length of posterior tibial slope (LPTS), and the length of tibial tunnel (LTT). The measurement results were analyzed according to the body height (divided into 3 groups: a 1.00 to 1.60 m group, a 1.61 to 1.70 m group, and a ≥1.71 m group) and gender using the software IBM SPSS 26. Results The primary measures: ATPT was 37.0°±4.5°, and L1 and L2 were respectively (57.8±7.4) mm and (34.5±3.3) mm. The secondary measures: PSA 128.1°±5.4°, ATAA 52.7°±4.1°, APST 89.1°±5.9°, APD was (32.9±2.6) mm, LPTS (20.5±2.4) mm, and LTT (40.9±5.7) mm. After grouping by gender, there was no significant difference in PSA between men and women (P>0.05) while there were significant differences in the other indexes between men and women (P<0.05). After grouping by body height, there was no significant difference in ATPT, PSA, APST or ATAA between the 3 groups (1.00 to 1.60 m group, 1.61 to 1.70 m group and ≥1.71 m group) (P>0.05) while there were significant differences in L1, L2, APD, LPTS and LTT between the 3 groups (P<0.05). Conclusions Based on the knee CT images, the primary measures of PCL low tibial tunnel are as follows: the angle between tibial plateau and tibial tunnel is 37.0°±4.5°, and the perpendicular distances from the tibial tunnel entrance and exit point to the tibial plateau are (57.8±7.4) mm and (34.5±3.3) mm, respectively. Objective: To explore the effect of external fixator and reconstituted bone xenograft (RBX) in the treatment of tibial bone defect, tibial bone nonunion and congenital pseudarthrosis of the tibia with limb shortening. Methods : Twenty patients ( 13 males and 7 females)with tibial bone defect, tibial bone nonunion or congenital pseudarthrosis of the tibia with limb shortening were treated with external fixation, Two kinds of external fixators were used: a half ring sulcated external fixator used in 13 patients and a combined external fixator in 7 patients.Foot-drop was corrected at the same time with external fixation in 4 patients. The shortened length of the tibia was in the range of 2-9 cm, with an average of 4.8 cm. For bone grafting, RBX was used in 12 patients, autogenous ilium was used in 3 patients and autogenous fibula was implanted as a bone plug into the medullary canal in 1 case,and no bone graft was used in 4 patients. Results: All the 20 patients were followed-up for 8 months to 7 years, averaging 51 months. Satisfactory function of the affected extremities was obtained. All the shortened extremities were lengthened to the expected length. For all the lengthening area and the fracture sites,bone union was obtained at the last. The average healing time of 12 patients treated with RBX was 4.8 months. Conclusions: Both the half ring sulcated external fixator and the combined external fixator have the advantages of small trauma, simple operation, elastic fixation without stress shielding and non-limitation from local soft tissue conditions, and there is satisfactory functional recovery of affected extremities in the treatment of tibial bone defects, tibial bone nonunion and congenital pseudarthrosis of the tibia combined with limb shortening.RBX has good biocompatibility and does not cause immunological rejections. It can also be safely used in treatment of bone nonunion and has reliable effect to promote bone healing.  相似文献

Objective: To evaluate the anatomy of femoral tunnels created by simulated transtibial technique in double-bundle anterior cruciate ligament (ACL) reconstruction. Methods: Two tibial tunnels, anteromedial (AM) and posterolateral (PL), were drilled 45° and 55° to tibial plateau respectively. On the femoral side, the AM and PL tunnels were drilled through anteriomedial portal. After the four tun- nels were established, the shaft of a reamer was introduced into the joint through tibial tunnel and reached against the lateral wall of intercondylar notch. The position that the reamer shaft can reach was marked and recorded. Results: Neither femoral AM nor PL tunnel opening can be fully or partially reached by the reamer shaft through the tibial AM tunnel in all cases. The evaluation through the tibial PL tunnel showed that only in 8 of 50 cases (16%) the femoral AM tunnel opening and in 4 cases (8%) the PL opening can be fully reached. On the other hand, in 12 cases (24%) the femoral AM tunnel opening and in 10 cases (20%) the PL opening can be partially reached by the shafts through the tibial PL tunnel. Conclusion: The result strongly suggests that transtibial technique is not well competent for femoral tun- nel drilling in anatomic double-bundle ACL reconstruction as we have hypothesized.  相似文献

Objective To evaluate the results of endovascular intervention for infrapopliteal arterial occlusion in 40 patients. Methods There were 41 affected limbs in these 40 patients receiving 44 times of endovascular intervention for infrapopliteal arterial occlusion during Nov. 2006 and Dec. 2007. The average age was 76±6. The ABI(ankle brachial index)before intervention was 0.39±0.20 in anterior tibial artery and 0.39±0.23 in posterior tibial artery. CLI (critical limb ischemia) was 80.49% (33/41). Results The after intervention ABI increased by 0.43±0.22 (P<0.01) in anterior tibial artery and 0.43±0.25(P<0.01)in posterior tibial artery. 35 patients (36 limbs) were followed-up for (6±3) months. The limbs of Fontaine Ⅰ and Fontaine Ⅱ A were 28 (77.78%), CLI decreased to 19.44% (7/36) (P<0.01). At follow-up the ABI in anterior tibial artery was 0.63±0.22 and 0.56±0.22 in posterior tibial artery. The difference were all significant when compared with that before intervention and after intervention. The perioperative amputation rate was 0. The perioperative mortality rate was 2.5%. The total mortality rate was 15%. The limb salvage rate were 100%. Conclusion The effect of endovascular intervention for infrapopliteal arterial occlusion is satisfactory.  相似文献

Objective To investigate the relationship between tibial anteroposterior axis and other anatomic landmarks in determining the rotation of tibial prosthesis in total knee arthroplasty using computed tomography. Methods Transverse CT scans of 40 volunteers' right knee in full extension were made. The anteroposterior axis of the tibia was defined as a line which was perpendicular to the transepicondylar axis and passing through the middle point of the posterior crnciate ligament. At the tibial plateau and optimum re-section level, the mean medial percentage width of intersection point of the patellar tendon anti the antero-posterior axis was measured. The mean angle between the anteroposterior axis and a line connecting the middle of the posterior cruciate ligament and the medial of the patellar tendon and the medial 1/3 of the patellar tendon were measured. Results At the tibial plateau level, the mean medial percentage width of intersection point of the patellar tendon was 10.1%±8.3%. At the optimum resection level, the mean medial percentage width of intersection point of the patellar tendon was 0.2%±10.0%. At the optimum resection lev-el, the mean angle between the anteroposterior axis and a line connecting the middle of the posterior cruciate ligament and the medial of the patellar tendon was 0.1°±2.7°. The mean angle between the anteroposterior axis and a line connecting the middle of the posterior cruciate ligament anti the medial 1/3 of the patellar tendon was 10.3°±3.6°. Conclusion There was a tendency to align the tibial component in external rotation relative to the femoral component when the medial 1/3 of the patellar tendon was used. The line connecting the middle of the posterior cruciate ligament and the medial of the patellar tendon can be used as a reliable axis for correct rotational orientation of the tibial component.  相似文献

Objective: To introduce the experience of treating nonunions of humeral fractures with interlocking intramedullary nailing.
Methods: Twelve patients with humeral nonunions were treated with interlocking intramedullary nailing. The time interval between trauma and surgery was 10.5 months on average. Open reduction with anterograde approach was performed. Axial compression was specially applied to the fracture site with humeral nail holder after insertion of distal locked screws. Iliac bone grafting was added.
Results: The average follow-up period was 21 months (ranging 9-51 months). All patients achieved osseous union 5.8 months after treatment on average. Eleven patients hadgood functions of the shoulder joints and the upper extremities. No patient experienced any permanent neurological deficit. Refracture of the original ununited region occurred in one patient after removal of the internal fixator one year later, but union was achieved after closed re-intramedullary nailing fixation.
Conclusion: Humeral interlocking intramedullary nailing is an effective alternative treatment for humeral nonunion.  相似文献

目的 评价超远端胫骨髓内钉治疗开放性或潜在开放性(Tscherne分型为2度、3度)的胫骨远端干骺端骨折的安全性与有效性.方法 2007年3月至2008年4月共收治15例胫骨远端骨折患者,男9例,女6例;年龄21~53岁,平均34.3岁.对于Gustilo Ⅰ型、Ⅱ型开放性骨折患者,于伤后6~48 h采用超远端胫骨髓内钉治疗;对于Gustilo Ⅲ型开放性骨折患者,将该髓内钉作为分期治疗策略中的最终治疗.对于闭合性骨折患者,先予消肿抗炎等对症处理,待肢体皮肤褶皱出现、局部软组织无红肿渗液时手术.通过随访患者影像学资料及术后1年Johner-Wruhs评分标准评估其临床疗效.结果 15例患者术后获12~18个月(平均14个月)随访;骨折愈合时间为18~25周(平均21.1周),所有患者均获解剖或功能复位.术后1年根据Johner-Wruhs评分标准评价疗效:优11例,良3例,可1例,优良率为93.3%.并发症:伤口浅层感染3例,皮肤局部坏死2例,经清创换药后愈合.未发生深部及髓内感染,无畸形愈合或不愈合发生.结论 对于开放性及潜在开放性胫骨远端干骺端骨折,超远端胫骨髓内钉是一种安全、有效的选择.
Abstract:
Objective To evaluate the safety and efficacy of extra-distal tibial intramedullary nailing for open and potentially open fractures of distal metaphyseal tibia.Methods Between March 2007 and April 2008, 15 patients (9 men and 6 women), with a mean age of 34.3 (from 21 to 53) years, were admitted to this study.For Gustilo Ⅰ and Ⅱ fractures, extra-distal tibial intramedullary nailing was performed 6 to 48 hours after injury; for Gustilo Ⅲ fractures, the intramedullary nailing was conducted as the definitive procedure of a staged strategy; for closed fractures, the operation was not performed until swelling subsided and exudation stopped following symptomatic treatments.All the patients were followed up by periodic X-ray examination.Functional outcomes were evaluated according to Johner-Wruhs criteria one year after surgery.Results The patients obtained a mean follow-up of 14 months (range, 12 to 18) after surgery.Anatomic or functional reduction was achieved and maintained in all the cases.The fractures healed 21.1 weeks (range,18 to 25) after surgery.By Johner-Wruhs criteria, the final outcomes were excellent in 11 cases (73.3%),good in 3 (20%), and fair in one(6.7% ).Complications included 3 cases of superficial infection and 2 cases of local skin necrosis, all responding to debridement and dressing.No deep infection, malunion or nonunion occurred.Conclusion Extra-distal tibial nail is a safe and effective alterative solution for open and potentially open fractures of distal metaphyseal tibia.  相似文献

At follow-up the ABI in anterior tibial artery was 0.63±0.22 and 0.56±0.22 in posterior tibial artery. The difference were all significant when compared with that before intervention and after intervention. The perioperative amputation rate was 0. The perioperative mortality rate was 2.5%. The total mortality rate was 15%. The limb salvage rate were 100%. Conclusion The effect of endovascular intervention for infrapopliteal arterial occlusion is satisfactory.  相似文献

Objective To evaluate the results of endovascular intervention for infrapopliteal arterial occlusion in 40 patients. Methods There were 41 affected limbs in these 40 patients receiving 44 times of endovascular intervention for infrapopliteal arterial occlusion during Nov. 2006 and Dec. 2007. The average age was 76±6. The ABI(ankle brachial index)before intervention was 0.39±0.20 in anterior tibial artery and 0.39±0.23 in posterior tibial artery. CLI (critical limb ischemia) was 80.49% (33/41). Results The after intervention ABI increased by 0.43±0.22 (P<0.01) in anterior tibial artery and 0.43±0.25(P<0.01)in posterior tibial artery. 35 patients (36 limbs) were followed-up for (6±3) months. The limbs of Fontaine Ⅰ and Fontaine Ⅱ A were 28 (77.78%), CLI decreased to 19.44% (7/36) (P<0.01).
