Three green mills and two dry mills were studied for personal exposure to wood dust and biohazards associated with wood dust and their correlation to lung function and work-related symptoms among sawmill workers. The levels of exposure to endotoxin, (1-->3)-beta-D-glucan, bacteria and fungi were high in green mills compared with dry mills. Compared with dry mill workers, green mill workers had significantly high prevalence of regular cough, chronic bronchitis, regular blocked nose, regular sneezing, sinus problems, flu-like symptoms, and eye and throat irritation. Significant positive correlations were found among endotoxin and Gram (-)ve bacteria, (1-->3)-beta-D-glucan and fungi, and endotoxin and (1-->3)-beta-D-glucan exposure levels. Significant dose-response relationships were found for personal exposures and lung function, and lung function and work-related respiratory symptoms. The significant correlations found for respirable fractions show that not only inhalable but also respirable fractions are important in determining potential health effects of exposure to wood dust. The management and employees of the sawmilling industry should be educated on the potential health effects of wood dust. Wood dust should be controlled at the source. 相似文献

A cross-sectional study including 54 furniture factories and three control factories was conducted to survey lung function and prevalence of respiratory symptoms among woodworkers. Spirometry was performed on 2423 persons. Questionnaires regarding respiratory symptoms and wood dust exposure were completed by 2033 woodworkers and 474 controls. Personal passive dust measurements were performed on 1579 persons. The arithmetic mean +/- SD for equivalent inhalable dust was relatively low (1.19 +/- 0.86 mg/m3). Woodworkers had increased frequency of coughing with negative interaction between dust exposure and smoking. A dose-response relationship was seen between dust exposure and asthma symptoms, and a positive interaction for asthma was seen between female gender and dust exposure. Increased frequency of wheezing and a cross-shift decrease in forced expiratory volume in 1 second among workers using pinewood was seen. In conclusion, wood dust exposure might cause respiratory symptoms, despite a relatively low exposure level. 相似文献

A case-control study of respiratory cancer, nested within a cohort of male woodworkers, was updated in Finland. The update extended the initial follow up of 3805 workers from 19 plants to 7307 workers from 35 plants. Each case of respiratory cancer (n = 136) diagnosed between 1957 and 1982 within the cohort was matched by year of birth with three controls (n = 408) from the cohort. Chemical exposures were assessed for the cases and the controls by a plant and period specific job exposure matrix. An excess of respiratory cancer was associated with phenol. Concomitant exposures to several other agents occurred as well, however, and no exposure-response relation for phenol was seen. An excess risk and an increasing exposure-response relation were found for engine exhaust from petrol and diesel driven factory trucks. The excess risk associated with pesticides was lower than in our previous study, an indication of qualitative and quantitative differences in exposure between the initial and augmented cohorts. Slightly increased risks were found for terpenes and mould spores, which may be due to chance although the contribution of occupational exposure cannot be ruled out. Exposure to wood dust, mainly from pine, spruce and birch, at a level of about 1 mg/m3, was not associated with lung cancer, upper respiratory cancer, or adenocarcinoma of the lung. 相似文献

BACKGROUND: If the occurrence of disease monotonically increases with the degree of exposure in an epidemiologic study, a dose-response (or exposure-response) relationship is indicated and facilitates the interpretation that the exposure has a causal role. It is not uncommon, however, that there is some effect in terms of an overall increased relative risk but no clear dose-response relationship. METHODS: Models presented here show that cumulative exposure, as involving the duration of exposure, is not an adequate parameter when more recent exposure or the intensity of the exposure plays the greater role for the disease outcome. CONCLUSIONS: In lack of a dose-response pattern by cumulative exposure, the interpretation of an overall increased risk might well be that there is no definite effect. The proper consideration should be, however, that the measure of exposure could be inadequate, suggesting a need for further analyses and evaluations of the material studied. 相似文献

In 186 cases of nasal cancer diagnosed over the decade 1965-74, in a population of 2-0 million, 114 of 157 ectodermal tumours were found in men. Adenocarcinoma was found in 17 patients, two women and 15 men; 12 of these had a history of occupational exposure to wood dust in the furniture industry. The period of latency was from 28 to 57 years. Among the remaining 99 tumours in men there was occupational exposure to wood dust in 10 cases. Wood dust concentrations in the breathing zone, respiratory symptoms and upper airway function were studied in 68 workers in the furniture industry. The dust concentrations affecting 63% of the workers were higher than 5 mg/m3 (TLV). Middle ear inflammation and common colds were more frequent at high dust concentrations, and the number of workers with nasal mucostasis was directly proportional to the wood dust concentration. 相似文献

We report results of two pilot studies of the relationship between musculoskeletal disorders and work tasks in sheet metal workers. These two studies were undertaken as pilot investigations to determine if symptoms of carpal tunnel syndrome or other work-related musculoskeletal symptoms are present in sheet metal workers, and to determine if we could begin to identify the factors in sheet metal work that cause them. In the first study, telephone interviews were conducted with 18 disabled sheet metal workers; the second survey entailed self-administered questionnaires completed by 47 active and retired sheet metal workers. Among the disabled workers, rotator cuff cases reported the greatest percent of time spent hanging duct, an overhead task commonly carried out during field work. Carpal tunnel cases reported more hand tool use than the rotator cuff cases. The questionnaire survey of active and retired workers found the proportion of time spent in a sheet metal shop (contrasted with field work) to be positively associated with hand symptoms; sheet metal workers who spent at least 65% of their time in the shop had an odds ratio of 3.4 for symptomatic hand cumulative trauma disorder (CTD) (p = 0.12). The proportion of time spent hanging duct was positively associated with both neck and shoulder symptoms, with odds ratios of 7.9 (p = 0.08) and 2.7 (p = 0.16), respectively. The results show that symptoms of neck, arm, and hand pain are common in sheet metal workers who are actively working, that carpal tunnel syndrome or hand pain is positively associated with more time spent working in the shop, and that shoulder pain or shoulder injuries is associated with work overhead (hanging duct). Ergonomic risks and cumulative trauma disorders have not been well characterized in the construction industry; these data strongly suggest that a subset of work tasks in construction increase the risk of cumulative trauma disorders. 相似文献

Sewage operatives at five sewage treatment plants (n = 59) and controls not exposed to sewage (n = 55) were examined to determine work-related symptoms and inflammatory responses. Symptoms were elicited using a questionnaire, and spirometry was performed. Inflammatory markers were determined in blood and nasal lavage. Workplace endotoxin and hydrogen sulfide were measured and adeno- and enterovirus antibodies were evaluated in blood. Gastrointestinal and airway symptoms, joint pains, unusual tiredness, and toxic pneumonitis were more common among operatives, and the proportion of blood neutrophils was higher among operatives as compared with controls. A relationship was found between several reported symptoms and the inflammatory markers. Hydrogen sulfide levels were very low. Endotoxin levels were generally low, but high at some work sites. 相似文献

The authors conducted a cross-sectional study of 474 rice-granary workers and 235 non-granary worker controls in a rural area near Shanghai, the People's Republic of China. Responses to a respiratory-symptom questionnaire and pre- and post-shift spirometry were obtained for all subjects. Area sampling was performed for total and vertically elutriated (= 15 micrometer) dust levels. Total dust levels were high, ranging from 6.6 mg/m(3) to 59.8 mg/m(3), with vertical elutriated dust concentrations ranging from 2.0 to 10. 4 mg/m(3). The granary workers reported significantly more respiratory symptoms, including chronic cough, sputum production, chronic bronchitis, grain fever (ODTS), and nasal and skin irritation. Grain dust and tobacco smoking were more than additive for the prevalence of chronic cough and chronic bronchitis. After adjusting for confounders, the granary workers had lower mean FEV&inf1;/FVC values both pre- and post-shift, indicating an association between chronic grain-dust exposure and chronic airway obstruction. The results suggest that exposure to rice dust can induce pulmonary responses similar to those observed with exposures to other types of grains. 相似文献

Objective
Research on the respiratory effect of exposure to solder fumes in electronics workers has been conducted since the 1970s, but has yielded inconsistent results. The aim of this meta-analysis was to clarify the potential association.Methods
Effect sizes with corresponding 95% confidence intervals (CIs) for odds of respiratory symptoms related to soldering and spirometric parameters of solderers were extracted from seven studies and pooled to generate summary estimates and standardized mean differences in lung function measures between exposed persons and controls.Results
Soldering was positively associated with wheeze after controlling for smoking (meta-odds ratio: 2.60, 95% CI: 1.46, 4.63) and with statistically significant reductions in forced expiratory volume in 1 s (FEV1) (−0.88%, 95% CI: −1.51, −0.26), forced vital capacity (FVC) (−0.64%, 95% CI: −1.18, −0.10), and FEV1/FVC (−0.35%, 95% CI: −0.65, −0.05). However, lung function parameters of solderers were within normal ranges [pooled mean FEV1: 97.85 (as percent of predicted), 95% CI: 94.70, 100.95, pooled mean FVC: 94.92 (as percent of predicted), 95% CI: 81.21, 108.64, and pooled mean FEV1/FVC: 86.5 (as percent), 95% CI: 78.01, 94.98].Conclusions

Wastewater treatment plant workers are exposed to microbes, including Salmonella, but the prevalence of antibodies against Salmonella species or serovars in their serum samples has not been studied. Antibodies against Salmonella Infantis and lipopolysaccharide antigen common to S. Enteritidis and S. Typhimurium in immunoglobulin classes IgA, IgM and IgG were measured from 79 serum samples of wastewater treatment plant workers and from 79 blood donor samples. Faecal samples for Salmonella and Campylobacter were studied. Gastrointestinal, dermal and other symptoms were compared between 81 wastewater treatment plant workers and 89 food-processing workers. The blood donors had more antibodies against all of the tested antigens expect for S. Infantis in IgM and IgA classes, even though the wastewater treatment plant workers had more gastrointestinal symptoms than the controls. No Salmonella or Campylobacter were found in any faecal samples. Salmonella is not a probable cause of symptoms among wastewater treatment plant workers. 相似文献

The objective of this work was to determine whether the prevalence of respiratory symptoms differed among workers exposed to different types of metal-working fluids. As part of a mandatory surveillance system for occupational illness, from 1988-1994, the Michigan Department of Public Health received, 86 occupational disease reports of work-related asthma secondary to exposure to metal-working fluids. As part of a public health program, follow-up industrial hygiene inspections, including medical interviews of the workforce, were performed at companies where the reported cases had become ill. Metal-working fluids were the second most common cause of work-related asthma reported in the state. Most of the reports were from the automobile industry. Follow-up inspections were conducted at 37 facilities where the individuals with work-related asthma had worked. Seven hundred and fifty-five workers at these facilities were interviewed. Only one facility was above the allowable oil mist standard. Despite the exposure levels being within the legal limits, approximately 20% of the fellow workers of the reported cases had daily or weekly respiratory symptoms suggestive of work-related asthma. Workers exposed to emulsified, semisynthetic, or synthetic machining coolants were more likely to have chronic bronchitis; to have visited a doctor for shortness of breath; to have visited a doctor for a sinus problem; to be bothered at work by nasal stuffiness, runny nose, or sore throat; and to have an increased prevalence of respiratory symptoms consistent with work-related asthma, compared to workers exposed to mineral oil metal-working fluids. These findings were found in individuals who currently smoked, had never smoked or were ex-cigarette smokers. Our objectives were to measure the prevalence of work-related and nonwork-related respiratory symptoms in a group of New Zealand mussel openers who open green-lipped mussels, and to relate these to demographic factors, work history, smoking history, and pulmonary function measurements. A cross-sectional study of respiratory symptoms and lung function was performed on 224 New Zealand mussel openers (99.6% of the study population) at nine work sites. In addition, peak expiratory flow (PEF) change across-shift was measured at one work site in 19 workers. The mean age of all mussel openers was 33.4 years and the mean duration of mussel opening was 5.0 years; 25% were male, 54.7% were current smokers, and 13.9% were ex-smokers. The reported symptom prevalences were: any wheeze, 35%; work-related wheeze, 23%; any chest tightness, 30.5%; work-related chest tightness, 20.2% (work-related symptoms were defined as symptoms improving on rest days or worse at work). Seventy-two mussel openers (32.3%) answered positively to at least 1 of 4 questions concerning work-related symptoms. The mean predicted FEV1 (SD) for this group was 74.3% (14.5), and the mean predicted FVC (SD) was 79.2% (16.0). Nineteen workers completed serial PEF, and the mean percentage change was +1.5% at 7 hr, but 8 workers had falls ranging between 1.1–14% after either 1 or 7 hr of work. Duration of mussel opening of greater than 2 years, but less than 7 years (OR = 2.29; 95% CI, 1.07–4.91), and duration of mussel opening greater than 7 years (OR = 3.72; 95% CI, 1.52–9.11), were significant predictors of work-related respiratory symptoms. Female sex (OR = 1.73; 95% CI, 0.83–3.60) was also associated with the presence of work-related symptoms. No relationship was found with measured hygiene parameters or cleaning agents used. In conclusion, duration of work as a mussel opener was associated with the presence of work-related respiratory symptoms, after adjustment for age, sex, and smoking habit. Health hazards associated with wood dust exposure have been investigated in various industries. This study surveyed wood dust exposure levels and pulmonary effects among joss stick workers. Greater dust concentrations, as measured by six-stage cascade impactors, were observed in work areas where joss sticks were produced and incense was mixed than in other work areas. Total dust concentrations for these two high dust activities ranged from 9.9 to 42.7 mg/m3, and respirable proportions were between 2.0% and 54.6%. Higher dust levels were observed for dry joss stick production methods than for wet production methods. Dust levels for all other performance areas were lower than the permissible exposure level of 10 mg/m3. Although symptoms of cough and phlegm were higher in smoking workers than in nonsmoking workers, the prevalence of respiratory symptoms for exposed workers was not significantly higher than for the controls. The prevalence of pulmonary function deficits and the values of FEF25% and FEF75% in the exposed workers were significantly worse than those in the controls. But no difference was found between the male controls and the male exposed workers, the high-exposure group. Respiratory symptoms and pulmonary function also did not show a dose-response trend with the exposure levels estimated by correlation with worker job titles and duration of employment. No suspected case of pneumoconiosis was found from the chest radiographs. These results suggest that wood dust exposure in the joss stick industries might not lead to significant pulmonary damage. 相似文献

Respiratory and allergic symptoms were evaluated in 66 Iranian carpet weavers and 66 controls with similar age and gender characteristics using a questionnaire including questions on work-related respiratory symptoms in the past year, allergies, smoking habits, and work exposure duration. A total of 28 carpet weavers (42%) reported work-related respiratory symptoms. Incidences of allrespiratory symptoms and most allergic symptoms were significantly higher in carpet weavers than in controls (p<0.05 - p<0.001). Moreover, most respiratory and allergic symptoms in carpet weavers were significantly more prominent during working hours (p<0.01 - p<0.001). Pulmonary function test results of the carpet weavers showed significant impairment compared with controls (p<0.05 - p<0.001). 相似文献

In a 13 year follow up study conducted among residents of Cracow the relation of annual rate of decline in FEV1 to occupational exposures was analysed. The study group consisted of 696 men and 983 women aged 19-60 at the start of the study in 1968. They were interviewed three times, in 1968, 1973, and 1981, and decline in FEV1 was estimated for each subject from spirometric measurements in 1968 and 1981. The interviews provided data on exposure at the workplace to dusts, variable temperature, and chemicals or irritating gases, which established duration and time of the exposure. The FEV1 mean level, height, and smoking habits were considered as confounders in the analysis. The study indicated that the most pronounced influence on decline in FEV1 was prolonged and continuing exposure to variable temperature. The effects of dusts, independent of exposure to variable temperature, were much smaller but analysis in occupational subgroups suggest that dust may be important in some, such as workers in the building materials and pottery industry. Relatively immediate effects of exposure to chemicals were detected independently of effects of other exposures. The estimated effects of occupational exposures were of a similar magnitude as those of tobacco smoking though related to much smaller groups. Both effects were additive in accelerating decline in lung function. These results, obtained in the general population and less biased by selection than studies performed in industrial settings, show the importance of occupational factors in the natural history of limitation of airflow. 相似文献

Human respiratory health hazards for people working in livestock confinement buildings have been recognized since 1974. However, before comprehensive control programs can be implemented, more knowledge is needed of specific hazardous substances present in the air of these buildings, and at what concentrations they are harmful. Therefore, a medical epidemiological and exposure-response study was conducted on 207 swine producers using intensive housing systems (108 farms). Dose-response relationships between pulmonary function and exposures are reported here. Positive correlations were seen between change in pulmonary function over a work period and exposure to total dust, respirable dust, ammonia, respirable endotoxin, and the interactions of age-of-producer and dust exposure and years-of-working-in-the-facility and dust exposure. Relationships between baseline pulmonary function and exposures were not strong and therefore, not pursued in this study. The correlations between exposure and response were stronger after 6 years of exposure. Multiple regression models were used to identify total dust and ammonia as the two primary environmental predictors of pulmonary function decrements over a work period. The regression models were then used to determine exposure concentrations related to pulmonary function decrements suggestive of a health hazard. Total dust concentrations ? 2.8 mg/m3 were predictive of a work period decrement of ? 10% in FEV1. Ammonia concentrations of ? 7.5 ppm were predictive of a ? 3% work period decrement in FEV1. These predictive concentrations were similar to a previous doseresponse study, which suggested 2.5 mg/m3 of total dust and 7 ppm of NH3 were associated with significant work period decrements. Therefore, dust ? 2.8 mg/m3 and ammonia ? 7.5 ppm should be considered reasonable evidence for guidelines regarding hazardous exposure concentrations in this work environment. 相似文献
