Objective. To evaluate if the presence of periodontitis is associated with carotid arterial calcifications diagnosed on panoramic radiographs in an elderly population. Materials and methods. Study individuals were randomly selected from the Swedish civil registration database representing the aging population (60–96 years) in Karlskrona, Sweden. Bleeding on probing (BOP) and the deepest probing measurement at each tooth were registered. The proportions of teeth with a probing depth ≥5 mm and the proportion of teeth with bleeding on probing were calculated. Analog panoramic radiographs were taken and the proportion of sites with a distance ≥5 mm between the alveolar bone level and the cement–enamel junction (CEJ) were assessed. A diagnosis of periodontitis was declared if a distance between the alveolar bone level and the CEJ ≥5 mm could be identified from the panoramic radiographs at >10% of sites, probing depth of ≥5 mm at one tooth or more and with BOP at >20% of teeth. Results. Readable radiographs were obtained from 499 individuals. Carotid calcification was identified in 39.1%. Individuals were diagnosed with periodontitis in 18.4%. Data analysis demonstrated that individuals with periodontitis had a higher prevalence of carotid calcifications (Pearson χ2 = 4.05 p < 0.05) and with a likelihood of 1.5 (95% CI = 1.0, 2.3, p < 0.05). Conclusions. Data analysis demonstrated a significant association between periodontitis and carotid calcification.  相似文献

The present study assessed the progression rate of periodontal disease over 8 years in a group of 52 adult patients with various forms of cleft lip, alveolus, and palate considered at risk for progression of periodontal disease. Of special interest was the evaluation of periodontal disease progression at sites adjacent to cleft regions compared to changes found at control sites not directly affected by such defects. High incidences of generalized plaque accumulation and bleeding on probing were noted at both examinations in 1979 and 1987. A mean apical shift of the clinical attachment level amounting to 0.2 mm had occurred over the 8-year observation period. A slight apical displacement of the mesial and distal mean crestal alveolar bone was also noted. The rate of progression of periodontal disease over the 8 years was not found to be different at statistically significant levels at cleft sites compared to control sites. However, the results of this study documented that the cumulative periodontal destruction at 26 to 28 years of age was statistically significant and more pronounced at cleft sites as revealed by greater probing pocket depth and loss of clinical attachment. The differences between test and control sites amounted to 0.3 and 0.4 mm respectively for probing depth and 0.6 mm for loss of clinical attachment. In addition, the discrepancy between alveolar bone height and the levels of the clinical attachment at cleft sites demonstrated the presence of a long supracrestal connective tissue attachment adjacent to cleft defects. Therefore, the alveolar bone height as visualized in radiographs at such sites was considered an unreliable diagnostic tool for the assessment of the degree of periodontal destruction.  相似文献

The present study was performed to determine radiographically the normal distance between the cemento-enamel junction (CEJ) and the alveolar bone crest (AC) as a background for diagnostic decision on bone loss in adolescents. For this purpose, the normal appearance of the alveolar crest was studied in 30, 18-year-olds reported to have had a clinically healthy gingiva at the age of 16 years. A clinical examination was performed and 4 posterior bite-wing radiographs were taken. Sites with bleeding on probing, loss of probing attachment, defective fillings and calculus were excluded from the analysis. At the remaining 737 sites, the distance CEJ-AC was measured on radiographs. Also, the appearance of the marginal bone (normal, sclerotic, notched and/or irregular image, widening of the periodontal ligament space) was recorded. The mean distances CEJ-AC for the separately-recorded maxillary surfaces amounted to 0.9 or 1.0 mm. In the mandible, the distances amounted to 0.7 or 0.8 mm except for the mesial surface of the mandibular first premolar, where a mean distance of 0.5 mm was noted. At maxillary surfaces, 84-90% of the sites showed a distance of 1 mm or less and 94-100% a distance of 1.5 mm or less. At mandibular surfaces, 93-100% of the sites showed a distance of 1.0 mm or less and 100% a distance of 1.5 mm or less. At 4 sites, located at the mesial surface of the first and second premolars in the maxilla, the CEJ-AC distance was recorded as 2 mm. Periodontal bone loss in 126 individuals between 9 and 20 years of age was screened radiographically by means of 2 standarized interproximal bite-wings radiographs. This group was selected from a population of 1253 young people according to the criteria of "having one or more probing pocket depths equal or higher than 4 mm". "Bone loss" was measured when the distance from the CEJ to the alveolar crest was greater than 2 mm per cent of this sample had bone loss, being more prevalent in females (34.3%) than in men (18.75%). Vertical defects predominated in the mesial aspect of mandibular first molars. Only one individual had radiographic features typical of localized juvenile periodontitis, representing 0.7% of the selected sample and 0.007% of the population studied.  相似文献

A 6-year study on the pattern of periodontal disease progression   总被引:2,自引:0,他引:2  
Abstract. The present longitudinal radiographic investigation was designed to study the change in the alveolar bone height over 6 years in relation to tooth type, age and the presence of previous bone loss in a group of 142 subjects (age 18–67 years) who were not under systematic periodontal treatment. The subjects were examined radiographically at baseline, and after 2 and 6 years, and sites showing significant (≥ 1 mm) change in the alveolar bone height during 2 consecutive examinations were identified. 90.2% of all examined sites showed no bone loss during the 6 years, while 8.6% and 1.2% of the sites exhibited bone loss during one or both examination periods, respectively, 3 groups of subjects within the studied population were identified. Approximately 5% of the subjects had high rates of periodontal disease progression whilst ∼ 70% demonstrated very few or no sites with bone loss, and ∼ 25% had a moderate level of disease progression during the study period. The results also showed that the number of sites exhibiting alveolar bone loss during the study period varied with respect to tooth type, age of the patient and presence of bone loss at baseline. In addition, significantly different rates of bone loss were disclosed with relation to tooth type ( P <0.01), age ( P <0.0001) and initial bone loss ( P <0.01). It was concluded that periodontal disease progression occurs infrequently and may take the form of discrete or recurrent episodes of loss of periodontal support which can be influenced by the location of the site, age of the subject and previous periodontal disease experience.  相似文献

SUMMARY The alveolar bone height in a total of 713 15–16-year-old children consisting of 364 males and 349 females was assessed using posterior bitewing radiographs. The distance between the alveolar crest (AC) and amelo-cemental junction (ACJ) on all molar and premolar teeth was determined from magnified images of the radiographs. Analyses of variance models were fitted in order to assess the dependence of the overall ACJ to AC distance on the inter-subject variables of gender and social class and the intra-subject variables of site, missing adjacent tooth, condition of the approximal tooth surface, and for mesial surfaces, gingivitis and pocket depth. The overall geometric mean for bone height for the whole population was 0.542mm. The ACJ to AC distance for the vast majority of sites was below 2 mm. Site had a highly significant effect on bone height ( P ≤ 0.001): ACJ to AC distances were greater in the maxilla compared to the mandible ( P ≤ 0.001), greater for distal sites compared to mesial ( P ≤ 0.001) and significantly affected by tooth type ( P ≤ 0.001). Sites with missing adjacent teeth also had significantly increased distances ( P ≤ 0.001). The condition of the approximal tooth surface and the presence of pocketing or gingivitis did not have a significant association with bone height. The geometric mean AC to ACJ distance, adjusted for subject, site and social class was 0.577mm for males and 0.506mm for females ( P ≤ 0.001), but the main effect of social class was not significant.  相似文献

Objective: The aim of this study was a radiographic mesiodistal analysis of the shape of the bone crest 3 months after tooth removal. Material and methods: One hundred single tooth extractions were performed on 100 patients because of orthodontic or prosthetic causes. Bite blocks were used for two radiographs: one on the day of extraction and the other after healing of the socket, 3 months later. These X‐rays were used to determine: (1) the most apical distance of alveolar ridge resorption, with baseline as the line between bone‐to‐teeth contact (the greatest distance in bone resorption height) and (2) the mesiodistal distance (MDD) and mesial and distal angles arising after bone tissue modeling. Results: Significant differences (P<0.05) emerged between the MDDs of multiple‐ [8 mm, 95% confidence interval (CI): 6.09, 9.90] and single‐root teeth (5.60 mm, 95% CI: 4.80, 6.50). However, mesial or distal angles or the most apical distance of alveolar ridge resorption did not differ (mean distance in height=4.32 mm, 95% CI: 3.85, 4.78; mean angle=24°). Conclusions: In this study, the post‐extraction mesiodistal bone distance between teeth adjacent to the edentulous ridge depends on the size of the edentulous space. Nevertheless, the distance does not affect the distance in bone loss height. The distance of bone resorption height reaches a balance at the midpoint, which we consider indicative of stable healing. This resorption process must be considered when placing dental implants in fresh extraction sockets, especially in aesthetic sites, because the implant surfaces could be exposed after 3 months. Abstract The aim of the present study was to determine the progression rate of periodontal disease in patients treated for localized or generalized mild to moderate adult periodontitis. 52 patients with a mean age of 53.7 years (S.D. 12. 6 years) were instructed in optimal home care procedures and exposed to initial periodontal therapy, before reconstructive therapy was initiated. Following completion of the prosthetic procedures, supportive therapy was offered to a limited extent and maintenance visits were irregularly scheduled corresponding to traditional dental care. Clinical periodontal parameters from 4 sites per tooth were assessed at the initial examination, at the time of reevaluation after initial therapy and at the re-examination after 8-years. Full sets of intraoral radiographs from the initial and the 8-year re-examination were analyzed with respect to changes in the radiographic alveolar bone height as a % of the total tooth length. As the result of the home care instructions, the mean plaque index (plaque control record) amounted to 21% at the end of initial periodontal therapy. 8 years later, the re-examination revealed a mean plaque index of 49% and a mean gingival bleeding index of 24%. At the initial examination, the 52 patients presented with an average of 18.7 teeth. During treatment. 26 teeth were sacrificed and 19 teeth were lost over the 8 years of supportive therapy. Bicuspids were the most frequent teeth to be lost over the observation period. As a result of initial therapy, the mean pocket probing depths decreased significantly. However, after 8 years, only minor differences were found when compared to the initial examination. At all examinations, the buccal and the oral aspects presented with shallower pockets compared to the interproximal sites. The comparison of the changes in the mean probing attachment levels over the observation period at mesial, buccal, distal and oral sites revealed a gain in clinical attachment after initial therapy and a mean loss of attachment ranging from 0.48 mm to 0.79 mm from the initial to the re-examination after 8 years. The buccal aspects demonstrated the highest mean clinical attachment loss followed by the oral sites. The radio-graphic assessment of the changes in the mean alveolar bone height revealed a statistically significant, but a clinically insignificant, loss of alveolar bone height of less than 2% of the tooth length. The patient population was grouped into 21 patients seeking supportive therapy less than once per year over 8 years. 14 patients having had one maintenance visit per year and 17 patients who were recalled more than once per year. With respect to the distribution of the number of sites with different changes in probing attachment level, no statistically significant difference between the groups was observed. When frequency analyses of the radiographically assessed changes in the alveolar bone height were performed, similar results were obtained. With the parameters and the statistical methods applied in this study, no significant influence of the age nor the recall frequency on the progression rate of periodontal disease could be detected in this group of patients presenting initially with mild to moderate adult periodontitis. Recall visits corresponding to conventional dental care did not prevent further loss of attachment, resulting in a deterioriation of the periodontal conditions compared to the results achieved after initial therapy and in 45 out of 52 patients compared to the baseline examination.  相似文献

The aim of the present retrospective study was to evaluate alterations of the alveolar bone level over a 10-year period at tooth sites with "angular" and "even" patterns of bone loss, in subjects who were not exposed to systematic periodontal therapy. A further objective was to evaluate whether the presence of an angular defect can serve as a predictor of additional bone loss. 201 subjects in ages 25-70 years were examined radiographically on 2 occasions 10 years apart. 194 were dentate on the 2nd examination. The radiographic bone height at the mesial and distal aspect of all teeth was assessed by measuring the distance between the cemento-enamel junction and the bone crest. The morphologic pattern of alveolar bone loss at baseline was assessed for each tooth site. Angular defects were classified as degree 1, 2 and 3, according to increasing defect depth. The change in periodontal bone height over the 10-year period was calculated for each site. The results demonstrated an increased frequency of tooth loss among teeth showing presence of an angular bony defect at baseline; whereas 13% of the teeth with an even pattern of bone loss were lost between the 2 examinations, this proportion was 22%, 46% and 68% for teeth with an angular defect of degree 1, degree 2 and degree 3, respectively. Longitudinal bone loss of greater than or equal to 2 mm occurred more often among sites showing an angular defect when compared to sites with an even alveolar bone morphology.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

BACKGROUND: The aims of this study were to investigate the anti-inflammatory effect and the effect on bone regeneration of hyaluronan in surgical and non-surgical groups. METHODS: In each of 15 individuals, 2 teeth with defects of similar character and magnitude in the upper or lower jaw were chosen. There were at least 2 teeth between the test and the control sites. In the surgical group, a bioabsorbable membrane was used for both test and control sites, and hyaluronan was placed in the intrabony pocket of the test site. In the non-surgical group, the periodontal pockets were scaled and hyaluronan was administered 3 times with an interval of 1 week in the test pockets. Alveolar bone height and bone healing patterns were analyzed using digital intraoral radiographs. Measurements of bone height were performed in the original digital black-and-white radiographs to obtain quantitative data on bone gain or loss. Bone healing patterns were studied with color-coded radiographs, using specially designed software in a personal computer with subsequent combinations of radiographs. Gingival crevicular fluid immunoglobulin (Ig)G, C3, and prostaglandin E2 (PGE2) responses; periodontal probing depth; bleeding on probing; and the presence of plaque were studied to evaluate the anti-inflammatory effect. Data were obtained at baseline before treatment, and at 2 weeks, and 1, 3, 6, and 12 months after treatment. RESULTS: For the surgical treatments, bone height was increased in the test group treated with hyaluronan (mean value 2.2%, corresponding to an average increase of approximately 0.5 mm) and reduced in the control group (mean value -1.8%, corresponding to an average decrease of approximately - 0.4 mm) (P<0.05) after 12 months. For the non-surgical treatments, bone height was reduced by a mean value of -1.1% (corresponding to an average decrease of approximately -0.25 mm) in the test group treated with hyaluronan and -3.3% (corresponding to an average decrease of approximately -0.75 mm) in the control group after 12 months (N.S.). According to the digital color-coded radiographs, the test sites in the surgical and non-surgical groups showed apposition of bone minerals. Immune responses showed no differences during the 12 months studied for the surgical and non-surgical sites. Mean periodontal probing depths were reduced between 2.5 mm and 4.1 mm in the surgical and non-surgical groups. CONCLUSIONS: The observed difference in bone height between test and control sites in the surgical group after 12 months was less than 1 mm, which was only detectable on radiographs. No statistical difference was found on radiographs in the non-surgical group, where a decrease in bone height was found for both groups after scaling. Probing depth reduction after the surgical treatment, as well as after scaling and root planing, was as expected. Hyaluronan in contact with bone and soft tissues had no influence on the immune system in this study. Further studies are needed to determine the extent to which hyaluronan can lead to clinically significant healing of periodontal lesions.  相似文献

Abstract The purpose of the present study was to examine longitudinal alterations in the periodontal conditions of regular dental care attendants, 225 randomly selected patients (age 18–65 years) at 12 community dental clinics in the county of Värmland, Sweden, were subjected to a baseline clinical and radiographic examination in 1978 and to a re-examination in 1990. During the study period, all participants received preventive and therapeutic measures according to decisions made by the dentist on duty in the clinics. The examinations involved assessments of number of remaining teeth, plaque, gingivitis, probing pocket depth, loss of probing attachment and periodontal bone height. The results showed that during the 12 years of monitoring, an average of 0.4 teeth were lost, The % of tooth sites with gingivitis was lower in 1990 (4%) than in 1978 (15%), but no major changes were found for the mean probing pocket depth. The mean probing attachment loss during the 12 years amounted to 0.5 mm. The tooth site analysis revealed that buccal sites had experienced more loss of attachment than lingual and approximal surfaces. Whereas no differences were observed between age groups with respect to longitudinal loss of attachment at lingual and approximal tooth sites, the youngest age group demonstrated more pronounced loss at buccal surfaces than older subjects. The radiographic assessments of the alveolar bone height revealed a mean longitudinal loss amounting to 0.2–0.4 mm in the various age groups. The time-lag comparison revealed less tooth mortality, lower gingivitis scores, less probing attachment loss and less periodontal bone loss in 1990 than in 1978 for subjects of the age 30–53 years, whereas the data were similar for older subjects. The improvement was the result of less breakdown at lingual and approximal sites, whereas no difference was evident for buccal surfaces.  相似文献

PURPOSE: The aim of this study was to indicate the necessity of overcorrection regarding the occurrence of bone height relapse at the end of consolidation period in distracted alveolar bone. MATERIALS AND METHODS: Eleven patients with a total of 17 distractions performed and 43 implants placed were included in this study. Bone height was evaluated on computed tomography before the procedure and on orthopantomographic radiographs following distraction and consolidation. Measurement was performed on the aproximal surfaces of implants and on identical points before and after distraction. RESULTS: The mean of distraction performed was 6.08 +/- 1.82 mm at mesial points and 6.18 +/- 1.90 mm at distal points of measurement. The mean of bone relapse following consolidation period was 1.57 +/- 1.82 mm at the mesial and 1.79 +/- 1.68 mm at the distal aspects of implants. Statistical evaluation revealed that alveolar bone distraction should include 20% of overcorrection for both mesial and distal points of measurements plus 0.34 mm for mesial and 0.52 mm for distal points of measurement. CONCLUSIONS: Occurrence of relapse found in this study indicates that overcorrection should be included when performing alveolar distraction osteogenesis.  相似文献

Objectives: The aim of this study was to evaluate and compare marginal bone loss and clinical outcomes of conventionally and immediately loaded two implants supporting a ball‐retained mandibular overdenture. Materials and methods: Thirty six completely edentulous patients (22 males and 14 females) were randomly assigned into two groups. Each patient received two implants in the canine area of the mandible after a minimal flap reflection. Implants were loaded by mandibular overdentures either 3 months (conventional loading group) or the same day (immediate loading group) after implant placement. Ball attachments were used to retain all overdentures to the implants. Vertical and horizontal alveolar bone losses were evaluated in both groups 1 and 3 years after implant placement using multislice computed tomography, which allow evaluation of peri‐implant buccal and lingual alveolar bone. Plaque scores, gingival scores, probing depths and periotest values (PTVs) were evaluated at 4 months (baseline), 1 and 3 years after implant placement. Clinical and radiographic evaluations were performed at distal, labial, mesial and lingual peri‐implant sites. Results: After 3 years of follow‐up period, the immediate loading group recorded significant vertical bone loss at distal and labial sites than the conventional loading group and no significant differences in horizontal bone loss between groups were observed. Probing depth at distal and labial sites in the immediate loading group were higher than the conventional loading group, while plaque scores, gingival scores and PTVs showed no significant differences between the two groups. A low level of positive correlation between plaque scores, gingival scores, probing depths and vertical bone loss was noted. Conclusion: Immediately loaded two implants supporting a ball‐retained mandibular overdenture are associated with more marginal bone resorption and increased probing depths when compared with conventionally loaded implants after 3 years. The bone resorption and probing depths at distal and labial sites are significantly higher than those at mesial and lingual sites. A cross-sectional epidemiologic survey was conducted of 618 Navajo Indians, aged 14-19, resident in a boarding school in New Mexico. Periodontal status was assessed by clinical measurements of attachment level and gingival bleeding, and evidence of alveolar bone loss from standardized bitewing radiographs. Attachment level and gingival bleeding were measured at 24 posterior interproximal sites (six sites in each quadrant): the mesio-buccal aspect of the second molar; the disto-buccal and mesio-buccal aspects of the first molar and second premolar; and the disto-buccal aspect of the first premolar. Alveolar bone level was measured from radiographs at the corresponding approximal surfaces of the same teeth. Attachment loss was considered present when the distance from the CEJ to the base of the pocket was greater than 1 mm; bone loss was considered present when the radiographic distance from the CEJ to the alveolar crest was greater than 2 mm, and gingival bleeding was considered present if bleeding occurred immediately after gentle probing. Attachment loss was evident at one or more sites in 88.7% of the population, 45.9% of the subjects had attachment loss at eight or more sites, and 101 subjects (16.3%) had one or more sites with at least 4.0 mm of attachment loss. Bone loss was present at one or more sites in 89.2% of the population, 28.6% had eight or more affected sites, and 4.7% (29 subjects) had one or more sites with at least 2.0 mm of bone loss. The aim of this study was to compare the prevalence and severity of periodontal disease in age- and sex-matched adult long- and short-duration insulin-dependent diabetics and non-diabetics. The study involved 82 subjects with long- and 72 with short-duration diabetes and 77 non-diabetics, all aged 20-70 years. The clinical and radiographic examination comprised recordings of the number of existing teeth, absence or presence of plaque and supra- and subgingival calculus, gingival conditions, probing pocket depth and alveolar bone level. There were no significant differences in the number of existing teeth or presence of plaque and supra- and subgingival calculus between long- and short-duration diabetics and non-diabetics. Diabetics, irrespective of the duration of the disease, had a higher prevalence of sites with gingivitis than non-diabetics. Overall, there were no significant differences between the groups regarding the prevalence of tooth surfaces with probing pocket depths of 4 and 5 mm. However, on comparison between age subgroups, long-duration diabetics younger than 45 years had significantly more 4 and 5 mm pockets than non-diabetics. Long-duration diabetics altogether had significantly more tooth surfaces with probing depth greater than or equal to 6 mm than non-diabetics. The radiographs of alveolar bone height exhibited significantly more extensive alveolar bone loss in long-duration diabetics aged 40-49 years than in short-duration diabetics and non-diabetics. This, together with the increased number of subjects belonging to classification groups with severe periodontal disease experience among long-duration diabetics, indicates more periodontal disease in these diabetics.  相似文献

BACKGROUND, AIMS: The purpose of this 2-year longitudinal clinical study was to determine the impact of smoking on alveolar bone height and density changes in postmenopausal females. METHODS: 59 postmenopausal women completed this study, including 38 non-smokers and 21 smokers. All subjects had a history of periodontitis, participated in 3- to 4-month periodontal maintenance programs and were within 5 years of menopause at the study outset. 4 vertical bite-wing radiographs of posterior sextants were taken at baseline and 2-year visits. Radiographs were evaluated using computer-assisted densitometric image analysis (CADIA); changes in interproximal alveolar bone density and changes in alveolar bone height were determined. Relative clinical attachment levels (RCAL) and presence/absence of plaque and bleeding on probing were recorded. RESULTS: Smokers exhibited a higher frequency of alveolar bone height loss (p<0.05) and crestal (p<0.03) and subcrestal (p<0.02) density loss relative to non-smokers. Smokers exhibited a trend (p<0.08) toward a higher frequency of > or =2.0 mm RCAL loss over the 2-year period. Plaque and bleeding on probing did not differ between smokers and non-smokers. A significant interaction, determined by repeated measures ANOVA, was noted between systemic bone mineral density (BMD) at the lumbar spine and smoking on alveolar bone density change (p<0.05). Only non-smoking patients with normal BMD realized a mean net gain in alveolar bone density; osteoporotic/osteopenic subjects (n=25) and smokers lost alveolar bone density. CONCLUSION: Postmenopausal female smokers were more likely to lose alveolar bone height and density than non-smokers with a similar periodontitis, plaque and gingival bleeding experience. Abstract The aim of this investigation was to determine the relationship between loss of radiographic crestal bone height and probing attachment loss. As part of this aim, we are introducing a new method for determination of a threshold for significant probing attachment loss which takes into account the error contributed by alteration in tissue tone. 57 adults with established periodontitis were selected. Radiographs were taken using the Rinn alignment system. Crestal change was determined from bone height measurements on digitized images of pairs of radiographs using the “side-by-side” technique of analysis developed recently by us. Probing attachment loss was measured using the Florida electronic probe system. Radiographic and probing measurements were made at baseline and after 1 year. No treatment was given during this period. A direct and significant relationship was observed between radiographic bone loss and probing attachment loss on a site basis (p= 0.0001, r2= 0.018) and between subject means (p= 0.0014, r2= 0.16). Radiographic and probing attachment change at all categories of sites, dichotomously classified as to not changing or loosing indicated 13% of sites were loosing by measurement of radiographic change and 9.6% were Loosing by measurement of attachment change. Concordance in radiographic and attachment level change was found in 82% of sites examined. The relative diagnostic import of change in probing attachment or change in radiographic bone height requires treatment outcome studies based on use of diagnostic information of the 2 measuring techniques used singly and in combination.  相似文献

The aim of the present study was to use a decision making model in order to assess the periodontal treatment needs of a random sample of employees in a large Swedish industrial corporation. The model used provided data on critical bone loss limits for different tooth types and ages, beyond which treatment must be initiated, in order to fulfill the goal of maintaining all teeth in a functional state throughout life. A sample comprising 192 subjects belonging to four age strata (31-35 yr, 41-45 yr, 51-55 yr, and 61-65 yr) was involved. From each subject, a full mouth series of intra-oral radiographs were available. The radiographic bone height was assessed at the mesial and distal aspect of all teeth by measuring the distance between the cementoenamel junction and the bone crest. The clinical examination included assessments of plaque, gingivitis, probing pocket depth, and probing attachment level. The results revealed that (i) only 3.1% of all approximal tooth sites exhibited radiographic bone loss exceeding the critical limit, (ii) all individuals and 70% of the approximal tooth sites were in need periodontal treatment when presence of gingival inflammation (bleeding on probing) was employed as the single criterion for therapeutic intervention, (iii) the proportion of individuals and tooth sites requiring treatment amounted to 98% and 27%. respectively, when a probing pocket depth of at least 4 mm was included as an additional criterion, and 54% and 4.1%, respectively, if a probing depth threshold of 6mm was used, while (iv) the use of bleeding on probing in combination with radiographic bone loss beyond the critical limits disclosed a need of treatment in 40% of the subjects and 2.5% of the approximal tooth sites.  相似文献

The longitudinal effects of periodontal therapy in patients without a frequent periodontal maintenance program have been minimally documented. In this study we used Duckworth's modification of the Schei Ruler Technique to assess the difference in bone level around individual teeth treated for periodontal disease in subjects receiving infrequent posttherapy maintenance (less than or equal to 1 time/year). Crestal bone height differences were evaluated using the initial presenting series of long cone parallel radiographs of 23 subjects with their subsequent posttherapy analogous radiographic series taken 5.4 +/- 2.9 years later. Bone loss was defined as a reduction in the alveolar crest of greater than 50% of the radiographic crown height which corresponds to approximately 4 mm in posterior and 5 mm in anterior teeth. The loss for each tooth was expressed as a per cent of the measured height of the crown after conversion from millimeters using the mean crown-root ratio for each tooth. We found increased alveolar bone loss and tooth loss in subjects examined posttherapy when compared with conditions present when each subject initially presented for periodontal treatment. Our data suggest that molar teeth are more at risk than incisors and cuspids and that a lack of periodontal maintenance care and inadequate plaque control contribute to progressive bone loss following treatment.  相似文献
