BackgroundThe aim of this prospective, randomized trial was to evaluate whether the use of coronary computed tomography angiography (CCTA) as the first-line anatomical test in patients with suspected significant coronary artery disease (CAD) may reduce the number of coronary invasive angiographies (ICA), and expand the use of CCTA in patients currently diagnosed invasively.Methods120 patients (age:60.6 ± 7.9 years, 35% female) with indications to ICA were randomized 1:1 to undergo CCTA versus direct ICA. Outcomes were evaluated during the diagnostic and therapeutic periods.ResultsThe number of invasively examined patients was reduced by 64.4% in the CCTA group as compared to the direct ICA group (21vs59,p < 0.0001). The number of patients with ICAs not followed by coronary intervention was reduced by 88.1% with the CCTA strategy (5vs42,p < 0.0001). Over the diagnostic and therapeutic course there were no significant differences regarding the median volume of contrast (CCTA 80.3 ml[65.0–165.0] vs ICA 90.0 ml[55.0–100.0], p = 0.099), while a non-significant trend towards higher radiation dose in the CCTA group was observed (9.9 mSv[7.0–22.1] vs 9.4 mSv[5.2–14.0], p = 0.05). There were no acute cardiovascular events.ConclusionsCCTA may hypothetically act as an effective ‘gatekeeper’ to the catheterization laboratory in the diagnosis of stable patients with current indications for ICA. This strategy may result in non-invasive, outpatient-based triage of two thirds of individuals without actionable CAD, obviating unnecessary invasive examinations. BackgroundCoronary CT angiography (CTA) has emerged as an effective noninvasive method for direct visualization of the coronary arteries, with high diagnostic performance compared with invasive coronary angiography (ICA). However, coronary CTA is prone to artifacts, including coronary motion, which may reduce its diagnostic performance. Intracycle motion compensation algorithms (MCAs) from a combination of software and hardware techniques now allow for correction of coronary motion, but the diagnostic performance of MCAs compared with traditional coronary CTA reconstruction methods remains unexplored.MethodsViCTORY (Validation of an Intracycle CT Motion CORrection Algorithm for Diagnostic AccuracY) is a prospective international multicenter trial of 218 patients which is designed to evaluate the performance of MCAs for the diagnosis of anatomically obstructive coronary artery disease (CAD) compared with an ICA reference standard, on a per-patient, per-vessel, and per-segment basis. Patients enrolled into ViCTORY will undergo investigational coronary CTA and clinically indicated ICA and will not receive heart rate-lowering medications before coronary CTA. Coronary CTA images will be reconstructed by conventional standard methods as well as by MCAs. Blinded core laboratory interpretation will be performed for coronary CTA and ICA in an intent-to-diagnose fashion.ResultsThe primary end point of ViCTORY is the per-patient diagnostic accuracy of MCAs for the diagnosis of anatomically obstructive CAD compared with ICA. Secondary end points will include other per-patient, per-vessel, and per-segment diagnostic performance characteristics, including accuracy, sensitivity, specificity, positive predictive value, and negative predictive value. Other key secondary end points will include diagnostic interpretability, image quality, the upper heart rate threshold of utility of MCAs, and the additive value of MCAs to traditionally reconstructed coronary CTA.ConclusionViCTORY will determine whether MCAs improve the diagnosis of obstructive CAD in patients undergoing coronary CTA who are not receiving heart rate-lowering medications.  相似文献

目的 以有创冠状动脉造影(ICA)为参考标准,探讨人工智能(AI)辅助的冠状动脉CT血管成像(CCTA)诊断阻塞性冠状动脉狭窄的效能。 方法 回顾性收集行CCTA检查并于3个月内行ICA检查的50例疑患冠状动脉疾病(CAD)的病人,男34例,女16例,平均年龄(61.8±8.5)岁。AI软件、不同年资医师(低/中/高年资)及AI+不同年资医师分别对入组病人CCTA影像进行后处理并解读。将ICA和CCTA上冠状动脉管腔狭窄≥50%定义为阻塞性冠状动脉狭窄。采用Agatston积分法测量病人的钙化积分值,并将病人分为低钙化组(钙化积分<100)和高钙化组(钙化积分≥100)。采用独立样本t检验对AI、医师及AI+医师的图像后处理和解读时间进行两两比较。以ICA为参考标准,分析AI在不同研究水平和高/低钙化组的诊断价值,并比较AI、不同年资医师和AI+不同年资医师的诊断敏感度、特异度、阳性预测值、阴性预测值、准确度及受试者操作特征(ROC)曲线下面积(AUC)。采用Pearson卡方检验或Fisher精确概率检验比较组间差异,采用DeLong检验比较AUC。 结果 50例病人共分析195支血管424个节段。AI和AI+医师的平均后处理和解读时间均低于单独医师诊断的时间(均P<0.05),AI的时间较低/中/高年资医师分别减少了80%、76.8%和75%;AI+低/中/高年资医师较单独医师分别减少了67%、64%、57.9%。在病人、血管及节段水平,AI诊断阻塞性冠状动脉狭窄的敏感度分别为93.7%、83.1%、67.7%,特异度为50.0%、89.0%、91.0%,准确度为92%、86.7%、85.6%,阳性预测值为97.8%、83.1%、69.8%,阴性预测值为25%、89.0%、90.2%,AUC为0.87、0.89、0.83;在血管及节段水平,AI对低钙化组的特异度高于高钙化组(均P<0.05)。在血管水平,AI诊断的AUC值均低于中/高年资医师(均P<0.05);其余研究水平,AI与其他不同年资医师诊断的AUC值差异均无统计学意义(均P>0.05)。3种研究水平下,AI+低/中/高年资医师诊断的AUC值与单独不同年资医师诊断的AUC值差异均无统计学意义(均P>0.05)。 结论 AI辅助的CCTA诊断阻塞性冠状动脉狭窄具有较好的诊断效能,且明显缩短后处理时间,可能成为临床医师诊断阻塞性冠状动脉狭窄的有效辅助工具。  相似文献

IntroductionThe clinical implications of a widespread adoption of guideline recommendations for patients with stable chest pain and low pretest probability (PTP) of obstructive coronary artery disease (CAD) remain unclear. We aimed to assess the results of three different testing strategies in this subgroup of patients: A) defer testing; B) perform coronary artery calcium score (CACS), withholding further testing if CACS ​= ​0 and proceeding to coronary computed tomography angiography (CCTA) if CACS>0; C) perform CCTA in all.MethodsTwo-center cross-sectional study assessing 1328 symptomatic patients undergoing CACS and CCTA for suspected CAD. PTP was calculated based on age, sex and symptom typicality. Obstructive CAD was defined as any luminal stenosis ≥50% on CCTA.ResultsThe prevalence of obstructive CAD was 8.6% (n ​= ​114). In the 786 patients (56.8%) with CACS ​= ​0, 8.5% (n ​= ​67) had some degree of CAD [1.9% (n ​= ​15) obstructive, and 6.6% (n ​= ​52) nonobstructive]. Among those with CACS>0 (n ​= ​542), 18.3% (n ​= ​99) had obstructive CAD. The number of patients needed to scan (NNS) to identify one patient with obstructive CAD was 13 for strategy B vs. A, and 91 for strategy C vs. B.ConclusionsUsing CACS as gatekeeper would decrease CCTA use by more than 50%, at the cost of missing obstructive CAD in one in 100 patients. These findings may help inform decisions on testing, which will ultimately depend on the willingness to accept some diagnostic uncertainty.  相似文献

目的:研究320排动态容积冠状动脉CT成像(CCTA)诊断冠状动脉疾病(CAD)的可行性。方法:5961例临床诊断CAD的患者进行CCTA检查,其中186例接受常规X线冠状动脉造影术(CAG),对照分析其检查结果。1490例接受CCTA检查的健康查体者作为对照。结果:成像质量均在I~III级,无呼吸伪影图像。在186例CAD患者中,CCTA诊断冠状动脉狭窄(狭窄度≥50%)的敏感性为96.72%,特异性98.95%,阳性预测值95.16%,阴性预测值99.30%,准确度98.56%。结论:320排动态容积CT冠状动脉成像图像清晰,对诊断CAD具有重要的临床价值。  相似文献

BackgroundHigh amounts of coronary artery calcium (CAC) pose challenges in interpretation of coronary CT angiography (CCTA). The accuracy of stenosis assessment by CCTA in patients with very extensive CAC is uncertain.MethodsRetrospective study was performed including patients who underwent clinically directed CCTA with CAC score >1000 and invasive coronary angiography within 90 days. Segmental stenosis on CCTA was graded by visual inspection with two-observer consensus using categories of 0%, 1–24%, 25–49%, 50–69%, 70–99%, 100% stenosis, or uninterpretable. Blinded quantitative coronary angiography (QCA) was performed on all segments with stenosis ≥25% by CCTA. The primary outcome was vessel-based agreement between CCTA and QCA, using significant stenosis defined by diameter stenosis ≥70%. Secondary analyses on a per-patient basis and inclusive of uninterpretable segments were performed.Results726 segments with stenosis ≥25% in 346 vessels within 119 patients were analyzed. Median coronary calcium score was 1616 (1221–2118). CCTA identification of QCA-based stenosis resulted in a per-vessel sensitivity of 79%, specificity of 75%, positive predictive value (PPV) of 45%, negative predictive value (NPV) of 93%, and accuracy 76% (68 false positive and 15 false negative). Per-patient analysis had sensitivity 94%, specificity 55%, PPV 63%, NPV 92%, and accuracy 72% (30 false-positive and 3 false-negative). Inclusion of uninterpretable segments had variable effect on sensitivity and specificity, depending on whether they are considered as significant or non-significant stenosis.ConclusionsIn patients with very extensive CAC (>1000 Agatston units), CCTA retained a negative predictive value ​> ​90% to identify lack of significant stenosis on a per-vessel and per-patient level, but frequently overestimated stenosis.  相似文献

AimsCoronary CT angiography (CCTA) is an accurate non-invasive tool for the evaluation of coronary artery bypass graft (CABG). However, inability to sustain a long breath-hold, high heart rate (HR) and atrial fibrillation may affect image quality. Moreover, radiation exposure is still a matter of some concern. A scanner combining 0.23-mm spatial resolution, new iterative reconstruction and fast gantry rotation time has been recently introduced in the clinical field. The aims of our study were to evaluate interpretability, radiation exposure and diagnostic accuracy of CCTA performed with the latest generation of cardiac-CT scanners compared to invasive coronary angiography (ICA) in the assessment of bypass grafts, and non-grafted and post-anastomotic native coronary arteries.Methods and resultsWe prospectively enrolled 300 patients undergoing clinically indicated CCTA with a 16-cm z-axis coverage, 256-detector rows, and 0.28-sec gantry rotation time scanner. Coronary artery and graft interpretability, image quality and effective dose (ED) were assessed in all patients and diagnostic accuracy was evaluated in a subgroup of 100 patients who underwent ICA.Mean HR during the scan was 69.6 ± 10.8. Sinus rhythm was present in 118 patients with HR < 75 bpm and in 112 patients with HR ≥ 75 bpm, while 70 patients had atrial fibrillation. CABG interpretability was 100%. Compared to ICA, CCTA was able to correctly detecting occlusions or significant stenoses of all CABG segments. Overall interpretability of native coronary segments was 95.6%. Sensitivity, specificity, positive predictive value, negative predictive value and accuracy of coronary arteries were 98.3%, 97.4%, 93.1%, 99.3% and 96.5%, respectively. The diagnostic accuracy in a patient based analysis was 95.2%. Mean ED was 3.14 ± 1.7 mSv.ConclusionsThe novel whole-heart coverage CT scanner allows to evaluating CABG and native coronary arteries with excellent interpretability and low radiation exposure even in the presence of unfavorable heart rhythm.  相似文献

BackgroundRecent studies demonstrated a significant improvement in the diagnostic performance of coronary CT angiography (CCTA) for the evaluation of in-stent restenosis (ISR). However, coronary stent assessment is still challenging, especially because of beam-hardening artifacts due to metallic stent struts and high atherosclerotic burden of non-stented segments. Adenosine-stress myocardial perfusion assessed by CT (CTP) recently demonstrated to be a feasible and accurate tool for evaluating the functional significance of coronary stenoses in patients with suspected coronary artery disease (CAD). Yet, scarce data are available on the performance of CTP in patients with previous stent implantation.Aim of the studyWe aim to assess the diagnostic performance of CCTA alone, CTP alone and CCTA plus CTP performed with a new scanner generation using quantitative invasive coronary angiography (ICA) and invasive fractional flow reserve (FFR) as standard of reference.MethodsWe will enroll 300 consecutive patients with previous stent implantation, referred for non-emergent and clinically indicated invasive coronary angiography (ICA) due to suspected ISR or progression of CAD in native coronary segments. All patients will be subjected to stress myocardial CTP and a rest CCTA. The first 150 subjects will undergo static CTP scan, while the following 150 patients will undergo dynamic CTP scan. Measurement of invasive FFR will be performed during ICA when clinically indicated.ResultsThe primary study end points will be: 1) assessment of the diagnostic performance (diagnostic rate, sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy) of CCTA, CTP, combined CCTA-CTP and concordant CCTA-CTP vs. ICA as standard of reference in a territory-based and patient-based analysis; 2) assessment of sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy of CCTA, CTP, combined CCTA-CTP and concordant CCTA-CTP vs. invasive FFR as standard of reference in a territory-based analysis.ConclusionsThe ADVANTAGE study aims to provide an answer to the intriguing question whether the combined anatomical and functional assessment with CCTA plus CTP may have higher diagnostic performance as compared to CCTA alone in identifying stented patients with significant ISR or CAD progression.  相似文献

目的:评价冠状动脉CT血管成像(CCTA)和运动平板试验(TET)对冠心病的诊断价值。方法:以常规冠状动脉造影(CAG)为诊断冠心病(冠脉狭窄≥50%)的"金标准",对同期先后行TET、CCTA和CAG 3种检查的75例疑似冠心病患者进行回顾性分析,将其TET和CCTA的结果与CAG进行比较。结果:TET和CCTA诊断冠心病的敏感度分别为45.2%和90.5%,特异度为69.7%和93.9%,阳性预测值为65.5%和95.0%,阴性预测值为50.0%和88.6%,准确率为56.0%和92.0%,P<0.01。在冠状动脉血管水平CCTA对右冠状动脉、左主干、前降支、回旋支狭窄诊断的准确率分别为86.7%、100.0%、88.0%和76.0%。冠心病患者中TET诊断阳性率与病变血管支数呈正相关(r=0.440,P=0.004);冠心病患者TET诊断结果阳性与阴性仅与血管狭窄程度≥75%狭窄的节段数目有统计学差异(P=0.016)。结论:CCTA诊断冠心病较TET有更高的诊断准确性和较低的诊断假阳性和假阴性,对有症状的疑诊冠心病患者CCTA的诊断具有更重要作用。  相似文献

BackgroundCTA based FFR, a software based application, enhances diagnostic value of coronary computed tomography angiography (CTA) examination. However it remains unknown whether it improves accuracy over the gold standard of invasive coronary angiography (ICA) in predicting functionally significant coronary stenosis. The aim of our study was to compare diagnostic accuracies of coronary CTA, CTA based FFR, and ICA, with invasive FFR as the reference standard in patients with intermediate stenosis on CTA.Methods96 intermediate stenoses (50–90%) from 90 subjects, with intermediate pre-test probability of CAD, who underwent coronary CTA were analyzed. Each patient had subsequent ICA with FFR. CTA based FFR (cFFR v2.1, Siemens) analysis was performed on-site. The stenoses with invasive FFR≤0.8 were considered hemodynamically significant.Results41/96 stenoses were hemodynamically significant (FFR≤0.8). While the area under ROC curves (AUC) for identification of significant stenosis evaluated on QCA (0.653), visual ICA (0.652), qCTA (0.690) and visual CTA (0.660) did not significantly differ, the AUC for CTA based FFR (0.835) was significantly higher (p = 0.004, p = 0.004, p = 0.010, p = 0.007, respectively). The accuracies of CTA based FFR, qCTA and QCA were 76%, 63% and 58% respectively.ConclusionOur results suggest that diagnostic potential of routine coronary CTA, augmented with CTA based FFR analysis, is superior to ICA in patients with intermediate stenosis.  相似文献

BackgroundThis study aimed to investigate the diagnostic value of comprehensive on-site coronary computed tomography angiography (CCTA) using stenosis and plaque measures and subtended myocardial mass (Vsub) for fractional flow reserve (FFR) defined hemodynamically obstructive coronary artery disease (CAD). Additionally, the incremental diagnostic value of off-site CT-derived FFR (FFRCT) was assessed.MethodsProspectively enrolled patients underwent CCTA followed by invasive FFR interrogation of all major coronary arteries. Vessels with ≥30% stenosis were included for analysis. On-site CCTA assessment included qualitative and quantitative stenosis (visual grading and minimal lumen area, MLA) and plaque measures (characteristics and volumes), and Vsub. Diagnostic value of comprehensive on-site CCTA assessment was tested by comparing area under the curves (AUC). In vessels with available FFRCT, the incremental value of off-site FFRCT was tested.ResultsIn 236 vessels (132 patients), MLA, positive remodeling, non-calcified plaque volume, and Vsub were independent on-site CCTA predictors for hemodynamically obstructive CAD (p < 0.05 for all). Vsub/MLA2 outperformed all these on-site CCTA parameters (AUC = 0.85) and Vsub was incremental to all other CCTA predictors (p = 0.02). In subgroup analysis (n = 194 vessels), diagnostic performance of FFRCT and Vsub/MLA2 was similar (AUC 0.89 and 0.85 respectively, p = 0.25). Furthermore, diagnostic performance significantly albeit minimally increased when FFRCT was added to on-site CCTA assessment (ΔAUC = 0.03, p = 0.02).ConclusionsIn comprehensive on-site CCTA assessment, Vsub/MLA2 demonstrated greatest diagnostic value for hemodynamically obstructive CAD and Vsub was incremental to all evaluated CCTA indices. Additionally, adding FFRCT only minimally increased diagnostic performance, demonstrating that on-site CCTA assessment is a reasonable alternative to FFRCT.  相似文献

ObjectiveTo assess the clinical safety and effectiveness of coronary revascularization in patients who underwent coronary artery bypass grafting (CABG) based exclusively on coronary computed tomography angiography (CCTA) results.Methods53 patients (62.3 ± 7.1 years) underwent CCTA before a CABG surgery without prior invasive coronary angiography (ICA). Primary endpoints were all-cause mortality and major adverse cardiovascular events (MACE). The secondary endpoint was quality of life (QoL) assessed with the Minnesota Living with Heart Failure Questionnaire (MLHFQ). All were collected one year after the surgery.ResultsCCTA revealed multivessel coronary artery disease (CAD) in 52 patients. Indication for bypass surgery was made exclusively based on CCTA results. 136 distal anastomoses were performed. Assessment at 1 year (13.3 ± 1.4 months) was completed in 98.1% of the patients. MACE and mortality rates were 0%. The MLHFQ total score was 21.8 ± 8.7, and active lifestyle was maintained in all patients.ConclusionsIn this proof of concept prospective pilot study, we observed that non-invasive coronary angiography may provide adequate anatomic detail to guide CABG surgery. BackgroundThe role of change in fractional flow reserve derived from CT (FFRCT) across coronary stenoses (ΔFFRCT) in guiding downstream testing in patients with stable coronary artery disease (CAD) is unknown.ObjectivesTo investigate the incremental value of ΔFFRCT in predicting early revascularization and improving efficiency of catheter laboratory utilization.MaterialsPatients with CAD on coronary CT angiography (CCTA) were enrolled in an international multicenter registry. Stenosis severity was assessed as per CAD-Reporting and Data System (CAD-RADS), and lesion-specific FFRCT was measured 2 ?cm distal to stenosis. ΔFFRCT was manually measured as the difference of FFRCT across visible stenosis.ResultsOf 4730 patients (66 ?± ?10 years; 34% female), 42.7% underwent ICA and 24.7% underwent early revascularization. ΔFFRCT remained an independent predictor for early revascularization (odds ratio per 0.05 increase [95% confidence interval], 1.31 [1.26–1.35]; p ?< ?0.001) after adjusting for risk factors, stenosis features, and lesion-specific FFRCT. Among the 3 models (model 1: risk factors ?+ ?stenosis type and location ?+ ?CAD-RADS; model 2: model 1 ?+ ?FFRCT; model 3: model 2 ?+ ?ΔFFRCT), model 3 improved discrimination compared to model 2 (area under the curve, 0.87 [0.86–0.88] vs 0.85 [0.84–0.86]; p ?< ?0.001), with the greatest incremental value for FFRCT 0.71–0.80. ΔFFRCT of 0.13 was the optimal cut-off as determined by the Youden index. In patients with CAD-RADS ≥3 and lesion-specific FFRCT ≤0.8, a diagnostic strategy incorporating ΔFFRCT >0.13, would potentially reduce ICA by 32.2% (1638–1110, p ?< ?0.001) and improve the revascularization to ICA ratio from 65.2% to 73.1%.ConclusionsΔFFRCT improves the discrimination of patients who underwent early revascularization compared to a standard diagnostic strategy of CCTA with FFRCT, particularly for those with FFRCT 0.71–0.80. ΔFFRCT has the potential to aid decision-making for ICA referral and improve efficiency of catheter laboratory utilization.  相似文献

The National Institute for Health and Care Excellence (NICE) provides independent evidence-based guidance for England's National Health Service. Its 2010 guideline for the “assessment and diagnosis of recent onset chest pain or discomfort of suspected cardiac origin” (CG95) recommended a variety of first-line investigations in stable patients, depending on the pre-test probability (PTP) of obstructive coronary artery disease (CAD). Following a limited review, NICE produced an updated version of CG95 in 2016. Formal calculation of PTP is no longer advised. Coronary computed tomographic angiography (CCTA) is recommended as the first-line investigation for all patients with angina (or non-anginal pain but an abnormal electrocardiogram) and no prior CAD, with second-line functional imaging if the CCTA is equivocal.Notwithstanding some controversies regarding NICE's methodology, the updated version of CG95 can be justified on several levels. The focus on angina reflects evidence that patients with non-anginal pain have a similar prevalence of CAD to an asymptomatic population, and may not benefit from further investigation. The elimination of PTP is reasonable in patients required to have cardiac-sounding (anginal) symptoms. The ability of CCTA to identify non-obstructive atheroma, invisible to functional testing, might lead to improved medical treatment. Conversely the argument sometimes made for first-line functional testing, that ischemia-guided coronary revascularization leads to improved outcomes, has little hard evidence to support it. The performance of a separate functional test following equivocal CCTA may improve diagnostic specificity, and similar information is now obtainable from the CT study itself via computational flow dynamics.  相似文献

BackgroundA diminished coronary lumen volume to left ventricle mass ratio (V/M) derived from coronary computed tomography angiography (CCTA) has been proposed as factor contributing to impaired myocardial blood flow (MBF) even in the absence of obstructive disease on invasive coronary angiography (ICA).MethodsPatients underwent CCTA, and positron emission tomography (PET) prior to ICA. Matched global V/M, global, and vessel specific hyperaemic MBF (hMBF), coronary flow reserve (CFR), and, FFR were available for 431 vessels in 152 patients. The median V/M (20.71 mm3/g) was used to divide the population into patients with either a low V/M or a high V/M.ResultsOverall, a higher percentage of vessels with an abnormal hMBF and FFR (34% vs. 19%, p = 0.009 and 20% vs. 9%, p = 0.004), as well as a lower FFR (0.93 [interquartile range: 0.85–0.97] vs. 0.95 [0.89–0.98], p = 0.016) values were observed in the low V/M group. V/M was weakly associated with vessel specific hMBF (R = 0.148, p = 0.027), and FFR (R = 0.156, p < 0.001). Among vessels with non-obstructive CAD on ICA (361 vessels), no association between V/M and vessel specific hMBF nor CFR was noted. However, in the absence of obstructive CAD, V/M was associated with (R = 0.081, p = 0.027), and independently predictive for FFR (p = 0.047).ConclusionOverall, an abnormal vessel specific hMBF and FFR were more prevalent in patients with a low V/M compared to those with a high V/M. Furthermore, V/M was weakly associated with vessel specific hMBF and FFR. BackgroundAngina is a frequent symptom in patients with hypertrophic cardiomyopathy (HCM); however, it is often not because of significant epicardial coronary artery stenosis. Coronary CT angiography (CCTA) is an excellent modality to rule out significant coronary artery stenosis in the low- and intermediate-risk patients; however, its value in patients with HCM has not been explored. We sought to assess the utility of CCTA in the assessment of patients with HCM and stable anginal symptoms and compare the incidence of epicardial coronary artery stenosis to an age- and gender-matched control group.MethodsConsecutive outpatients with HCM referred for CCTA over a 3-year period because of stable anginal symptoms (chest pain or shortness of breath) were identified retrospectively. Age- and gender-matched patients without HCM referred for CCTA because of similar symptoms over a 6-month period were used as controls. All patients had CCTA using an Aquilion ONE 320 scanner. The coronary arteries were evaluated independently by 2 blinded observers, and any luminal narrowing was scored quantitatively as follows: >70% = severe; 50% to 70% = moderate; <50% = mild; and none. For the HCM group, results of cardiac single-photon emission CT (SPECT) or cardiac magnetic resonance perfusion studies as well as catheter angiograms were recorded where available.ResultsA total of 91 patients with HCM and 91 controls were included. No significant difference in cardiac risk factors was present between the 2 groups. The CCTA was of diagnostic quality in all patients. The median (interquartile range) calcium score was lower in patients with HCM (0 [0–50] vs 2 [0–189]) but did not reach statistical significance (P = .23). The incidence of moderate-to-severe coronary artery stenosis was significantly lower in patients with HCM than in controls (6.6% vs 33.0%; P < .001). The incidence of left anterior descending artery luminal narrowing overall was also significantly lower in the HCM patients (7.0% vs 20.9%; P = .002). There was a higher incidence of myocardial bridging in patients with HCM (40.7% vs 6.6%; P < .001), with longer and deeper bridged segments. Among a subgroup of HCM patients (n = 24) who had either stress perfusion CMR or cardiac single-photon emission CT studies performed, 15 of 24 had false-positive perfusion abnormalities without evidence of luminal obstruction on CCTA.ConclusionWe demonstrate the use of CCTA for the assessment of stable anginal symptoms in patients with HCM. The incidence of moderate-to-severe coronary artery stenosis was significantly lower in our HCM patients in comparison to our age-matched, gender-matched, and risk factor–matched control group. Given the high incidence of false-positive findings on perfusion stress studies, we propose that CCTA may be useful for appropriate triage to coronary angiography in the HCM patient with anginal symptoms.  相似文献

ObjectiveTo review the imaging features of coronary artery-to-pulmonary artery fistula (CPAF) on CT coronary angiography (CTCA) and evaluate its diagnostic performance compared with coronary catheter angiography (CCA) and transthoracic echocardiography (TTE).Materials and methodsWe retrospectively reviewed with a diagnosis of CPAF from among 19855 consecutive CCTA performed with 256-slice MDCT scanner for suspected coronary artery disease. CT images were evaluated for – origin, number, size and course (tubular/worm-like dilation/significant aneurysm formation/wall attachment sign) of fistula vessels, drainage site, drainage site imaging features (pierced sign, isodensity sign, smoke sign, jet sign), and main pulmonary artery (MPA) enlargement. 25 patients of CPAF also underwent CCA and 47 patients underwent TTE.ResultsThere were 72 patients with CPAF (0.36%) in our study, of which 44 were men and 28 were women, with mean age of 55.8 ± 13.2 years (range 22–85 years). CPAF originated from conus artery, left anterior descending artery (LAD), combined conus artery and LAD in 55, 67, 50 cases, respectively. Tubular dilation, worm-like dilation and aneurysm was seen in 14, 58 and 35 cases, respectively. Wall attachment sign was noted in 69 cases. All the cases demonstrated only a single drainage site, with left lateral wall, left anterolateral, anterior, right lateral and right anterolateral walls of MPA in 44, 21, 5, 1 and 1 cases, respectively. The mean diameter of the fistula drainage site was 2.6 ± 1.3 mm. Pierced sign, jet sign, smoke sign, isodensity sign was seen in 72, 46, 41 and 24 cases, respectively. MPA enlargement was seen in 20 patients. CCA showed CPAF in only 20 cases among 25 patients; while TTE showed CPAF in only 9 patients among 47 patients.ConclusionCTCA is competent in detecting and characterizing CPAF with an excellent diagnostic performance as the first imaging modality of choice, which is valuable for giving a distinct and intuitive explanation to patients and physicians and making an objective and exact assessment for further management.  相似文献

Objective

To evaluate the diagnostic performance of stress perfusion cardiac MR (CMR) for detecting significant CAD (≥70% narrowing) in comparison with invasive coronary angiography (ICA) as a reference standard.

Methods

Examinations of 54 patients who underwent both stress perfusion CMR and ICA for investigation of CAD between 2007 and 2009 were evaluated. The CMR protocol included dipyridamole stress and rest perfusion, stress and rest cine MRI for assessment of ventricular function and delayed gadolinium enhancement for assessment of myocardial viability and detection of infarction. CMR interpretation was performed by 2 observers blinded to the results of ICA and the clinical history.

Results

From a total of 54 patients, 37 (68.5%) showed significant CAD in 71 coronary territories. A perfusion defect was detected in 35 patients and in 69 coronary territories. Individual stress perfusion CMR evaluation showed the highest accuracy (83%) of the CMR techniques. The combined analysis using all sequences increased the overall accuracy of CMR to 87%.

Conclusion

Combination of perfusion and cine-MR during stress/rest, associated to delayed enhancement in the same protocol improves CMRI diagnostic accuracy and sensitivity for patients with significant coronary stenosis, and may therefore be helpful for risk stratification and defining treatment strategies.  相似文献

Background

Multidetector computed tomography (MDCT) has been proposed as a noninvasive method for the diagnosis of obstructive coronary artery disease (CAD). In patients with high risk of iodinated contrast adverse effects such as acute allergic-type reactions, the use of gadolinium could be an alternative.

Objective

We sought to evaluate the feasibility of gadolinium-enhanced MDCT for the diagnosis of obstructive CAD.

Methods

Twenty patients (mean age, 61 years; range, 50-73 years) referred for X-ray coronary angiography were studied by both gadolinium and iodine-enhanced 16-row MDCT coronary angiography. The degree of enhancement and the accuracy for detection of obstructive CAD (>50% diameter reduction) were evaluated with X-ray coronary angiography as the standard. Renal nephrotoxicity was strictly monitored.

Results

Gadolinium- and iodine-enhanced MDCT showed adequate visualization of the coronary arteries in 310 of the 312 coronary artery segments that were available by X-ray angiography, respectively. The average density of the coronary arteries in both iodine and gadolinium CT scans was 253.65 Hounsfield unit (HU) and 135.20 HU, respectively. In a per-coronary segment analysis, gadolinium- and iodine-enhanced MDCT showed sensitivities of 89% vs 84%, specificities of 96% vs 95%, and negative predictive values of 97% vs 96%, respectively. In a per-patient analysis, both gadolinium- and iodine-enhanced MDCT showed sensitivities of 92.85% vs specificities of 83.33%. Intermethod agreement between gadolinium- and iodine-enhanced MDCT (κ) was 0.95 (P < 0.0001).

Conclusion

Our preliminary results indicate lower attenuation with gadolinium but similar diagnostic accuracy for the detection of obstructive CAD when compared with iodine-enhanced MDCT. Therefore, gadolinium is a feasible alternative contrast agent for patients with iodine contrast allergy referred for MDCT coronary angiography.  相似文献

BackgroundThe Duke clinical score (DCS) is commonly used to estimate the pretest probability of coronary artery disease (CAD). However, the criterion was developed in a population undergoing catheter angiography.ObjectiveTo test the hypothesis that DCS overestimates the CAD probability when applied to patients evaluated with coronary CT angiography (CCTA). A second objective is to compute an adjustment of the calculated DCS to apply to this population.MethodsThe DCS was calculated for the 3996 consecutive CCTA studies (February 2009 to April 2013) performed for symptomatic patients with no known CAD. Performance of the DCS for the detection of CAD was evaluated by the area under the receiver operating characteristic curve. Using the training cohort (n = 2789), a linear regression line between the calculated probability and the observed prevalence of CAD identified a modified DCS cutoff for a better risk categorization; this was internally validated by a separate cohort (n = 1207).ResultsThe DCS showed a good discrimination (area under the receiver operating characteristic curve = 0.71) for the detection of CAD (prevalence = 23.3%). The calibration analysis showed an overall 2.4-fold overestimation by DCS with a DCS < 23% corresponding to the low-risk category (ie, observed prevalence of CAD < 10%). There was no appropriate DCS cutoff to define high-risk category (ie, prevalence > 90%). The validation cohort showed a prevalence of 9.4% when DCS < 23% was used to define low risk.ConclusionAmong patients who underwent CCTA, DCS overestimated the pretest probability by at least 2-fold; the DCS < 23% should define the lower risk probability. The DCS poorly identifies high-risk population and thus development of new CCTA-based criteria is warranted.  相似文献
