The patellar component of a total knee replacement (TKR) is the most frequent source of non-septic complications after total knee arthroplasty. Fracture of patellar pegs in all polyethylene patellar components is a very rare occurrence. We report such a case of a patellar polyethylene fracture in a 72-year-old female patient 10 years after TKR. Due to patellar malalignment and high level of activity, the patellar components failed in this patient. This was treated arthroscopically by removing the components that failed and leaving the patella unresurfaced. We followed up the patient postoperatively and her symptoms were substantially resolved.  相似文献

Purpose

To demonstrate that lateral to medial patellofemoral force and pressure ratios could be a surrogate marker of retinacular tension and patellar tracking.

Methods

The patellofemoral forces of six knees from three fresh-frozen half-body female cadavers were evaluated with a capacitive sensor under simulated operative conditions in six staged clinical scenarios: native knees, knee arthroplasty without patellar resurfacing, resurfaced knee and patella, resurfaced knee and patella with lateral release, gender-specific knee arthroplasty with patella resurfacing, and gender-specific knee arthroplasty with lateral release. Maximum force and peak pressure were simultaneously recorded during three to four ranges of motion. Average values were compared between lateral and medial patellofemoral compartments as an objective measure of patellar tracking for the different settings.

Results

Significant differences in lateral and medial force and pressure differentials were seen in most scenarios despite clinically normal patellar tracking. Lateral to medial ratios of maximum force and peak pressure significantly increased after TKA (2.9, 2.1) and after patella resurfacing (2.8, 2.6) compared to the native knee (1.6, 1.8). Addition of a lateral release in resurfaced knees decreased the ratio of lateral to medial patellofemoral forces and pressures as did gender knee arthroplasty (1.5 and 1.1, 2 and 1.3, respectively). Pressure and force values most closely resembled the native knee in the resurfaced knee/resurfaced patella with lateral release and in the gender knee arthroplasty scenarios.

Conclusions

Use of lateral to medial patellofemoral force ratios as a surrogate objective marker for patellar tracking was validated in this study by decreasing ratios observed after lateral release in TKA and with gender-specific implants.  相似文献

Standard surgical exposure reduces blood flow to the patella during total knee arthroplasty (TKA). Reduction of patellar blood flow has resulted in patellofemoral complications including osteonecrosis and patellar fracture, necessitating revision surgery. In TKA, avoiding patellar eversion is one type of minimally invasive surgery (MIS) technique. This study is the first to measure patellar blood flow during MIS TKA with the knee in both extension and 90° of flexion followed by lateral retraction and then eversion of the patella. Patellar blood flow was measured using laser Doppler flowmetry in 40 patients during MIS TKA. A significant reduction in flow was noted when the leg was flexed from full extension to 90°. Eversion of the patella reduced patellar blood flow. MIS TKA without patellar eversion may be useful for preventing a reduction in patellar blood flow.  相似文献

It is often suggested that patella tracking after total knee arthroplasty (TKA) with an asymmetrical patella groove is more physiological than with a symmetrical patella groove. Therefore, this study tried to address two questions: what is the effect of TKA on patella tracking, and is patella tracking after asymmetrical TKA more physiological than patella tracking after symmetrical TKA? The patellar and tibial kinematics of five cadaveric knee specimens were measured in the intact situation, after the incision and suturing of a zipper, and after placement of a symmetrical TKA and an asymmetrical TKA, respectively. The patellae were not resurfaced. The flexion-extension kinematics were measured with an internal and external tibial moment to determine the envelope of motion (laxity bandwidth) of the tibio-femoral and patello-femoral articulation. The kinematics after TKA showed statistically significant changes in comparison to the intact situation: patellar medio-lateral translation, patellar tilt and tibial rotation were significantly affected. No statistically significant differences in knee kinematics were found between the symmetrical and the asymmetrical TKAs. We conclude that conventional TKA significantly changes physiological patello-femoral kinematics, and TKA with an asymmetrical patella groove does not improve the non-physiological tracking of the patella.  相似文献

An increased stress in the patella due to the implantation of a patellar button may also be another potential source of pain in total knee arthroplasty patients. This study assessed the location inside the patella having largest stress change after implantation of an ultra high molecular polyethylene patella button. Finite elements models of the patellae before and after implantation of patellar button were created. Experimentally determined spring constants of muscles and ligaments, and patellofemoral contacting loads were applied to the models at 30 degrees , 60 degrees , and 90 degrees of knee flexion. The Von Mises stress of the intact patella decreased with increased knee flexion, while that of implanted patella increased. Also, the stress range in the implanted patella was 3~9 times higher than in the intact one. The highly stressed region of the intact patella moved proximally with higher knee flexion angles, while that of the implanted model stayed near the central anterior patella. At 90 degrees of knee flexion, the stress in the anterodistal patella increased considerably after implantation of a patella button so that the anterodistal patella may be susceptible to be painful source after the total knee replacement.  相似文献

Early arthroplasty designs were associated with a high level of anterior knee pain as they failed to cater for the patello-femoral joint. Patellar resurfacing was heralded as the saviour safeguarding patient satisfaction and success but opinion on its necessity has since deeply divided the scientific community and has become synonymous to topics of religion or politics. Opponents of resurfacing contend that the native patella provides better patellar tracking, improved clinical function, and avoids implant-related complications, whilst proponents argue that patients have less pain, are overall more satisfied, and avert the need for secondary resurfacing. The question remains whether complications associated with patellar resurfacing including those arising from future component revision outweigh the somewhat increased incidence of anterior knee pain recorded in unresurfaced patients. The current scientific literature, which is often affected by methodological limitations and observer bias, remains confusing as it provides evidence in support of both sides of the argument, whilst blinded satisfaction studies comparing resurfaced and non-resurfaced knees generally reveal equivalent results. Even national arthroplasty register data show wide variations in the proportion of patellar resurfacing between countries that cannot be explained by cultural differences alone. Advocates who always resurface or never resurface indiscriminately expose the patella to a random choice. Selective resurfacing offers a compromise by providing a decision algorithm based on a propensity for improved clinical success, whilst avoiding potential complications associated with unnecessary resurfacing. Evidence regarding the validity of selection criteria, however, is missing, and the decision when to resurface is often based on intuitive reasoning. Our lack of understanding why, irrespective of pre-operative symptoms and patellar resurfacing, some patients may suffer pain following TKA and others may not have so far stifled our efforts to make the strategy of selective resurfacing succeed. We should hence devote our efforts in defining predictive criteria and indicators that will enable us to reliably identify those individuals who might benefit from a resurfacing procedure. Purpose

Patellar tendon rupture after total knee replacement is a rare and highly limiting injury with multifactorial aetiology. Many reconstruction techniques have been described with not very predictable results. The use of allografts has been accepted as a suitable solution.

Methods

A series of seven patients with patellar tendon rupture treated with fresh-frozen tendon allograft reconstruction after knee arthroplasty is presented.

Results

Median follow-up is 25 months (20–31). Functional assessment improved, and the knee society score and knee functional score improved from 26 and 16 to 82 and 55, respectively. Median extension lag was 5° (0°–20°), with a median range of motion of 95° (70–100). Radiological study showed a rise of the patella of 22.26 mm.

Conclusion

The use of fresh-frozen allografts as a solution to patellar tendon ruptures after knee arthroplasty seems to provide acceptable results. Increased patellar height does not seem to affect functionality.

The complications associated with surgery on the extensor mechanism of the knee were reviewed. These complications may be avoided by being mindful of the importance of patient selection, establishing specific diagnoses, and then carrying out proper surgical procedures. Also reviewed were the complications associated with the extensor mechanism in total knee arthroplasty. Patellofemoral complications are the most common complications of total knee arthroplasty. The importance of precise surgical technique in aligning the extensor mechanism was emphasized. Caution is probably advisable in the use of metal-backed patellar components for the time being. Lastly, the relatively rare, but important, complications of reflex sympathetic dystrophy, infrapatellar contracture syndrome, and patella infra were reviewed.  相似文献

Several controversies exist regarding the surgical difficulties and the results of total knee arthroplasty performed after failed valgus high tibial osteotomy (HTO), and the main subject is the change in patellar height that results as patella baja or infera. The purpose of this retrospective study was to evaluate patellar height after valgus HTO and the measurement methods that were actually used. Eighty-five knees that were subjected to valgus HTO were evaluated both preoperatively and postoperatively according to the Insall-Salvati Index (ISI), Blackburne-Peel Index (BPI), and Caton Index (CI) to assess any alteration of patellar tendon height that was present. All cases underwent closing wedge osteotomy with three staples or plate internal fixation and were allowed early range of motion. Significant decrease in mean patellar height ratios was detected according to all three indexes at 85 months of mean follow-up. The percentages of the decrease were 8.26% in ISI, 9.08% in BPI, and 6.34% in CI. Two knees showed patella infera according to ISI, one according to BPI and three according to CI. There were no significant correlations between the indexes and clinical status of the patients. Elevation of patella relative to the femur in closing wedge valgus HTO procedure due to the shortening of the segment between tibial tuberosity and joint line is normally expected. A significant decrease in patellar height according to ISI suggests that there should be patellar tendon shortening as patellar height cannot be changed. BPI and CI for determining patellar height in valgus HTO do not accurately measure the alteration of patella because they may affect the tibial inclination and antero-posterior translation of the proximal fragment. Another measuring system based on femoral reference points should be proposed to determine the exact change of patellar height in the valgus HTO procedure.  相似文献

Although the results of total knee arthroplasty continue to improve, problems related to the patellofemoral joint remain significant. This study examined the factors affecting patellar alignment after total knee arthroplasty and subsequent changes in 56 knees during a postoperative period of 5.3 years. None of the knees examined displayed any clinical complications of the patellofemoral joint; no revision surgeries were necessary, with acceptable patellar alignment on average. The patellar resection angle had a strong influence on patellar alignment. Thinning of the patellar remnant on the medial side can increase postoperative lateral tilt, which leads to a need for lateral retinacular release. Although the changes in patellar alignment were minimal, the tendency that postoperative varus alignment resulted in patellar lateral tilt was observed. As postoperative femorotibial misalignment can lead to patellofemoral problems after total knee arthroplasty, surgeons need to pay scrupulous attention to femorotibial alignment and proper patellar preparation to decrease patellofemoral complications.  相似文献

BACKGROUND: Medial subluxation of the patella is a rare, disabling condition that has iatrogenic and traumatic causation. To date, only open reconstructions have been reported for operative treatment of this condition, but these procedures have a high rate of complications, revisions, and subsequent surgery. This is the first study to present the results of arthroscopic medial retinacular release for treatment of this problem. HYPOTHESIS: Arthroscopic release will alleviate painful medial patellar subluxation. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Nine knees (7 patients) with painful, recurrent medial subluxation of the patella that occurred spontaneously (2 knees) or after a lateral release (5 knees) or an injury (2 knees) were treated with an arthroscopic medial retinacular release. The retinaculum was released 2 cm medial to the superior pole of the patella down to the anteromedial portal. At a mean follow-up of 2.7 years (range, 1-8 years), all knees were evaluated, and the results were graded according to the Merchant and Mercer rating scale. RESULTS: The mean age of the patients was 25 years (range, 15-38 years), and the mean duration of the subluxations before surgery was 28 months (range, 6-48 months). In all 7 patients (9 knees), the medial release relieved their medial subluxation and knee pain, and there were 6 excellent and 3 good results. There were no complications or further realignment surgery needed after this procedure. CONCLUSION: An arthroscopic medial retinacular release will successfully treat painful medial subluxation of the patella.  相似文献

Permanent post-traumatic patellar dislocation is a rare condition, and total knee arthroplasty (TKA) can be performed for osteoarthritic knees with this condition. There have been only a few reports about using the standard medial parapatellar approach and extensive lateral release or extensor mechanism realignment for patients with permanent post-traumatic patellar dislocation. We performed two TKAs with using a subvastus approach and lateral release for osteoarthritic knees with permanent post-traumatic patellar dislocation. The patients were well satisfied with their results. The patellae tracked very well in the femoral trochlear groove throughout the full range of motion after the operations. Bone scans were done 1 year postoperatively on both patients, and the scans showed normal vascularity for the patella.  相似文献

Purpose

Considering the discrepant results of the recent biomechanical studies, the purpose of this study was to simulate dynamic muscle-loaded knee flexion with a large number of specimens and to analyse the influence of total knee arthroplasty (TKA) without and with patellar resurfacing on the patellofemoral pressure distribution.

Methods

In 22 cadaver knee specimens, dynamic muscle-loaded knee flexion (15°–90°) was simulated with a specially developed knee simulator applying variable muscle forces on the quadriceps muscles to maintain a constant ankle force. Patellofemoral pressures were measured with flexible, pressure-sensitive sensor foils (TEKSCAN) and patellofemoral offset with an ultrasound motion-tracking system (ZEBRIS). Measurements were taken on the native knee, after total knee arthroplasty and after patellar resurfacing. Correct positioning of the patellar implant was examined radiologically.

Results

The maximal patellofemoral peak pressure partly increased from the native knee to the knee with TKA with intact patella (35°–90°, p < 0.012) and highly increased (twofold to threefold) after patellar resurfacing (20°–90°, p < 0.001). Concurrently, the patellofemoral contact area decreased and changed from a wide area distribution in the native knee, to a punctate area after TKA with intact patella and a line-shaped area after patellar resurfacing. Patellar resurfacing led to no increase in patellar thickness and patellofemoral offset.

Conclusions

Despite correct implantation of the patellar implants and largely unchanged patellofemoral offset, a highly significant increase in pressure after patellar resurfacing was measured. Therefore, from a biomechanical point of view, the preservation of the native patella seems reasonable if there is no higher grade patellar cartilage damage.  相似文献

The condition referred to as patella infera is characterized by a permanent shortening of the patellar ligament and is associated with a severely limited range of motion of the knee joint. Patella infera is a common complication of injury or surgery to the knee joint, and it is generally considered a condition that presents after a variable, albeit considerable time after injury. A review of the knee roentgenograms of patients with arthrofibrosis and patella infera showed this condition to be an immediate complication of knee fractures. Therefore, in order to better define the development of patella infera, the patellar height was studied during treatment of 146 knee joint fractures (46 supracondylar fractures, 50 patellar fractures and 50 tibial plateau fractures). Using the Caton-Deschamps method patelaa infera was detected in eight cases (17.39%) immediately after supracondylar fractures, in six cases (12%) after patellar fractures, and only in two cases (4%) after tibial plateau fractures. The patellar ratio remained unchanged or worsened further after treatment, and its incidence was unaffected by the type of treatment. These observations rule out etiologies such as inflammatory or algodystrophic phenomena and quadriceps inhibition, because the reduction of aptellar height was evident immediately and persisted at follow-up examinations.  相似文献

We hypothesized that changes in patellar thickness following patellar resurfacing affect patellar tilt in total knee arthroplasty (TKA) patients. The study enrolled 272 TKAs and categorized them into four groups according to change in patellar thickness: (A) thinner by 1 mm or more, (B) equal or thinner by less than 1 mm, (C) thicker by 1 mm or less, and (D) thicker by more than 1 mm. Patellar tilt angle was measured postoperatively using Merchant radiography. There were no significant differences in postoperative patellar tilt among groups A, B, and C (n.s). However, the postoperative patellar tilting angle of group D was significantly higher than that of all other groups (P < 0.05). Postoperative patellar tilt increased when the postoperative patella was >1 mm thicker than the preoperative patella.  相似文献

The position of the patella was studied prospectively in both knees of 45 consecutive patients (21 male and 24 female patients aged 16-48 years who were competitive [n = 17] or recreational [n = 28] athletes) who had unilateral patellofemoral pain syndrome without symptoms or signs of patellar instability at initial examination. In each knee, standardized anteroposterior, lateral, and tangential radiographs were obtained and six indexes of patellar position (the ratio of the patellar tendon to the greatest diagonal length of the patella, sulcus angles, lateral patellofemoral angle, lateral patellar displacement, patellofemoral index, and knee angle) were measured. When healthy and affected knees were compared, high riding of the patella due to long patellar tendon (patella alta) was the only definite finding in the affected knees. The shape of the intercondylar sulcus and the mediolateral position of the patella were identical in both knees, providing no evidence for patellofemoral incongruence or lateral patellar tilt. Results of this study strongly suggest that idiopathic retropatellar pain is closely associated with patella alta.  相似文献

Patellar tendon shortening after anterior cruciate ligament reconstruction may be associated with anterior knee pain or patellofemoral arthritis. The present study was designed to compare postoperative changes in patellar tendon length after anterior cruciate ligament reconstruction between patellar tendon and hamstring tendon autograft. Magnetic resonance images of both knees (operated and healthy) and functional outcome were documented at least 1 year postoperatively in 16 patellar tendon harvested patients and in 32 hamstrings harvested patients. Patellar tendon length, patella length and Insall–Salvati ratio were measured. The operated knee values were compared to the respective values of the non-operated control knees. A significant 4.2 mm or 9.7% patellar tendon shortening in patellar tendon group and a non-significant 1.14 mm or 2.6% shortening in hamstrings group was detected. No significant difference was detected in terms of major shortening—patella baja—(6% for the patellar tendon group vs. 0% for the hamsting group). There was no significant difference in anterior knee problems between the two groups as evidenced by the Shelbourne score (94 for the patellar tendon group vs. 98 for the hamsting group). Harvesting of the patellar tendon for anterior cruciate ligament reconstruction resulted in a significant shortening of the remaining tendon. In contrast harvesting of the hamstring tendons did not affect significantly the patellar tendon length. However, the incidence of patella baja and overall functional outcome was not significantly different between the two groups. Patellae in small knees, or after severe patellar erosion, may be vulnerable to fracture after resection during arthroplasty. The patellar remnant may be thin, while the polyethylene component has a standard thickness. Anterior patellar bone strain was measured in cadaver knees loaded via the quadriceps, from 0 to 90° flexion, with the patella intact, and after resections to 16, 13 and 11 mm thick and replacement by an 8-mm-thick polyethylene component. Strain increased significantly with knee flexion with constant 500 N quadriceps tension. Resection caused significant changes from intact values in knee flexion and extension and no significant effect at 30° flexion. In flexion, bending caused the anterior surface to become more convex, with high tensile bone strains. In extension, resection caused negative anterior strains, representing bending in the opposite direction, with large tensile strains on the cut posterior surface. For normal activities, such as rising from a chair (1.8 kN quadriceps tension) the patella appears safe against fracture with a minimal resection to 16 mm thick. An eroded patella resected to 11 mm thickness may be at risk of fracture with that loading.  相似文献

BACKGROUND: High tibial osteotomy has been associated with significant complications, including delayed union or nonunion, loss of correction, arthrofibrosis, and patella infera. HYPOTHESES: A technique for opening wedge osteotomy that incorporates an autogenous iliac crest bone graft will prevent delayed union or nonunion, allow early rehabilitation and weightbearing, and prevent knee arthrofibrosis and patella infera. Secondly, the authors' methods for calculating the desired correction of valgus alignment prevent undesired alterations in tibial slope. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: A total of 55 consecutive patients who underwent high tibial osteotomy were observed at a mean of 20 months postoperatively. Preoperative and postoperative measurements of radiographs were conducted by independent examiners for bony union, tibial slope, and patellar height. The osteotomy opening size ranged from 5 to 17.5 mm; 35 knees (64%) had openings < or =10 mm, and 20 knees (36%) had openings >11 mm. RESULTS: The osteotomy united in all patients. Three patients had a delay in union, which resolved by 6 to 8 months postoperatively. A loss of fixation occurred in 1 patient, who admitted to full weightbearing immediately after surgery; the osteotomy required revision. The iliac crest graft site healed without complications, and there were no infections, loss of knee motion, nerve or arterial injuries, alterations in tibial slope, or cases of patellar infera postoperatively. Full weightbearing was achieved at a mean of 8 weeks (range, 4-11 weeks) postoperatively. CONCLUSIONS: The operative technique including use of an autologous iliac crest bone graft in addition to a progressive rehabilitation program successfully prevented nonunion, change in tibial slope, and knee arthrofibrosis in this study.  相似文献

Radiography of three boys 5-6 years old with anterior knee pain revealed increased density and irregularity of the patellar ossification centers. A search of the literature suggested that it was osteochondrosis of the primary ossification center of the patella, named after K?hler. Continuing search also revealed that it might be a normal variant instead of an osteochondrosis. A short course of activity modification led to marked improvement in all. The patients were either asymptomatic or experienced minor symptoms 18-34 months after presentation. It was concluded that the process, either physiological or pathological, has a benign course and favorable prognosis.  相似文献
