A study of the prevalence, intensity and risk factors for soil-transmitted helminth infection was undertaken among school children aged 5-9 years attending a primary school in the fishing village in Peda Jalaripet, Visakhapatnam, South India. One hundred and eighty nine (92.6%) of 204 children were infected with one or more soil transmitted helminth parasites. The predominant parasite was Ascaris lumbricoides (prevalence of 91%), followed by Trichuris trichiura (72%) and hookworm (54%). Study of age-specific prevalence and intensity of infection revealed that the prevalence and intensity of A. lumbricoides infection was higher among younger children than older children. While aggregation of parasite infection was observed, hookworm infection was more highly aggregated than either A. lumbricoides or T. trichiura. Multivariate analysis identified parental occupation, child's age and mother's education as the potential risk factors contributing to the high intensity of A. lumbricoides infection. Children from fishing families with low levels of education of the mother had the highest intensity of A. lumbricoides infection. As the outcome of chemotherapy programs to control soil transmitted helminth infection is dependant on the dynamics of their transmission, there is a need for further studies to better define the role of specific factors that determine their prevalence, intensity and aggregation in different epidemiological settings.  相似文献

A population-representative lot quality assurance sampling (LQAS) survey was conducted in 2009 to determine the prevalence of intestinal helminth infections among schoolchildren across Osh oblast, Kyrgyzstan. The diagnostic approach consisted of duplicate Kato-Katz thick smears from a single stool sample and an adhesive tape test. A questionnaire was administered to identify risk factors for infections. A total of 1262 schoolchildren aged 6-15 years were recruited; 41% of them harboured at least one of the eight identified helminth species. The two most prevalent helminths were Ascaris lumbricoides (23.1%) and Enterobius vermicularis (19.3%). Lower prevalences were found for Hymenolepis nana (4.4%), Fasciola hepatica (1.9%) and Dicrocoelium dendriticum (1.8%). Washing raw vegetables was a protective factor with regard to A. lumbricoides infection (odds ratio (OR) = 0.69, p = 0.022); tap water was borderline protective (OR = 0.56, p = 0.057). Children of the richest families were at a lower risk of E. vermicularis infection than the poorest ones (OR = 0.41, p = 0.011). Sharing the bed with more than one person was a risk factor for E. vermicularis infection (OR = 2.0, p = 0.002). The results call for targeted interventions against intestinal helminths in Osh oblast. In a first stage, annual deworming of schoolchildren and other high-risk groups with albendazole or mebendazole should be implemented, and reliable diagnosis and additional anthelminthic drugs should be made available. Subsequently, transmission control including locally-adapted health education, improved water supply and adequate sanitation should become the central features.  相似文献

In Zanzibar, the prevalence and intensity of helminth infections have markedly declined over the past 25 years, which is generally attributed to morbidity control programmes emphasising ‘preventive chemotherapy’. Here we provide an update of the current situation of soil-transmitted helminthiasis and urinary schistosomiasis on Unguja Island, and highlight socioeconomic development, improvement in sanitation and health education as additional drivers against helminthiases. Our data were obtained from cross-sectional surveys carried out between 2006 and 2009 with stool and urine samples from 2858 and 879 individuals, respectively, examined for helminth eggs and larvae. Additionally, several hundred people were interviewed on sanitation and hygienic behaviour. Data on Unguja's economic growth and improvements in access to clean water and household latrines in the recent past were derived from the extant literature. Pooled prevalences of Trichuris trichiura, Schistosoma haematobium, Ascaris lumbricoides, hookworm and Strongyloides stercoralis were 35%, 29%, 12%, 10% and 6%, respectively. However, there were considerable differences in prevalences between different age-groups and at village and district level. Moreover, some hotspots for A. lumbricoides, T. trichiura and S. haematobium were identified with prevalences above 60% among school-aged children. Availability of a latrine and washing hands before eating did not significantly lower the risk of helminth infections in our study population. Nevertheless, a considerable increase in access to household latrines (from 49% to 72%) and piped water (from 45% to 71%) between 1991 and 2005 is likely to have contributed to reducing the force of transmission of helminthiases in Zanzibar. The next logical step in Unguja is to change the tactics from morbidity control to interruption of helminth transmission and ultimately local elimination. Hence, ‘preventive chemotherapy’ needs to be further consolidated, placing particular emphasis on health education and improved access to clean water and sanitation.  相似文献

We review, for the first time, a 20-year Chinese story of research and development pertaining to tribendimidine, a promising anthelmintic agent that is safe and exhibits a broad spectrum of activity. Tribendimidine was first synthesized at the National Institute of Parasitic Diseases in Shanghai in the mid 1980s. In laboratory studies, tribendimidine showed high efficacy against Nippostrongylus braziliensis in rats, Necator americanus in hamsters, Ancylostoma caninum and Toxocara canis in dogs, and Syphacia mesocriceti in mice. Activity was also found against several species of cestodes in chicken. In clinical trials, a single oral dose of 400 mg tribendimidine, administered to patients infected only with N. americanus, or with N. americanus and Ancylostoma duodenalis, resulted in cure rates of 85.7% (132/154) and 89.8% (53/59), respectively. In comparison, a single oral dose of 400 mg albendazole resulted in significantly lower cure rates, namely 65.5% (91/139; chi(2) = 16.47, P < 0.001) and 71.7% (43/60; chi(2) = 6.29, P = 0.012), respectively. Single oral doses of tribendimidine (300 mg) and albendazole (400mg) were equally effective against Ascaris lumbricoides infections; cure rates were 96.0% (97/101) and 98.1% (101/103), respectively. In 5-14-year-old children with an Enterobius vermicularis infection, treated with a single oral dose of 200 mg tribendimidine, a cure rate of 81.6% (93/114) was observed. Tribendimidine was well-tolerated as only mild and transient side effects were observed. It would be of great public health significance if these findings are confirmed in other epidemiological settings, as more than one-quarter of the world population is currently affected by intestinal nematodes, with only very few drugs currently available on the market.  相似文献

Fishborne zoonotic trematodes (FZT) are endemic in humans and cultured fish in Vietnam but little is known about FZT in domestic animals. A study was designed to determine FZT prevalence and species diversity, and risk factors for infection, in dogs, cats and pigs. Faecal samples from 186 dogs, 94 cats and 168 pigs belonging to 132 households in Nghia Hung district, Nam Dinh province, were examined for small trematode eggs; those were trematode eggs with length less than 50 μm. Prevalence of FZT varied significantly between cats (70.2%), dogs (56.9%) and pigs (7.7%). Forty-nine of the egg-positive animals (25 dogs, 20 cats and 4 pigs) were necropsied to obtain adult trematodes for identification. The liver fluke, Clonorchis sinensis, and 11 species of intestinal flukes including Haplorchis, Stellantchasmus, Stictodora and Centrocestus were recovered from the infected animals. The practice of feeding raw fish to the animals was a significant risk factor for infection; this risk was reduced if the animals were periodically treated with anthelmintics. Based on the high prevalence of FZT and certain risky husbandry practices, domestic animals are likely to be major contributors of FZT eggs to the environment. Therefore, education of farmers to avoid feeding raw fish and to perform regular anthelmintic treatment of dogs, cats and pigs is needed in integrated FZT control programs.  相似文献

To compare the efficacy of stool examination for the detection of Strongyloides stercoralis and hookworm, a total of 634 stool samples from the routine laboratory service of the Pharmacia Faculty, Federal University of Bahia, Brazil, were examined by agar plate culture (APC), Baermann-Moraes and spontaneous sedimentation. The sensitivity of agar plate culture, calculated by combining results of all 3 methods, was 95% for S. stercoralis and 77.6% for hookwoorm. Moreover, APC had superior accuracy than Baermann-Moraes and spontaneous sedimentation for S. stercoralis and hookworm diagnosis, respectively. The S. stercoralis and hookworm positive samples from the laboratory routine, obtained after the previous analysis, along with those initially selected, were used to evaluate the concordance between microscopic examination and both the type of furrows left by larvae and the time for culture positivity using the APC method. Of 115 stool samples positive for S. stercoralis and 92 positive for hookworm, 110 (95.7%) and 89 (96.7%), respectively, had concordant results for furrows and morphological characteristics. The cumulative percentage of positivity increased to 94% by the third day of observation; at this time, only 19.6% of hookworm-positive samples had positive culture plates. Analyses of 74 S. stercoralis-positive stool samples stored at 4 °C for 24, 48 and 72 h showed the presence of larvae in 48.6%, 28.4% and 23% of samples, respectively when re-examined by the APC. As a definitive diagnosis of strongyloidiasis depends on the microscopic demonstration of parasites, increasing the sensitivity of the detection requires the use of different parasitological methods, including APC.  相似文献

Whilst infections with soil-transmitted helminths are common across China, the public-health significance of Schistosoma japonicum and food-borne helminths is more focalized. Only few studies have investigated the local epidemiology of helminth infections in rural China, including risk factor analysis. We collected stool and blood samples from 3220 individuals, aged 5-88 years, from 35 randomly selected villages in Eryuan county, Yunnan province, China. Stool samples were subjected to the Kato-Katz technique and examined for helminth eggs. Blood samples were tested for Trichinella spp., S. japonicum and cysticerci-specific antibodies. Data on individual and family-level risk factors were collected using questionnaires. The prevalence of Ascaris lumbricoides, Taenia spp., Trichuris trichiura and hookworms was 15.4%, 3.5%, 1.7% and 0.3%, respectively. The seroprevalence of Trichinella spp. was 58.8% and that of cysticercosis 18.5%. The egg positivity rate of S. japonicum in the 13 known endemic villages was 2.7%, and the corresponding seroprevalence was 49.5%. We observed a strong spatial heterogeneity in the families' economic status. S. japonicum infections were more prevalent among the Han than Bai nationality (odds ratio (OR)=3.77, 95% confidence interval (CI)=1.97-7.23) and tobacco growers (OR=3.66, 95% CI=1.77-7.60) and was only found at elevations below 2150 m above sea level. A. lumbricoides and Taenia spp. infections were more prevalent at altitudes above 2150 m when compared to lower settings (OR=1.51, 95% CI=1.24-1.84 and OR=5.32, 95% CI=3.42-8.28, respectively). The opposite was found for T. trichiura (OR=0.31, 95% CI=0.14-0.70). Our findings can guide the design and spatial targeting of control interventions against helminth infections in Eryuan county.  相似文献

The prevalence and intensity of Ascaris lumbricoides in 492 children from five rural villages in the Northern Area of Pakistan was examined. The overall prevalence of A. lumbricoides was 91% (95%CI 88.6-93.6) with geometric mean (GM) egg count intensities of 3985 eggs per g (epg). The most intense A. lumbricoides infections were found in children aged 5-8 years. We also investigated selected socio-cultural and behavioral variables for A. lumbricoides infections that might be relevant for the design of appropriate prevention and control programs. Univariate analysis associated A. lumbricoides intensity with age (P=0.004), location of household (P<0.01), defecation practices (P=0.02), soil eating habit (P<0.01), hand washing after defecation (P<0.01), and living with children under 5 years old (P=0.02). Multivariate analysis identified the children's age 5-8 (P<0.01), location of household in Surngo, Askole, and Stakchun where the pilot health care model activities were not done (P<0.01), and living with children under 5 years old (P=0.03) as variables statistically associated with the intensity of A. lumbricoides. The results indicated that there were certain clear risk factors in A. lumbricoides transmission, and that its intensity was influenced by age-related behavioral and environmental factors that contribute to exposure.  相似文献
