新生儿淋菌性结膜炎又称新生儿脓漏眼,今遇一例,报告如下。石×,女,出生五天,因双眼有大量脓性分泌物就诊。幼儿于出生2天时发现双眼大量分泌物,曾拟诊为“先天性泪囊炎”,用氯霉素眼水治疗两天无效,且分泌物异常增多。检查:发育正常,双眼睑轻度浮肿,睑裂处可见大量脓性分泌物,呈淡黄色,较稠,  相似文献

我院1997年5月~2005年8月共诊治新生儿淋菌性结膜炎56例,其中男26例,女30例;单眼9例,双眼47例。年龄2~20d,平均11d。所有病例均行结膜囊分泌物涂片或结膜刮片革兰染色,或培养结果确诊。本组56例患儿中顺产52例,剖宫产4例。1资料与方法1.1一般资料潜伏期:以发现有分泌物的时间计,出生即有者2眼,生后24h之内11眼,生后2~3d者36眼,4~5d者34眼,6~7d者12眼,8d以上者8眼,最长者15d发病。所有病例均有脓性分泌物,呈浆液性或粘液性,色黄;所有病例均有眼睑肿胀充血,有假膜形成8眼,出现结膜水肿57眼,有角膜病变者5例,其中角膜浸润4眼,角膜溃疡者1…  相似文献

新生儿淋菌性结膜炎又称新生儿脓漏眼。随着性传播疾病的发生 ,淋球菌引起的眼病也不断出现 ,1 990~ 2 0 0 0年 ,笔者共发现 42例新生儿淋菌性结膜炎 ,报告如下。临床资料本组 42例为出生 4~ 2 0天的新生儿。男 2 8例 ,女 1 4例 ,单眼患病 9例 ,双眼 3 3例 ,共 75只眼。均因眼睑红肿、有脓性分泌物而来眼科就诊。查 :患眼均有不同程度的眼睑潮红、肿胀 ,睁眼困难。睑裂部被稀薄或粘稠的脓汁粘附 ,分开眼睑时均有大量脓汁流出。脓汁呈淡白色 1 1例 ,血水样 7例 ,淡绿色 9例 ,黄白色 1 5例。结膜轻度水肿 1 0例 ,中度 1 9例 ,高度水肿 1 3例…  相似文献

Purpose: To describe the challenging diagnosis of ocular nocardiosis in a patient with anaplastic astrocytoma

Methods: A 56-year-old patient with recurrent cerebral anaplastic astrocytoma and a history of surgical resection, who underwent radiotherapy and chemotherapy, presented with a unilateral white subretinal mass at the posterior pole of his left eye. Vitrectomy and fine-needle biopsy of the vitreous as well as the subretinal mass were performed.

Results: Magnetic resonance imaging verified progression of the lesion’s size at follow-up. Histopathological workup of the material revealed gram-positive filamentous bacteria consistent with Nocardia cyriacigeorgica.

Conclusion: Endogenous Nocardia should be suspected in case of choroidal abscess in immunocompromised patients. Clinical diagnosis is challenging; therefore, early subretinal biopsy should be considered to facilitate treatment decision.  相似文献

Purpose: To report a case of acute retinal necrosis (ARN) caused by varicella-zoster virus (VZV) in an elderly patient with ocular sarcoidosis after oral corticosteroid indication.

Methods: Retrospective case report.

Results: A 75-year-old male with a past history of ocular sarcoidosis came with blurred left vision. Ocular findings in the left eye were consistent with ocular sarcoidosis, while no inflammation in the right eye. On day 14, intraocular inflammation in the left eye resolved by topical corticosteroid, but inflammatory cells were found in the right eye. Suspecting recurrence of ocular sarcoidosis, systemic corticosteroid was initiated. On day 21, inflammation worsened, and the presence of extended yellowish white peripheral retinal lesion in the right eye suggested ARN. Polymerase chain reaction (PCR) testing using ocular fluid detected 3.0 × 107 copies/ml of VZV DNA.

Conclusions: In the case of poor response to immunosuppressive therapy in elderly uveitis, infection including ARN should be considered. Immediate PCR testing for pathogen screening is required.  相似文献

Purpose: To report the diagnosis of a case of tubercular sclerouveitis, which masqueraded as an ocular tumor.

Design: Case report.

Methods: Retrospective medical chart review including serological, radiological, and histopathological investigative results.

Results: A patient presented with recurrent episodes of sclerouveitis in his right eye. Serological investigations for infective and connective tissue disorders were negative. Radiological investigation was suggestive of a scleral perforation. The eye was enucleated and histopathological examination showed chronic inflammation. Polymerase chain reaction of the ocular tissue was positive for IS1160 for Mycobacterium tuberculosis.

Conclusion: Infective scleritis should be suspected in cases of scleritis that progress despite treatment. Polymerase chain reaction is instrumental in diagnosing challenging cases.  相似文献

Afatinib is a second-generation epidermal growth factor receptor (EGFR) inhibitor that has been shown to be effective against EGFR-mutated non-small cell lung cancer (NSCLC) resistant to conventional EGFR inhibitors such as gefitinib and erlotinib. Although ocular side effects of gefitinib and erlotinib have been reported, those for afatinib have yet to be definitively established. This report presents details on the first case of unilateral iridocyclitis associated with the side effects of afatinib therapy. A 75-year-old Japanese male ex-smoker with EGFR-mutated NSCLC underwent afatinib therapy for multiple metastases. At 2 weeks, bilateral conjunctivitis developed. Topical medication and a 1-week afatinib washout period resulted in the improvement of the conjunctivitis. However, 3 days after the resumption of afatinib, the patient developed unilateral granulomatous anterior uveitis in his right eye. Best-corrected visual acuity (BCVA) measurement indicated a decimal visual acuity of 0.2, while the slit-lamp findings were characterized by granulomatous inflammation, keratic precipitates, Koeppe nodules and posterior synechiae. There was no evidence suggesting other intraocular inflammatory disease or metastatic tumor. The left eye was intact. The use of topical medication including steroids and a washout of afatinib resulted in a gradual subsiding of the anterior uveitis. After resolution of the anterior uveitis, oral afatinib was resumed. BCVA of the right eye finally recovered to a decimal acuity of 1.0. Ophthalmologists should be aware of the possibility that side effects associated with afatinib could cause granulomatous anterior uveitis.Key Words: Afatinib, Epidermal growth factor receptor, Side effect, Anterior uveitis, Koeppe nodule, Unilateral granulomatous anterior uveitis  相似文献
