This study describes breastfeeding practices with children born in a university hospital in the city of S o Paulo, Brazil, and identifies factors associated with duration of breastfeeding and exclusive breastfeeding. A cohort of 506 children was identified; of these, it was possible to analyze information on feeding practices for 450 infants at least until the second month of life. Daily information on infant feeding was recorded by mothers in a food frequency questionnaire. Survival analysis techniques (Kaplan-Meier and Cox) were used. Median duration of breastfeeding and exclusive breastfeeding were 205 and 23 days, respectively. The principal factors related to exclusive breastfeeding were mother's age (hr young/old = 1.22; 95% CI = 1.006-1.486) and mother's schooling (hr primary/university = 2.13; 95% CI = 1.381-3.307 and hr secondary/university = 1.78; 95% CI = 1.145-2.792), which could be related to knowledge concerning the benefits of exclusive breastfeeding.  相似文献

OBJECTIVE: Few published data are available on the feeding practice of younger children in Tibet. Our study investigated feeding pattern of younger Tibetan children. DESIGN AND SUBJECTS: The study was a cross-sectional survey conducted in Tibet. A sample of 1655 children younger than 36 months old and their mothers was obtained by stratified multistage cluster random sampling method. Mothers were interviewed to collect information on feeding practice by a designed family questionnaire including initial time and duration of breastfeeding and time, frequency and types of complementary foods introduced. RESULTS: Most of the children was ever or being breastfed at the time of interview. The median duration of breastfeeding was 26 months. The prevalence of almost exclusive breastfeeding for 4 months was 20.1%. Mothers' age and education, child's residence, and introduction of egg, fresh milk and milk powder were strongly associated with the duration of breastfeeding. The partial feeding was a main pattern in the first 6 months of life. Zanban and porridge were offered at about 1 month after birth. By 6th month, less than 25% of children were fed protein foods such as egg and meats and only near 20% of children were fed fresh vegetables. Bean products, fish and chicken were seldom offered to children. CONCLUSIONS: We conclude that although they were breastfed for long time, complementary feeding practice of Tibetan children was disadvantaged, especially of rural children. Improvement of monotonous complementary foods and timely introduction should be stressed in local programmatic nutritional interventions.  相似文献

This study aims to evaluate the situation with breastfeeding in the State of Piauí, Brazil, at the time of the 2006 vaccination campaign. The sample consisted of 1,963 infants (< 1 year of age) from 45 municipalities. Medians and probabilities of breastfeeding categories were calculated by probit analysis. Multivariate logistic analysis was used to analyze the association between independent variables and partial and exclusive breastfeeding. The probabilities were: 16% for exclusive breastfeeding and 18% for predominant breastfeeding at 180 days of age and 58% for breastfeeding at 361 days. Median duration was 67 days for exclusive breastfeeding and 200 days for breastfeeding. The breastfeeding rate was higher for children whose mothers belonged to social classes C, D, and E (p = 0.024) and lived in the countryside (p = 0.009). Not nursing during the first 24 hours of life, using a pacifier, and bottle-feeding reduced the prevalence of breastfeeding (p < 0.001). The prevalence rates are below the recommended standards and reflect the need to prioritize actions to promote breastfeeding in the State.  相似文献

Breastmilk contains many important nutrients, anti-inflammatory agents, and immunomodulators. It is the preferred nutrition source for infants. However, the association of the duration of exclusive breastmilk feeding (BMF) with asthma development is unclear. Data on children from the United States who participated in the National Health and Nutrition Examination Survey (NHANES) from 1999 to 2014 were obtained. We examined the association between the duration of exclusive BMF and asthma in 6000 children (3 to 6 years old). After calculating the duration of exclusive breastfeeding according to answers to NHANES questionnaires, the estimated duration of exclusive BMF was divided into five categories: never breastfed or BMF for 0 to 2 months after birth; BMF for 2 to 4 months after birth; BMF for 4 to 6 months after birth; and BMF for ≥6 months after birth. The overall prevalence of asthma in children aged 3 to 6 years was approximately 13.9%. The risk of asthma was lower in children with an exclusive BMF duration of 4 to 6 months (aOR, 0.69; 95% CI, 0.48–0.98), after adjustment for potentially confounding factors. Subgroup analysis revealed that children of younger ages (3 to 4 years old) benefited most from the protective effects of exclusive BMF for 4 to 6 months (aOR, 0.47; 95% CI, 0.27, 0.8). We found that exclusive BMF, especially BMF for 4 to 6 months, is associated with a decreased risk of asthma in preschool-age children. The protective effect appeared to be diminished in older children. The potential mechanism needs further investigation.  相似文献

目的 了解婴儿期母乳喂养持续时间和纯母乳喂养对学龄前期儿童蔬菜和水果摄入的影响,为婴幼儿喂养指导提供更为有力的科学依据。方法 使用已经进行的儿童肥胖研究的基线调查数据进行分析,数据为通过问卷调查的方式了解儿童婴儿期喂养基本情况和儿童膳食现状,使用Logistic回归模型对可能关联进行验证。结果 在1 189名调查儿童婴儿期,纯母乳喂养持续超过6个月的占17.1%,母乳喂养持续时间超过12个月的占9.6%。纯母乳喂养持续6个月以上的儿童在学龄前期超重肥胖发生率明显低于纯母乳喂养持续时间<6个月的儿童(2.2% &3.7%,P<0.05)。学龄前儿童摄入每周≥6次的水果和蔬菜的频率比例分别为87.5%及67.7%。婴儿期持续母乳喂养12个月及以上儿童在学龄前期摄入蔬菜频率高于母乳喂养持续时间<12个月的儿童(OR=1.251; 95%CI1.115~2.563),纯母乳喂养≥6个月的儿童较纯母乳喂养时间<6个月的儿童在学龄前期摄入蔬菜的频率高,两者存在统计学关联(OR=1.015;95%CI1.002~1.105)。母乳喂养模式与学龄前期儿童摄入水果频率差异无统计学意义(OR=0.882;95%CI0.412~1.514) &(OR=0.657;95%CI0.442~1.019)。结论 长期母乳喂养及纯母乳喂养与学龄前期儿童蔬菜摄入量存在正向关联,但与学龄前期儿童水果摄入无任何关联。生命早期科学喂养方式对于儿童生命后期营养习惯的形成乃至对营养发育的影响都十分重要的,因而社会和家庭对于母乳喂养的认识和支持也需进一步深入和提高。  相似文献

Introduction: The admission of neonates to Neonatal Intensive Care Units (NICUs) has been identified as a primary inhibiting factor in the establishment of breastfeeding. The aims of this study were to (1) estimate the prevalence and duration of breastfeeding in infants/toddlers who had been admitted to an NICU in Greece and (2) to investigate factors, associated with the NICU stay, which affected the establishment and maintenance of breastfeeding in infants/toddlers previously admitted to the NICU. Materials and methods: Data for this cohort study were retrieved from interviews with mothers of infants/toddlers who had been admitted to our NICU as neonates during the period of 2017–2019. Interviews were conducted based on a questionnaire regarding the child’s nutrition from birth to the day of the interview, including previous maternal experience with breastfeeding. Information related to the prenatal period, gestation age, delivery mode, duration of NICU stay, and neonatal feeding strategies during their hospital stay were recorded. Results: The response rate to the telephone interviews was 57%, resulting in 279 mother–infant pairs being included in this study. The results showed that 78.1% of children received maternal milk during their first days of life. Of all infants, 58.1% were exclusively breastfed during their first month, with a gradual decrease to 36.9% and 19.4% by the end of the third and sixth months of life, respectively. The prevalence of breastfed children reached 14.7% and 7.5% at the ages of twelve and eighteen months, respectively. In the multivariate analysis, prematurity emerged as an independent prognostic factor for the duration of exclusive and any breastfeeding (aHR 1.64, 95% CI: 1.03–2.62; and 1.69, 95% CI: 1.05–2.72, respectively; p < 0.05). Additionally, the nationality of the mother, NICU breastfeeding experience, the administration of maternal milk during neonatal hospital stay, and previous breastfeeding experience of the mother were independent prognostic factors for the duration of breastfeeding. Conclusions: Although breastfeeding is a top priority in our NICU, the exclusive-breastfeeding rates at 6 months were quite low for the hospitalized neonates, not reaching World Health Organization (WHO) recommendations. Mothers/families of hospitalized neonates should receive integrated psychological and practical breastfeeding support and guidance.  相似文献

BACKGROUND: According to Canadian public health recommendations, newborns should be fed exclusively with breast milk during their first 4 to 6 months of life. The aim of this paper is to identify the main social determinants of initiation, duration and exclusivity of breastfeeding from birth to 4 months, at the population level. The results will help the development of public health interventions aimed at improving the prevalence of exclusive breastfeeding for at least the first 4 to 6 months of babies' life. METHODS: The analyses were performed with the data of the Longitudinal Study of Child Development in Quebec (ELDEQ 1998-2002). The study follows a representative sample (n = 2,223) of the children born in 1998 in the province of Quebec, in Canada. The relations between breastfeeding (all and exclusive) and mothers' education level, annual family income, family type, parents' working situation and mothers' age group have been evaluated by crude and adjusted odds ratios. Multivariate analyses were used to identify the most influential factors. RESULTS: In 1998, nearly three quarters (72%) of the Quebec newborns were breastfed at birth. When they reached 4 months, only 6% of the Quebec children were exclusively breastfed, thus following public health recommendations. Mothers' education level remains the strongest factor of influence on breastfeeding from birth to 3 months, and its impact increases with baby's age. At 4 months, it is also the most important factor for all breastfeeding, but mothers' age is more important for exclusive breastfeeding. When all the studied factors are considered equal for all children, being breastfed in accordance with public health recommendations, which refers to exclusive breasfeeding for at least 4 months, is mainly influenced by mothers' age, followed by mothers' education level. Family income, family type and parents' working situation do not influence exclusive breastfeeding at 4 months. It should be noted that when considering all breastfed children, if mothers' age and education level are equal, annual family income shows a negative relationship with breastfeeding at any of the studied ages. This negative relationship is also observed at 2 months for exclusive breastfeeding (dollar 40,000-dollar 59,999). CONCLUSION: From a public health perspective, it is important that newborns be breastfed at birth and exclusively breastfed during their first 4 to 6 months of life. The analysis indicates that once the decision to breastfeed is made, different social factors influence the duration of breastfeeding and its exclusivity. Public policy targets are necessary to evaluate whether the situation is improving from one year to another and whether disparities are increasing or decreasing for newborns accumulating the impact of material and social deprivation early in life.  相似文献

This paper describes a rapid assessment of current infant feeding practices conducted during a mass immunization campaign in an urban area of S?o José do Rio Preto, S?o Paulo State, Brazil. Parents or guardians of all infants (<1 year) attending the 8 randomly-selected health centers were interviewed. Median duration of breastfeeding practices was estimated by probit analysis. Of the 719 infants, 692 (96.2%) began breastfeeding, and only 63 (8.9%) were in exclusive breastfeeding. Median duration of total breastfeeding (exclusive plus partial) was 205.93 days. Infants born by cesarean section showed a higher risk of early weaning: OR (95%CI)=1.51 (1.09-2.09) after adjusting for confounding variables (mother's educational level, gestational age, birth weight, whether breastfed). Median duration of total breastfeeding was higher among infants born by vaginal delivery (227.38 days; 95%CI=195.19-271.16) as compared to cesarean section (192.84 days; 95%CI=128.23-273.34). Early introduction of weaning foods such as water/herbal tea and cow's milk was observed with a median age at introduction of 30 days. These findings indicate the need to improve strategies by the public health services to promote exclusive breastfeeding.  相似文献

目的 描述2002-2015年浙江省舟山市母乳喂养情况,探索可能的影响因素。方法 提取2002-2015年舟山市妇幼保健院电子医疗记录,应用Logistic回归分析各因素对母乳喂养的影响。结果 以2011年为节点,舟山市产妇的母乳喂养率呈现先下降后上升的趋势。调整相关变量后,市区的产妇第1、3、9个月的母乳喂养率更高。较高学历的产妇第1、3个月母乳喂养率较高,但第9个月则相对较低。职业类型为脑力型的产妇第6、9、12个月母乳喂养率较高。巨大儿在第6、9个月母乳喂养率较高。剖宫产、早产、低体重儿、初产妇、产妇年龄大于30岁或小于25岁均为母乳喂养的危险因素。冬春季出生的婴儿其母乳喂养率在第1、3、6个月均较低。结论 来自市区、职业类型为脑力型的产妇、巨大儿为母乳喂养的保护因素。较高的学历在第1、3个月为母乳喂养的保护因素,但在第9个月为危险因素。剖宫产、早产、低体重出生儿、初产妇、产妇年龄大于30岁或小于25岁、冬春季出生的婴儿均为母乳喂养的危险因素。  相似文献

OBJECTIVE: To investigate whether the duration of exclusive breastfeeding and maternal body mass index (BMI) are associated with children's developmental status at the time of beginning elementary school. DESIGN AND SUBJECTS: The subjects in the sample came from a longitudinal study on infant nutrition in Iceland. Food records were made once a month from birth to the age of 12 months, from which duration of exclusive breastfeeding could be determined. Mothers filled in The Icelandic developmental inventory for evaluation of motor and verbal development close to their children's sixth birthday (n = 85). Maternal self-reported weight and height was recorded. Information on socioeconomic factors was gathered by a questionnaire. RESULTS: Duration of exclusive breastfeeding, in months, was positively related to children's motor component (B = 0.5+/-0.5, P = 0.054) and to the total developmental index (B = 1.0+/-0.5, P = 0.044) at 6 y, adjusting for gender and socioeconomic factors (maternal and paternal education, and family income). Children's learning score was negatively related to maternal BMI (B = -0.5+/-0.2, P=0.047). An inverse association also appeared between maternal BMI and two out of the three developmental composite scores, that is, verbal component and the total developmental index (B = -0.6+/-0.3, P=0.049) and (B = -0.4+/-0.2, P=0.057), respectively. In multiple regression the developmental indexes were most strongly independently associated with maternal BMI (negatively) and infants' birth weight (positively). CONCLUSION: Maternal BMI and duration of breastfeeding were associated with verbal and motor development of 6-y-old children, independent of socioeconomic factors. Birth weight was also an independent determinant for developmental scores.  相似文献

OBJECTIVE: To estimate the prevalence and duration of breastfeeding, and factors associated with them, in Asturias (Spain). METHODS: A cross-sectional study was carried out. A stratified random sample of 453 infants was drawn on a population of 4,326 infants aged 0-8 months at the time of the study. Sociodemographic data, and information on pre- and perinatal care were obtained through a telephone survey in which mothers were also asked whether they were breastfeeding on the day of the telephone call. A univariate logistic regression model was used to estimate the prevalence curve of breastfeeding according to infant age, and multiple logistic regression was used to determine the associations between the prevalence and covariates. The discontinuity index (DI) and the cumulative breastfeeding index (CBI) were also estimated as indicators of duration. RESULTS: A total of 418 telephone calls were made. The prevalence of initial exclusive breastfeeding was 51.4% (95% CI: 46.6-56.2%) which decreased to 14.9% at day 90 and to 8.7% at day 120. The DIs were 28.6% at day 30 and 71% at day 90. In the multivariate analysis the prevalence of exclusive breastfeeding was higher among mothers living in a rural or suburban area (OR = 5.69 and OR = 3.55, respectively), than among those living in urban areas. The prevalence was also higher among mothers with a university degree (OR = 3.90; 95% CI: 1.77-8.58), and among those who had been seen by a midwife during pregnancy (OR = 2.13; 95% CI: 1.04-4.38). A negative correlation was found between exclusive breastfeeding and the newborn having received food supplements during the first hours after birth (OR = 0.43; 95% CI: 0.20-0.95). CONCLUSIONS: The prevalence of breastfeeding in Asturias, and especially its duration, is much lower than that recommended by the World Health Organization. Mothers living in urban areas do not follow the most appropriate patterns of breastfeeding.  相似文献

Recent data from the World Health Organization showed that about 60% of all deaths, occurring among children aged less than five years (under-five children) in developing countries, could be attributed to malnutrition. It has been estimated that nearly 50.6 million under-five children are malnourished, and almost 90% of these children are from developing countries. Bangladesh is one of the countries with the highest rate of malnutrition. The recent baseline survey by the National Nutrition Programme (NNP) showed high rates of stunting, underweight, and wasting. However, data from the nutrition surveillance at the ICDDR,B hospital showed that the proportion of children with stunting, underweight, and wasting has actually reduced during 1984-2005. Inappropriate infant and young child-feeding practices (breastfeeding and complementary feeding) have been identified as a major cause of malnutrition. In Bangladesh, although the median duration of breastfeeding is about 30 months, the rate of exclusive breastfeeding until the first six months of life is low, and practice of appropriate complementary feeding is not satisfactory. Different surveys done by the Bangladesh Demographic and Health Survey, United Nations Children's Fund (UNICEF), and Bangladesh Breastfeeding Foundation (BBF) showed a rate of exclusive breastfeeding to be around 32-52%, which have actually remained same or declined over time. The NNP baseline survey using a strict definition of exclusive breastfeeding showed a rate of exclusive breastfeeding (12.8%) until six months of age. Another study from the Abhoynagar field site of ICDDR,B reported the prevalence of exclusive breastfeeding to be 15% only. Considerable efforts have been made to improve the rates of exclusive breastfeeding. Nationally, initiation of breastfeeding within one hour of birth, feeding colostrum, and exclusive breastfeeding have been promoted through the Baby-Friendly Hospital Initiative (BFHI) implemented and supported by BBF and UNICEF respectively. Since most (87-91%) deliveries take place in home, the BFHI has a limited impact on the breastfeeding practices. Results of a few studies done at ICDDR,B and elsewhere in developing countries showed that the breastfeeding peer-counselling method could substantially increase the rates of exclusive breastfeeding. Results of a study in urban Dhaka showed that the rate of exclusive breastfeeding was 70% among mothers who were counselled compared to only 6% who were not counselled. Results of another study in rural Bangladesh showed that peer-counselling given either individually or in a group improved the rate of exclusive breastfeeding from 89% to 81% compared to those mothers who received regular health messages only. This implies that scaling up peer-counselling methods and incorporation of breastfeeding counselling in the existing maternal and child heath programme is needed to achieve the Millennium Development Goal of improving child survival. The recent data showed that the prevalence of starting complementary food among infants aged 6-9 months had increased substantially with 76% in the current dataset. However, the adequacy, frequency, and energy density of the complementary food are in question. Remarkable advances have been made in the hospital management of severely-malnourished children. The protocolized management of severe protein-energy malnutrition at the Dhaka hospital of ICDDR,B has reduced the rate of hospital mortality by 50%. A recent study at ICDDR,B has also documented that home-based management of severe protein-energy malnutrition without follow-up was comparable with a hospital-based protocolized management. Although the community nutrition centres of the NNP have been providing food supplementation and performing growth monitoring of children with protein-energy malnutrition, the referral system and management of complicated severely-malnourished children are still not in place.  相似文献

The National Breastfeeding Survey 2001 was the first comprehensive study on breastfeeding conducted on a national level in Singapore. It aimed to establish the prevalence of breastfeeding among Chinese, Malay and Indian mothers and to identify factors influencing breastfeeding. A total of 2098 mothers were interviewed in this two-phase study, with the first interview conducted 2 months after delivery and the second interview 6 months after birth among mothers who were still breastfeeding at 2 months. Frequency distributions of breastfeeding prevalence and types of breastfeeding practices at different time intervals (from birth to 6 months) were produced. Multivariate logistic regression was carried out to construct a model with predictive information on factors which influence continued breastfeeding till 2 months and 6 months after delivery respectively. The study found that about 94.5% of the mothers attempted breastfeeding. At 1 month, 71.6% were still breastfeeding, 49.6% continued to do so at 2 months, and 29.8% persisted till 4 months. By 6 months, the breastfeeding prevalence rate fell to 21.1%. The results of this study show higher breastfeeding prevalence rates compared to past studies in Singapore. Despite this, exclusive breastfeeding is still not a common practice. Various factors were found to be significant in influencing mothers' decision to breastfeed. Factors such as ethnicity, age, educational attainment, religion and baby's sex are non-modifiable in the short term or at an individual level. However, factors such as awareness of breastfeeding benefits, advice from health professionals and previous breastfeeding experience are potentially modifiable. Efforts aimed at promoting breastfeeding in Singapore need to take these modifiable factors into consideration so as to better tailor health promotion efforts on breastfeeding to women.  相似文献

Socio-demographic characteristics of mothers have been associated with exclusive breastfeeding duration, but little is known about the association with maternal full- and part-time employment and return to work in European countries. To study the associations between breastfeeding, any and almost exclusive (infants receiving breast milk as their only milk) breastfeeding, at 4 months of infant’s age and the socio-demographic and occupational characteristics of mothers. We used the EDEN mother–child cohort, a prospective study of 2002 singleton pregnant women in two French university hospitals. We selected all mothers (n = 1,339) who were breastfeeding at discharge from the maternity unit. Data on feeding practices were collected at the maternity unit and by postal questionnaires at 4, 8 and 12 months after the birth. Among infants breastfed at discharge, 93 % were still receiving any breastfeeding (83 % almost exclusive breastfeeding) at the 3rd completed week of life, 78 % (63 %) at the 1st completed month, and 42 % (20 %) at the 4th completed month. Time of return to work was a major predictor for stopping breastfeeding: the sooner the mothers returned to work, the less they breastfed their babies at 4 months of infant’s age, independently of full-time or part-time employment. The association was stronger for almost exclusive breastfeeding mothers than for any breastfeeding ones. In a society where breastfeeding is not the norm, women may have difficulties combining work and breastfeeding. Specific actions need to be developed and assessed among mothers who return to work and among employers.  相似文献

In Malaysia, the National Breastfeeding Policy recommends exclusive breastfeeding for the first six months of life and continued up to two years. Since the 1990s, several breastfeeding promotion programmes had been implemented in the country. This article reports the findings on the prevalence of breastfeeding practice from The Third National Health and Morbidity Survey (NHMS III) which was conducted in 2006. A total of 2167 mothers or carers of children below two years old were interviewed representing 804,480 of the estimated population of children aged below 2 years in Malaysia. Respondents were asked whether various types of liquid or solid food were given to the child at any time during the preceding 24-hour period. The overall prevalence of ever breastfed among children aged less than 12 months was 94.7% (CI: 93.0 - 95.9). The overall prevalence of exclusive breastfeeding below 6 months was 14.5% (CI: 11.7 - 17.9). Prevalence of timely initiation was 63.7% (CI: 61.4 - 65.9) and the continued prevalence of breastfeeding up to two years was 37.4% (CI: 32.9 - 42.2). The findings suggest that the programmes implemented in the last ten years were effective in improving the prevalence of ever breastfeeding, timely initiation of breastfeeding and continued breastfeeding up to two years. However, the challenge is to improve exclusive breastfeeding practice. Longterm community-based interventions need to be carried out in partnership with the existing health care system, focusing on discouraging the use of water and infant formula, especially in the first few months of life.  相似文献

Breast milk is the most natural and best type of nutrition for almost all infants. Moreover, breastfeeding is associated with health benefits for mother and child: breast milk supports the development of the infant and protects it against diseases; the health benefits for the mothers also extend beyond the period after birth. Therefore, the WHO recommends exclusive breastfeeding for at least six months. Until now, no representative data on the breastfeeding rate have been available in Germany. The KiGGS results show an increase in the breastfeeding rate from 1986 to 2005. Across all age groups studied, 76.7 % (95 % CI: 75.2 %-78.1 %) of the children were ever breastfed. The rate of ever-breastfed children was significantly lower in mothers from socially disadvantaged population groups, children of mothers who had smoked during pregnancy or in situations with problems after birth. Children with migration background were more frequently ever-breastfed than children without migration background. The average duration of breastfeeding was - across all age-groups - 6.9 months (95 % CI: 6.8-7.0); the duration of full breastfeeding was 4.6 months (95 % CI: 4.5-4.7) on average. 22.4 % (95 % CI: 21.4 %-23.5 %) of all children from the KiGGS study population were exclusively breastfed for a period of six months. The results emphasises the necessity to further promote breastfeeding, especially to support the socially disadvantaged, and most of all to encourage a positive attitude towards breastfeeding in the society.  相似文献

Longitudinal study based on a birth cohort in Viçosa, MG, Southeastern Brazil. In 2011/2012, 168 new mothers accessing the public health network were followed. Three interviews, at 30, 60, and 120 days postpartum, with the new mothers were conducted. Exclusive breastfeeding abandonment was analyzed in the first, second, and fourth months after childbirth. The Edinburgh Postnatal Depression Scale was applied to identify depressive symptoms in the first and second meetings, with a score of ≥ 12 considered as the cutoff point. Socioeconomic, demographic, and obstetric variables were investigated, along with emotional conditions and the new mothers’ social network during pregnancy and the postpartum period.

RESULTS

The prevalence of exclusive breastfeeding abandonment at 30, 60, and 120 days postpartum was 53.6% (n = 90), 47.6% (n = 80), and 69.6% (n = 117), respectively, and its incidence in the fourth month compared with the first was 48.7%. Depressive symptoms and traumatic delivery were associated with exclusive breastfeeding abandonment in the second month after childbirth. In the fourth month, the following variables were significant: lower maternal education levels, lack of homeownership, returning to work, not receiving guidance on breastfeeding in the postpartum period, mother’s negative reaction to the news of pregnancy, and not receiving assistance from their partners for infant care.

CONCLUSIONS

Psychosocial and sociodemographic factors were strong predictors of early exclusive breastfeeding abandonment. Therefore, it is necessary to identify and provide early treatment to nursing mothers with depressive symptoms, decreasing the associated morbidity and promoting greater duration of exclusive breastfeeding. Support from health professionals, as well as that received at home and at work, can assist in this process.  相似文献

The aim of this study was to assess the association between promotion, protection, and support for breastfeeding in primary health care services and prevalence of exclusive breastfeeding in the first six months of life. A cross-sectional study was conducted in a representative sample of 1,029 mothers of infants younger than six months treated at primary health care facilities in the city of Rio de Janeiro, in southeastern Brazil. Prevalence ratios (PR) were estimated by Poisson regression. Prevalence of exclusive breastfeeding was 58.1%. Factors associated with increased prevalence of exclusive breastfeeding were: white skin color (PR = 1.20; 95% CI: 1.05-1.36); schooling (PR = 1.19; 95% CI: 1.05-1.35); marital status (married or in common-law marriage) (PR = 1.72; 95% IC:1.02-2.90); previous breastfeeding (PR = 1.27; 95% CI: 1.08-1,49); exclusive breastfeeding at the time of hospital discharge (PR = 2.01; 95% CI: 1.20-3.36); group support for the mother (PR = 1.14; 95% CI: 1.01-1.28); and orientation on breastfeeding (PR = 1.20; 95% CI: 1.08-1.33). Prevalence of exclusive breastfeeding decreased 17% per month in the infant's life. Support groups and orientation by primary health care for breastfeeding were associated with increased exclusive breastfeeding prevalence.  相似文献

目的 分析上海市某区产妇住院期间母乳喂养服务对产褥期纯母乳喂养行为的影响。 方法 在全区随机抽取13家社区卫生中心中的5家儿保门诊,招募1月龄的婴儿母亲进行问卷调查。 结果 共收集到合格问卷456份,住院期间母婴同室率为96.1%;11.4%没有使用奶瓶、奶粉;45.2%的婴儿产后1 h内吸吮母乳,分娩后30 min内开始母婴早接触率为47.6%。住院期间纯母乳喂养率23.9%,1月龄婴儿纯母乳喂养率55.3%。多元logistics回归分析显示,出生后第一口食物为母乳的婴儿在出生后一个月的纯母乳喂养率较高(aOR=1.932,95%CI:1.254~2.978,P=0.003)。分娩后30 min内进行母婴接触的婴儿在出生后一个月的纯母乳喂养率较高(aOR=1.624,95%CI:1.033~2.552,P=0.036)。 结论 住院期间母乳喂养服务与产褥期纯母乳喂养行为密切相关,需加强爱婴医院长效管理机制的落实,提升母乳喂养服务质量,提高纯母乳喂养率。  相似文献

A cross-sectional study was conducted to investigate the weight status and the relationship of infant-feeding variables, birthweight and birth order with BMI in a group of Iranian children. Five hundred and eleven students of both sexes at the first grade in elementary schools (aged 7 years) were recruited randomly from all 19 educational districts of Tehran. Weights and heights of children and their mothers were measured. Data on breastfeeding (BF), formula-feeding, the timing of introduction of complementary foods (CF), birthweight, and birth order were collected from the mothers. The 2007 WHO reference value was used for determining child''s weight status. Regression analysis in single and a 2-level linear regression models was used for examining the independent relationships of infant-feeding variables, birthweight and birth order with childhood BMI. The prevalence of underweight and overweight in this group of children was 7.6% and 19.7%, respectively. Total time of BF and duration of exclusive BF were not associated with childhood BMI. The timing of introduction of CF was inversely related to childhood BMI after controlling for other variables (β:-0.34; 95% CI:-0.58,-0.10). Children with an early introduction of CF had significantly higher mean BMI (p for linear trend=0.012). Birth order and birthweight were related to childhood BMI significantly. These data suggest that overweight and obesity are nutritional problems among 7 years old Teharani children. The timing of introduction of CF, birth order, and birthweight were independent predictors of childhood BMI.
