BACKGROUND: Elevated plasma triglycerides (TG) and waist girth (hypertriglyceridemic waist (HTGW)) has been associated with elevated insulin, small dense low-density lipoprotein (sLDL) particles, and Apo B in men. The HTGW has not been reported for women and the effect of cardiorespiratory fitness ("fitness") on associations between HTGW and coronary risk factors is unknown. PURPOSE: To determine the prevalence of HTGW and the influence of fitness on the relationship between HTGW and coronary risk among 137 healthy women (54+/-9 year; body mass index (BMI)=28+/-6 kg/m(2)). METHODS: HTGW was defined as waist girth >88 cm and TG >150 mg/dl. The metabolic triad was defined as insulin >31 pmol/l, Apo B >69 mg/dl and LDL-C >84 mg/dl. Fitness was assessed with a maximal treadmill exercise test. RESULTS: The sample prevalence of HTGW (n=15) was 11% (95% CI=5.7-16.0%). Apo B (P=0.04) and insulin (P=0.0001) increased across quintiles of waist girth, and LDL-C (P=0.004) increased across quintiles of TG. Metabolic triad prevalence was highest (67%, n=10) among HTGW women and lowest (22%, n=26) among non-HTGW women. A trend for higher coronary heart disease CHD risk factors was observed among HTGW compared with non-HTGW women. Among the HTGW group, a trend for lower CHD risk factors was observed among fit (>or=6.5 METs, n=7) versus unfit women (<6.5 METs, n=8). Sample size limitations prohibited meaningful tests of significant differences in CHD risk factors when stratified simultaneously on HTGW and fitness status. CONCLUSIONS: HTGW is associated with increased coronary risk factors similarly among women as reported for men. Higher fitness may improve the CHD risk profile among women with HTGW. 相似文献

目的探讨女性高甘油三酯血症腰(HTGW)的临床特征。方法根据血清甘油三酯(TG)和腰围(WC)、空腹血糖(FPG)〈5.6mmol/L,将395例女性患者分为HTGW(TG≥1.70mmol/L和腰围≥80cm)组和非HTGW组,FPG≥5.6mmol/L 82例为高FPG组,采用HOMA-IR指数评价胰岛素抵抗。结果①多元逐步回归分析显示腰围、TG和年龄与HOMA-IR指数独立相关;②HTGW组HOMA-IR与高FPG组比较差异无显著性;③与高FPG组比较,HTGW组体重指数、腰围、血压、TG升高,高密度脂蛋白胆固醇(HDL-C)降低,与非HTGW组比较,HTGW组上述指标变化更为明显。结论女性HTGW患者有与高FPG相似的胰岛素抵抗程度,有肥胖、血压高、血脂紊乱(TG升高、HDL-C降低)的临床特征。 相似文献

The present study aimed to evaluate the relationship between the hypertriglyceridemic waist (HTGW) phenotype and hyperuricemia. We undertook a cross-sectional study with a sample of 11,576 adults from China. The HTGW phenotype was defined as elevated waist circumference (WC) and elevated triglyceride (TG) concentration. Logistic regression analysis was used to evaluate the association between the HTGW phenotype and hyperuricemia. The prevalence of hyperuricemia was significantly higher in individuals with the HTGW phenotype, as compared to those with the normal waist normal triglyceride (NWNT) group (25.9 vs. 6.1%, respectively, P < 0.001). After adjusting for age, sex, and race, the HTGW phenotype was associated with hyperuricemia (OR 6.09; 95% CI 5.18–7.17). Further adjusted for potential confounders, the HTGW phenotype was still significantly associated with hyperuricemia (adjusted OR 4.54; 95% CI 3.37–5.54). Moreover, the association between the HTGW phenotype and hyperuricemia was stronger in men (adjusted OR 4.59; 95% CI 3.53–5.98) than in women (adjusted OR 3.55; 95% CI 2.60–4.86). This study indicated that the HTGW phenotype was strongly associated with hyperuricemia, and uric acid should be clinically monitored in individuals with the HTGW phenotype. We can speculate that the HTGW phenotype might be a marker for identifying individuals at high risk of hyperuricemia. 相似文献

The purpose of this study was to explore the possibility that the recently described “hypertriglyceridemic waist” (HTGW) phenotype, a risk for future coronary artery disease, is associated with the presence of low-grade inflammation. This is a cross-sectional study in a cohort of apparently healthy nondiabetic employed individuals in whom the presence of low-grade inflammation was determined by using the Dade Behring high-sensitivity C-reactive protein (hs-CRP) assay. We have presently analyzed the results obtained in 9842 apparently healthy individuals, at a mean (SD) age of 44 (11) years. We identified 1249 individuals (70.0% men) with HTGW phenotype according to the cutoff points of waist girth of at least 90 cm for men and at least 85 cm for women and triglycerides levels of at least 177 mg/dL for men and at least 133 mg/dL for women. In addition, we identified 1164 individuals (69.3% men) with the metabolic syndrome (MetS) according to the updated Adult Treatment Panel III criteria. The mean (SD) of hs-CRP was 1.3 (2.9) mg/L for the 8055 individuals who had neither the HTGW phenotype nor the MetS, 2.1 (2.7) mg/L for those who had the HTGW phenotype and no MetS, and 2.5 (2.7) for 538 individuals with the MetS and no HTGW phenotype, whereas those who had both atherogenic disorders presented an hs-CRP concentration of 2.9 (2.3) mg/L. In this cohort of apparently healthy nondiabetic employed individuals, the HTGW phenotype had a similar prevalence as the MetS and was associated with the presence of low-grade inflammation. This inflammation could be a pathophysiologic link between this dysmetabolism and atherothrombosis. In addition, the HTGW phenotype is relatively prevalent and could be a simple and inexpensive way to single out individuals at risk for future coronary artery disease. 相似文献

目的探讨高甘油三酯腰围表型(HTGW)与急性胰腺炎(AP)病情程度及预后的相关性。方法选取西南医科大学附属医院2018年9月-2019年8月收治的356例AP患者为研究对象,将患者分为甘油三酯和腰围正常组(NWNT)、单纯高甘油三酯组(NWET)、单纯腹型肥胖组(EWNT)、高甘油三酯腰围表型组(HTGW)4个组,对比4组患者的临床特点,包括年龄、性别、实验室指标、病情分级、并发症等。符合正态分布的计量资料组间比较采用one-way ANOVA检验,组内两两比较采用LSD-t检验;不符合正态分布的计量资料组间及组内两两比较均采用Kruskal-Wallis H检验。计数资料二分类变量组间及组内两两比较均采用χ2检验,有序多分类变量组间比较采用Kruskal-Wallis H检验,组内两两比较采用Mann-Whitney U检验。患者发生器官衰竭和胰腺感染坏死的影响因素采用单因素及多因素logistic回归分析。结果HTGW组中性粒细胞计数水平高于NWNT组,CRP水平高于NWNT、NWET组,肌酐水平高于NWNT组,总胆固醇、TG、低密度脂蛋白水平高于NWNT、EWNT组,高密度脂蛋白水平低于NWNT、EWNT组,血糖水平高于NWNT组,差异均有统计学意义(P值均<0.05)。HTGW组重型急性胰腺炎发生率高于其他3组,差异有统计学意义(χ2=189.519,P<0.001)。HTGW组感染性胰腺坏死和器官衰竭并发症比例均高于其他3组,差异均有统计学意义(χ2值分别为11.770、19.457,P值分别为0.008、<0.001)。单因素logistic回归分析显示性别、复发史是感染性胰腺坏死的危险因素,校正危险因素后,HTGW组发生感染性胰腺坏死的风险为NWNT的3.347倍[95%可信区间(95%CI):1.538~7.283,P=0.002];BMI、腰围甘油三酯指数、吸烟、糖尿病、HTGW是器官衰竭的危险因素(P<0.05),校正危险因素后,HTGW组发生器官衰竭的风险为NWNT的4.143倍(95%CI:1.299~13.219,P=0.016)。结论HTGW是重型急性胰腺炎发生的独立危险因素,影响患者的病情程度及并发症的发生,应早期识别和诊断此型患者,积极改善脂代谢紊乱,改善患者的病情及预后。 相似文献

目的:回顾性地分析高甘油三酯腰围表型(HTWP)在早发冠心病患者中的临床意义。 方法:收集本院经临床及冠状动脉造影确诊的早发冠心病患者123例,其中合并HTWP组71例,无HTWP组52例。测定低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、C反应蛋白(CRP)、空腹血糖、尿酸、血压等指标。结果:合并HTWP组的LDL-C、空腹血糖、CRP显著高于无HTWP组(P<0.05);合并HTWP组急性冠脉综合征(ACS)发生率高于无HTWP组(39.44%比19.23%,P<0.05),冠脉病变Gensini评分高于无HTWP组[(36.61±31.22)比(25.31±27.44),P<0.05]。结论:HTWP在早发冠心病患者可作为评估冠心病的危险因素;合并HTWP与ACS发生率及冠状动脉病变严重程度有关。 相似文献

目的 研究非ST段抬高型急性冠脉综合征合并高甘油三酯血症腰围表型(HTWP)患者的临床特征和冠状动脉评分。方法 回顾性分析2014年4月至2015年12月在秦皇岛市第一医院心内科就诊的非ST段抬高型急性冠脉综合征患者的临床特征和冠状动脉评分及其危险因素。结果 共入选226例患者,年龄(60.5±9.6)岁,男性155例,HTWP组99例(43.8%)。HTWP组腰围、甘油三酯、总胆固醇、低密度脂蛋白胆固醇和空腹血糖均高于无HTWP组(98.7±9.0比93.2±10.1、2.71±1.46比1.31±0.46、4.78±1.03比4.07±1.00、3.16±0.98比2.55±0.84和6.79±2.74比6.06±1.75,所有P<0.05),高密度脂蛋白胆固醇则低于无HTWP组(1.05±0.66比1.15±0.30,P<0.05),而冠状动脉评分高于无HTWP组(18.3±15.5比15.2±14.3,P<0.05)。多元回归分析显示,冠状动脉评分与HTWP、空腹血糖和甘油三酯相关,其标准化回归系数分别为0.192、0.179和0.143(P<0.05)。结论 非ST段抬高型急性冠脉综合征合并HTWP患者冠状动脉评分较高,且冠状动脉病变评分与HTWP、空腹血糖和甘油三酯相关。 相似文献

This study tested the hypothesis that the "hypertriglyceridemic waist" phenotype (waist girth >90 cm [35.4 inches] in men and >85 cm [33.5 inches] in women, along with a plasma triglyceride concentration of >or=2.0 mmol/L [177 mg/dl]) as a covariate of metabolic syndrome features (hyperinsulinemia, hyperapolipoprotein B, and small low-density lipoprotein particles), is predictive of premature coronary artery disease (CAD) among patients with glucose intolerance or type 2 diabetes. Glucose intolerance and type 2 diabetes were assessed after an oral glucose tolerance test among 1,190 men and women using the American Diabetes Association criteria. Glycemic control was evaluated using hemoglobin A1c levels. CAD was considered present on the basis of a clinical history of retrosternal pains on exertion, electrophysiologically and clinically documented myocardial infarction, or angiographic evidence of coronary lesions. More than 53% of men (n = 103) with a waist circumference >or=90 cm (35.4 inches) and nearly 80% of women (n = 122) with a waist circumference >or=85 cm (33.5 in.) with triglyceride levels >or=2 mmol/L (177 mg/dl) were diagnosed with glucose intolerance or type 2 diabetes. Survival models revealed that those with glucose intolerance or type 2 diabetes with the "hypertriglyceridemic waist" phenotype experienced their first CAD symptoms 5 years earlier than those without this phenotype. This elevated and earlier risk of CAD was statistically significant (hazard ratio 2.0, 95% confidence interval 1.2 to 3.7, p = 0.02). In conclusion, the "hypertriglyceridemic waist" phenotype, an inexpensive and simple tool identifying subjects with metabolic syndrome features, is a significant marker of CAD manifestations occurring at an earlier age in those with glucose intolerance or type 2 diabetes. 相似文献

Background
Recent studies have shown that Vitamin D deficiency is very common globally. Vitamin D deficiency is associated with lipid metabolism. A relationship between vitamin D levels and waist circumference (WC) has been observed. The purpose of this study is to evaluate the relationship between vitamin D status and metabolic dyslipidemia and the hypertriglyceridemic waist phenotype.Methods
This cross-sectional study was conducted on 265 healthy Tehran adults. Hypertriglyceridemic waist phenotype (HTGWP) was described as serum triacylglycerol concentrations >150?mg/dL and concurrent WC?>?88?cm (women) and >102?cm (men). Dyslipidemia was defined as: 1) TG level of >150?mg/dL 2) HDL?>?40?mg/dL for men or >50?mg/dL for women, as has been previously described.Results
Current study’s results demonstrated that HTGWP was significantly associated with weight, age, WC, hip, fat percent, TG, lipid profile, ALT and BMI. We found 77% reduction in the chances of developing metabolic dyslipidemia in suficient satus of vitamin D in compare to deficiency, although the significancy was mariginal, OR: 0.33, 95% CI of 0.09 to 1.21, P?=?0.09. However, our results revealed that vitamin D deficiency, compared with normal status, can increase the risk of phenotype 1 (high TG/high WC); OR: 3.86 and 95% CI from 0.86 to 0.99, p for trend?=?0.05.Conclusions
Significant associations were found between vitamin D status and HTGWP. In addition, there was a relationship between vitamin D and lipid profiles. There is a direct correlation between TG and waist circumference in insulin resistance in healthy Tehran adults. 相似文献11.

Background and aimsLipid alterations in overt hypothyroidsm (OH) were well known, but its changes in subclinical hypothyroidism (SCH) and postprandial period were not clear. The aim of this study is to evaluate postprandial lipemia by oral lipid tolerance test (OLTT) in patients with OH and SCH.Materials and methodologyTwenty-five OH and 27 SCH, totally 52 hypothyroid patients [mean age 38.3 ± 12.8 year, body mass index (BMI): 29.0 ± 5.8 kg/m2] and 23 BMI- and age-matched healthy controls (mean age 36.7 ± 11.9 years; BMI: 27.1 ± 6.9 kg/m2) were included to the study. Anthropometric measurements and HOMA-IR levels were measured. Basal and postprandial lipid profile at 2nd, 4th, 6th and 8th hours were determined by oral lipid tolerance test.ResultsThere were not any statistical differences among three groups (control, OH and SCH) in terms of mean fasting levels of total cholesterol, LDL-cholesterol, VLDL-cholesterol, and triglyceride. On the contrary, mean triglyceride levels at postprandial 8th hour in both OH and SCH patients were higher than control subjects (p = 0.017 and p = 0.049, respectively). Again mean postprandial 8th hour VLDL-cholesterol levels in OH group were also higher than control subjects (p = 0.05). In addition mean HOMA-IR value of SCH and OH patients was similar with control subjects (1.5 ± 1.4 in OH; 1.3 ± 0.8 in SCH; 2.2 ± 2.2 in control group).ConclusionsAlthough total, LDL and VLDL-cholesterol, and triglyceride levels were not different from healthy controls, triglyceride and/or VLDL-cholesterol levels apparently increased with OLTT in both OH and SCH patients. The hypertriglyceridemic waist phenotype, the National Cholesterol Education Program-Adult Treatment Panel III (NCEP-ATP III) criteria, and the International Diabetes Federation (IDF) criteria have been proposed as screening tools to identify subjects with features of the metabolic syndrome and therefore at increased cardiometabolic risk. The aim of the present study was to compare the ability of these 3 clinical approaches to identify individuals at increased cardiometabolic risk as suggested by the presence of deteriorated markers such as hyperinsulinemia, elevated apolipoprotein B levels, small low-density lipoprotein particles, high C-reactive protein concentrations, and low adiponectin levels. For that purpose, physical and cardiometabolic characteristics of a sample of 272 white men recruited for various metabolic investigations were studied. The hypertriglyceridemic waist phenotype was defined as having both a high waist circumference (≥90 cm) and increased fasting triglyceride levels (≥2.0 mmol/L). Having at least 3 of the 5 NCEP-ATP III criteria or waist circumference of at least 94 cm plus any 2 of the 4 additional IDF criteria was also used to identify individuals at increased cardiometabolic risk. A large proportion of men with the hypertriglyceridemic waist phenotype also met the NCEP-ATP III (82.7%) or IDF (89.2%) criteria. Men with the hypertriglyceridemic waist phenotype were characterized by alterations in their lipoprotein-lipid profile that included small low-density lipoprotein particles, increased apolipoprotein B and insulin levels, as well as reduced adiponectin concentrations, which were similar to individuals meeting the NCEP-ATP III or the IDF criteria. Moreover, the Framingham risk score of men meeting any of the 3 screening tools criteria was higher and was similar across the 3 approaches (4.2, 3.8, and 3.7, respectively). These results suggest that hypertriglyceridemic waist may be as discriminant as the NCEP-ATP III or the IDF criteria and could be used as an initial screening approach to identify individuals with deteriorated cardiometabolic risk markers. 相似文献

The present study aimed to evaluate the relationship between the hypertriglyceridemic waist (HTGW) phenotype and hypertension. We undertook a cross‐sectional study with a sample of 9015 adults from China. The HTGW phenotype was defined as elevated waist circumference (WC) and elevated triglyceride (TG) concentration. Logistic regression analysis was used to evaluate the association between the HTGW phenotype and hypertension. The prevalence of hypertension was significantly higher in individuals with the HTGW phenotype, than in those with the normal waist normal triglyceride (NWNT) phenotype (89.9% vs 75.3%, respectively, P < .001). After adjusting for age, sex, BMI, current smoker, and current alcohol consumption, the HTGW phenotype was associated with hypertension (Odds Ratio (OR)1.53; 95% CI 1.25–1.87). After further adjustment for potential confounders, the HTGW phenotype was still significantly associated with hypertension (adjusted OR1.28; 95% CI 1.04‐1.58) regardless of sex. The subgroup analyses generally revealed similar associations across all subgroups. This study indicated that the HTGW phenotype was strongly associated with hypertension, and blood pressure should be clinically monitored in individuals with the HTGW phenotype. We suggested a combined use of hypertriglyceridemia waist phenotype in identifying participants who are at high risk of hypertension. 相似文献

The atherogenic dyslipidemia of the insulin resistance syndrome is characterized by hypertriglyceridemia (hyperTG), elevated apolipoprotein (apo) B levels, reduced high-density lipoprotein (HDL) cholesterol concentrations and by an increased proportion of small, dense low-density lipoprotein (LDL) particles. Although the hyperTG-low HDL cholesterol dyslipidemia has been associated with an impaired clearance of dietary fat, the contribution of the small, dense LDL phenotype as an independent predictor of postprandial triglyceride (TG) clearance remains uncertain. We have therefore compared the postprandial TG response among three subgroups of men characterized by small, intermediate or large LDL particles in a total sample of 69 men (mean age +/- SD; 45.1 +/- 10.5 years). To identify men with small versus large LDL particles, the first (LDL peak particle diameter < 251.9 A) and the third (> 257.6 A) tertiles of the distribution of LDL particle diameters were used as cutoff points. Men with small, dense LDL particles had the expected fasting dyslipidemic profile (high TG-low HDL cholesterol levels) compared to men with large, buoyant LDL particles. The oral lipid tolerance test revealed that men with small, dense LDL particles had significantly higher total-, large-, and medium-TG-rich lipoprotein (TRL) responses to a fatty meal than men with large LDL particles (P < 0.03). In addition, within a subgroup of normolipidemic men (TG < 2.3 mmol/l and HDL cholesterol > 0.9 mmol/l), those with small, dense LDL particles had higher levels of total-, medium- and small-TRL responses compared to men with large, buoyant LDL particles (P < 0.05). Moreover, normotriglyceridemic men with small, dense LDL had higher levels of small-TRLs measured 8 h after the ingestion of the fat meal (P < 0.05) compared to normolipidemic men with large, buoyant LDL particles. Results of the present study suggest that the dense LDL phenotype may be an additional fasting marker of an exaggerated postprandial TG response and of an impaired clearance of TRLs. 相似文献

OBJECTIVES: To determine the prevalence of 'hypertriglyceridemic waist' (HTGW) in Oji-Cree, to examine its interaction with hepatic nuclear factor-1alpha (HNF1A) in association with type 2 diabetes, and to characterize its putative genetic determinants. METHOD: The presence or absence of HTGW was determined in 522 Oji-Cree subjects (223 males, 299 females), >or=18 years of age, in whom physical measurements and fasting plasma analyte concentrations were gathered, and a 75-g oral glucose tolerance test was administered, as part of a cross-sectional study. Subjects were genotyped for HNF1A codon 319, angiotensinogen (AGT) codons 174 and 235, G-protein beta3-subunit (GNB3) nucleotide 825, fatty acid-binding protein (FABP2) codon 54, nucleotides -455 and -482 of the apolipoprotein (apo) C-III (APOC3) promoter, and a 5-bp insertion/deletion polymorphism within the 3'-untranslated region of protein phosphatase 1 regulatory subunit 3 (PPP1R3). RESULTS: The unadjusted prevalence of HTGW in Oji-Cree adults was 20.5%, with more males affected than females (27.8 vs 15.1%, P=0.0004). Logistic regression analysis, adjusted for age and gender, showed type 2 diabetes was associated with both HNF1A G319S (odds ratio (OR) 4.85, 95% CI 2.45, 9.58) and HTGW (OR 4.96, 95% CI 2.49, 9.88). When the HNF1A mutation and HTGW were present in combination, the OR for type 2 diabetes was markedly increased (OR 43.2, 95% CI 12.4, 150). In women only, both GNB3 825C>T and FABP2 A54T genotypes were significantly associated with HTGW (OR 2.02, 95% CI 1.01, 4.05 and OR 1.95, 95% CI 1.01, 3.74, respectively). CONCLUSIONS: HTGW is prevalent in Oji-Cree, especially in men. The ORs for type 2 diabetes were similar ( approximately 5-fold) for subjects with either the presence of HTGW or the private HNF1A G319S mutation. The present study tested whether in patients with type 2 diabetes mellitus (DM) the combination of increased waist circumference and increased plasma triglyceride (TG) levels can predict the presence of coronary artery disease (CAD) as assessed by multidetector computed tomographic coronary angiography (CTA). In 202 patients with type 2 DM who were clinically referred for CTA, waist circumference and TG levels were measured. Patients were divided into 4 groups according to waist circumference measurements and TG levels. Increased waist circumference and TG levels (n = 61, 31%) indicated the presence of the hypertriglyceridemic waist phenotype. Patients with low waist circumference and TG (n = 49, 24%) were considered the reference group. Physical examination and blood measurements were performed. CTA was used to determine presence and severity of CAD. In addition, plaque type was evaluated. Plasma cholesterol levels were significantly increased in the group with increased TG levels and waist circumference, whereas high-density lipoprotein cholesterol was significantly lower than in the reference group. There was a significant increase in the presence of any CAD (odds ratio 3.3, confidence interval 1.31 to 8.13, p <0.05) and obstructive CAD (≥50%, odds ratio 2.9, confidence interval 1.16 to 7.28, p <0.05) in the group with increased TG level and waist circumference. In addition, a significantly larger number of noncalcified and mixed plaques was observed. In conclusion, in patients with type 2 DM, presence of the hypertriglyceridemic waist phenotype translated into a deteriorated blood lipid profile and more extensive CAD on CTA. Accordingly, the hypertriglyceridemic waist phenotype may serve as a practical clinical biomarker to improve risk stratification in patients with type 2 DM. 相似文献

The present study was conducted to investigate whether the fatty liver phenotype could be helpful in the identification of subgroups with distinct metabolic properties and lipid profiles within familial combined hyperlipidemia (FCHL). One hundred eighty-five FCHL family members participated in the current study; 38 subjects were found to be hypertriglyceridemic, of whom 66% showed evidence of fatty liver as measured with ultrasound. A detailed comparison between the hypertriglyceridemic FCHL subjects with (n = 25) and without (n = 13) fatty liver revealed that, despite very similar plasma triglyceride levels (3.5 vs 3.2 mmol/L in subjects with and without fatty liver, respectively), the fatty liver subgroup presented with significantly higher body mass index, visceral adipose tissue (ultrasound), insulin, and alanine aminotransferase levels. Moreover, very low-density lipoprotein (VLDL) subclass analysis showed that the VLDL2 fraction of the fatty liver subgroup contained significantly less cholesterol and triglycerides (P = .02 for both parameters), which was likely explained by a decreased VLDL2 particle number because VLDL2 apolipoprotein B levels tended to be lower (P = .08). These data indicate that hypertriglyceridemic FCHL subjects may belong to metabolically distinct subgroups and suggest that a refinement of the hypertriglyceridemic FCHL phenotype by adding information on fatty liver will eventually facilitate the elucidation of its complex genetic background. 相似文献
