Rhinoplasty in male patients

The complexities of rhinoplasty are further emphasized when this operation is undertaken in the male patient, and a growing percentage of the rhinoplasty population comprises men. We review the senior author's approach to rhinoplasty analysis, preoperative consultation, and surgical techniques that have been successfully employed for over 40 years. Specific recommendations and modifications to these techniques are discussed as they relate to the rhinoplasty operation in male patients.     

Rhinoplasty in the older patient

Rhinoplasty is often indicated in the older patient, either as an isolated procedure or in continuous or staged combination with other facial rejuvenative operations. The operations must be tempered with conservatism and artistic judgment. The nose of an older person should look natural and inconspicuous and fit the patient's face. Because of possible coronary or hypertensive problems, epinephrine in the local anesthetic solution should be used in lesser concentrations, requiring a longer wait for its full vasoconstrictive effect. Nasal cartilage is tougher, and the nasal bone more brittle in the older patient. In addition to the usual complete rhinoplasty, the partial rhinoplasty is a useful operation for the older individual.     

Rhinoplasty in the aging patient

Rhinoplasty in the aging patient presents a unique challenge to the facial plastic surgeon. The effects of age on the skin, ligaments, and cartilage of the nose conspire with the relentless pull of gravity to create a ptotic tip and collapsing nasal sidewalls. Combined with the increased incidence of other comorbidities in the aged patient, the resulting functional and cosmetic problems that ensue can be difficult to overcome. The astute plastic surgeon, however, will not vary from a systemic and patient-centered approach to each individual problem. Moreover, armed with common tools such as cartilage grafting, tip-suturing techniques, and meticulous handling of fragile tissues, the surgeon can restore the nose to a more youthful appearance and functional state. Although once taboo, rhinoplasty in the aging patient is increasingly a rewarding procedure for both patient and surgeon alike.     

Rhinoplasty and the nasolabial area

The nasolabial area is important to consider in achieving a satisfactory relationship between the nose and the other features of the face; this will result in a natural-looking nose. However, the importance of the entire nose on a particular face should be kept foremost in mind.     

HPV in the male patient

Human papillomavirus (HPV) is a DNA-containing virus associated with a wide variety of clinical and subclinical diseases. These HPV lesions may resolve spontaneously or progress to benign (condyloma acuminata) or malignant (genital carcinoma) neoplasms. The incidence of HPV genital infection has risen dramatically over the past 30 years, and it is now the most common viral sexually transmitted disease. Many therapeutic options are available to the urologist with new treatments currently being investigated. The history, etiology, pathogenesis, carcinogenesis, and guidelines for evaluation and management are discussed.

Rhinoplasty

Rhinoplasty--surgery to reshape the nose--is a common procedure for both cosmetic and functional requests. In this article, the author provides an overview of rhinoplasty, with special emphasis on the surgical anatomy and preoperative analysis of appearance. A review of some of the surgical techniques at the disposal of the rhinoplasty surgeon is also provided and illustrated with patient examples.     

Secondary Rhinoplasty

Revision rhinoplasty is a complex aesthetic and reconstructive procedure in which both functional and cosmetic principles must be considered in the planning of an appropriate operation. Different techniques must be modified according to the specific defects. The modifications may vary from simple integration of a previous poorly performed surgery to complex grafting of homologous or heterologous material. The authors report their experience with 311 cases of revision rhinoplasty.     

Facial resurfacing of the male patient

Cosmetic facial plastic procedures, surgical and nonsurgical alike, continue to increase in number. Recent statistics from the American Society of Aesthetic Plastic Surgery indicate that approximately 78% of the nearly 8.3 million cosmetic procedures performed in the United States each year are nonsurgical. Similar statistics have been demonstrated by the American Academy of Facial Plastic and Reconstructive Surgery who report that over the past 5 years there has been a 47% increase in nonsurgical procedures compared with a 19% increase in surgical procedures. The demographic of those seeking cosmetic procedures is dramatically changing also. Although cosmetic surgery was once predominantly sought by females, more and more male patients are now seeking treatment.     

Rhinoplasty and the aesthetic of the smile

The resection of the columella and nasal depressor muscles is a simple operation to perform and one which allows an improvement in the facial physiognomy of many patients. This operation can be done alone or in conjunction with the classic rhinoplasty, thus achieving an improvement in the aesthetics of the smile. It has also been proved, contrary to common belief, that the action of these muscles has no connection with physiological breathing mechanisms.
