目的研究将脐动脉血气分析用于新生儿窒息预测的价值。方法本实验共选取150例新生儿为研究对象,根据新生儿窘迫情况分成两组,Ⅰ组(n=45胎儿宫内窘迫)和Ⅱ组(n=105胎儿无宫内窘迫);根据Apgar评分的三个梯度≥8分、3~7分和≤3分即分成A组(n=110正常)、B组(n=33轻度窒息)和C组(n=7重度窒息)。所有患儿均接受脐动脉血抽取操作,通常抽取的剂量以0.1mL最适宜用于测定患儿的血气分析,除此之外所有新生儿还接受1min Apagar评分,对三组患儿进行不同指标比较。结果Ⅰ组pH值指标高于Ⅱ组(P<0.05);PCO_2随着1min Apgar评分减少不断升高,两者呈负相关系;PO2、pH值、HCO_3~-及BE指标数据均随着1min Apgar评分的减少而减少,呈正相关系(P<0.05);相比于p H值≥7.20新生儿缺氧缺血脑病发生率,p H值<7.20发病率更高(P<0.05);相比于评分≤7新生儿缺氧缺血脑病发生率,Apgar评分≥8分发病率更高(P<0.05)。结论将脐动脉血气分析应用于新生儿窒息的临床预测当中具有重要作用,通过不同脐动脉血气指标数据变化情况及Apgar评分数据能够用于病症诊断参考,尽早确定患儿缺氧缺血情况,对于制定科学有效治疗方案、提高新生儿生存质量均具有重要作用。  相似文献

目的 研究超声联合胎心无负荷试验对胎儿窘迫的诊断价值.方法 选取接受胎心监护84例孕妇作为研究对象,回顾性分析其临床诊治资料,对比胎儿宫内窘迫者无负荷试验、S/D与Apgar评分.结果 21例NST检查结果表明是胎儿宫内窘迫,11例孕妇NST检查表明无反应型,并且脐血流S/D≥3,4例产妇胎儿大脑中动脉多勒普诊断显示血流S/D≤4,4例新生儿具体Apgar评分≤7分;NST有反应型者与无反应型者、脐动脉血流S/D≥3者与S/D<3者、大脑中动脉血流S/D>4与S/D≤4者Apgar评分比较差异具有显著性(P<0.05).结论 超声联合胎心无负荷试验能够提高新生儿窒息临床诊断正确率,减小剖宫产率或者假阳性率,具有积极应用意义.  相似文献

目的 探讨脐血血气分析、胎心监护及Apgar评分的相关性,为预测和诊断新生儿窒息及减低新生儿窒息发生提供重要依据.方法 对我院205例新生儿出生后立即采集脐动脉血进行血气分析,结合产前胎心监护及Apgar评分进行统计分析.结果 1.Apgar评分≤7分的新生儿与评分≥8分新生儿在血气分析结果中pH值、PaCO2、PaO2、BE值及HCO3-均有显著性差异,(<0.05~0.01).2.随着pH值下降,Apgar低评分增多,产前胎心电子监护异常发生率也随之增加,新生儿窒息发生率增高.结论 联合监测脐血血气分析、胎心监护及Apgar评分可提高新生儿窒息诊断的正确性,降低围产儿患病率和死亡率.  相似文献

目的分析脐动脉血气分析联合呼吸评分在评估新生儿缺血缺氧性脑病(HIE)患儿病情严重程度中的临床价值。方法选取本院儿科2016年1月~2018年12月住院治疗的103例HIE患儿作为研究对象,根据临床分度相关标准将上述患儿分为轻度组(34例)、中度组(37例)和重度组(32例),同期选取30例正常新生儿作为对照组。检测各组新生儿脐动脉血p H值、二氧化碳分压(PCO_2)、氧分压(PO_2)、碱剩余(BE)、碳酸氢根(HCO3-)等血气分析指标,采用新生儿呼吸评分法对各组新生儿呼吸状态予以评估。比较HIE患儿及正常新生儿脐动脉血气分析相关指标及呼吸评分,比较不同病情严重程度HIE患儿脐动脉血气分析相关指标及呼吸评分。结果 HIE患儿pH值、PO_2、HCO_3~-均明显低于正常新生儿(P 0.05),而PCO_2、BE、呼吸评分均明显高于正常新生儿(P 0.05);不同病情严重程度HIE患儿脐动脉血气分析相关指标及呼吸评分比较均差异有统计学意义(P 0.05),且随着病情加重pH值、PO_2、HCO_3~-等指标明显降低(P 0.05),而PCO_2、BE、呼吸评分明显升高(P 0.05)。结论脐动脉血气分析联合呼吸评分在新生儿HIE患儿病情严重程度评估中具有较高的临床价值。  相似文献

目的通过脐动脉血流测定联合胎心监护对足月妊娠胎儿窘迫进行预测,以寻求更为准确可靠的诊断方法。方法将研究对象分为三组,联合组即脐动脉血流测定S/D值≥2.5且NST无反应型组;脐血流组即单纯行脐动脉血流测定S/D值≥2.5组和NST组即单纯行胎心监护NST无反应型组;分别了解分娩结局:记录羊水污染,新生儿Apgar评分等指标。结果联合组的羊水污染率,胎儿窘迫率,新生儿窒息率明显高于脐血流组以及NST组。结论胎儿脐血流监护与胎心监护联合应用,可及早发现胎儿窘迫,及时采取适当的干预措施,改善胎儿的预后,减少围产儿的病死率。  相似文献

目的探讨产时使用连续性胎心监护联合脐动脉血气分析在评估围生期窒息的临床应用。方法随机选取2011年1月至2012年12月在我院住院并进入产房待产的孕产妇140例为研究对象,分为两组:胎心监护正常的58例为对照组,胎心监护异常的82例为观察组,比较两组孕产妇产时脐动脉血气分析及分析在评价围生期窒息中胎心监护、脐动脉血气分析、Apgar评分三者之间的关系。结果①观察组的脐动脉血气分析的PH值明显低于对照组(P〈0.05)。②随着PH值的下降,Apgar低评分增多,新生儿窒息发生率增高,而且产时胎心监测异常发生率也增高。结论胎心监护对预测胎儿窘迫有重大意义,联合脐动脉血气分析能为产前验证,准确评估、诊治围生期窒息及其预后提供有力依据。  相似文献

目的 通过新生儿脐动静脉血血气分析,探讨其对新生儿窒息的预警价值.方法 对95例足月新生儿出生即时脐动静脉血血气分析与Apgar评分、羊水性状进行比较.结果 新生儿Apgar评分越低,脐动脉血pH值、PO2值也越低,PCO2值越高,且酸中毒发生率较临床诊断宫内窘迫、窒息率愈高.羊水粪染程度较高者比羊水清者窒息发生率高,两者差异有统计学意义(P<0.05).结论 脐血血气分析可比Apgar评分更客观、更灵敏地反映胎儿缺氧程度.且脐血血气分析可先于羊水粪染改变,作为预警新生儿窒息具有一定的临床应用价值.  相似文献

目的及早、正确地诊断胎儿宫内窘迫,预测胎儿预后。方法 415例发生胎儿宫内窘迫患者随机分为监护组240例和对照组175例。监护组给予胎心监护,对照组常规处理。观察2组新生儿Apgar评分、正常产率、侧切胎吸及产钳助产率等。结果监护组新生儿Apgar评分0~3分及4~7分者所占比例均低于对照组,差异有统计学意义(P〈0.01);8~10分者所占比例为89.2%高于对照组的66.9%,差异有统计学意义(P〈0.01)。监护组正常产率为51.6%高于对照组的41.1%,差异有统计学意义(P〈0.05);侧切胎吸及产钳助产率明显低于对照组(P〈0.05)。结论应早期重视可导致胎儿宫内窘迫的高危因素,正确应用胎心监护,采取合适的分娩方式适时终止妊娠。  相似文献

目的探讨胎儿窘迫与新生儿窒息的关系。方法对2053例产妇中因胎心率异常、羊水污染及胎儿电子监护诊断的胎儿窘迫及出生后Apgar评分进行分析。结果(1)胎心率与羊水均异常者新生儿窒息率可达40.32%;重度窒息率达11.29%;(2)胎心率减慢程度与新生儿窒息率密切相关,胎心率低于60次/min,新生儿重度窒息率达61.54%;(3)胎儿窘迫持续时间与新生儿窒息率呈正相关,胎儿窘迫超过60min,新生儿窒息率可达88.33%,与胎儿窘迫少于30min者相比,差异非常显著(P<0.01)。结论胎儿窘迫是新生儿窒息的先兆,通过对胎心率及羊水性状综合分析,可以对新生儿窒息发生的可能性做出预先判断,以便提早防范。  相似文献

Some 2-alkyldithio, 2-aralkyldithio, 2-aryldithio benzoic acids, their methyl esters and N-monosubstituted amides were prepared and tested in vitro against Candida albicans and Trichophyton mentagrophytes. Some N-monosubstituted amides displayed activities similar to those of clotrimazole and pyrrolnitrin. 2-Aminoanthraquinone (AA), l-amino-2-methylanthraquinone (l-Am-2-MeA), and 2-methyl-l-nitroanthraquinone (2-Me-1-Na) were tested for carcinogenicity in Fischer 344 rats and B6C3F1 mice. In rats, all the 3 chemicals increased the incidence of hepatocellular neoplasms. Renal neoplasms were increased with 1-Am-2-MeA, and 2-Me-1-NA increased the incidence of subcutaneous fibromas in a dose-related manner. In mice, incidences of hepatocellular carcinomas were increased with feeding of AA, and 2-Me-l-NA increased the incidence of subcutaneous hemangiosarcomas. 1-Am-2-MeA did not appear to be carcinogenic in the mouse. Nephrotoxicity was associated with feeding of AA to female rats and 1-Am-2-MeA to mice.  相似文献

The stabilities of tritium-labeled T-2, HT-2, and T-2 tetraol were studied in blood and urine at -70 degrees, 4 degrees, and 23 degrees C for 6 months in the presence of EDTA or NaF. Samples were counted with a radiochromatographic scanner and results indicated the stability of T-2 tetraol greater than T-2 greater than HT-2. Toxins were most stable when stored at -70 degrees C, in the presence of NaF, and in urine (pH 6). They were less stable in saline (control, pH 7) and least stable in blood (pH 8). These results suggest that urine and T-2 tetraol are the biological fluid and metabolite of choice for diagnostic purposes.  相似文献

Lithium confers cell protection against stress and toxic stimuli. Although lithium inhibits a number of enzymes, the antiapoptotic mechanisms of lithium remain unresolved. Here, we report a novel role of lithium on the blockage of ceramide- and etoposide-induced apoptosis via inhibition of protein phosphatase 2A (PP2A) activity. Overexpression of PP2A resulted in caspase-2 activation, mitochondrial damage, and cell apoptosis that were inhibited by okadaic acid (OA) and lithium. Lithium and OA abrogated ceramide- and etoposide-induced Bcl-2 dephosphorylation at serine 70. Furthermore, ceramide- and etoposide-induced PP2A activation involved methylation of PP2A C subunit, which lithium suppressed. Lithium caused dissociation of PP2A B subunit from the PP2A core enzyme, whereas ceramide caused recruitment of the B subunit. Taken together, lithium exhibited an antiapoptotic effect by inhibiting Bcl-2 dephosphorylation and caspase-2 activation, which involved, at least in part, a mechanism of down-regulating PP2A methylation and PP2A activity.  相似文献

A set of four types of benzazoles, 1, 2, 4-triazole, and pyridine-2-carbonitrile/-2-carbothioamide substituted with 1-naphthylmethylsulfanyl or pyridylmethylsulfanyl was prepared to modify the structure of benzylsulfanyl derivatives of the above-mentioned heterocycles. The compounds were evaluated for in vitro antimycobacterial activity against Mycobacterium tuberculosis, M. avium, and two strains of M. kansasii. The activities were expressed as the minimum inhibitory concentration (MIC). The MIC values fall into a range of 2 to >1000 micromol/L. Introduction of a pyridyl moiety into the molecule mostly decreased the activity. A naphthyl moiety did not influence the activity in comparison with a phenyl. The most active substances were 4-(3-pyridylmethylsulfanyl)pyridine-2-carbothioamide (7b) (MIC = 2 - >62.5 micromol/L) and 4-(1-naphthylmethylsulfanyl)pyridine-2-carbothioamide (7d) (MIC = 2 - >32 micromol/L).  相似文献
