This study assessed individual-level effects of adding micro-enterprise services to a peer-mediated HIV/AIDS intervention
among 227 female sex workers (FSWs) in Kenya. Survey data were collected in May–July 2003 and July–August 2005. Two-thirds
of participants had operational businesses by end-line survey. Nearly half reported to have stopped sex work. Self-reported
weekly mean number of all sexual partners changed from 3.26 (SD 2.45) at baseline to 1.84 (SD 2.15) at end-line survey (P < 0.001). Weekly mean number of casual partners did not change significantly. Weekly mean number of regular partners changed
from 1.96 (SD 1.86) to 0.73 (SD 0.98) over the follow-up period (P < 0.001). Consistent condom use with regular partners increased by 18.5% and remained above 90% with casual partners. Micro-enterprise
services may empower FSWs by giving them an alternative livelihood when they wish to exit or reduce reliance on sex work.
Determinants of successful business operation by FSWs deserve further research. 相似文献

Stigma associated with HIV and risk behaviors is known to be a barrier to health care access for many populations. Less is known about female sex workers (FSW) in Russia, a population that is especially vulnerable to HIV-infection, and yet hard-to-reach for service providers. We administered a questionnaire to 139 FSW to better understand how stigma and discrimination influence HIV service utilization. Logistic regression analysis indicated that HIV-related stigma is negatively associated with uptake of HIV testing, while sex work-related stigma is positively associated with HIV testing. HIV-positive FSW are more likely than HIV-negative FSW to experience discrimination in health care settings. While decreasing societal stigma should be a long-term goal, programs that foster inclusion of marginalized populations in Russian health care settings are urgently needed. 相似文献

The aim of this article is to provide a nuanced analysis of the transformation of sexuality that occurs with age. Drawing on several ethnographic narratives of married men and women over the age of 60 in Japan, this article provides insight into gender-specific experiences of marital sexuality, the impact of extra-marital sexual activity on marital relationships, and the development of “sibling-like” relationships as couples grow older. It also highlights the continuing importance of sexual desire in later life, especially to men’s sense of self. This article contributes to the growing scholarship on the sexuality of older persons around the world and makes a case for the importance of the life history interview as a methodology for illuminating the complex relationship between gender, sexuality, and aging. 相似文献

The purpose of this study was to develop a soap opera video, A Story about Toni, Mike, and Valerie, designed to communicate HIV risk reduction themes. The study evaluated viewing the video and responding to audio computer
assisted self-interview (ACASI) on a handheld computer. The sample was 76 predominately African American women, aged 18–29,
in sexual relationships with men. Data were collected in urban neighborhoods in the northeastern United States. A pretest-posttest
control group design with systematic assignment indicated statistically significant reduction in expectations to engage in
unprotected sex in the experimental group. The handheld computer was found to be acceptable to view the near feature length
video and complete ACASI. To date, no study has reported on use of video and ACASI on a handheld device to reduce HIV risk.
The significance is the potential to stream health promotion videos to personal devices, such as cell phones. 相似文献

The purpose of this study was to examine whether HIV-positive women experience regret as a consequence of disclosing their
HIV serostatus. Participants for this study were 73 HIV-positive women involved in a longitudinal study of HIV disclosure.
Results revealed that overall, participants experienced little regret. Fifty nine percent of women experienced no regret and
71% had regret percentages that were less than 10%. Results indicated that all estimated odds ratios were not statistically

Little is known about female condom use among female sex workers who inject drugs (FSW-IDUs) in Northern Mexico, where HIV/STI prevalence is high. We examined the prevalence and correlates of female condom use and interest in female condom use among FSW-IDUs aged?>18?years in Tijuana and Ciudad Juárez, Mexico enrolled in a behavioral intervention designed to reduce high-risk sexual and injection behaviors. Of 621 FSW-IDUs, 8?% reported ever using female condoms, and 67.2?% expressed interest in trying female condoms. Factors independently associated with female condom use were having had a client become angry at the suggestion of using condoms and having engaged in unprotected vaginal sex with non-regular clients. Factors independently associated with interest in using female condoms were lifetime physical abuse and lifetime sexual abuse. Increasing the availability of female condoms and providing education on their use in the context of drug use and violence is recommended. 相似文献

While national HIV prevalence remains low in China, female sex workers (FSWs) have become infected at high rates. Free voluntary
HIV counseling and testing (VCT) has been offered in recent years; however, its utilization rate is low. This study explored
factors related to FSWs’ willingness to utilize a VCT clinic. Qualitative interviews informed by the Ecological Perspective
were conducted to interview 17 FSWs and 12 managers from 23 selected entertainment establishments in Jinan, the capital of
Shandong province in northern China. While the majority of FSWs professed willingness to use VCT services, they described
barriers to actual utilization of services which included: misunderstandings about HIV; low perceived risk and HIV prevalence;
mistrust of the free VCT; and especially anxiety about the implications of possible test results. This research suggests that
increasing FSWs’ utilization of VCT will require increased knowledge of HIV and VCT, and acceptance of testing and on-site

Rigorous HIV-related data for the homeless population in Jamaica is limited. A cross-sectional survey using a venue-based sampling approach was conducted in 2015 to derive HIV prevalence and associated risk factors. Three hundred twenty-three homeless persons from the parishes of St. James, St. Ann, Kingston, and St. Andrew (the main urban centers) participated. HIV prevalence was 13.8%, with a difference in gender (males 11.6%, females 26.7%, P?=?.007). Sex work, multiple partnerships, incarceration, non-injecting drug use, and female rape were common among the participants. Long-term, multilayered, HIV-specific, female-focused interventions are required for the population, along with additional female-centric research. 相似文献

This intervention sought to improve overall quality of life and health behavior in women living with human immunodeficiency
virus (HIV). We contrasted the effect of a group cognitive behavioral stress management expressive supportive therapy (CBSM+)
intervention plus a healthier lifestyles (HL) component with an individual educational/informational format plus HL on HIV-medication
adherence. Women, n = 237, predominantly African-American and Latina, living with HIV were recruited from Miami, New York and New Jersey and
randomized to group or individual conditions (ten weekly sessions) plus group or individual HL, i.e., four conditions. Women
reported relatively high levels of adherence at baseline. Participants in any of the group conditions increased self-reported
adherence and emotion-focused coping skills in comparison with individual participation. This study suggests that group interventions

Purpose of the review
For many years, competitive sport has been dominated by men. Recent times have witnessed a significant increase in women participating in elite sports. As most studies investigated male athletes, with few reports on female counterparts, it is crucial to have a better understanding on physiological cardiac adaptation to exercise in female athletes, to distinguish normal phenotypes from potentially fatal cardiac diseases. This review reports on cardiac adaptation to exercise in females.Recent findings
Recent studies show that electrical, structural, and functional cardiac changes due to physiological adaptation to exercise differ in male and female athletes. Women tend to exhibit eccentric hypertrophy, and while concentric hypertrophy or concentric remodeling may be a normal finding in male athletes, it should be evaluated carefully in female athletes as it may be a sign of pathology. Although few studies on veteran female athletes are available, women seem to be affected by atrial fibrillation, coronary atherosclerosis, and myocardial fibrosis less than male counterparts.Summary
Males and females exhibit many biological, anatomical, and hormonal differences, and cardiac adaptation to exercise is no exception. The increasing participation of women in sports should stimulate the scientific community to develop large, longitudinal studies aimed at a better understanding of cardiac adaptation to exercise in female athletes.15.

Non-consensual sex is associated with HIV infection in Africa, but there is little longitudinal data on this association. We describe reported non-consensual sex among women over two decades in southwest Uganda, including associations with incident HIV infection. Between 1990 and 2008, individuals in a population cohort who recently seroconverted to HIV were enrolled into a clinical cohort, along with randomly selected HIV-negative controls. Participants were invited to the study clinic every 3 months, and females asked about recent experiences of sex against their will. Associations of non-consensual sex with HIV status were analyzed prospectively using conditional logistic regression, adjusting for age and year of interview, allowing for within-woman correlation. 476 women aged 14–81 enrolled and attended 10,475 visits over 19 years. The results show high levels of repeated non-consensual sex, often long after HIV infection. There was more reporting among women living with HIV compared to HIV-negative women (22 vs 9 %; OR = 2.29, 95 %CI 1.03–5.09), with the strongest associations among married participants. HIV programmes should address repeated sexual coercion before and subsequent to HIV infection. 相似文献

BACKGROUND
Cigarette smoking is an important risk factor for adverse health events in HIV-infected populations. While recent US population-wide surveys report annual sustained smoking cessation rates of 3.4–8.5%, prospective data are lacking on cessation rates for HIV-infected smokers.OBJECTIVE
To determine the sustained tobacco cessation rate and predictors of cessation among women with or at risk for HIV infection.DESIGN
Prospective cohort study.PARTICIPANTS
A total of 747 women (537 HIV-infected and 210 HIV-uninfected) who reported smoking at enrollment (1994–1995) in the Women’s Interagency HIV Study (WIHS) and remained in follow-up after 10 years. The participants were mostly minority (61% non-Hispanic Blacks and 22% Hispanics) and low income (68% with reported annual incomes of less than or equal to $12,000).MEASUREMENTS AND MAIN RESULTS
The primary outcome was defined as greater than 12 months continuous cessation at year 10. Multivariate logistic regression was used to identify independent baseline predictors of subsequent tobacco cessation. A total of 121 (16%) women reported tobacco cessation at year 10 (annual sustained cessation rate of 1.8%, 95% CI 1.6–2.1%). Annual sustained cessation rates were 1.8% among both HIV-positive and HIV-negative women (p = 0.82). In multivariate analysis, the odds of tobacco cessation were significantly higher in women with more years of education (p trend = 0.02) and of Hispanic origin (OR = 1.87, 95% CI = 1.4–2.9) compared to Black women. Cessation was significantly lower in current or former illicit drug users (OR = 0.42 95% CI = 0.24–0.74 and OR = 0.65, 95% CI = 0.49–0.86, respectively, p trend = 0.03) and women reporting a higher number of cigarettes per day at baseline (p trend < 0.001).CONCLUSIONS
HIV-infected and at-risk women in this cohort have lower smoking cessation rates than the general population. Many men who have sex with men (MSM) do not use condoms with their main partners, especially if both parties are of the same HIV status. However, significant proportions of MSM have never tested or recently tested and are unaware of their main partners’ HIV status. A cross-sectional survey was conducted among 524 MSM in Jiangsu, China in 2013–2014. Time-location sampling and online convenience sampling were used to recruit participants. We compared awareness of HIV status and recent HIV testing between participants who had main partners versus those who did not, and identified factors associated with recent HIV testing among men in main partnerships. Participants in main partnerships were significantly more likely to report recent HIV testing and being HIV-negative instead of HIV-unknown compared to participants in casual partnerships only. Overall, 74.5 % of participants were aware of their main partners’ HIV status. Among participants in main partnerships, those who had 2–5 male anal sex partners in the past 6 months and those who reported that their partners were HIV-negative had 2.36 (95 % CI 1.12, 4.97) and 4.20 (95 % CI 2.03, 8.70) fold greater odds of being tested in the past year compared to those who had main partners only and those whose partners were HIV-positive/unknown, respectively. Chinese MSM in main partnerships might be practicing serosorting and may be at lower risk for HIV infection due to increased awareness of main partners’ HIV status and higher uptake of recent testing. 相似文献

This mixed-methods study examined the acceptability of a hypothetical couples-based HIV prevention program for female sex workers and their intimate (non-commercial) male partners in Mexico. Among 320 participants, 67 % preferred couples-based over individual programs, particularly among men. Reasons cited for preferring couples-based programs included convenience and health benefits for both partners. Participants reported that they would benefit from general health information and services, HIV counseling and testing, job training (particularly for men) and other services. However, qualitative interviews revealed that barriers relating to the environment (i.e., poor access to services), providers (i.e., lack of a therapeutic alliance), and intimate relationships (i.e., mistrust or instability) would need to be addressed before such a program could be successfully implemented. Despite women’s concerns about privacy and men’s preferences for gender-specific services, couples-based HIV prevention programs were largely acceptable to female sex workers and their intimate male partners. 相似文献

Identifying venues where women meet sexual partners, particular partners who increase women’s risk of acquiring HIV, could inform prevention efforts. We categorized venues where women enrolled in HPTN 064 reported meeting their last three sex partners as: (1) Formal, (2) Public, (3) Private, and (4) Virtual spaces. We used multinomial logistic regression to assess the association between these venues and women’s individual characteristics and reports of their partners’ HIV risk characteristics. The 2099 women reported meeting 3991 partners, 51 % at Public, 30 % Private, 17 % Formal and 3 % at Virtual venues. Women meeting partners at Formal venues reported more education and condom use than women meeting partners at other venues. Fewer partners met through Formal venues had “high” risk characteristics for HIV than through other venues and hence may pose less risk of HIV transmission. HIV prevention interventions can help women choose partners with fewer risk characteristics across all venue types. 相似文献
