目的:探讨窦口鼻道复合体(OMC)各结构间隙与中鼻甲、鼻中隔间隙狭窄与慢性鼻窦炎的关系及狭窄的主要原因。 方法:采用鼻窦冠状位CT扫描,高空间分辨率算法成像分析经鼻内镜手术治疗的89例(171侧)患者的正常鼻窦和炎症鼻窦的OMC各结构间隙的变异情况。 结果:正常鼻窦与有炎症鼻窦间OMC各结构间隙狭窄的发生率差异均有统计学意义(P<0.01),OMC各结构间隙与慢性鼻窦炎分别行Logistic回归分析,差异均有统计学意义(P<0.01)。 结论:OMC间隙狭窄为鼻窦炎发生的重要危险因素;在鼻窦内窥镜手术治疗鼻窦炎疾病时,必须矫正OMC间隙狭窄。  相似文献

197例鼻窦炎螺旋CT扫描结果分析及临床意义   总被引:2,自引:1,他引:1  
目的:用高分辨率CT对鼻窦炎进行影像学分析,探讨窦口鼻道复合体(OMC)的解剖变异与鼻窦炎发生的关系。方法:选取197例有明显症状、初诊为鼻窦炎的患者(鼻窦炎组),年龄4~74岁;选取无鼻部疾病史的654例为对照组,年龄4~76岁;两组均行鼻窦冠、轴状位CT扫描,对OMC的几种常见的解剖变异进行评估,并统计解剖变异与年龄的关系。结果:存在OMC区解剖变异的患者在11~20岁;慢性鼻窦炎患者的年龄11~20岁;鼻中隔偏曲、钩突变异和下鼻甲肥大的发生在鼻窦炎组和对照组之间差异有统计学意义(P〈0.05)。结论:OMC解剖变异常见,且与慢性鼻窦炎密切相关,特别是鼻中隔偏曲、钩突变异及下鼻甲肥大,纠正这三处的异常对鼻窦炎的预防和治疗有重大意义。  相似文献

目的 探讨鼻腔、鼻窦解剖异常与慢性鼻窦炎的关系。 方法 分析450例患者的CT片,从中分出鼻窦炎组和对照组(无鼻窦炎组),对450例患者存在的鼻腔、鼻窦解剖异常, 如中鼻甲气化及反向歪曲、鼻丘气房、鼻中隔高位偏曲、Haller’s气房、钩突肥大及钩突移位等进行统计分析。 结果 鼻腔、鼻窦解剖异常在鼻窦炎组与对照组的发生率差异有统计学意义(P<0.01)。结论 鼻腔、鼻窦解剖异常与慢性鼻窦炎有相关关系。  相似文献

目的:探讨慢性鼻鼻窦炎及鼻息肉与鼻部解剖变异的关系。方法:分析411例鼻窦冠状位CT资料,分为正常组(A)、慢性鼻鼻窦炎组(B)、鼻息肉组(C)3组。统计中鼻甲、钩突变异及Haller′s气房、鼻中隔偏曲等在3组的发生率,并应用统计软件SPSS 12.0分析其与慢性鼻鼻窦炎及鼻息肉的关系。结果:中鼻甲气化、反向弯曲及直形中鼻甲在A组的发生率分别为23.39%、8.06%、3.23%; 在B组为38.52%、18.58%、12.84%;C组为37.45%、21.70%、17.02%。A组中鼻甲各解剖变异与B、C两组差异有统计学意义(P<0.01)。钩突气化、上端分叉及反向弯曲在A组中的发生率为4.03%、4.83%、0.00%;B组中为7.10%、2.73%、2.18%;C组为8.51%、3.83%、7.23%。钩突反向弯曲在A组与C组间、B组与C组间分布差异有统计学意义(P<0.01)。结论:中鼻甲各解剖变异与慢性鼻鼻窦炎、鼻息肉的发生有关;钩突反向弯曲较易引发鼻息肉;钩突气化、钩突上端分叉、鼻中隔偏曲及Haller′s气房与慢性鼻鼻窦炎、鼻息肉的发生无相关性。  相似文献

鼻中隔偏曲两侧窦口鼻道复合体解剖变异的差异   总被引:29,自引:0,他引:29  
目的探讨鼻中隔偏曲患者两侧窦口鼻道复合体(ostiomeatalcomplex,OMC)的解剖变异。方法对103例不同部位鼻中隔偏曲的患者作鼻内窥镜检查和鼻窦冠状位、轴位CT扫描,比较两侧OMC解剖变异发生率和鼻窦炎发病率的差异。结果偏曲对侧中、下鼻甲肥大的发生率明显高于同侧(P<0.05),筛泡和鼻丘气房的宽度也明显大于同侧(配对t检验,P<0.01);中鼻甲反常曲线偏曲同侧的发生率明显高于对侧(配对t检验,P<0.01),其它解剖变异偏曲两侧差异无显著性。偏曲两侧鼻窦炎的发病率差异无显著性(P>0.05)。结论鼻中隔偏曲可能引起偏曲对侧中鼻甲及鼻腔外侧壁结构的代偿性改变,这种改变可能在偏曲对侧鼻窦炎的发病中起着一定的作用。  相似文献

目的:进一步探讨鼻窦骨结构变异与鼻窦炎发生的关系。方法:对109例临床疑为鼻窦炎患者进行鼻窦冠状位CT扫描。结果:鼻窦粘膜异常改变者93例(85.3%),16例粘膜呈正常影像(14.7%)。在93例鼻窦粘膜异常改变者中,鼻窦骨结构变异为70例(75.3%),其中Haller气房出现率为31.2%,中鼻甲气化为31.2%,中鼻甲反和弯曲为10.8%,钩突气化为2.2%。结论:鼻窦骨结构变异可能是鼻窦炎致病因素之一,鼻窦炎患者骨结构变异情况应对每个患者进行具体分析,同时也要考虑到其它致病因素。  相似文献

应用PCR技术研究真菌在鼻息肉及鼻窦炎发病中的作用   总被引:1,自引:1,他引:0  
目的:探讨真菌在慢性鼻窦炎、鼻息肉患者中的致病作用。方法:采用PCR方法分别检测66例慢性鼻窦炎患者(鼻窦炎组)生理盐水冲洗前、后的鼻息肉及鼻窦黏膜的真菌,同时检测21例正常志愿者(对照组)的鼻腔分泌物中的真菌,应用Y。检验比较其差异性。结果:鼻息肉组真菌阳性率:息肉为89.2%,黏膜为89.4%,分别与对照组(66.7%)比较,差异均有统计学意义(均P〈0.05);用生理盐水冲洗后,鼻息肉组真菌阳性率:息肉为7.14%,黏膜为2.63%,分别与冲洗前比较,差异均有统计学意义(均P〈0.05)。结论:真菌在鼻息肉和鼻窦炎的发病中可能发挥重要作用,PCR技术在真菌检测中有重要意义。  相似文献

目的:评价中鼻甲部分切除术在慢性鼻-鼻窦炎患者鼻内镜手术后的疗效。方法对122例已行鼻内镜手术的慢性鼻-鼻窦炎患者的临床资料进行回顾性分析,采用视觉模拟量表(VAS)、鼻腔鼻窦结局测试-20中文版(SNOT-20 CV)量表、Lund-Mackay CT方法和Lund-Kennedy鼻内镜方法进行主客观评分,综合评价中鼻甲部分切除患者与中鼻甲保留患者治疗前后的临床疗效。结果①两组患者术前合并鼻息肉和/或鼻中隔偏曲发生率差异有统计学意义(P〈0.05),而单纯合并变应性鼻炎和/或支气管哮喘的发生率差异无统计学意义(P〉0.05);②两组患者术前CT及鼻内镜评分差异有统计学意义(P〈0.05),而VAS及SNOT-20CV评分差异无统计学意义(P〉0.05);③两组患者术后半年与术前鼻内镜评分的差异有统计学意义(P〈0.05),而VAS及SNOT-20CV评分差异无统计学意义(P〉0.05)。结论行中鼻甲部分切除的慢性鼻-鼻窦炎患者多合并鼻息肉和/或鼻中隔偏曲,与中鼻甲保留患者术前及术后的生存质量无明显差异,而行中鼻甲部分切除患者术后的内镜评分明显提高。  相似文献

目的:探讨儿童鼻窦炎对咽鼓管与中耳传音功能的影响及程度,观察治疗鼻窦炎后,中耳传音功能障碍的恢复情况。方法:对儿童鼻窦炎100例和正常儿童50例行耳科检查、咽鼓管咽口观察、声阻抗检查、纯音测听,对比结果;对鼻窦炎合并耳病变的68例患者(128耳)行有针对性的治疗。结果:鼻窦炎患者中鼓膜异常率为64%、咽鼓管咽口异常率为62%、咽鼓管功能异常率为63.5%、鼓室导抗图异常率为62.5%,听力减退47.5%,与正常儿童组相比,差异有统计学意义(P<0.01)。急性鼻窦炎、亚急性鼻窦炎与慢性鼻窦炎中耳病变的发生率和程度相比,差异有统计学意义(P<0.05)。治疗鼻窦炎后,中耳传音功能障碍有明显改善。结论:儿童鼻窦炎引起咽鼓管功能的改变,中耳病变发生率较正常儿童显著增高;随病程的延长,发病率增高且程度加重。  相似文献

目的 用高分辨率CT对鼻腔鼻窦炎进行影像学分析,探讨窦口鼻道复合体(OMC)区的解剖变异与儿童鼻窦炎、成人慢性鼻-鼻窦炎的关系。 方法 回顾性分析有明显临床表现且经CT扫描确诊为慢性鼻-鼻窦炎167例的临床资料,年龄4~74岁,并将其影像学资料分为儿童组和成人组,并对其分析结果进行比较。 结果 (1)儿童组和成人组的窦腔炎症最常累及上颌窦、前组筛窦和额窦;(2)儿童组的窦口鼻道复合体区解剖变异与慢性鼻-鼻窦炎的发生没有明显关系,而成人组的窦口鼻道复合体区解剖变异与慢性鼻-鼻窦炎的发生关系显著(P<0.05)。 结论 窦口鼻道复合体区的解剖变异对儿童慢性鼻-鼻窦炎的发生不如对成人的影响大,儿童慢性鼻-鼻窦炎的诊断和手术指征不能单凭异常的鼻窦CT扫描图作为惟一依据,应该切实联系临床症状。  相似文献

OBJECTIVE: To explore the influence of the deviated septum on the development of ostiomeatal complex (OMC). METHODS: Nasal endoscopic examination and CT scan of the paranasal sinuses of 103 patients with nasal septal deviation were analyzed. The differences of anatomic variations in OMC between ipsilateral and contralateral sides in relation to the direction of septal deviation. The incidence of sinusitis on either side of the deviated septum was evaluated. RESULTS: On the side opposite to the deviated septum, the incidence of middle and inferior turbinate hypertrophy was higher than that of the deviated side(P < 0.05). The width of agger nasi and ethmoid bulla was on the contralateral side larger than that of the ipsilateral side (P < 0.01). The incidence of paradoxical curvature of middle turbinate on the ipsilateral side was higher than that of contralateral side (P < 0.01). Other anatomic variations showed no significant differences between two sides of the deviated septum (P > 0.05), nor was there any difference between the incidence of sinusitis on the two sides. CONCLUSION: Some compensatory changes in middle and inferior turbinates and lateral nasal-wall on the opposite side to the deviation might be caused by the deviated septum.  相似文献

CONCLUSIONS: Anatomical variations in the nose and paranasal sinuses are common in children. Due to the absence of a definitive relationship between anatomical variations and sinus disease, local, systemic or environmental factors appear to be more significant in pediatric sinusitis than the anatomic variations. OBJECTIVE: Chronic sinusitis is increasing in the pediatric population. Following the initial reports of functional endoscopic sinus surgery (FESS) for children, the majority of patients undergoing this procedure have experienced improved quality of life. Thus, we need to know the precise anatomy and anatomical variations of the nose, as well as the relationship between such anatomy and chronic sinusitis. PATIENTS AND METHODS: To determine the extent and distribution of disease, as well as associated anatomic abnormalities, computed tomography (CT) scans were performed on 113 children with persistent symptoms of sinusitis after failure of medical therapies. RESULTS: The maxillary sinus was the most commonly involved sinus, followed by the ethmoid, sphenoid, and frontal sinuses. There was no relationship between age and severity of sinusitis. Agger nasi cell was the most common anatomical variation, followed by septal deviation, Haller cell, concha bullosa, paradoxical middle turbinate, and Onodi cell. The prevalence of septal deviation increased with age. There was no significant relationship between the sinusitis and anatomic variations.  相似文献

Conclusions. Anatomical variations in the nose and paranasal sinuses are common in children.
