To our knowledge, the simultaneous coronal fractures of the hamate, capitate, and trapezoid have not been previously reported
in the literature. There were few reports about coronal fractures of the hamate caused only by axial loading to the ulnar
column. We report 43-year-old man who presented with painful swelling on the dorso-ulnar side of the wrist after striking
a wall with his right clenched fist. The computed tomography scan demonstrated coronal fractures through the hamate, capitate,
and trapezoid. We performed closed reduction and fixation with a percutaneous compression screw. At the last follow-up, the
patient was able to return to sports activities without any symptoms. 相似文献

Giant cell tumors in the small bones of the hand are unusual, particularly in the carpus. A 28-year-old woman developed a giant cell tumor in the capitate. After biopsy, she was treated with complete excision of the capitate with no reconstruction. Five years later she had excellent hand function and no tumor recurrence. A careful roentgenographic and pathologic examination will distinguish between aneurysmal bone cysts and giant cell reparative granulomas. Complete excision of the involved bone is recommended. 相似文献

Fractures of the carpal bones in the coronal plane are encountered rarely and their diagnosis usually is delayed. Coronal hamate fractures are relatively well described but a coronal capitate fracture occurring simultaneously with a coronal hamate fracture is extremely rare. Giant Cell Tumors are rare tumors in the young adult and localisation in the hand is even more exceptional. In the absence of adjuvant treatment, the literature reports a strong potential for local recurrence of between 75 and 86%. These tumours also have a risk of sarcomatosis degeneration and pulmonary metastasis. The case report concerns a 24-year-old patient, with a Giant Cell Tumour of the capitate initially diagnosed on simple curettage, and we describe her treatment together with the treatment of a subsequent reccurrence. A literature review will also highlight the current knowledge of this disease. 相似文献

Giant cell tumors of the flexor sheath are the second most common tumors of the hand. We present a case that is unusual in the extent of the tumor and the pronounced bony invasion, which is rare. Preoperative diagnosis was complicated by a history of gout in our patient. 相似文献

We report a patient with a unique complex carpal dissociation involving complete and divergent dislocation of the hamate, capitate, and trapezium with total ligamentous disruption of the distal carpal row and midcarpal joint, associated with a transverse shaft fracture of the fifth metacarpal. The mechanism of injury was a sustained torsional force resulting in a simultaneous distraction and bidirectional dorsopalmar (ulnar)-palmodorsal (radial) crushing force at the carpus. 相似文献

Metastatic tumors of the hand are rare, with fewer than 200 cases reported in the literature. Renal cell carcinoma is among the most common tumors to metastasize, but we are not aware of reports of this tumor metastasizing to the carpal bones. We describe a case of renal cell carcinoma that metastasized to the triquetrum to draw attention to the potential for such lesions developing within the hand and wrist. 相似文献

IntroductionGiant cell tumors of soft tissue (GCTs) are a relatively rare entity. It is a distinct but uncommon group of neoplasms morphologically identical to osseous giant cell tumor. The diffuse type of extra-articular GCT arising within muscle is a rare benign soft tissue tumor with a wide spectrum of clinical presentation.Presentation of caseThis article reports a rare case of a 44-year-old woman with a mass arising from her right thigh. MRI showed only a few areas of low T2 signal in a mass that was hyper intense to muscle. Histopathology of this lesion located within the right quadriceps muscle revealed admixture of multinucleated giant cell with mononuclear cells. This patient was treated by surgical resection and followed up for recurrence.DiscussionDiffuse-type GCTs are commonly located in the periarticular soft tissues, but on rare occasions these lesions can be purely intramuscular or subcutaneous and can be challenging to diagnose. Characteristic findings include gradient echo secondary to hemosiderin deposition, and the low signal on T2.ConclusionBecause extra-articular diffuse-type GCTs are rare, the differential diagnosis is challenging. The clinical outcomes of diffuse-type GCTs are unclear because of their rarity. Benign clinical course is expected if the lesion is excised adequately. 相似文献

Clear cell meningioma is a rare morphologic form of meningioma. This case report describes a very rare case of multifocal
clear cell meningioma in the thoracic and lumbar spine. A 9 year old patient presented with back pain and popliteal pain,
which were aggravated by trunk motion and knocking. Magnetic resonance imaging (MRI) suggested a meningioma at the L2 level.
An intradural extramedural tumor attached to the cauda equina was surgically removed and pathological diagnosis was clear
cell meningioma. The patient began to complain of back pain at the Th12 level 4 months postoperatively. Retrospective analysis
of MRI before the first operation revealed the small tumors at Th12 and L5 levels; comparison with a new MRI showed the rapid
growth in 4 months. Adjunctive surgical resection was carried out and the back pain disappeared. These tumors had the same
pathological findings. In conclusion, in young patients with suggested meningioma in the lumar spine, clear cell meningioma
should also be considered, and the possibility of multifocal occurrence and postoperative recurrence must be taken into account. 相似文献

Multicentric giant cell tumour (GCT) is extremely rare; no case has been previously reported where two lesions occurred in the same foot at different sites. We report a case involving the calcaneus and subsequently the 3rd toe of the same foot and review the reported literature. In established cases of multicentricity, the histopathology has to be properly reviewed and the patient has to be followed up for a longer time with serial whole body assessment to pick up any subsquent lesions.
