糖尿病联络护士工作模式的探讨

目的探讨糖尿病联络护士工作方法、效果和运作模式。方法在全院开展以专科护士为核心、各病区有1名护士参与的糖尿病联络护士工作小组,糖尿病联络护士对本科糖尿病患者进行转介,专科护士对转介患者进行会诊、个体化的指导。结果提高了在非糖尿病专科住院的糖尿病患者的遵医行为,提高了患者的糖尿病相关知识水平,纠正非糖尿病专科护理人员对糖尿病患者的不正确护理,新发现糖尿病患者的糖尿病专科门诊就诊率100%。结论提高兼职糖尿病专科护士的工作效率,提高非糖尿病专科住院的糖尿病患者的教育效果。

Attitudes towards the Infection Prevention and Control Nurse: an interview study

(2012) Journal of Nursing Management  20, 648–658 Attitudes towards the Infection Prevention and Control Nurse: an interview study Aim A study was undertaken involving nursing students and nurse mentors to investigate the experiences and learning needs of nursing students in relation to infection prevention. One of the objectives was to consider the views of both nursing students and mentors towards the Infection Prevention and Control Nurse (IPCN) as an important staff member in infection prevention and control. Background Infection prevention and control is a national and international priority but compliance with precautions can be low. One reason for this is staff attitudes. Infection Prevention and Control Nurses have an important role to play in the management of patient care through clinical staff and it is therefore important that they are seen as approachable and effective in their role. Methods Using a qualitative approach, data were obtained through semistructured interviews with 31 nursing students and 32 nurse mentors. Interviews were recorded, transcribed and analysed using framework analysis. Findings Three themes emerged: attitudes towards the IPCN, effects of the presence of the IPCN and preferred qualities in IPCNs. Conclusions Areas for future research are identified and recommendations made to address areas where attitudes may affect both clinical practice and the education of nursing students in clinical placements. Implications for nursing management Nurse specialists or practitioners, who are often seen within a management role, need to consider how they work with clinical staff in order to foster more collaborative relationships.

The effectiveness of a diabetes nurse clinic in treating older patients with type 2 diabetes for their glycaemic control

Aims. The aim of this study is to determine the effectiveness of a diabetes nurse clinic intervention in controlling the poor glycaemia of older patients with type 2 diabetes. Research method. This is a quasi‐experimental design with pre‐ and follow‐up tests. The study was conducted in a regional acute hospital in Hong Kong. A total of 150 (75 controls, 75 cases) poor glycaemic control older patients with diabetes were recruited in the study. Outcome measures. Biomedical and psychological data were collected at pre‐ and follow‐up period and compared between groups. Results. The study results show an effective intervention of diabetes nurse clinic in giving consultation and education to the type 2 diabetic patients. Subjects in the nurse follow‐up group showed an improvement in the HbA1c, and systolic blood pressure and reduction on healthcare utilization. Conclusion. The study provided evidence to support the diabetes nurse clinic in treating the older patients with diabetes. This study has provided confidence to the diabetes nurse to treat the unstable older patients with diabetes. Relevance to clinical practice. Diabetes mellitus is a serious health concern that most commonly affects older people. As indicated by the results of this study, this educational programme can act as an effective nursing intervention to the type 2 diabetic patients.

Nurse migration from Zimbabwe: analysis of recent trends and impacts

The migration of nursing professionals from developing countries such as Zimbabwe to industrialised countries is taking place at an alarming rate, with little signs of slowing down. In Africa, nurses form the backbone of the healthcare delivery system and their migration has a huge negative impact on health service provision. Drawing on evidence from selected health institutions, the paper shows the magnitude of migration of nurses from Zimbabwe. The paper also shows that public to private health sector migration of nurses is occurring at a significant rate. The effects of such movements are examined in detail. For instance, at the health institution level, nurse migration has led to staff shortages, and health institutions located in the disadvantaged areas have been the worst affected. The paper calls for the adoption of an integrated approach in managing and addressing the concerns of the nursing professionals.     

临床护理岗位设置的分析与思考

护理岗位的配置与管理已成为目前研究的热点问题,但由于护理人才发展的不平衡,实施过程中存在诸多难以在现阶段能够解决的实际问题。文章就临床护理岗位设置的现状与存在问题进行分析并提出建议,科学设置临床护理岗位是基础,实施规范的护理人才培训是实现临床护理岗位管理的保证。     

临床护理人员对专科护士认知的调查

目的 了解临床护理人员对专科护士的认知情况,为护理人才专科化发展及专科护士培养提供依据.方法 采用自行设计的问卷,对苏州市4家医院650名临床护理人员进行调查.结果 临床护理人员缺乏对专科护士相关信息的了解,92.2%临床护理人员支持设置专科护士岗位,30.6%临床护理人员有报名参加专科护士培训的计划,但由于名额有限和护理工作忙等原因限制临床护理人员参加专科护士进修.结论 加强对专科护士认知教育,大力发展专科护理,争取行政支持,加强专科护士队伍建设.     

Pharmacological management of akathisia in combination with psychological interventions by a mental health nurse consultant

The article describes the management of akathisia by a mental health nurse (MHN) prescriber, working in partnership with the patient. A single-case design was used to evaluate this. It highlights three features: first, MHN can safely prescribe psychiatric medication in combination with concordance therapy. Second, the value base underpinning prescribing practice is partnership, honesty and choice for the patient. Finally, the pharmacological mechanism of antipsychotic medication, which contributes towards akathisia, requires further analysis.     

Attitudes of registered nurses toward nurse practitioners

PURPOSE: Registered nurses (RNs) work closely with nurse practitioners (NPs) in all facets of patient care and often have a direct influence on the patient's perception of the professional who is directing the care, and a patient's perceptions may affect clinical outcomes. However, few studies have been conducted to measure RN acceptance of the NP. The authors surveyed RNs in southern Illinois to assess their attitudes concerning the level of care provided by NPs. Southern Illinois was chosen because of the limited number of NPs in the area and because of concerns that limited exposure might negatively influence RN attitudes about the NP role. DATA COLLECTION: A 26-item research questionnaire, previously used with school nurses, was modified to reflect RNs in general. Seven items related to demographic information, and 19 items created a five-response Likert-formatted scale (r = .93). The questionnaire was mailed to 500 randomly selected RNs from the 11 southernmost counties of Illinois. CONCLUSIONS: The results showed support of the NP role. RNs believed that NPs were knowledgeable, competent health care providers. RNs were also comfortable working with NPs and often consulted them for advice and information. They saw the role of the NP as a positive addition to the health care team. IMPLICATIONS FOR PRACTICE: One identified concern was that scores indicated RNs felt that they received limited respect from NPs. Although the scores were high in this category, some RNs had concerns that NPs might not respect or understand the difficulty of the RN role in providing patient care. NPs need to recognize their collegial and professional ties to RNs and should take time to provide positive feedback to RNs as they collaborate to provide health care. Such behavior would only solidify the health team approach.     

Advanced practice nursing: A concept analysis

A variety of terms are used to describe advanced practice nursing roles internationally. This has resulted in confusion in terminology around these roles. The aim of this concept analysis was to clarify what is meant by advanced practice nursing internationally, what attributes signify advanced practice nursing and what are its antecedents, consequences, references and related terms. Rodgers's evolutionary method of concept analysis was used. Data sources included Medline, CINAHL, Applied Social Sciences Index and Abstracts (ASSIA), Cochrane Library, Science Direct, SCOPUS, Web of Science, Dissertation Abstracts and DARE as well as relevant nursing texts and professional organization websites. The analysis reveals that there are many different articulations of the advanced practice nursing role outlined in the literature. This variety in terminology hinders developments in advanced practice nursing roles. Consensus on advanced practice nursing definitions, terminology, educational requirements and regulatory approaches is integral to the implementation of the advanced practice nursing role internationally.
