OBJECTIVE: To assess the effects of routine intraoperative transesophageal echocardiography (TEE) on surgical management of patients undergoing all types of cardiac surgery. DESIGN: Prospective, observational. Setting: A single-institution, clinical investigation, university-affiliated hospital. PARTICIPANTS: Two hundred eighty-three consecutive patients undergoing cardiac surgery. INTERVENTIONS: A comprehensive TEE examination was performed in every patient after the induction of anesthesia. An appropriate surgical plan was then developed. A focused TEE examination was also performed at the conclusion of surgery. Whether or not TEE findings represented new information and whether or not this new information altered surgical management was documented. MEASUREMENTS AND MAIN RESULTS: There were 106 new TEE findings in 87 patients (31%). Half of the new findings involved the mitral valve, and a quarter involved the tricuspid valve. The new TEE information altered surgical management 77 ways in 71 patients (25%). Half of the altered surgical managements involved the mitral valve, and a third involved the tricuspid valve. In 8 patients (3%), TEE information influenced decisions regarding use/nonuse of cardiopulmonary bypass (CPB). In 2 patients, TEE examination after the separation from CPB prompted reinitiation of CPB. In 1 patient, TEE examination after the induction of general anesthesia prompted cancellation of surgery. CONCLUSIONS: The routine use of TEE during cardiac surgery revealed new cardiac pathology in 1 of every 3 patients and led to altered surgical management in 1 of every 4 patients. TEE information also influenced decisions regarding use/nonuse of CPB in 3% of patients. Thus, the authors suggest that intraoperative TEE should be used routinely in all patients undergoing cardiac surgery.  相似文献

Transesophageal echocardiography (TEE) is an invaluable intraoperative diagnostic monitor that is considered to be relatively safe and noninvasive. Insertion and manipulation of the TEE probe, however, may cause oropharyngeal, esophageal, or gastric trauma. We report the incidence of intraoperative TEE-associated complications in a single-center series of 7200 adult cardiac surgical patients. Information related to intraoperative TEE-associated complications was obtained retrospectively from the intraoperative TEE data form, routine postoperative visits, and cardiac surgical morbidity and mortality data. The overall incidences of TEE-associated morbidity and mortality in the study population were 0.2% and 0%, respectively. The most common TEE-associated complication was severe odynophagia, which occurred in 0.1% of the study population. Other complications included dental injury (0.03%), endotracheal tube malpositioning (0.03%), upper gastrointestinal hemorrhage (0.03%), and esophageal perforation (0.01%). TEE probe insertion was unsuccessful or contraindicated in 0.18% and 0.5% of the study population, respectively. These data suggest that intraoperative TEE is a relatively safe diagnostic monitor for the management of cardiac surgical patients. IMPLICATIONS: The overall morbidity (0.2%) and mortality (0%) rates of intraoperative transesophageal echocardiography (TEE) were determined in a retrospective case series of 7200 adult, anesthetized cardiac surgical patients. The most common source of TEE-associated morbidity was odynophagia (0.1%), which resolved with conservative management. These results suggest that TEE is a safe diagnostic tool for the management of cardiac surgical patients.  相似文献

Transesophageal echocardiography (TEE) is a monitoring and diagnostic tool for the care of children undergoing cardiac surgery. We analyzed reports from 865 routine TEE examinations performed between January 1994 and March 2002 in patients younger than 17-yr-old who were undergoing surgery for congenital heart disease. Patients' median age was 36 mo (range, 1 day-16 yr). The primary end-point of the study was the incidence of surgical and medical management decisions changed as a result of TEE findings; secondary end-points were diagnostic impact (diagnostic exclusions and new diagnoses) and surgical outcome. Fifty percent of the examinations were performed by anesthesiologists with an advanced level of training in perioperative TEE; all of the examiners had an experience of >or=>500 TEE examinations. Supervision by an anesthesiologist with an advanced level of training was requested in 36.7% of cases; supervision by a cardiologist was requested in 3.8%. Surgical alterations of management were reported in 12.7% of cases and included the need for a repeat bypass run in 7.3%; medical alterations of management were required in 19.4% of cases. We observed a diagnostic impact of TEE in 18.5% of cases and a suboptimal but acceptable surgical outcome in 27.6%; TEE findings predicted postoperative difficulties in 4.0%. Our results confirm the utility of routine TEE to assess repair of congenital heart defects. Furthermore, this service was competently performed by a regular team of cardiac anesthesiologists appropriately trained in TEE. IMPLICATIONS: Transesophageal echocardiography (TEE) is an essential monitoring and diagnostic device for the care of children undergoing cardiac surgery. The surgical and medical impact of TEE is demonstrated in a large series of patients. This service can be performed by appropriately trained cardiac anesthesiologists.  相似文献

Transesophageal echocardiography (TEE) is a semi-invasive diagnostic tool which allows a unique intraoperative visualization of cardiac chambers and valves. It is mainly used for intraoperative monitoring during cardiac surgery and in cardiopathic patients undergoing major non cardiac surgical procedures. TEE gives a beat-to-beat assessment of cardiac morphology and function, it is less invasive and more readily performed than pulmonary artery catheterization, and it allows an etiological diagnosis of hemodynamic problems. Based on TEE data, the anesthesiologist is able to modify in real time vasoactive therapy and fluid management. TEE data concern: 1) preload: left ventricular endodiastolic area is measured and this is considered an index of volemia; 2) systolic function: the circumferential shortening fraction, which correlates with LV ejection fraction, is measured; 3) onset of ischemia: ischemia is detected as a regional modification of normal kinetics; 4) causes of hemodynamic instability; 5) assessment of sovrahepatic anastomoses during liver transplantation.  相似文献

BACKGROUND: The American Society of Anesthesiologists (ASA) has published practice guidelines for the use of perioperative transesophageal echocardiography (TEE) but the role and impact of TEE performed by anesthesiologists outside the cardiac operating room (OR) is still poorly explored. We report our experience in the use of TEE in the noncardiac OR, the recovery room and in the intensive care unit (ICU) in a university hospital, and analyze the impact of TEE on clinical decision making. METHODS: Two hundred fourteen patients were included and TEE indications were classified prospectively according to the ASA guidelines. The examinations and data sheets were reviewed by two anesthesiologists with advanced training in TEE. For each examination, it was noted if TEE altered the management according to five groups: 1) changing medical therapy; 2) changing surgical therapy; 3) confirmation of a diagnosis; 4) positioning of an intravascular device; and 5) TEE used as a substitute to a pulmonary artery catheter. RESULTS: Eighty-nine (37%), 67 (31%) and 58 (27%) patients had category I, II and III indications. The impact was more significant in category I where TEE altered therapy 60% of the time compared with 31% and 21% for categories II and III (P < 0.001). The most frequent reason for changing management was a modification in medical therapy in 53 instances (45%). CONCLUSION: Our results confirm a greater impact of TEE performed by anesthesiologists on clinical management for category I compared to category II and III indications in the noncardiac OR surgical setting and in the ICU.  相似文献

OBJECTIVE: To evaluate the role of intraoperative real-time transesophageal echocardiography (TEE) for the anesthetic and surgical management of patients with renal cell carcinoma and vena cava extension. METHODS: Retrospective analysis of the intraoperative application of TEE in a series of 4 patients. RESULTS: Real-time TEE with a multiplane probe allowed visualization of inferior vena cava tumor extensions, accurate assessment of the distal extent of vena cava invasion into hepatic veins and right atrium, monitoring of embolism and evaluation of cardiac preload and function in all patients. CONCLUSION: Intraoperative TEE is a useful adjunct to the anesthetic and surgical management of patients with renal cell carcinoma and vena cava extension.  相似文献

The beneficial effect of transesophageal echocardiography (TEE) on medical and surgical treatment of children with congenital heart disease has been established. Its cost-effectiveness, however, has not been extensively studied. We analyzed reports of 580 routine TEE examinations performed in our institution between January 1994 and December 2003 in patients younger than 17 yr who required congenital cardiac surgery. After excluding patients who died immediately postoperatively, we identified 33 patients (5.7%) who required a second bypass run on clear-cut indication, i.e., surgical reoperation, and who clearly benefited from TEE findings. An estimate of both fixed and variable costs revealed a savings of 850 to 2655 Swiss francs (CHF) ($690 to $2130 US) per child. This figure undoubtedly underestimates the true cost-effectiveness of routine intraoperative TEE in this setting because we used mostly conservative estimates of the benefits and liberal estimates of the costs. The potential benefits of TEE in hemodynamic monitoring and medical management, in reduction of postoperative morbidity, and in improvement in the quality of life are intangible and were not considered. Although benefits and costs vary according to market conditions, patient populations, surgical practice, and technical expertise with TEE, our analysis demonstrates substantial cost-effectiveness in the use of routine TEE during pediatric cardiac surgery.  相似文献

PURPOSE: To investigate 1) if clinical indications match diagnostic findings from urgent transesophageal echocardiography (TEE) in hemodynamically unstable patients after cardiac surgery and 2) the clinical impact of the TEE findings. METHODS: Retrospective review of all postcardiac surgical intensive care patients who received an urgent TEE over a three- year period from July 1(st) 1997 until June 30(th) 2000. The clinician's presumed diagnosis based on hemodynamic and clinical evaluation was compared to TEE diagnosis. Surgical and medical interventions based on TEE results and the associated mortality were correlated. RESULTS: A hundred and thirty TEEs were performed for hemodynamic instability or suspected intracardiac vegetation or thrombus, all category I indications according to ASA guidelines. In 41.5% of patients the echocardiographic finding matched the presumed diagnosis. Patient management was significantly changed as a result of TEE findings in 58.5% of patients; 43.3% had changes in pharmacological therapy and 15.3% had a surgical intervention. Mortality was significantly lower in those who received a surgical intervention when compared to those who had changes in drug treatment (P <0.05). CONCLUSIONS: The results of urgent TEE in hemodynamically unstable patients or patients with thromboembolic phenomena in the postcardiac surgical intensive care unit are unpredictable in over half of cases. Inappropriate management decisions may result without the information obtained from TEE examination. Clinical management is often modified as a result of TEE findings. TEE is essential in the management of hemodynamically unstable postcardiac surgical patients.  相似文献

Purpose

The purpose of this review is to introduce the uninitiated to transoesophageal echocardiography (TEE): how it works, and what it can do. Sufficient detail is provided to serve as a reference guide to anaesthetists already using TEE in clinical practice.

A Medline search of English language literature up to and including August 1995 was conducted using the key words echocardiography and TEE. Reference echocardiography textbooks were also utilized in the preparation of this review.

Principle findings

All information available from TEE is derived from either cardiac imaging or analysis of blood flow velocity using various Doppler modes. To understand the diagnostic capabilities of TEE we review clinically useful views of the heart as well as modes of cardiac imaging. Sufficient basic physics is presented to allow proper use of adjustment features on the echocardiography machine so that cardiac imaging can be optimized. Available Doppler modes are explained along with an overview of their clinical applications. Figures illustrating clinically useful views obtainable with omniplane TEE are included along with colour prints demonstrating clinical applications of colour flow Doppler.

Conclusion

TEE is becoming increasingly important in the management of cardiac patients for cardiac and non-cardiac surgery. An understanding of the capabilities of the technology as well as the underlying physics allows the anaesthetist to glean the most information from this valuable technique, both quantitatively and qualitatively.  相似文献

According to guidelines established by the American Society of Anesthesiologists and the Society of Cardiovascular Anesthesiologists, life-threatening hemodynamic disturbances are classified as a category I indication for the intraoperative use of transesophageal echocardiography (TEE). However, the usefulness of TEE during intraoperative cardiac arrest and its impact on patient management have not been rigorously investigated. Using our departmental TEE database, we identified a population of 22 patients who underwent noncardiac surgical procedures and experienced unexpected intraoperative hemodynamic collapse requiring the initiation of Advanced Cardiac Life Support procedures between the time of induction of general anesthesia and the termination of the surgical procedure. Results of TEE examinations, patient records, detailed operative records, and outcome of patients were reviewed for the utility of TEE to diagnose the etiology of the hemodynamic collapse. Furthermore, the impact on subsequent patient management was evaluated. A primary suspected diagnosis of the underlying pathological process was established in 19 of 22 patients with TEE, including 9 with thromboembolic events, 6 with acute myocardial ischemia, 2 with hypovolemia, and 2 patients with pericardial tamponade. A definitive diagnosis could not be made in 3 patients with TEE. In 18 patients, TEE guided specific management beyond implementation of Advanced Cardiac Life Support protocols, including the addition of surgical procedures in 12 patients. Fourteen patients survived to leave the operating room, and 7 of these patients were eventually discharged from the hospital. Thus, TEE may provide additional diagnostic information in patients with intraoperative cardiac arrest and may directly guide specific, potentially life-saving therapy.  相似文献

Echocardiography has evolved to become one of the most versatile modalities for diagnosing and guiding treatment of critically ill patients. Both transthoracic (TTE) and transesophageal echocardiography (TEE) provide real-time bedside information about a variety of structural and functional abnormalities of the heart as well as contractility, filling status and cardiac output, rendering it the method of choice for the assessment of cardiac function in the intensive care unit (ICU). Both approaches have its benefits and limitations. Although TTE remains the approach of choice, TEE has been shown to be of additional value in many instances in critically ill patients due to its ability to provide excellent visualisation of cardiac structures, its impact on patient management, and its low complication rate (2.6%). The present status of TEE in adult critical care is reviewed with special emphasis on its role as a diagnostic tool in several clinical scenarios, underlining its effects on clinical decision making but also as a monitoring adjunct. Conditions and settings in which TEE provides the most definitive diagnosis in the critically ill and injured are hemodynamically unstable patients with suboptimal TTE images or if mechanically ventilated, patients with suspected aortic dissection or aortic injury and other conditions in which TEE is superior to TTE (such as suspected endocarditis, cardiac or aortic source of emboli. The diagnostic, therapeutic and surgical impact on patient management in critically ill patients ranged from 44% to 99% (weighted mean 67.2%), 10% to 69% (weighted mean 36%), and 2% to 29% (weighted mean 14.1%), respectively, depending on patients and type of ICU. Since echocardiography provides different information than other devices for hemodynamic monitoring such as the pulmonary artery catheter the methods are therefore not competitive but rather complementary. The present body of evidence supporting the use of TEE in critically ill patients lacks prospective, randomized controlled studies focusing on end-points like cost-effectiveness, morbidity or mortality. However, present evidence as well as experience, points to the significant benefits which may be gained by the availability of echocardiography and especially TEE in ICUs, as well the necessity for a training of intensive care physicians.  相似文献

Transesophageal echocardiography (TEE) is sometimes used in renal cell carcinoma excision for evaluating the extension of tumor in the inferior vena cava (IVC), characterizing the tumor anatomy, monitoring the tumor during surgical mobilization, and assessing cardiac function. Although the risk for embolization is small, when embolization does occur, its consequences can be catastrophic. In this case report, we describe the crucial role of TEE in diagnosing an intraoperative migratory embolus from the IVC to the pulmonary artery and also provide both single-frame photographs and Internet-accessible videos of the event. Our case illustrates the key role that TEE played in the intraoperative management of a patient with renal cell carcinoma undergoing surgical excision of tumor. TEE aided in accurately defining the cephalad extent of the thrombus, provided continuous monitoring of the thrombus during surgical manipulation, and allowed immediate identification of its embolization and proper notification of the surgeons. This case illustrates the crucial role TEE played in the management of a migratory tumor embolus and argues for its routine use during excision of renal cell carcinomas invading the IVC.  相似文献

BACKGROUND: The efficacy of transoesophageal echocardiography (TEE) has been evaluated predominantly in medical and cardiac surgical ICUs. This article reviews the pertinent literature and evaluates the impact of TEE in a general surgical ICU. METHODS: Twenty studies on TEE in the ICU were evaluated for complications, indications, diagnostic, therapeutic, and surgical impact on patient management. Diagnostic impact was defined as identification of the underlying cardiovascular pathology, therapeutic impact as changes in patient management and surgical impact as indication for operative procedures. In addition, we reviewed the TEE reports and patient charts of 216 critically ill patients in a 16-bed multidisciplinary surgical ICU at our university hospital, who underwent a TEE for differential diagnosis of hemodynamic instability from July 1995 to December 1998 to assess the impact of TEE on patient management in a general surgical ICU. RESULTS: The diagnostic, therapeutic and surgical impact in a total of 2,508 patients ranged from 44 to 99% (weighted mean 67.2%), 10-69% (36.0%), and 2-29% (14.1%), respectively. The complication rate was 2.6%, with no examination related mortality. In our series in a general surgical ICU, a diagnostic, therapeutic and surgical impact was inferred in 191 (88.4%), 148 (68.5%) and 12 (5.6%) patients, respectively. Adverse effects were observed in 5.6%. CONCLUSION: TEE is safe, well-tolerated and useful in the management of critically ill patients. This applies as well for hemodynamically unstable patients in a general surgical ICU.  相似文献

PURPOSE: To determine the relative impact of each category-based TEE indication according to the ASA guidelines. METHODS: In 851 patients undergoing cardiac surgery, TEE clinical indications were classified as category I or II according to the ASA guidelines. Category I indications are patients in which TEE is considered useful and category II are those where TEE is potentially useful but indications are less clear. All TEE examinations were reviewed by two anesthesiologists with advanced training in TEE. For each patient, the clinical impact of TEE in the clinical management was assessed using five criteria: 1) change of medical therapy; 2) change in the surgical procedure; 3) confirmation of a suspected diagnosis; 4) positioning of an intravascular device, and 5) substitute to a pulmonary artery catheter (PAC). RESULTS: TEE had greater utility in category I than in category II indications (15/53 (28%) vs. 110/798 (14%) respectively) (P<0.01). The nature of the clinical impact was as follows: modification of medical therapy in 67/125 (53%), modification of planned surgical intervention in 38/125 (30%), confirmation of a diagnosis in 34/125 (27%). The impact on therapy was higher in complex surgical procedures (39%) than in valvular replacement (19%) (P<0.01) and coronary artery bypass surgery (10%) (P<0.001). CONCLUSIONS: Our findings validate the usefulness of the ASA practice guidelines demonstrating a greater impact of TEE on clinical management for category I indications than for category II. TEE also had a greater clinical impact in complex surgical procedures and in valvular replacement.  相似文献

In this study we have surveyed the rate of use of pulmonary artery catheter (PAC) in 319 cardiovascular operations from 1997 to 2001. The rate was decreasing year by year from 100%(6/6) in 1997, 77%(39/51) in '98, 32%(24/75) in '99, 20%(19/93) in 2000, to 8.5%(8/94) in 2001. Especially in elective coronary artery bypass grafting (CABG), an extreme reduction was found: 100%(3/3), 95% (19/20), 17%(6/35), 2.2%(1/45), 0%(0/53), respectively. There was no statistical significance between the use and hospital mortality. We consider that this reduction is due to improvement of the surgical technique and anesthetic management. As transesophageal echocardiography (TEE) has improved anesthetic management of the cardiac patients, we have employed it for all cases. We are sure that TEE contributed to the reduction in the catheter use. Cardiac functions monitored with PAC can be largely substituted with TEE except mixed venous oxygen saturation, but it is not continuous monitoring. Current indications of PAC is for patients with pulmonary hypertension and low cardiac output, as well as those who are predicted to be difficult for postoperative management, and those who have off pump CABG with inverted heart position, at our institution. Routine use of PAC for cardiac surgery should be reconsidered.  相似文献

We experienced pheochromocytoma resection and coronary artery bypass grafting under cardiopulmonary bypass (CPB). The patient was a 69-year-old man who was first diagnosed with atherosclerotic angina. During operation, his blood pressure increased at induction and manipulation of the tumor under CPB, associated with an increased serum noradrenaline concentration. Starting operation, we monitored using transesophageal echocardiography (TEE), and used that view for diagnosis and anesthetic or hemodynamic management. It was especially useful after tumor resection. Surgical and hemodynamic management was facilitated by TEE. TEE was useful to make a diagnosis of cardiac pheochromocytoma, to determine the area of resection, to determine the surgical repair, and to make a decision of hemodynamic management in this complicated patient. We suggest that perfoming these cases under CPB and TEE is recommended for stabilization of hemodynamic states.  相似文献

BACKGROUND: The goal of resuscitation is to correct the mismatch between oxygen delivery and that of cellular demands. The pulmonary artery catheter (PAC) is frequently used to gauge the adequacy of resuscitation and guide therapy based on ventricular filling pressures. Transesophageal echocardiography (TEE) has emerged as a potential tool in assessing adequacy of acute hemodynamic resuscitation. The purpose of this study was to evaluate the role of TEE in assessing preload during ongoing volume resuscitation in trauma patients. METHODS: A retrospective review was conducted of acutely injured patients undergoing TEE during resuscitation from hemorrhagic shock from January 2002 to 2004 at a Level I trauma center. The indication for TEE was persistent hemodynamic instability in the absence of ongoing surgical hemorrhage. Variables included hemodynamic and PAC parameters, pre-TEE resuscitation volume, and vasopressor requirements. The impact of TEE findings on therapeutic decisions was evaluated. RESULTS: Twenty-five patients underwent TEE, 18 (72%) had an indwelling PAC with a mean pulmonary artery occlusion pressure of 19.3 mm Hg (range, 12-29 mm Hg) and mean cardiac index of 2.9 L/min/m2 (range, 1.6-4.6 L/min/m2). Twelve patients (48%) were receiving inotropes and/or vasopressors for hypotension at the time of TEE. Resuscitation volume within 6 hours before TEE included a mean of 6.5 L of crystalloid and 12.2 units of blood products (packed red blood cells, fresh frozen plasma, and platelets). TEE revealed left ventricular hypovolemia in 13 patients (52%) and altered therapy in 16 patients (64%), including additional volume (n = 13), addition of an inotrope (n = 4), and addition of a vasodilator (n = 1) in one patient with ventricular overdistention. Comparison of the abnormal and normal TEE groups revealed that only cardiac index was significantly different (2.6 L/min/m2 in the abnormal group vs. 3.9 L/min/m2 in the normal group; p = 0.005). Significant mitral valve regurgitation leading to valve replacement was identified in one patient. No clinically relevant pericardial effusion was identified. CONCLUSION: TEE altered resuscitation management in almost two thirds of patients. Many patients with "acceptable" pulmonary artery occlusion pressure parameters may in fact have inadequate left ventricular filling. In addition, TEE offers the advantage of direct assessment of cardiac valve competency, myocardial wall contractility, and pericardial fluid.  相似文献

Since the introduction of intraoperative echocardiography into clinical practice in the 1970’s its use and utility in the perioperative period has become increasingly more evident. Especially in patients undergoing cardiac surgical procedures intraoperative echocardiography has gained great diagnostic importance. Intraoperative transesophageal echocardiography (TEE) and epiaortic ultrasound are two important and complementing diagnostic modalities in this patient population. The clinical information obtained with intraoperative TEE in certain cases might have a direct impact on surgical decision-making and therefore may positively influence patient outcome. In patients undergoing non-cardiac surgical procedures, TEE can be a valuable tool in high-risk patients, in patients experiencing hemodynamic instability or in those suffering intraoperative cardiac arrest. Intraoperative TEE might allow a primary diagnosis of the underlying etiology and facilitate the institution of further therapeutic interventions. In addition TEE can be performed during ongoing cardiopulmonary resuscitation and does not interfere with patient management. This review introduces the clinician to the current evidence of the impact of intraoperative echocardiography on intraoperative surgical decisions during surgical procedures. It helps the clinician to identify indications and realize the potential applications of intraoperative echocardiography.  相似文献

Transesophageal echocardiography (TEE) has been used over the last 10 years (1982-1992) to study the heart and thoracic aorta. We set out to evaluate the diagnostic applications of TEE in patients with thoracic trauma. Specifically, TEE was performed on patients suspected of having either a cardiac contusion or an injury of the thoracic aorta. Fifty-eight patients admitted with thoracic trauma underwent TEE. Fifty of those patients suspected of having a cardiac contusion also underwent transthoracic echocardiography (TTE). The two diagnostic modalities were compared. In 21 of these patients a wide mediastinum was apparent on admission chest x-ray films. Nineteen of this latter group underwent thoracic angiography in addition to TEE. Two patients underwent post-mortem examination. Of the 50 patients undergoing both TEE and TTE, a cardiac contusion was detected by TEE in 26 patients. Transthoracic echocardiography detected only six contusions in this group. Of the 21 patients with a wide mediastinum, TEE detected three obvious aortic disruptions. These findings were confirmed in each case by angiography. In 16 cases TEE showed the aorta to be normal. This was confirmed on the angiogram in 14 cases and by autopsy in two cases. Transesophageal echocardiography revealed an aortic intimal irregularity distal to the left subclavian artery in two cases. The results of aortography were normal in these last two cases. As a diagnostic modality, TEE more accurately detected cardiac contusions than TTE (p less than 0.001) and was a very sensitive screening tool in the early evaluation of patients with a wide mediastinum.  相似文献
