目的观察肝动脉切除重建在肝门部胆管癌治疗中的价值。方法1998年1月至2005年12月计收治125例肝门部胆管癌,其中行肝动脉切除13例,对该资料进行分析。结果在行肝动脉切除13例中,同时合并门静脉切除重建3例,其中部分肝固有动脉+右或左肝动脉切除联合左或右半肝及尾状叶切除10例,局部切除联合肝固有动脉切除1例,部分肝固有动脉+右或左肝动脉切除联合扩大左或右半肝及尾状叶切除各1例,肝动脉切除后未重建2例。术后胆肠吻合口漏4例,围手术期肝功能衰竭死亡1例,其余12例病人术后随访4个月至6年,平均20个月,其中最长的1例已存活5年5个月。结论肝动脉切除重建可提高肝门部胆管癌的治愈切除率,改善术后病人预后;肝脏大部切除联合肝动脉切除在中、重度黄疸病人须重建动脉血供。  相似文献

目的分析联合肝切除术结合肝动脉重建治疗肝门部胆管癌10例患者的应用效果。方法回顾性分析2016年1月至2017年2月10例均接受联合肝切除术结合肝动脉重建治疗的肝门部胆管癌患者资料,分析手术情况、围术期并发症及随访结果。结果10例患者中实施左半肝联合尾状叶切除4例(Ⅲb型),右半肝联合尾状叶切除3例(Ⅲa型),尾状叶切除2例(Ⅱ型),切除肝门部胆管及部分左内叶、右前叶及尾状叶1例(Ⅳ型);接受肝右动脉切除重建6例,肝固有动脉切除重建4例;R0切除率为80.0%,围术期均无死亡病例,术后胆瘘、消化道出血、肝动脉血栓继发胆道感染各1例(10.0%),均经保守治疗后症状好转;术后随访9~24个月,3例患者分别因肿瘤复发、肝动脉血栓、肝脓肿而死亡,术后24个月的生存率为70.0%(7/10)。结论给予肝门部胆管癌患者联合肝切除术结合肝动脉重建治疗可提高R0切除率,改善肝功能,且患者围术期并发症少、术后生存率高。  相似文献

Cholangiocarcinoma(CCA) is a malignant tumor of the biliary system and includes, according to the anatomical classification, intra hepatic CCA(iCCA),hilar CCA(hCCA) and distal CCA(dCCA). Hilar CCA is the most challenging type in terms of diagnosis, treatment and prognosis. Surgery is the only treatment possibly providing long-term survival, but only few patients are considered resectable at the time of diagnosis. In fact, tumor's extension to segmentary or subsegmentary biliary ducts, along with large lymph node involvement or intrahepatic metastases, precludes the surgical approach. To achieve R0 margins is mandatory for the disease-free survival and overall survival. In case of unresectable locally advanced hCCA, radiochemotherapy(RCT) as neoadjuvant treatment demonstrated to be a therapeutic option before either hepatic resection or liver transplantation. Before liver surgery, RCT is believed to enhance the R0 margins rate. For patients meeting the Mayo Clinic criteria, RCT prior to orthotopic liver transplant(OLT) has proved to produce acceptable 5-years survivals. In this review, we analyze the current role of neoadjuvant RCT before resection as well as before OLT.  相似文献

目的 探讨肝脏罕见肿瘤的诊断和治疗方法.方法 回顾性分析我院2005年5月至2010年1月收治的25例肝脏罕见肿瘤患者的临床病理资料.结果 25例患者中肝局灶性结节性增生6例,肝血管平滑肌瘤、肝门部神经鞘瘤、肝左叶动脉瘤、肝胆管囊腺瘤、肝错构瘤、肝胆管绒毛状腺瘤、肝弥漫性大B细胞淋巴瘤各1例,肝血管平滑肌脂肪瘤2例,肝原发间质瘤2例,肝母细胞瘤5例,肝胚胎性肉瘤3例;术前行B超检查24例,CT检查22例,MRI检查6例,仅有3例(16.7%)检查和术后病理结果一致.术前诊断和术后病理符合5例(20%).25例均行手术切除治疗,包括半肝切除术7例,肝叶切除术7例,肝段切除术9例,肿瘤局部挖除2例.肝脏良性、低度恶性肿瘤及1例弥漫性大B细胞淋巴瘤术后无复发,5例恶性肿瘤随访中3例术后复发行再次手术切除,术后随访无复发;另2例死亡,平均术后生存期4个月.结论肝脏罕见肿瘤影像学诊断率低,手术切除是主要的治疗手段,对能切除的复发性肿瘤性病变应争取再次手术切除.  相似文献

背景与目的:肝门部胆管癌是指发生在左右肝管、汇合部以及肝总管上段,起源于胆管上皮细胞的一种恶性肿瘤。由于肝门区结构复杂,肿瘤与门静脉、肝动脉等紧邻,故肝门部胆管癌容易出现血管、神经侵犯以及淋巴结转移;加之位置隐匿,早期缺乏特异性症状,患者往往因出现黄疸等晚期症状才会就诊。目前手术切除仍是改善预后的主要有效治疗方式,但对肝胆外科医生而言,肝门部胆管癌的外科治疗仍然是最为困难的挑战之一。笔者报告1例肝门部胆管癌侵犯胃、十二指肠、胰腺的患者施行全胰腺十二指肠联合全肝脏切除、异体肝移植术的治疗经过,以为该病的诊疗提供更多的参考依据。方法:回顾分析中国人民解放军火箭军特色医学中心与河北医科大学第三附属医院共同完成治疗的1例肝门部胆管癌病例的临床资料,并复习相关文献,总结相关的经验教训。结果:患者为51岁男性,有乙型肝炎病史,因腹腔积液就诊。剖腹探查(肿块为涉及肝脏、胰头、肝十二指肠韧带的一个完整无法分离的区域)与PET/CT检查(肝右叶稍低密度伴FDG代谢增高,伴门静脉主干及右支累及可能,未见明显远处转移)均考虑恶性肿瘤,但术前穿刺活检未能诊断。经讨论后对患者实施了全胰腺十二指肠联合肝脏切除与异...  相似文献

IntroductionThis case report is intended to inform pancreas surgeons of our experience in operative management of aberrant pancreatic artery.Presentation of caseA 63-year-old woman was admitted to our institute’s Department of Surgery with obstructive jaundice, and the pancreas head tumor was found. To improve liver dysfunction, an endoscopic retrograde nasogastric biliary drainage tube was placed in the bile duct. Endoscopic fine-needle aspiration showed a pancreas head carcinoma invading the common bile duct, the aberrant right hepatic artery arising from the superior mesenteric artery, and the portal vein. Enhanced computed tomography showed the communicating artery between the right and left hepatic artery via the hepatic hilar plate. By way of imaging preoperative examination, a pancreaticoduodenectomy combined resection of the aberrant right hepatic artery and portal vein was conducted without arterial anastomosis. Hepatic arterial flow was confirmed by intraoperative Doppler ultrasonography, and R0 resection without tumor exposure at the dissected plane was achieved. The patient’s postoperative course was uneventful.DiscussionIn this case report, perioperative detail examination by imaging diagnosis with respect to hepatic arterial communication to achieve curative resection in a pancreas head cancer was necessary. Non-anastomosis of hepatic artery was achieved, and the necessity of R0 resection was stressed by such management.ConclusionBy the preoperative and intraoperative imaging managements conducted, combined resection of the aberrant right hepatic artery without anastomosis was achieved by pancreaticoduodenectomy for pancreas head cancer. BackgroundCombined resection of the right hepatic artery (RHA) is sometimes required to achieve complete resection of hilar cholangiocarcinoma. The present study aimed to evaluate the feasibility of combined resection and subsequent reconstruction by continuous suture of the RHA during left hepatectomy for cholangiocarcinoma.Materials and methodsWe retrospectively compared the outcomes after left hepatectomy with biliary reconstruction for cholangiocarcinoma between patients with and without RHA resection and reconstruction.ResultsOf the 25 patients who underwent left hepatectomy combined with biliary reconstruction, eight patients (32%) underwent combined resection and reconstruction of the RHA (AR group). The demographic characteristics were not different between the AR and non-AR groups. The amount of intraoperative bleeding was significantly greater in patients with AR (2350 mL vs. 900 mL, p = 0.017). The prevalence of early complications above grade III in Clavien–Dindo classification and late complications were not significantly different between the AR and non-AR groups. In the AR group, complications directly associated with AR, such as thrombosis or reanastomosis, were not observed. On Kaplan–Meier analysis, recurrence-free survival (p = 0.618) and overall survival (p = 0.803) were comparable between the two groups despite the advanced T stages in the AR group.ConclusionsCombined resection and subsequent reconstruction of the RHA during left-sided hepatectomy is a feasible treatment alternative for cholangiocarcinoma.  相似文献

目的 观察大鼠原位肝移植重建肝动脉对肝内胆管上皮细胞缺血再灌注损伤后超微结构及术后胆道并发症的影响.方法 228只SD大鼠分为假手术组(8只)、肝移植重建肝动脉组(55对)和未重建肝动脉组(55对).重建肝动脉组和未重建肝动脉组分别于肝脏复流后0.5、3、6、12、24、36、48 h取材,用透射电镜观察肝内胆管上皮细胞的超微结构,通过计算机图像分析系统对线粒体形态计量分析;观察术后胆道并发症.结果 两组肝内胆管上皮细胞损伤均有加重,表现为线粒体肿胀、嵴模糊或消失、微绒毛减少等超微结构改变,至24 h达高峰,以后逐渐恢复.术后两组线粒体平均面积和周径随时间的延长逐渐增大,线粒体数密度随时问延长而减少.在24 h,两组缺血再灌注损伤最显著,之后均开始缓解.在24、36、48 h,两组线粒体平均面积、平均周径比较,差异均有统计学意义(t=-3.566,-7.780,-4.730,-4.610,-2.599,-5.370,P<0.05);在36、48 h,两组线粒体平均数密度比较,差异有统计学意义(t=-4.619,4.000,P<0.05).重建肝动脉组的胆道并发症发生率低于未重建肝动脉组(x2=4.286,P<0.05).结论 大鼠肝移植重建肝动脉对肝内胆管上皮细胞缺血再灌注损伤后的超微结构具有保护作用,有利于术后恢复和减少胆道并发症的发生.  相似文献

背景与目的:中下段胆管癌临床上主要以下段胆管癌多见,下段胆管癌一般采用胰十二指肠切除术,中段胆管癌可以采用胰十二指肠切除术或胆管癌根治、胆肠吻合术。中下段胆管癌因胆管紧邻肝动脉和门静脉,因此更容易发生门静脉侵犯,因肝动脉有动脉外鞘,因此肝动脉受侵犯相对较少,但一旦侵犯,因为涉及肝动脉切除吻合重建,具有较高技术难度,常需联合肝动脉切除重建才能实现R0切除。目前肝动脉切除重建在临床逐渐成熟,但腹腔镜下完成肝动脉切除重建经验缺乏,需要进一步积累。因此,本研究对3例完成腹腔镜下联合肝动脉切除重建的胆管癌患者的临床资料进行回顾性分析并评估短期结果,以期为临床实践提供初步经验。方法:回顾性分析2021年11月—2022年11月中国人民解放军陆军军医大学第二附属医院肝胆外科的3例行联合肝动脉切除重建的中下段胆管癌根治术患者的临床资料。结果:3例患者中女性1例,男性2例,年龄分别为61、65、69岁;病例1为胆管中段癌,因肿瘤侵犯右肝动脉和门静脉,且胆管下端切缘阴性,行联合右肝动脉切除重建、门静脉切除重建、胆管癌切除、胆肠吻合术、肝门部胆管整形术、淋巴结清扫术;病例2为胆管下段癌,因肿瘤侵犯替代右肝动...  相似文献

Background

Preoperative biliary drainage (PBD) prior to liver resection for hilar and intrahepatic cholangiocarcinoma (CCA) is common. While PBD for those with distal obstructions has been studied extensively and is associated with increased infectious complications, the impact of PBD among patients undergoing hepatectomy for non-disseminated proximal CCA has yet to be clearly elucidated.

Methods

Patients undergoing liver resection between 2014 and 2016 for non-disseminated hilar and intrahepatic CCA were analyzed using the American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) database. Associations between PBD (percutaneous or endoscopic) and 30-day outcomes were evaluated.

Results

There were 905 liver resections performed, with 186 (20.6%) for hilar CCA and 719 (79.4%) for intrahepatic CCA. Of those, 251/897 (28.0%) patients underwent PBD. Independent preoperative predictors of PBD were hilar CCA, major hepatectomy, open surgery, lower BMI, and higher preoperative bilirubin. Adjusting for preoperative variables, extent of resection, and bilirubin, PBD was independently associated with increased wound infection (OR 2.93), organ space infection (OR 3.63), sepsis (OR 3.17), renal insufficiency (OR 4.25), transfusion (OR 2.40), bile leak (OR 3.23), invasive intervention (OR 2.72), liver failure (OR 3.20), readmission (OR 3.01), reoperation (OR 2.32), and mortality (OR 4.24, all p?<?0.05).

Conclusions

Among patients undergoing hepatectomy for proximal CCA, PBD is associated with increased postoperative complications. These data suggest that avoidance of routine preoperative biliary drainage may decrease short-term complications.

Hilar bile duct carcinoma has a poor prognosis, but this has been improved in recent years by an aggressive surgical approach. We treated a 73-year-old woman who had obstructive jaundice due to bile duct carcinoma at the hepatic hilum. The jaundice decreased after percutaneous transhepatic biliary drainage. The tumor was resected with the left and caudate lobe of the liver and a part of portal vein. The right hepatic artery was located behind the common hepatic duct, and was suspected to be invaded by the tumor. We dissected the tumor from the arterial wall without carrying out combined resection of the hepatic artery. On the 6th postoperative day, the hepatic artery ruptured and the patient suffered hypovolemic shock. Resection of the hepatic artery and reconstruction were done, but the patient died 2 days later. Histological examination of the resected artery showed that the tumor had been curatively removed by dissection and that no tumor remained at the arterial wall. The rupture of the right hepatic artery was thought to have been caused by damage to the wall during the dissection procedure.  相似文献

Twenty-six patients are reviewed who had primary carcinomas involving the junction of the hepatic ducts. The majority had had an initial procedure of palliative biliary diversion elsewhere and were referred for further treatment. In three cases, en bloc resection of the tumor with total hepatectomy and orthotopic liver transplantation were performed. All tumor growth was encompassed in each case, but within 4 months all succumbed as a result of allograft rejection. Auxiliary (heterotopic) liver transplantation was performed in another patient because of recurrent disease after previous left hepatic resection in continuity with a hilar duct lesion. Five patients underwent hepatic lobectomy with en bloc resection of the hepatic duct junction. When adequate tumor excision was not feasible, biliary diversion could provide good palliation in some instances for extended periods of time. This is demonstrated by one patient who lived for 4 years and 4 months after the initial operation. In the meantime, the patient underwent 6 subsequent procedures of dilating of constricted bile ducts and tube cannulation of the biliary tree. Biliary diversion was achieved in 4 cases by intrahepatic cholangiojejunostomy. One of these patients, who is on chemotherapy, is asymptomatic one year after surgery.  相似文献

IntroductionWe describe successful two-step hepatic artery reconstruction in a patient whose graft site hepatic artery was too short for the use of a microclamp in living donor liver transplantation.Presentation of caseA 57-year-old woman was diagnosed as having hepatitis C and liver cirrhosis. Her 26-year-old son was the living liver donor. The living donor underwent right lobectomy. The dissected graft hepatic artery was too short for the use of a microclamp. The recipient right hepatic artery was cut and used as an arterial graft. The graft right hepatic artery was sutured to the right hepatic artery of the arterial graft and the graft posterior branch of the right hepatic artery was sutured to the middle hepatic artery of the arterial graft. After reconstruction of the portal vein and hepatic vein was completed, anastomosis was performed between the graft right hepatic artery and right hepatic artery. The patency of the vessels was checked using color Doppler ultrasonography for 1 week postoperatively. No postoperative complications involving blood flow of the hepatic artery were observed.DiscussionIn our case, the recipient hepatic artery was cut and used as an arterial graft. Although the number of anastomotic sites of the hepatic artery increased, we could perform hepatic artery reconstruction safely and easily.ConclusionTwo-step hepatic artery reconstruction is a useful method in cases where the recipient hepatic artery does not have enough length.  相似文献

目的 总结胃十二指肠动脉代替肝动脉重建在肝门部胆管癌根治术中的应用经验.方法 回顾性分析2004-2008年9例肝门部胆管癌根治术中,胃十二指肠动脉代替肝动脉重建临床资料及随访结果.结果 9例行肝门部胆管癌根治术肝动脉切除超过1 cm,利用胃十二指肠动脉代替肝动脉进行重建,其中1例联合门静脉部分楔形切除,自身大隐静脉移植修复,8例行肝内胆管支撑.9例术后全身炎症反应综合征于2~3 d后明显缓解,1例术后3 d出现上消化道出血治愈,无手术死亡和住院死亡.术后2周彩色超声临测显示重建肝动脉通畅.9例随访1~4年,中位生存期为23(6~32)个月.结论 胃十二指肠动脉能较好地代替肝动脉重建,减少术后并发症的发生.  相似文献

A 31 year-old patient presented with a retention jaundice from a centrally located hepatoma invading the hilum. Because of the patient's age and the severity of the pruritus and jaundice, palliative treatment was performed by percutaneous catheterization of the intrahepatic biliary tracts to drain the right and left hepatic canals. As the hilar stenosis could not be overcome, the external drainage was transformed into internal drainage by implantation of a Kron's biliary prosthesis linking the intrahepatic biliary tracts, following segment III hepatotomy, to the duodenal lumen, with burying of the prosthesis in the gastric antrum region. Jaundice was reduced until death of the patients from metastases 6 months later. This case demonstrates that the use of Kron's biliary prosthesis to perform a biliodigestive shunt from intrahepatic biliary pathways is a valid palliative procedure in patients with limited life expectancies.  相似文献

目的 探讨血管切除重建在肝门部胆管癌切除术中的价值.方法 2000年1月至2009年9月收治的肝门部胆管癌手术切除患者中,17例合并血管切除或重建,其中男性10例,女性7例,年龄30~72岁,平均53岁.病程4~30 d,平均(21±8)d.门静脉部分切除端端吻合6例,门静脉壁楔形切除、缝合修补3例,肝动脉结扎切除1例,肝动脉切除端端吻合2例,门静脉动脉化1例,1例同时行门静脉壁楔形切除+肝动脉结扎切除,2例同时行门静脉部分切除端端吻合+肝动脉部分切除端端吻合,1例同时行门静脉部分切除端端吻合+肝右动脉、胃十二指肠动脉端端吻合.对患者的临床资料进行分析.结果 住院死亡4例,病死率4/17,3例为术后出现肾功能不全后继发多器官功能衰竭,1例死于感染性休克.未死亡的13例患者中,6例恢复过程顺利,无并发症;7例发生并发症:3例胆瘘,1例呼吸衰竭,1例因U管阻塞发生胆管炎,1例腹腔内感染、门静脉血栓形成,1例远期门静脉狭窄、肝脓肿.中位生存期18个月,4例至今尚存活.结论 肝门部胆管癌切除联合血管切除重建有利于提高切除率但术后风险仍高,术后应警惕并发症的发生;肝动脉切除重建可能有利于降低术后风险.
Abstract:
Objective To investigate the value of vascular resection and reconstruction in resection of hilar cholangiocarcinoma.Methods The clinical data of 17 patients with hilar cholangiocarcinoma received resection in combination with vascular resection and reconstruction from January 2000 to September 2009 was retrospectively analyzed.Among the 17 patients,6 underwent portal vein segmental resection and end-to-end anastomosis,3 underwent portal vein wedge resection,1 underwent hepatic artery ligature,2 underwent hepatic artery segmental resection and end-to-end anastomosis,1 underwent portal vein arterialization,1 underwent portal vein wedge resection and hepatic artery ligature simultaneously,2 underwent portal vein segmental resection and heapatic artery segmental resection and end-to-end anastomosis simultaneously,1 underwent portal vein segmental resection and right heapatic artery and gastroduodenal artery end-to-end anastomosis simultaneously.Results Four patients died and the mortality was 4/17.Three patients died of renal dysfunction followed with multiple organ dysfunction and 1 patient died of sepsis shock.Among the 13 survive patients,6 had a smooth postoperative recover and 7 developed complications:3 had bile leakage,1 had respiratory failure,1 had cholangitis due to obstruction of U tube,1 had abdominal infection and thrombosis in portal vein system and 1 had portal vein stenosis and liver abscess.Follow-up investigation showed that the median survival time was 18 months and four patients still alive.Conclusions Combination of vascular resection and reconstruction in the resection of hilar cholangiocarcinoma may help to improve the resection rate but still have a high postoperative risk.The complications of renal dysfunction should be alert during the postoperative observation.The procedure of hepatic arterial reconstruction may help to reduce postoperative morbidity.  相似文献

We report a rare case of hepatolithiasis, which was diagnosed as hilar cholangiocarcinoma and treated with hepatectomy and extrahepatic bile duct resection. A 59-year-old woman presented to a local hospital with liver dysfunction. Diagnostic imaging revealed a biliary stricture at the hepatic hilum and middle bile duct. Hilar cholangiocarcinoma was diagnosed, and she was referred to our hospital for definitive surgical treatment. She underwent left hepatic trisectionectomy, total caudate lobectomy, and extrahepatic bile duct resection. Gross examination of the resected specimen revealed intrahepatic stones firmly adherent to the bile duct wall. Pathological examination revealed no malignant lesions. The epithelium of the bile duct was absent underneath the stone, and the boundary between the stone and bile duct wall was ill defined. To our knowledge, this is the first case report of hepatolithiasis with a biliary stricture caused by peculiar stone formation, mimicking hilar cholangiocarcinoma.  相似文献

Multiple reconstructions of the hepatic arteries (HA) after cancer resection presents a surgical challenge, not only because it is technically demanding, but also because attention must be paid to potential ischemic injury to the liver caused by the prolonged ischemia. We present a novel “preexcisional artery reconstruction” method for minimizing ischemic injury of the liver. A 65‐year‐old woman presented with cholangiocarcinoma invading the HA. Pancreatoduodenectomy, resection, and multiple reconstruction of the HA were performed. First, the left hepatic artery (LHA) was reconstructed prior to the tumor resection. During this procedure, blood supply was maintained to most of the liver via the right hepatic artery (RHA). Then, resection of the tumor en bloc with the HA was performed, followed by reconstruction of the RHA. During this procedure, blood supply was maintained via the already‐reconstructed LHA, thereby limiting the ischemic area. Use of this method allowed the ischemia time and region to be divided and minimized, thereby leading to a reduced risk of ischemia‐related complications. INTRODUCTIONThe coexistent biliary atresia with aneurysmal dilatation of hepatic artery is a rare association. To know these anomalies will avoid many per operative complications. It is also important to mention that these children require liver transplantation in the long run then these vascular anomalies become more relevant.PRESENTATION OF CASEA four month old male child presented with features of biliary atresia. On exploration a cystic expansile mass was detected beneath thread like common bile duct. Subsequent aspiration and studies proved it to be aneurysmal dilatation of hepatic artery.DISCUSSIONWith biliary atresia many vascular and cardiac malformations have been described but aneurysmal dilatation of hepatic artery is a rare association. These anomalies may have impact on aetiopathogenesis of biliary atresia and also future liver transplantation.CONCLUSIONAwareness of rare association of hepatic artery aneurysm with biliary atresia will help in understanding aetiopathogenesis of biliary atresia and planning liver transplantation in such cases.  相似文献

A 76-year-old male was referred for the treatment of liver tumors detected by abdominal computed tomography (CT). Dynamic CT revealed a low-density tumor with an irregularly enhanced rim in the left lateral sector, and a highly enhanced, well-circumscribed tumor in the caudate lobe, accompanied by dilation of the intrahepatic biliary ducts in the left lobe. Preoperative imaging studies led to the diagnosis of double cancers consisting of intrahepatic cholangiocarcinoma and hepatocellular carcinoma (HCC). Left hemihepatectomy with caudate lobectomy was performed. The postoperative course was uneventful. Microscopic evaluation revealed that the tumor in the left lateral sector was adenosquamous carcinoma (ASC), whereas that in the caudate lobe was HCC. This report presents the first case describing the resection of synchronous double cancers of primary hepatic ASC and HCC.  相似文献
