A guide to primary care of iron-deficiency anemia.

Iron deficiency, the most common cause of anemia, is prevalent in 10 percent to 30 percent of the world's population. Inadequate intake of iron may be an important causative factor, particularly when the body requires more iron than usual (e.g., during infancy, early childhood, adolescence, pregnancy and periods of blood loss). The popular increase of fiber in diets may increase the incidence of iron-deficiency anemia because too much fiber in the diet renders available iron unabsorbable. Symptoms in children include skin or conjunctival pallor, excessive sleepiness, learning disabilities, diminished attention span, tiredness, irritability or inappropriate behavior, and pica. Adults may have shortness of breath, decrease in exercise tolerance, palpitations, tachycardia, angina, congestive heart failure, orthopnea and edema. Iron deficiency occurs in sequential states and is measured by many laboratory tests. The levels of hemoglobin and hematocrit are both decreased, while the red blood cell count may be normal initially, but will decrease as the iron-deficiency state continues. The steps of treatment include correction of the underlying disorder, administration of the amount of iron needed and observation of the response to treatment.     

Clinico-immunological parallels in patients with iron-deficiency anemia

The main parameters of humoral and cellular immunity as well as indices of nonspecific body resistance were studied in 70 patients with iron deficiency anemia. A decrease in the content of the main classes of immunoglobulins and an increase in the CIC level were noted. Routine therapy brought about a tendency toward the restoration of indices of humoral immunity. With respect to cellular immunity, a relative increase in the level of B-lymphocytes and a decrease in the number of 0-lymphocytes were observed. A relative and absolute decrease in the level of T-helpers was revealed. Certain correlation between some clinical and immunological indices was observed.     

Clinical and functional cardiac characteristics in iron-deficiency anemia

AIM: To characterize clinically and electrocardiographically the condition of the heart in chronic iron deficient anemia (IDA), to study hemodynamic mechanisms of heart impairment for early detection of diastolic myocardial dysfunction in long-term course of sideropenia. MATERIAL AND METHODS: Clinical and ECG examinations were made in 400 IDA patients. Echocardiography and dopplerechocardiography were made in 95 and 27 patients, respectively. Patients with non-anemic myocardiodystrophy were withdrawn. Mean age of the patients was 35.4 +/- 3.8 years, mean Hb level--79.5 +/- 6.5 g/l, mean anemia duration 6.9 +/- 4.2 years. RESULTS: The severity of cardiac symptoms directly correlated with severity and duration of anemic sideropenic syndrome. 100% had heart complaints, 91.3% had mild chronic cardiac failure, 86.2% patients showed changes on the electrocardiogram. Diastolic insufficiency of both right and left ventricular myocardium was registered in almost all the examinees. CONCLUSION: Long-term anemia due to iron deficiency promotes metabolic and dystrophic alterations in the myocardium manifesting with diastolic dysfunction of both the ventricles prior to systolic failure. The study of cardiac diastolic function in IDA allows detection of early symptoms of cardiac failure which requires pharmacological correction.     

The characteristics of iron metabolism under iron-deficiency anemia and chronic disorders anemia

The study investigated the issues of iron metabolism under iron-deficiency anemia and chronic disorders anemia and dependencies of production of IL-1? and sICAM-1 immunoinflammatory markers from degree of severity and duration of anemia. The study data indicates that under iron-deficiency anemia lactoferrin and sICAM-1 are the negative regulators of hemopoiesis. The inhibition of transferrin expression by the proinflammatory cytokines is one of the causes of inefficient hemopoiesis under chronic disorders anemia.
