A patient who developed a subvalvular left ventricular paeudoaneurysm following mitral valve replacement is presented, the fourth such case in the literature. The etiology of this uncommon complication is felt to be caused by excessive surgical resection of the diseased nitral valve, which may weaken or cause rupture of the heart at the mitral annulus. The diagnosis can only be made by left ventriculography. Surgical treatment is indicated, because rupture of the pseudoaneurysm can occur.  相似文献

We report on a case of a false aneurysm at the access site after transapical aortic valve replacement. Although this is not the first case report about this issue, our case emphasizes the importance of follow-up echocardiography after transapical aortic valve replacement that focuses not only on prosthesis function, but also on detecting probable complications of the surgical access site.  相似文献

Despite safer surgical procedures, left ventricular rupture remains a rare but potentially lethal complication of mitral valve replacement. The use of fibrin sealant has substantially improved the outcome of many difficult bleeding episodes after cardiac surgery. We describe a case of left ventricular rupture successfully treated with fibrin sealant combined with external Teflon-pledgeted sutures.  相似文献

Disruption of the posterior mitral anulus is a rare complication of mitral valve replacement that may result in subvalvular left ventricular pseudoaneurysm formation. Such pseudoaneurysm formation was easily recognized by two-dimensional echocardiography in a 54 year old man 3 years after his second mitral valve replacement. The finding was confirmed by cineangiography and direct surgical inspection. Recognition of this rare complication of mitral valve replacement has therapeutic importance because surgical correction is necessary.  相似文献

目的 动态观察二尖瓣置换术(Mitral valve replacement,MVR)后巨大左心室的几何学变化,对比3种不同术式对左心室形态学逆转的影响。方法 回顾性分析1992年1月至2002年1月间48例巨大左心室病人施行二尖瓣置换术后的超声心动图随访资料。结果 保留二尖瓣装置对巨大左心室形态的逆转有效,其中保留全部瓣下结构术后左窒缩小最显,部分保留其次,无保留术后早期缩小晚期再扩大。结论 对于二尖瓣关闭不全为主合并巨大左室。应尽量采用保留瓣下结构的术式,有利于术后晚期左心室形态的逆转。  相似文献

Herein are reported two cases of late left ventricular rupture following mitral valve replacement at 10 and 48 days, respectively, after the initial surgery. We present the case of a 65 year-old female who was admitted to the hospital because of deterioration of chronic dyspnea. Twenty-three years prior to this admission, mitral valve replacement for chronic mitral stenosis was performed using a Starr-Edwards caged-ball prosthesis. There was severe pulmonary hypertension. On transthoracic echocardiography and on cineradiography, the function of the mitral valve prosthesis was unimpaired. However, cineradiography showed extensive mural calcification of the left atrium; we report this remarkable finding.  相似文献

We report about a woman with a rare complication after mitral valve replacement 24 years ago. The patient had a massive hemorrhage some hours after operation. We performed invasive diagnostics because of an increasing pressure gradient across the prosthesis and revealed a left ventricular pseudoaneurysm. Before the planned reoperation the patient died suddenly. As the cause of death, we assumed a rupture of the pseudoaneurysm.  相似文献
