Two groups of individuals were patch tested for frequencies of contact allergy to dental materials. The patient group with orofacial and distant symptoms and complaints referred to the Department of Prosthetic Dentistry were compared with an age-sex matched control group of individuals referred to the Department of Dermatology. The results did not show any differences in patch test reactions between the patient and control groups. AIM: To describe successful root canal treatment of a patient with a true zinc oxide allergy and to discuss allergic reactions to dental materials. SUMMARY: Dental materials have been reported as aetiologic agents for both local and systemic allergic reactions. It is essential for the oral healthcare provider to recognize the clinical symptoms associated with allergic reactions and to modify dental treatment, if necessary, to prevent these reactions from occurring. This article describes an unusual case of a patient with an allergy to zinc oxide. To our knowledge, this is the first case of successful root canal treatment of a patient with confirmed zinc oxide allergy to be reported in the dental literature. KEY LEARNING POINTS: Medical and dental histories must be evaluated to prevent medical complications secondary to dental treatment. Any patient suspected of having an allergy to dental materials should be referred to a healthcare professional capable of performing and interpreting allergy tests prior to dental treatment.  相似文献

Contact allergic reactions in and around the mouth are not frequent, but they can have very unpleasant consequences. Discussed are the most important materials that can cause reactions in patients and dental personnel. Particulary the enormous increase in the use of latex gloves has caused a rise in latex allergy and contact dermatitis by rubber additives. Bacterial endocarditis (BE) is a rare and life-threatening heart infection that can be caused by oral microorganisms. Patients with specific cardiac valvular abnormalities as well as those with a history of recurrent episodes of endocarditis are considered to be at high-risk for developing BE. Antibiotic prophylaxis is recommended for high-risk individuals when bleeding is anticipated during dental procedures. Penicillins are the antibiotics of choice in preventing endocarditis, while other medications are indicated for patients with penicillin allergies. This case presentation outlines antibiotic prophylaxis prior to restorative care for a 44-year-old man who had a prosthetic heart valve, history of recurrent infective endocarditis and penicillin allergy. Intravenously administered van-comycin and gentamicin were prescribed due to the patients' level of risk and bleeding propensity. This article was written to raise the awareness of dental practitioners to the antibiotic prophylaxis options available for the treatment of patients with cardiac and associated systemic conditions.  相似文献

A patient was treated with a maxillary gold-palladium alloy fixed partial denture (FPD). Shortly after placement, the patient contracted severe contact mucositis. Patch-testing revealed a positive reaction to palladium chloride (PdCl 2 , 1% petrolatum) but not to any other component of the prosthesis. After removal of the FPD and placement of a provisional acrylic resin restoration, all signs and symptoms disappeared. A definitive metal-ceramic (titanium-porcelain) prosthesis was placed, and no signs of lesions appeared. This clinical report demonstrates that titanium may be a satisfactory alternative for patients who require prostheses and are sensitive to other metals.  相似文献

Penicillin is the drug that most often leads to allergic reactions and anaphylaxis. The incidence of adverse events triggered by penicillins is believed to be between 1% and 10%. Up to one-tenth of these episodes are life-threatening, with the most serious reactions occurring in patients with no history of allergy. The case of a 5 year, 3 month-old female who had a severe allergic reaction to amoxicillin prior to a dental appointment is described. The literature on penicillin hypersensitivity is reviewed and recommendations for management of an allergic reaction in the pediatric dental office are discussed.  相似文献
